Aortic stenosis(AS)is the most common valvular disease in the western coun-tries and is associated with aging.[1,2]Transcatheter aortic valve replacement(TAvR)now offers a therapeutic option for elderly patients who are deemed inoperable and is an excellent alternative for those who are considered high-risk for surgical valve replacement.[3,4]However,mortality after TAvR is still high with one-year and two-year mortality rates of 24.3%and 33.9%,respectively.[5]Thus,util-ization of effective risk stratification tools before TAvR cannot be overstated.

Bacterial biofilms have emerged as potential critical triggers in the pathogenesis of bisphosphonate（BP）-related osteonecrosis of the jaw（ONJ） or BRONJ. BRONJ lesions have shown to be heavily colonized by oral bacteria, most of these difficult to cultivate and presents many clinical challenges. The purpose of this study was to characterize the bacterial diversity in BRONJ lesions and to determine host immune response. We examined tissue specimens from three cohorts（n530）; patients with periodontal disease without a history of BP therapy（Control, n510）, patients with periodontal disease having history of BP therapy but without ONJ（BP, n55） and patients with BRONJ（BRONJ, n515）. Denaturing gradient gel electrophoresis of polymerase chain reaction（PCR）-amplified 16 S r RNA gene fragments revealed less bacterial diversity in BRONJ than BP and Control cohorts. Sequence analysis detected six phyla with predominant affiliation to Firmicutes in BRONJ（71.6%）, BP（70.3%） and Control（59.1%）. Significant differences（P,0.05） in genera were observed, between Control/BP, Control/BRONJ and BP/BRONJ cohorts. Enzyme-linked immunosorbent assay（ELISA）results indicated that the levels of myeloperoxidase were significantly lower, whereas interleukin-6 and tumor necrosis factor-alpha levels were moderately elevated in BRONJ patients as compared to Controls. PCR array showed significant changes in BRONJ patients with downregulation of host genes, such as nucleotide-binding oligomerization domain containing protein 2, and cathepsin G, the key modulators for antibacterial response and upregulation of secretory leukocyte protease inhibitor, proteinase 3 and conserved helix–loop–helix ubiquitous kinase. The results suggest that colonization of unique bacterial communities coupled with deficient innate immune response is likely to impact the pathogenesis of ONJ.

Results of this form are also established when 0 < p ≤ 1 where no independence or mean 0 conditions are placed on the random elements and without any geometric conditions placed on the underlying Banach space.

We consider the density dependent diffusion Nagumo equation, where the diffusion coefficient is a simple power function. This equation is used in modelling electrical pulse propagation in nerve axons and in population genetics (amongst other areas). In the present paper, the δ-expansion method is applied to a travelling wave reduction of the problem, so that we may obtain globally valid perturbation solutions (in the sense that the perturbation solutions are valid over the entire infinite domain, not just locally; hence the results are a generalization of the local solutions considered recently in the literature). The resulting boundary value problem is solved on the real line subject to conditions at z →±∞. Whenever a perturbative method is applied, it is important to discuss the accuracy and convergence properties of the resulting perturbation expansions. We compare our results with those of two different numerical methods (designed for initial and boundary value problems, respectively) and deduce that the perturbation expansions agree with the numerical results after a reasonable number of iterations. Finally, we are able to discuss the influence of the wave speed c and the asymptotic concentration value α on the obtained solutions. Upon recasting the density dependent diffusion Nagumo equation as a two-dimensional dynamical system, we are also able to discuss the influence of the nonlinear density dependence (which is governed by a power-law parameter m) on oscillations of the travelling wave solutions.

A Tutorial on Common Differential Equations and Solutions Useful for Modeling Epidemics Like COvID-19: Linear and Non-Linear Compartmentation Models

Purpose: To review some of the basic models, differential equations and solutions, both analytic and numerical, which produce time courses for the fractions of Susceptible (S), Infectious (I) and Recovered (R) fractions of the population during the epidemic and/or endemic conditions. Methods: Two and three-compartment models with analytic solutions to the proposed linear differential equations as well as models based on the non-linear differential equations first proposed by Kermack and McKendrick (KM) [1] a century ago are considered. The equations reviewed include the ability to slide between so-called Susceptible-Infected-Recovered (SIR), Susceptible-Infectious-Susceptible (SIS), Susceptible-Infectious (SI) and Susceptible-Infectious-Recovered-Susceptible (SIRS) models, effectively moving from epidemic to endemic characterizations of infectious disease. Results: Both the linear and KM model yield typical “curves” of the infected fraction being sought “to flatten” with the effects of social distancing/masking efforts and/or pharmaceutical interventions. Demonstrative applications of the solutions to fit real COvID-19 data, including linear and KM SIR fit data from the first 100 days following “lockdown” in the authors’ locale and to the total number of cases in the USA over the course of 1 year with SI and SIS models are provided. Conclusions: COvID-19 took us all by surprise, all wondering how to help. Spreading a basic understanding of some of the mathematics used by epidemiologists to model infectious diseases seemed like a good place to start and served as the primary purpose for this tutorial.

Extracellular microvesicles present in the epididymal fluid have been named epididymosomes. Many epididymosome-associated proteins are transferred to spermatozoa during their maturation in the excurrent duct. Epididymosomes are heterogeneous, with their size varying between 50 and 250 nm. Two distinct population ofepididymosomes characterized by different protein compositions and diameters have been isolated from the bovine epididymal fluid using different centrifugation protocols. One subpopulation of epididymosomes was characterized by CD9 and other tetraspanin partners. Transfer of proteins from these epididymosomes to maturing spermatozoa in co-incubation experiments was inhibited by antibodies against tetraspanin proteins. This suggests that this subpopulation of epididymosomes is involved in the acquisition of proteins involved in maturation by spermatozoa in the epididymis. The other population of epididymosomes was characterized by ELSPBPl （epididymal sperm binding protein 1）, known for its affinity for the phospholipid choline group. Flow cytometric analyses showed that ELSPBP1-positive epididymosomes only interacted with dying or dead epididymal spermatozoa in a Zn^2＋-dependent manner. BLvRA （biliverdin reductase） was identified as a partner of ELSPBP1. This enzyme reduces biliverdin to bilirubin： two molecules with powerful anti-oxidant properties. We hypothesize that BLvRA is involved in an ROS-scavenging mechanism protecting live epididymal spermatozoa against detrimental molecules （ROS） released by dying cells. Therefore, it appears that there are at least two epididymosome population with distinct functions： targeting specific proteins to transiting spermatozoa by tetraspanin-mediated membrane fusion, and protection of epididymal spermatozoa against ROS released from dying cells. Further work is needed to understand functions of epididymosomes in epididymal physiology and sperm maturation and storage.

AIM To examine whether high-flow nasal oxygen(HFNO) availability influences the use of general anesthesia(GA) in patients undergoing endoscopic retrograde cholangiopancreatography(ERCP) and endoscopic ultrasound(EUS) and associated *** In this retrospective study, patients were stratified into 3 eras between October 1, 2013 and June 30, 2014 based on HFNO availability for deep sedation at the time of their endoscopy. During the first and last 3-mo eras(era 1 and 3), no HFNO was available, whereas it was an option during the second 3-mo era(era 2). The primary outcome was the percent utilization of GA vs deep sedation in each period. Secondary outcomes included oxygen saturation nadir during sedation between periods, as well as procedure duration, and anesthesia-only time between periods and for GA vs sedation cases *** During the study period 238 ERCP or EUS cases were identified for analysis.
