Assessment of Safety and Effectiveness of Glaritus<sup>®</sup>(Wockhardt’s Insulin Glargine) in a Prospective, Multi-Centric Post Marketing Observational Study in Nepalese Having Type 2 Diabetes Mellitus

Background: Nepal is one of the fastest urbanizing countries in South Asia and is facing the consequences of urban lifestyle leading to obesity and metabolic syndrome. Type 2 diabetes mellitus (T2DM) is currently a high-burden disease in Nepal with a prevalence of 8.4%. Of these 8% - 18% patients are on insulin and 42% patients were reported to have uncontrolled diabetes in the past year. This suggests a need for better therapy options in terms of efficacy and safety. The current study was designed to investigate the effects of Insulin glargine-based therapy in Nepalese with T2DM who could not achieve adequate glycemic control with oral and non-glargine-insulin therapy. Methods: This is a prospective, multi-centric, single arm and post marketing observational study to assess the safety and effectiveness of Glaritus®(Wockhardt’s Insulin Glargine) in 52 T2DM patients from 3 (three) different study sites in Nepal (Bharatpur, Kathmandu and Pokhra) from September 2015 to December 2016. The primary objective of the study was to evaluate the safety of Glaritus®, mainly in terms of hypoglycemia, renal function tests and liver function tests. The secondary objectives were to evaluate the effectiveness of Glaritus®in terms of percentage of patients achieving HbA1c goal of less than 7%, mean changes in HbA1c & fasting plasma glucose (FPG) levels from baseline till the end of study. Results: 3.85% of subjects experienced hypoglycemia during first 3 months of therapy whereas 1.92% had similar experience in next 3 months of therapy. The mean HbA1c values reduced from 9.16% to 7.15% at the end of study. 21.05% of the enrolled subjects achieved the goal of HbA1c ®was well tolerated by the study patients. Conclusion: In patients with type 2 diabetes mellitus inadequately controlled on oral hypoglycemic agents and/or insulin, initiation with Glaritus®significantly improved glycemic control with good tolerability and acceptability. Liposomal Iron for Iron Deficiency Anemia in Women of Reproductive Age: Review of Current Evidence

Iron deficiency anemia is the most frequent nutritional deficiency disorder. Conventionally administered oral iron is associated with gastrointestinal intolerance that affects the outcomes and compliance. Liposomal iron is associated with increased absorption without causing significant adverse effects. In this review, we have discussed the technology of liposomal iron preparation, mechanisms of its absorption and clinical evidence on its utility in iron deficiency states in pregnant and non-pregnant women. Based on the available evidence, we compared liposomal iron to conventional oral iron. Encapsulation of micronized iron in liposomes is associated with lesser exposure to gastric contents, lesser interaction with food contents, no exposure to different digestive juices, targeted delivery of iron and allows lower doses to be administered in lieu of direct absorption without need for protein carriers. The available evidence suggests that liposomal iron significantly increases hemoglobin, ferritin levels in pregnant women as well as in women with iron deficiency.

Integrated approach of heavy metal pollution indices and complexity quantification using chemometric models in the Sirsa Basin, Nalagarh valley, Himachal Pradesh, India

Chemometric techniques and pollution assessment indices were applied to determine the source and intensity of pollution in the Sirsa River, Himachal Pradesh,India. Results show EC, Cr, Fe, Mn, and Ni were above the permissible limit as per the Bureau of Indian *** heavy metal pollution index(HPI) and contamination index(Cd) provided contrasting outcome and poor correlation was observed. A heavy metal evaluation index(HEI)method was developed using a multiple of the mean and correlation coefficient values to provide an alternative pollution classification. The criteria of HEI adopted for reclassification of HPI and Cdproduced comparable results; 40 % samples were labeled as low contamination,50 % as medium contamination, and 10 % as high contamination for all indices. Principal component analysis along with cluster analysis was used to identify the main factors responsible for degradation of water quality,namely discharge of industrial effluent, river bed mining,agricultural runoff, and minor natural or geogenic *** methods and chemometric study proposed here can be used as effective tools to gather information about water quality and water resource management.

Performance characterization of Bi_2O_3/Al nanoenergetics blasted micro-forming system

This paper reports a novel micro-blast driven manufacturing process for micro-forming of Aluminum foils. The micro-blast is realized by using a nanoenergetic material system comprising of Bi_2O_3 microrods and aluminum particles. There is an enhanced need of forming of thin aluminum foil structures in small regions from point of view of drug packaging etc. The process developed caters to this need by using a single shot forming process using a micro-blast source. The micro-blast that is generated from an energetic composite system is made highly tunable by modulating the peak pressure generated through the blasting process and their impact in micro-forming of thin aluminum foils is observed through parametric studies. The engineering challenge involved in these experiments is to tune the blast pressure properties in order to address the forming of thin metal sheets with limiting boundary values as defined by the failure criteria. A variety of characterization techniques related to a thorough analysis of the synthesized material viz. X-ray diffraction(XRD), Scanning Electron Microscopy(SEM) etc, are used to tune the functional properties like gauge blast pressure etc, of material system. We have found a material system that can generate a maximum peak pressure of 73.8 MPa with pressurization rate of 2460 GPas^(-1) and that is able to accomplish micro-forming on thin metal foils(around 0.3 mm thickness). Experimental investigations demonstrate that tunabilty aspect of the energetic composites when exercised can enable variant processes such as embossing, coining, drilling etc. which may be of significant utility to drug packaging industries. A proper mathematical modeling of the forming process and critical process parameters therein have also been detailed.

AIM:To access the association of forty-eight single nucleotide polymorphisms(SNPs)identified from Caucasian population with steroid-induced ocular hypertension(OHT)in India ***:Fifty-four triamcinolone-acetonide(TA)and for ty-seven dexamethasone(Dex)administered subjects were enrolled in the study after a written *** pressure(IOP)values were recorded for a period of 6-month post steroid injections and patients were grouped as steroid-responders(SR:IOP≥21 mm Hg)and non-responders(NR:IOP≤20 mm Hg).Genomic DNA was isolated from peripheral venous ***-eight SNPs identified in TA treated Caucasian patients by genome wide association study(GWAS)were genotyped using iPLEXTM MassA RRAY among TA as well as Dex administered Indian *** data of 48 general subjects from a previous study were considered as reference controls for statistical *** frequencies were calculated and P-value,Chi-square and odds ratio at 95%confidenceinterval of group A(steroid treated vs controls),group B(SR vs NR),group C(phenotype correlation:influence of time,severity and gender on IOP rise),were calculated.P30 mm Hg)and immediate(<1 wk)among TA-SR patients while it was noticed to be mild(<30 mm Hg)and between 1-2 mo among Dex-SR *** regression for risk factor correlation with OHT remained non-significant,hence these factors were not considered as confounding parameters for further ***133,rs34016742,rs274554,rs10936746,rs274547,rs804854,rs7751500,rs359498,and rs7547448 SNPs significantly varied even after Bonferroni corrections(P<0.0025;group A).rs1879370(TA)and rs6559662(Dex)were significantly(P<0.05)associated with OHT(group B).rs133(severe IOP rise),rs11047639 and rs1879370(male gender),and rs11171569(immediate IOP rise)significantly(P<0.05)infl
