Web-based system for training and dissemination of a magnification chromoendoscopy classification

AIM： To evaluate the use of web-based technologies to assess the learning curve and reassess reproducibility of a simplified version of a classification for gastric magnification chromoendoscopy （MC）. METHODS： As part of a multicenter trial, a hybrid approach was taken using a CD-ROM, with 20 films of MC lasting 5 s each and an ＂autorun＂ file triggering a local HTML frameset referenced to a remote questionnaire through an Internet connection. Three endoscopists were asked to prospectively and independently classify 10 of these films randomly selected with at least 3 d apart. The answers were centrally stored and returned to participants together with adequate feedback with the right answer. RESULTS： For classification in 3 groups, both intra- [Cohen＇s kappa （K） = 0.79-1.00 to 0.89-1.00] and inter-observer agreement increased from 1st （moderate） to 6th observation （k = 0.94）. Also, agreement with reference increased in the last observations （0.90, 1.00 and 1.00, for observers A, B and C, respectively）. Validity of 100% was obtained by all observers at their 4th observation. When a 4th （sub）group was considered, inter-observer agreement was almost perfect （K = 0.92） at 6th observation. The relation with reference clearly improved into K （0.93-1.00） and sensitivity （75%-100%） at their 6th observations. CONCLUSION： This MC classification seems to be easily explainable and learnable as shown by excellent intra- and inter-observer agreement, and improved agreement with reference. A web system such as the one used in this study may be useful for endoscopic or other image based diagnostic procedures with respect to definition, education and dissemination.

Obesity is a significant and escalating health issue both in Brazil and globally, with over 650 million overweight adults worldwide. The treatment of obesity can be performed clinically, endoscopically or surgically;surgical treatment proves to be safe and more effective in terms of weight loss and long-term maintenance. Objective: This study aimed to monitor the progress of weight loss and comorbidity control in patients undergoing sleeve gastrectomy with duodenal bipartition. Methods: This pilot project involved 8 patients divided into 2 arms. In the first arm, patients underwent sleeve gastrectomy with Roux-en-Y duodenoileal transit bipartition (S-RYDITB), while in the second arm, patients underwent sleeve gastrectomy with Roux-en-Y duodenojejunal transit bipartition (S-RYDJTB). Both procedures involved Roux-en-Y reconstruction without duodenal exclusion. In S-RYDITB, the duodenal-ileal anastomosis was performed 300 cm from the ileocecal valve (ICV), creating a 250 cm common channel and a 50 cm alimentary channel. In S-RYDJTB, a biliopancreatic loop was created 200 cm from the angle of Treitz, with a 1 m alimentary channel. Results: Five patients underwent the procedures, with one undergoing S-RYDITB and four undergoing S-RYDJTB. No adverse events such as hospitalizations, readmissions, reoperations, fistulas, bleeding, pulmonary embolism, diarrhea, dumping syndrome, or hypoglycemia occurred during the study period. The mean length of hospital stay was 2 days. The average BMI decreased from 37.27 kg/m2 preoperatively to 29.48 kg/m2 after 6 months. The significant percentage of weight loss was 21.22%, with excess weight loss of 63.6%. Ninety-five percent remission of comorbidities, including hypercholesterolemia, hypertriglyceridemia, diabetes, hypertension, steatosis, and pre-diabetes. Sustainable transit vehicle tracking service,using intelligent transportation system services and emerging communication technologies:A review

The most salient problems of transit vehicle service in Latin American intermediate cities include:the high number of passengers involved in traffic accidents;traffic congestion caused by transit vehicles,and pollution generated by these vehicles,which increases in high congestion *** improve upon this situation,a research was conducted on the transit vehicle tracking service,which is a basic service for implementing mobility solutions for the aforementioned problems,the most relevant characteristics of this service for the context of Latin American intermediate cities were identified,and an implementation was *** paper presents the four stages of the study:(a)a review of the state-of-the-art of services or systems related to vehicle tracking,including wireless communications technologies,implemented sustainability approaches,usage of special algorithms for efficiency improvement,and the intelligent transportation system(ITS)architecture used as a basis;(b)the process of identifying relevant characteristics of the service for a given context;(c)proposal of an ITS architecture for this service in an intermediate city,its requirements and the suggested technologies;and(d)development of experiments for validating usage of the key suggested *** review allowed to identify the main service characteristics,with regard to vehicle positioning technologies,the recommended wireless communication technology(long range,LoRa),energy consumption considerations,and use of artificial intelligence(AI)to calculate waiting time of users at bus ***,an ITS architecture for the city of Popayan(Colombian city)considering the aforementioned characteristics is proposed,and the experiments related to the use of these technologies are described in detail.

BACKGROUND The association between congenital heart disease and chronic kidney disease is well *** mechanisms of kidney damage associated with congenital heart disease have been *** etiology of kidneydisease has commonly been considered to be secondary to focal segmental glomerulosclerosis(FSGS),however,this has only been demonstrated in case reports and not in observational or clinical *** To identify baseline and clinical characteristics,as well as the findings in kidney biopsies of patients with congenital heart disease in our *** This is a retrospective observational study conducted at the Nephrology Depart-ment of the National Institute of Cardiology“Ignacio Chávez”.All patients over 16 years old who underwent percutaneous kidney biopsy from January 2000 to January 2023 with congenital heart disease were included in the *** Ten patients with congenital heart disease and kidney biopsy were *** average age was 29.00 years±15.87 years with pre-biopsy proteinuria of 6193 mg/24 h±6165 mg/24 *** most common congenital heart disease was Fallot’s tetralogy with 2 cases(20%)and ventricular septal defect with 2(20%)*** the 10 cases,one case of IgA nephropathy and one case of membranoproliferative glomerulonephritis associated with immune complexes were found,receiving specific treatment after histopathological diagnosis,delaying the initiation of kidney replacement *** remaining 8 cases(80%),one case of FSGS with perihilar variety was found,while the other 7 cases were non-specific *** Determining the cause of chronic kidney disease can help in delaying the need for kidney replacement *** 2 out of 10 patients in our study,interventions were performed,and initiation of kidney replacement therapy was *** studies are needed to determine the usefulness of kidney biopsy in patients with congenital heart disease.

Intelligent collision risk detection in medium-sized cities of developing countries,using naturalistic driving:A review

Traffic accidents are one of the most serious problems worldwide,being one of the leading causes of death and economic loss in the ***-and middle-income countries,mainly their medium-sized cities,are among the most affected by this problem.93%of traffic accidents occur in low and middle-income countries,even though these countries have approximately 60%of the world’s *** occurs mainly because in these types of countries,especially in medium-sized cities(target context),there are no ideal conditions for driving,such as adequate road infrastructure,good condition of vehicles,and rigorous safety *** data analysis techniques including machine learning(ML)have increasingly been used to solve this *** driving(ND)can be applied as a data collection method that provides information on traffic *** commonly uses a vehicle’s kinematic data to detect high-risk driving behaviors that could cause an *** objectives of this document are to present a review of different alternatives that help in data collection and creation of intelligent solutions related to detection of possible traffic accidents,principally using ND;and to propose an intelligent collision risk detection system(ICRDS)for identification of areas with a high probability of TA in the target *** the review,it was possible to analyze and evaluate the devices,variables and algorithms that help characterize a risk event in driving,considering the target *** development of a prototype of an ICRDS for a medium-sized city in a developing country is considered viable,considering the identified components,with the aim of identifying risk events in driving,and areas of high probability of accidents in the city.

Conversation Table as an Environment for Re-Signification of Subjectivity and Identities in Portuguese as a Foreign Language

As foreign language teachers, we always wondered about the silence of our students during the lessons. However, after we started the project ＂Conversation Table in Portuguese＂（CTP）, we realized that the students assumed a more active role during the interactions, probably due to the less hierarchical situation they were in. The CTP provided the students the opportunity to express themselves in a foreign language without the interdiction they felt inside the classroom. Within this new context, the lack of confidence they might have been facing when communicating in a foreign language started to disappear, giving place to a motivation to express what they felt in this ＂new＂ language. Therefore, the target language started to become a new locus, a new way that made them not feel scared, but free and curious to say things that they would not say in similar situations when communicating in their mother tongue. In other words, the foreign language can be the place where everything seems to be possible as we do not have the control or interdiction imposed by the social values presented in our society and manifested in our mother tongue. In this study, we analyze some discursive situations taken from a session of CTP in which the students show their new identities in the foreign language. The results show that acquiring another language is much more than acquiring new linguistic aspects of communication, but also giving birth to new identities that make us become new subjects.
