Global lessons on delivery of primary healthcare services for people with non-communicable diseases:convergent mixed methods

Objective To extract key lessons on primary healthcare(PHC)service delivery strategies for non-communicable diseases(NCD)from the work of researchers funded by the Global Alliance for Chronic Diseases(GACD).Design A convergent mixed methods study that extracted data using a standardised template from research projects funded by the GACD that focused on *** strategies implemented in these studies were mapped onto the PHC Performance Initiative *** qualitative interviews were conducted with researchers from purposefully selected projects to understand the strategies and contextual factors in more *** PHC contexts from low or middle-income countries(LMIC)as well as vulnerable groups within high-income *** came from all regions of the world,particularly East Asia and Pacific,sub-Saharan Africa,South Asia,Latin America and *** The study extracted data on 84 research projects and interviewed researchers from 16 research *** Research projects came from all regions of the world,and mainly focused on diabetes(35.3%),hypertension(28.3%)and mental health(27.6%).Mapped onto the PHC Performance Initiative framework:49.4%focused on high-quality PHC(particularly the comprehensiveness of NCD care,41.2%);41.2%on the availability of PHC services(particularly the competence of healthcare workers,36.5%);35.3%on population health management(particularly community-based services,35.3%);34.1%on facility organisation and management(particularly team-based care,20.0%)and 31.8%on access(particularly digital technology,23.5%).Most common strategies were task shifting and training to improve the comprehensiveness of NCD care through community-based *** factors related to inputs:infrastructure,equipment and medication,workforce(particularly community health workers),finances,health information systems and digital *** Key strategies and contextual factors to improve PHC service del

Therapeutic effect of Sinapic acid in aluminium chloride induced dementia of Alzheimer's type in rats

Objective:To evaluate the effect of sinapic acid against Aluminium chloride-induced dementia of Alzheimer's disease (AD) type in ***:The study was designed to induce dementia by chronic exposure of aluminium chloride at a dose of 175 mg/kg, p.o. for a period of 25 days in rats and then divided into different groups,i.e. Treatment group, negative control and two groups of sinapic acid, (at a dose of 20 and 40mg/kg, p.o.), where these groups treated and observed till the 35th day of experimental trial. The behavioural, neuronal and biochemical parameters were determined during or end of experiment. Histological changes in the brain were also ***:Aluminium chloride at a dose of 175 mg/kg, o.p. had significantly induced the dementia and sinapic acid, at a dose of 40 mg/kg, p.o., possessed therapeutic effect against Aluminium chloride induced-dementia of AD type in ***:Sinapic acid is a class of compound wide spread in plant kingdom and could be a better source of neutraceuticals in brain disorders. The compound showed anin vivo MAO-A and MAO-B inhibiting activity and their role in Alzheimer's disease type of dementia was unexplored. The article also provides information on acute toxicity of sinapic acid with no toxicological sign on brain with chronic dose of AlCl3.

Staphylococcal bullous impetigo in a neonate

An otherwise healthy, full-term neonate presented at day 15 of life to the pediatric emergency with generalized papulo-pustular rash for 2 d. This was finally diagnosed as bullous impetigo caused by Staphylococcus aureus(S. aureus). The skin lesions decreased significantly after starting antibiotic therapy and drainage of blister fluid. There was no recurrence of the lesions on followup. This case of generalized pustular eruption due to S. aureus in a neonate is reported, as it poses a diagnostic dilemma and can have serious consequences if left untreated.

In this study, we report the fabrication of engineered iron oxide magnetic nanoparticles (MNPs)functionalized with anti-human epidermal growth factor receptor type 2 (HER2) antibody totarget the tumor antigen HER2. The Fc-directed conjugation of antibodies to the MNPs aidstheir efficient immunospecific targeting through free Fab portions. The directional specificity ofconjugation was verified on a macrophage cell line. Immunofluorescence studies on macrophagestreated with functionalized MNPs and free anti-HER2 antibody revealed that the antibodymolecules bind to the MNPs predominantly through their Fc portion. Different cell lines with different HER2 expression levels were used to test the specificity of our functionalized nanoprobe formolecular targeting applications. The results of cell line targeting demonstrate that these engineered MNPs are able to differentiate between cell lines with different levels of HER2 expression.

Thermal Characterization of Se80-xTe20InxGlasses Using Iso-Conversional Methods

Alloys of Se80-xTe20Inx glassy system are obtained by quenching technique and crystallization kinetics has been studied using Differential Scanning Calorimetric [DSC] technique. Well defined endothermic and exothermic peaks are ob- served at glass transition temperature (Tg) and crystallization temperature (Tc). From DSC scans, Tc is obtained at dif- ferent heating rates (5, 10, 15, 20, 25 K/min). It is observed that Tc increases with increasing heating rate for a particular glassy alloy. Activation energy of crystallization (Ec) has been calculated by different Non-isothermal Iso-conversional methods, i.e., Kissinger-Akahira-Sunose [KAS], Friedman, Flynn-wall-Ozawa [FWO], Friedman-Ozawa [FO] and Sta-rink methods. It is observed that Ec is dependent on extent of crystallization (α). Activation energy is also found to vary with atomic percentage of In in ternary Se80-xTe20Inx glassy system. The compositional dependence of Ec shows a re-versal in the trend at x = 15 in Se80-xTe20Inx, which is explained in terms of mechanically stabilized structure at this composition.

The aim of this study was to investigate and characterize different crystal forms of arteether,a rapidly acting,highly potent antimalarial drug for the treatment of acute Plasmodium falciparum *** different crystal forms were prepared utilizing various polar and non-polar *** electron microscopy(SEM)revealed differences in the surface characteristics of the six forms from those of a commercial *** scanning calorimetry(DSC)revealed the absence of a desolvation endotherm indicating that the forms were neither hydrates nor *** X-ray diffraction(PXRD)patterns of the forms showed much weaker major peaks than in the commercial sample indicating them to be less *** and dissolution studies showed that the most amorphous form was the most soluble and possessed the highest antimalarial activity.

Strong Convergence of a New Three Step Iterative Scheme in Banach Spaces

In this paper, we suggest a new type of three step iterative scheme called the CR iterative scheme and study the strong convergence of this iterative scheme for a certain class of quasi-contractive operators in Banach spaces. We show that for the aforementioned class of operators, the CR iterative scheme is equivalent to and faster than Picard, Mann, Ishikawa, Agarwal et al., Noor and SP iterative schemes. Moreover, we also present various numerical examples using computer programming in C++ for the CR iterative scheme to compare it with the other above mentioned iterative schemes. Our results show that as far as the rate of convergence is concerned 1) for increasing functions the CR iterative scheme is best, while for decreasing functions the SP iterative scheme is best;2) CR iterative scheme is best for a certain class of quasi-contractive operators.

目的:从主客观方面比较超声乳化术(PKE)和手法小切口白内障手术(SICS)术后致干眼的研究,并对致病因素进行分析。方法:单中心前瞻性研究。选取2013-07/2014-12接受同一医生治疗白内障手术及人工晶体植入术患者,100例患者100眼分为两组(第一组:50例50眼手法小切口白内障手术患者;第二组:50例50眼超声乳化术患者)。术后所有干眼患者都作表记录,从主客观方面评估其第1、4、12wk术后干眼情况,并分析其致病诱因。结果:干眼测试(DET)值在主客观方面无显著差异(P>0.05)。相较于各自术前值,尽管期间无显著变化(P>0.05),两组在第12wk目标干眼测试值呈持续下降趋势(P<0.05)。超声乳化术在第12wk结束时干眼测试值更主观。结论:白内障术后干眼症是不可避免的。在术后12wk前超声乳化术组和手法小切口白内障手术组术后干眼测试值影响相似。进一步研究需要更多样本、更长随访时间的随机多中心临床试验来证实。

The High Frequency Decay Parameter κ(Kappa) in the Region of North East India

The high frequency decay parameter κ has been considered as one of the important parameters required in the simulation of earthquake strong ground motions necessary for the proper evaluation of seismic hazard of a region. The present study estimated “κ” for the highly seismic active region of North East India. The spectral analysis of 598 accelerograms of 32 earthquakes has been done using [1] approach for this purpose. The average values of “κ” have been found to be 0.049, 0.047 and 0.040 for L-, T- and V-component respectively. The distance dependence of κ is not significant in the region. The κ 0 (κ at R = 0) for soft rock stations is found to be more than those of hard rock sites in consistent with other similar studies. The correlation between “κ” and earthquake magnitude at most of the stations for the region under study is not significant which indicates that κ depends on the site conditions in the region. The κ values estimated in the present study are useful for the evaluation of seismic hazard of the region.

Plasmodium parasites causing malaria have developed resistance to most of the antimalarials in use,in-cluding the artemisinin-based combinations,which are the last line of defense against *** ne-cessitates the discovery of new targets and the development of novel *** falciparum alanyl aminopeptidase(PfA-M1)and leucyl aminopeptidase(PfA-M17)belong to the M1 and M17 family of metalloproteases respectively and play critical roles in the asexual erythrocytic stage of *** enzymes have been suggested as potential antimalarial drug *** we describe the devel-opment of peptidomimetic hydroxamates as PfA-M1 and PfA-M17 dual *** of the compounds described in this study display inhibition at sub-micromolar range against the recombinant PfA-M1 and *** importantly,compound 26 not only exhibits potent malarial aminopeptidases inhibitory activities(PfA-M1 K i=0.11±0.0002μmol/L,PfA-M17 K_(i)=0.05±0.005μmol/L),but also possesses remarkable selectivity over the mammalian counterpart(pAPN K_(i)=17.24±0.08μmol/L),which endows 26 with strong inhibition of the malarial parasite growth and negligible cytotoxicity on human cell *** structures of PfA-M1 at atomic resolution in complex with four different compounds including compound 26 establish the structural basis for their inhibitory ***,the terminal ureidoben-zyl group of 26 explores the S2' region where differences between the malarial and mammalian enzymes are apparent,which rationalizes the selectivity of ***,our data provide important insights for the rational and structure-based design of selective and dual inhibitors of malarial aminopeptidases that will likely lead to novel chemotherapeutics for the treatment of malaria.
