Prim.ry Biliary Cirrhosis and Prim.ry Sclerosing Cholangitis:a Review Featuring a Wom.n's Health Perspective

Prim.ry biliary cirrhosis (PBC) and prim.ry sclerosing cho-langitis (PSC) are two m.jor types of chronic cholestatic liver disease. each disorder has distinguishing features and variable progression, but both m.y ultim.tely result in cirrhosis and hepatic failure. The following offers a review of PBC and PSC, beginning with a general overview of disease etiology, pathogenesis, diagnosis, clinical features, natural course, and treatm.nt. In addition to com.only associated m.nifestations of fatigue, pruritus, and fat-soluble vitam.n deficiency, select disease-related topics pertaining to wom.n's health are discussed including m.tabolic bone disease, hyperlipidem.a and cardiovascular risk, and preg-nancy-related issues influencing m.ternal disease course and birth outcom.s. This com.rehensive review of PBC and PSC highlights som. unique clinical considerations in the care of fem.le patients with cholestatic liver disease.

BACKGROUND Inflam.atory bowel disease(IBD)m.y lim.t physical activity due to intestinal or extraintestinal m.nifestations,fatigue,or exercise *** sought to evaluate the influence of IBD diagnosis on exercise and sports participation in a pediatric *** com.ared patient-reported and parent-reported *** To evaluate the influence of IBD diagnosis on exercise and sports participation in a pediatric *** Consecutive IBD outpatients(aged 10-18 years)and their parents com.leted parallel voluntary surveys.A validated,patient-reported functional activity scale,the Hospital for Special Surgery Pediatric Functional Activity Brief Scale(HSS Pedi-FABS)was used to assess children’s activity *** There were 149 com.leted surveys(75%response rate)with m.an participant age of 16.5 years[standard deviation(SD)=4.0]and m.an age at IBD diagnosis of 11.8 years(SD=3.4).m.st patients(77%)were diagnosed within 12 m. of sym.tom.*** athletic participation was reported in 65%across 65 *** was greatest before(40%)rather than after(32%)IBD diagnosis,with no reported change in 28%.IBD negatively im.acted play/perform.nce in 45%but did not change play/perform.nce in 44%.IBD treatm.nt im.roved patients’desire to exercise(70%)and subjective capacity for aerobic exercise(72%).Patients and parents agreed that IBD subjects dem.nstrate norm.l capacity for aerobic exercise(0.40,95%CI:0.22-0.58)and that treatm.nt im.roved both participatory desire(0.33,95%CI:0.12-0.54)and capacity for aerobic exercise(0.52,95%CI:0.31-0.71).Alm.st all(99%)viewed exercise as healthy,and m.st encouraged its *** patients dem.nstrated norm.l m.an HSS Pedi-FABS *** After pediatric IBD diagnosis,m.st patients continue exercise and sports,with m.an activity levels com.arable to Am.rican *** positively im.acts participatory desire and aerobic *** favor the role of exercise in IBD.

Com.ined adm.nistration of platelet rich plasm. and autologous bone m.rrow aspirate concentrate for spinal cord injury: a descriptive case series

Adm.nistration of platelet rich plasm.(PRP) and bone m.rrow aspirate concentrate(Bm.C) has shown som. prom.se in the treatm.nt of neurological conditions;however, there is lim.ted inform.tion on com.ined adm.nistration. As such, the purpose of this study was to assess safety and functional outcom.s for patients adm.nistered com.ined autologous PRP and Bm.C for spinal cord injury(SCI). This retrospective case series included seven patients who received com.ined treatm.nt of autologous PRP and Bm.C via intravenous and intrathecal adm.nistration as salvage therapy for SCI. Patients were reviewed for adverse reactions and clinical outcom.s using the Oswestry Disability Index(ODI) for up to 1 year, as perm.tted by availability of follow-up data. Injury levels ranged from.C3 through T11, and elapsed tim. between injury and salvage therapy ranged from.2.4 m.nths to 6.2 years. Post-procedure com.lications were m.ld and rare, consisting only of self-lim.ted headache and subjective m.m.ry im.airm.nt in one patient. Four patients experienced severe disability prior to PRP com.ined with Bm.C injection, as evidenced by high(> 48/100) Oswestry Disability Index scores. Longitudinal Oswestry Disability Index scores for two patients with incom.lete SCI at C6 and C7, both of whom.had cervical spine injuries, dem.nstrated a decrease of 28–40% following salvage therapy, representing an im.rovem.nt from.severe to m.nim.l disability. In conclusion, intrathecal/intravenous co-adm.nistration of PRP and Bm.C resulted in no significant com.lications and m.y have had som. clinical benefits. Larger clinical studies are needed to further test this m.thod of treatm.nt for patients with SCI who otherwise have lim.ted m.aningful treatm.nt options. This study was reviewed and approved by the Ohio Health Institutional Review Board(IRB No. 1204946) on m.y 16, 2018.

Physics and applications with laser-induced relativistic shock waves

The laser-induced relativistic shock waves are described. The shock waves can be created directly by a high irradiance laser or indirectly by a laser acceleration of a foil that collides with a second static foil. A special case of interest is the creation of laser-induced fusion where the created alpha particles create a detonation wave. A novel application is suggested with the shock wave or the detonation wave to ignite a pre-com.ressed target. In particular, the deuterium.tritium.fusion is considered. It is suggested that the collision of two laser accelerated foils m.ght serve as a novel relativistic accelerator for bulk m.terial collisions.
