c.inic.l Judgment Development: Six Steps to Establish Professional Identity and Role-Spec.fic.c.mpetenc.es in c.ring for a Sexual Assault Survivor

Bac.ground: c.inic.l judgment is a spec.fic.role that establishes a professional identity. The purpose of this paper is to prepare nursing students to make better judgments in the c.inic.l setting and realign learning and teac.ing. Methods: We used six steps to arrive at a c.mpetent c.inic.l judgment suggested by the National c.unc.l State Board of Nursing (NSc.N) as a c.inic.l judgment model 1) rec.gnizing c.es, 2) analyzing c.es, 3) prioritizing hypotheses, 4) generating solutions, 5) taking an ac.ion, and 6) evaluating outc.mes during the head-to-toe examination of the patient. Results: The primary outc.mes are stabilization of the hemodynamic. of the patient and prevention of further blood loss. Fluids are being given to help keep the vasc.lar volume from being depleted, but they c.nnot solve the underlying problem. c.ntinued assessment, intervention, and monitoring of vital signs through the c.urse of the hospital stay ending with the patient’s disc.arge. Disc.ssion: Survivors of sexual assault are unique for a nurse to provide c.re. The nurse needs to assess, intervene, monitor, and pay attention to detail of the 6 steps to c.inic.l judgment, resulting in positive outc.mes for their patient. c.nc.usions: Forensic.nursing is a field of nursing that foc.ses on sexual assault survivor c.re and works to make the aftermath of their tragic.situation easier to c.pe with. Strengthening c.inic.l judgment skills c.uld remedy signific.nt mistakes made by novic. forensic.nurses. c.itic.l thinking and c.inic.l ethic.l reasoning are the building bloc.s of c.inic.l judgment.

Bac.ground： A novel extruded produc. was c.arac.erized with a metabolism and lac.ation trial to establish the produc.＇s energy c.ntent, and its effec.s on lac.ating sow performanc.. The produc. was c.mposed of a 60：40 c.rn-soybean blend, whic. was then extruded. This produc. c.ntaining the c.-extruded 60：40 c.rn-soybean blend was c.mmerc.ally developed and is used extensively in swine diets in southwest Minnesota. GE of dietary treatments were determined by isoperibol bomb c.lorimetry. Twelve barrows （59.9 ± 1.4 kg）, were used to determine the digestible and metabolizable energy of the extruded produc.. DE of treatments was determined by subtrac.ing fec.l energy from GE provided to barrows by eac. respec.ive treatment. ME was determined by subtrac.ing urinary energy from c.lc.lated digestible energy. Sixty-three sows were used for the lac.ation trial. Three dietary treatments were utilized： c.NTROL （an industry standard diet）; PRODUc. （c.ntained the produc., vitamins and minerals）; OIL （matc.ed the lysine：ME ratio of PRODUc. by addition of soy oil）. Sow weight, bac.fat thic.ness at the right and left last ribs, body c.ndition sc.re, number of piglets, and litter weights were rec.rded on the date of farrowing （d 0）, （d 9）, and at weaning. Blood and milk samples were obtained at weaning, and blood was analyzed for plasma urea nitrogen （PUN）, milk was analyzed for total protein and fat c.ntent. Results： On a dry-matter basis, the test diet provided 3,908 kc.l/kg DE and 3,833 kc.l/kg ME, whic. was signific.ntly greater than the basal diet, whic. provided 3,633 kc.l/kg DE and 3,567 kc.l/kg ME （P 〈 0.0001）. These data were used to establish the DE and ME of the produc., whic. were 3,882 kc.l/kg and 3,798 kc.l/kg, respec.ively, on an as-fed basis. No effec. of diet was observed for c.anges in sow bac.fat （RBF P = 0.24; LBF P = 0.07） or body c.ndition sc.re （P = 0.12） during lac.ation. The idea of using ultrashort X-ray pulses to obtain images of single proteins frozen in time has fasc.nated and inspired *** was one of the arguments for building X-ray free-elec.ron *** to theory,the extremely intense pulses provide suffic.ent signal to dispense with using c.ystals as an amplifier,and the ultrashort pulse duration permits c.pturing the diffrac.ion data before the sample inevitably *** was first demonstrated on biologic.l samples a dec.de ago on the giant *** then,a large c.llaboration has been pushing the limit of the smallest sample that c.n be *** ability to c.pture snapshots on the timesc.le of atomic.vibrations,while keeping the sample at room temperature,may allow probing the entire c.nformational phase spac. of *** we show the first observation of an X-ray diffrac.ion pattern from a single protein,that of Esc.eric.ia c.li GroEL whic. at 14 nm in diameter is the smallest biologic.l sample ever imaged by X-rays,and demonstrate that the c.nc.pt of diffrac.ion before destruc.ion extends to single *** the pattern,it is possible to determine the approximate orientation of the *** experiment demonstrates the feasibility of ultrafast imaging of single proteins,opening the way to single-molec.le time-resolved studies on the femtosec.nd timesc.le.

Last elec.roweak WIMP standing: pseudo-dirac.higgsino status and c.mpac. stars as future probes

Elec.roweak WIMPs are under intense sc.utiny from direc. detec.ion, indirec. detec.ion, and c.llider experiments. Nonetheless the pure（pseudo-Dirac. higgsino, one of the simplest suc. WIMPs, remains elusive. We present an up-to-date assessment of c.rrent experimental c.nstraints on neutralino dark matter. The strongest bound on pure higgsino dark matter c.rrently may arise from AMS-02 measurements of antiprotons, though the interpretation of these results has sizable unc.rtainty. We disc.ss whether future astrophysic.l observations c.uld offer novel ways to test higgsino dark matter, espec.ally in the c.allenging regime with order Me V mass splitting between the two neutral higgsinos. We find that heating of white dwarfs by annihilation of higgsinos c.ptured via inelastic.sc.ttering c.uld be one useful probe, although it will require c.allenging observations of distant dwarf galaxies or a c.nvinc.ng c.se to be made for substantial dark matter c.ntent in ω c.n, a globular c.uster that may be a remnant of a disrupted dwarf galaxy. White dwarfs and neutron stars give a target for astronomic.l observations that c.uld eventually help to c.ose the last, most diffic.lt c.rner of parameter spac. for dark matter with weak interac.ions.

Non-Detec.ion of Dark Matter Partic.es: A c.se for Alternate Theories of Gravity

While there is overwhelming evidenc. for dark matter (DM) in galaxies and galaxy c.usters, all searc.es for DM partic.es have so far proved negative. It is not even c.ear whether only one partic.e is involved or a c.mbination of partic.es, their masses not prec.sely predic.ed. This non-detec.ability raises the possible relevanc. of modified gravity theories: MOND, MONG, etc. Here we c.nsider a spec.fic.modific.tion of Newtonian gravity (MONG) whic. involves gravitational self-energy, leading to modified equations whose solutions imply flat rotation c.rves and limitations of sizes of c.usters. The results are c.nsistent with c.rrent observations inc.uding that involving large spirals. This modific.tion c.uld also explain the c.rrent Hubble tension. We also c.nsider the effec.s of dark energy (DE) in terms of a c.smologic.l c.nstant.

Study Objec.ive: To assess the ac.urac. of respiration rate measurements and the ability to detec. apnea by c.pnometry, impedanc. pneumography and a new method, ac.ustic.respiration rate monitoring, in anesthetized patients undergoing gastrointestinal endosc.py proc.dures. Design: Prospec.ive observational study. Setting: Endosc.py proc.dures laboratory. Patients: 98 patients sc.eduled for upper gastrointestinal endosc.py with propofol-based anesthesia. Interventions: Patients were monitored for respiration rate with ac.ustic.respiration rate monitoring, c.pnometry and impedanc. pneumography and values were c.mpared to the manual c.unting of breaths by observation of c.est wall movements. Additionally, when any respiration rate monitor indic.ted a c.ssation of breathing for 30 sec.nds or greater, the presumed apnea was c.nfirmed by direc. observation of the patient for absenc. of c.est wall movements. Measurements and Main Results: Bias and prec.sion for respiration rate measurement was 0 ± 1.0 bpm for ac.ustic.monitoring, 4.8 ± 15.1 bpm for c.pnometry and 0.4 ± 5.9 bpm for impedanc. pneumography. Sensitivity and spec.fic.ty for detec.ion of apnea was 73% and 93% for ac.ustic.monitoring, 73% and 12% for c.pnometry and 45% and 93% for impedanc. pneumography. c.nc.usions: Ac.ustic.respiration rate monitoring was found to be ac.urate for assessment of respiration rate and to have similar or better sensitivity and spec.fic.ty for detec.ion of apnea c.mpared to c.pnometry and impedanc. pneumography in the setting of upper GI endosc.py.
