Revascularization and outcomes in Veterans with moderate to severe ischemia on myocardial perfusion imaging

Background： The prevalence of ischemia on nuclear myocardial perfusion imaging （MPI） has been decreasing. Recent research has questioned the benefit of invasive revascularization for patients with moderate to severe ischemia. We hypothesized that patients with moderate to severe ischemia could routinely undergo successful revascularization. Methods： We analyzed data from 544 patients who underwent an MPI at a single academic Veterans Affairs Medical Center. Patients with moderate to severe ischemia, defined as a summed difference score （SDS） 8 or greater, were compared to the rest of the cohort. Results： Of the total cohort （n=544）, 39 patients had MPI studies with resultant moderate to severe ischemia. Patients with ischemia were more likely to develop coronary artery disease （74.4% versus 38.8%, P〈0.0001） and have successful revascularization （38.5% versus 4.0%, P〈0.0001） during the following year. Revascularization was attempted in 31 patients with moderate to severe ischemia, though only 15（47%） of these attempts were successful, Ischemia was predictive of myocardial infarction （5.1% versus 0.8%, P=0.01） within I year. Conclusion： Moderate to severe ischemia is an uncommon finding in a contemporary nuclear laboratory. Among patients with ischemia, revascularization is typically attempted but is frequently unsuccessful. Trial registration： This trial does not appear on a registry as it is neither randomized nor prospective.

Hepatocellular carcinoma(HCC)is the most common primary tumor of the liver and often evolves from regenerating nodules in liver *** there have been significant advances in the treatment of HCC,the overall prognosis in patients with a large tumor burden,vascular invasion,or extrahepatic spread remains poor.1 The magnitude of liver stiffness in cirrhosis has been consistently associated with an increased risk of development of HCC.

Temporal variability and sources of PFAS in the Rio Grande,New Mexico through an arid urban area using multiple tracers and highfrequency sampling

Per-and polyfluoroalkyl substances(PFAS)are ubiquitous in the environment but sources are not well defined for temporal and spatial aspects within an urban environment,and especially for an arid urban environment subject to seasonal short term high-intensity precipitation events.A focused diel sampling was conducted in the summer of 2021 to assess the temporal and spatial variability of PFAS in the Rio Grande near Albuquerque,New Mexico and showed an order of magnitude increase of PFAS as it flows through the Albuquerque urban *** samples were collected at two different locations on the Rio Grande in addition to wastewater treatment plant(WWTP)effluent that discharges directly to the Rio Grande between the sampling ***-term high-intensity precipitation events occurred during the study period and mobilized PFAS from urban *** organic matter composed of tryptophan-like organic substances and refined fuel and fuel byproducts,characteristic of an urban signature,were also related to the precipitation *** PFAS in discharge from the WWTP was consistent over a 24-h period with slight differences in some *** presence on the Rio Grande downstream of the WWTP was evidenced by a gadolinium anomaly as well as increases in several other trace elements,total dissolved nitrogen,and fluorescence indicators,in addition to *** varied depending on source contribution,where urban runoff was associated with PFOA,PFOS,and PFBA,whereas PFHxA and PFPeA were associated with wastewater *** addition,passive polar organic chemical integrative samplers(POCIS)using hydrophilic-lipid balance(HLB)sorption media were deployed for a month at two locations on the Rio Grande to assess longer term PFAS *** POCIS results show some compounds(PFPeA and PFHpA)were greater than the average concentration from discrete samples,whereas other compounds(PFHxA,PFOA,PFDA,and PFNA)were lower in the POCIS,and PFOS was very simila

Background:It has long been assumed that post-stroke intracranial pressure(ICP)elevation occurs because of large infarct and edema ***,we have repeatedly shown IcP elevation at 24 h post-stroke in the presence of little to no edema in *** processes are often conserved across species and types of ***,we aimed to determine if an ICP rise occurs at 24 h post-stroke in the presence of small infarct and edema volumes in ***:Mice were randomized by random number table to either photothrombotic stroke or sham surgery(n=15).Epidural ICP was recorded using a fiber optic catheter at 1 h post-stroke(baseline)and between 23 and ***:ICP was significantly higher at 24 h compared to baseline in stroke animals(n=6;10.71±6.45mmHg vs.3.74±2.20mmHg,respec-tively;p=0.03).ICP at 24 h was also significantly higher in stroke mice compared to sham(n=6;3.45±1.43mmHg;p=0.02).There was no change in ICP in sham mice(p=0.9).Edema volumes in stroke animals were small(0.04±0.04 mm3)and unlikely to have caused significant ICP ***:This study provides evidence of an edema-independent ICP elevation following small ischemic stroke in *** occurrence of this rise supports our findings in other species and suggests it is caused by a previously undescribed mechanism.

Growth and nutritional status are important issues in paediatric inflammatory bowel disease(IBD).While linear growth is easy to assess,nutritional status is more complicated,with reports often compromised by the use of simple measures,such as weight and the body mass index,to assess nutritional status rather than more appropriate and sophisticated techniques to measure body *** review is an update on what is currently known about nutritional status as determined by body composition in paediatric ***,this review will focus on the impact of biologics on growth in paediatric *** lean mass deficits have been reported in children with IBD compared with controls,and there is evidence these deficits persist over ***,data imply that gender differences exist in body composition,both at diagnosis and in response to *** respect to growth improvements following treatment with biologics,there are conflicting *** some studies report enhancement of growth,others do *** relationship between disease severity,impaired growth and the requirement for biologics needs to be considered when interpreting these ***,key features associated with improvements in growth appear to be successful clinical response to treatment,patients in early stages of puberty,and the presence of growth failure at the onset of treatment.

Nanoparticles for targeted cancer radiotherapy

Radiotherapy,where ionizing radiation is locally delivered either through an external beam or by surgically implanting radionuclide-based seeds in the tumor,is one of the gold standard treatments for *** to the non-selective nature of radiation,healthy tissue surrounding the cancerous region is usually affected by the ***,new strategies,including targeted alpha therapy,are being studied to improve the selectivity of the treatment and minimize side *** challenges,however,limit the current development of targeted radiotherapy,such as the functionalization of the therapeutic agent with targeting vectors and controlling the release of recoiling *** offer unique opportunities as drug delivery vehicles,since they are biocompatible,enhance the cellular uptake of drugs,and are easily functionalized with targeting *** this review,we examine how nanoparticles can be used for targeted radiotherapy,either as sensitizers of external beams or as delivery vehicles for therapeutic *** describe the clinical relevance of different types of nanoparticles,followed by an analysis of how these nanoconstructs can solve some of the main limitations of conventional ***,we critically discuss the current situation of nanoparticle-based radiotherapy in clinical settings and challenges that need to be overcome in the future for further development of the field.

Geographic variation in sexual selection and implications for speciation in the Barn Swallow

Barn Swallows(Hirundo rustica), a group of passerine birds comprised of six closely related subspecies, are well known throughout their nearly worldwide distribution, in part because of their close association with human settlements.A tractable species for both individual-based and population-level studies, Barn Swallows are a prominent model system in evolutionary, ecological, and behavioral research. Here we review work on sexual selection and population divergence in this species complex, focusing on comparative studies among populations and *** summarize variation in the targets of mate choice and in the information conveyed by sexually selected traits, and conclude that the benefits advertised by different traits may vary geographically. Finally, we consider the role of sexual selection as a driver of population divergence in this widespread and phenotypically variable species complex.

These organelles, which appear to vary between 200 nm–1 μm in diameter, originate from the maturation of endocytic vesicles and via the enrichment of newly synthesized lysosomal proteins in the trans-Golgi network.
