Ever since its earliest reports, portal venous gas (PVG) has been associated with numerous intraabdominal catastrophes and has served as an indication for urgent surgical exploration. It is traditionally regarded to be an ominous finding of impending death, with highest mortality reported in patients with underlying bowel ischemia. Today, computed tomography has demonstrated a wider range of clinical conditions associated with PVG, some of which are ‘benign’ and do not necessarily require surgery, unless when there are signs of intraabdominal catastrophe or systemic toxicity. One of these ‘benign’ conditions is Crohn’s disease. The present report describes a 19-year-old Chinese boy with Crohn’s pancolitis who presented with septic shock associated with PVG and portal vein thrombosis, and was successfully managed surgically. To our knowledge, this is the fi rst report of PVG and portal vein thrombosis associated with Crohn’s disease in a Chinese patient. In addition, we have also reviewed the reports of another 18 Crohn’s patients with PVG previously described in the English literature. Specifi c predisposing factors for PVG were identified in 8 patients, including barium enema, colonoscopy, blunt abdominal trauma, and enterovenous f istula. The patients who developed PVG following barium enema and blunt trauma were all asymptomatic and no specifi c treatment was necessary. Eleven patients (58%) who presented with signs of intraabdominal catastrophe or systemic toxicity required either immediate or eventual surgery. The overall mortality rate among the 19 patients was only 11%. The present literature review has shown that the f inding of PVG associated with Crohn’ s disease does not always mandate surgical intervention. It is the clinical features and the related complications that ultimately determine the treatment approaches. The overall outcome of PVG associated with Crohn’s disease has been favourable.

Surgical resection with a tumor-free margin is the onlycurative treatment for hilar cholangiocarcinoma (Klatskin tumor). However, over half of the patients present late with unresectable tumors. Radiotherapy using external beamirradiation or intraluminal brachytherapy (ILBT) has been used to treat unresectable hilar cholangiocarcinoma with satisfactory outcome. We reported a patient with unresectable hilar cholangiocarcinoma surviving more than 6 years after combined external beam irradiation and ILBT.

Lycium barbarum, a traditional Chinese anti-aging herb, has been shown to protect retinal ganglion cells （RGCs） in a rat chronic ocular hypertension （COH） model. Here, we investigated the expression of endothelin-1 （ET-1）, a strong vasoconstrictor, and its receptors, ETA and ETB, in the COH model and assessed the effects of Lycium barbarum on the ET-1 axis. Elevated intraocular pressure （IOP） was induced in the right eye of SD rats using argon laser photocoagulation. （1） The expression of ET-1, ETA and ETB in normal and COH retinas was studied. （2） Some COH rats were fed daily with Lycium barbarum Polysaccharides （LBP） using 1 mg/kg or phosphate-buffered saline （PBS） for 3 weeks （started 1 week before photocoagulation）. The effects of LBP on the expression of ET-1 and its receptors, ETA and ETB, in COH retina were evaluated. A semi-quantitative analysis of staining intensity was used to evaluate the expression levels of ET-1, ETA and ETB in retinal vasculature. We found that （1） Under COH condition, the immunoreactivity of ET-1 was increased in retina associated with an increase of ETB receptor immunoreactivity and a decrease of ETA receptor immunoreactivity. （2） After feeding COH rats with LBP, the expression of ET-1 was decreased with an increase of ETA expression and a decrease of ETB expression in the retina, especially in RGCs. （3） By comparing the staining intensity in the vasculature of COH retina in LBP-fed group with PBS-fed group, there was a decrease in the expression of ET-1 and ETA and an increase in ETB. In summary, ET-1 expression was up-regulated in the retina in COH model. LBP could decrease the expression of ET-1 and modulate the expression of its receptors, ETA and ETB, under the condition of COH. The neuroprotective effect of LBP on RGCs might be related to its ability to regulate the ET-1-mediated biological effects on RGCs and retinal vasculature.

Objective：This study was to investigate the variables in bone marrow harvesting procedure and individual donor factors which can potentially affect the yield of mesenchymal stromal cells（MSC）.Methods：We determined the yield of MSC from bone marrow under different clinical conditions by comparing the MSC colony numbers from：（1） donors of different ages;（2） healthy donors and patients with leukemia;（3） bone marrow aspirated at different time points during marrow harvesting;（4） bone marrow harvested by different ***：During the process of harvesting,the number of MSC significantly decreased with increase number of aspiration,from 675/ml at the initial decreased to 60/ml after 100 ml bone marrow aspirated,and 50/ml after 200 ml bone marrow *** number of MSC retrieved from leukemia patients（99/ml bone marrow） was significantly lower than that of healthy donors（708/ml bone marrow）.However,there was no significant difference in growth *** was no significant age-related difference of MSC yielded from donors 55 *** there was no significant difference in MSC number between the samples from single end-holed needle and those from multiple-side-hole ***：The optimal bone marrow samples for MSC collection should be obtained earlier in the process of harvesting *** marrow from donors 55 years was equally good as MSC *** autologous MSC from leukemia patients can be utilized for in-vitro MSC expansion.

＜正＞Objectives: Reactivation of varicella zoster virus （VZV）, clinically manifested as herpes zoster （HZ）, is an important complication after haematopoietic stem cell transplantation （HSCT）. At present, there is no effective prophylaxis for this condition. We have investigated the effect of vaccinating donors with a live-attenuated VZV vaccine before HSCT in terms of patients’ humoral and cellular immunity to VZV and the occurrence of HZ after HSCT.

Grasping Motion Control of Robotic Hands Using MIDS

A novel input and sensing system - the Micro Input Devices System (MIDS) is under development by merging MEMS sensors and existing wireless technologies. Previously,we have demonstrated MIDS to be applicable as a new form of humancomputer interface to potentially replace the mouse, keyboard, and light-pen. We have also shown that MIDS can be used as a generalized wireless distributed sensing system and successfully utilized it to measure the motion of the fingers of a robotic hand under manipulation. In this paper, we will present our recent results on using MIDS for grasping motion control of robotic hands. MIDS acts as a human-robot-interface that is used to detect and transduce an operator’s finger motion to control a five-finger robotic hand to perform grasping motion. Experimental results indicate that MIDS is capable of obtaining real-time acceleration signals and tilt angle data for the operator’s fingers. With the PD control algorithm, the robotic finger angles (measured by the joint angle sensors) tracked the angle data from MIDS, and hence, allowing the operator to control the robotic hand manipulator wirelessly using his own arm and finger motion.

目的探讨过氧化物酶体增殖物活化受体γ（PPARγ）的配体罗格列酮（RGZ）与全反式维甲酸（ATRA）对骨髓瘤细胞分化的影响及其可能机制。方法以RGZ和ATRA处理人骨髓瘤细胞U266和RPMI-8226，用^3H-掺入法检测细胞增殖变化；碘化丙啶（PI）染色后，以流式细胞仪检测细胞周期变化；瑞氏-姬母萨染色观察细胞形态学变化；流式细胞仪分析细胞表面CD49e的表达变化；Western blot方法检测p27^Kip1和p21^Waf1的表达变化。结果RGZ能显著抑制U266和RPMI-8226细胞的增殖，并且这种抑制作用呈剂量依赖性。经5μmol／LRGZ处理后，U266和RPMI-8226细胞G0／G1期细胞的比例分别为（45．2±6．7）％和（40．3±7．3）％，较对照组明显增加（均P〈0．05）；G2／M期和S期细胞的比例分别为（52．2±7．4）％和（57．4±9．5）％，较对照组明显减少（均P〈0．05）；CIM9e的表达率分别为（8．7±1．5）％和（7．9±1．6）％，较对照组明显上调（均P〈0．05）。以10μmol／LRGZ处理后，U266和RPMI-8226细胞的G0／G1期、G2／M期和S期细胞比例、CD49e的表达率均较5μmol／LRGZ组的变化更加显著。同时，经RGZ处理后，U266和RPMI-8226细胞的形态学变化也符合骨髓瘤细胞分化成熟的规律，p27^Kip1和p21^Waf1蛋白的表达量也较对照组明显增加。RGZ和ATRA联合对U266和RPMI-8226细胞影响较RGZ单独作用更加明显。结论RGZ可能通过上调p27^Kip1和p21^Waf1蛋白的表达，介导骨髓瘤细胞的周期阻滞，并诱导骨髓瘤细胞分化，抑制骨髓瘤细胞增殖。ATRA能增强RGZ对骨髓瘤细胞的上述作用，两者联合具有协同效应。
