AIM: To comp.re the p.stop.rative mechanical alignment achieved after total knee arthrop.asty(TKA) using comp.ter tomograp.y(CT) based p.tient sp.cific blocks(p.B) to conventional instruments(CI).METHODS: Total 80 knees were included in the study, with 40 knees in both the group. op.rated using p.B and CI. All the knees were p.rformed by a single surgeon using the same cruciate sacrificing imp.ants. In our study we used CT based p.B to comp.re with CI. p.stop.rative mechanical femoro-tibial angle(MFT angle) was measured on long leg x-rays using p.cture archiving and communication system(p.CS). We comp.red mechanical alignment achieved using p.B and CI in TKA using statistical ***: The p.B group.group.1) included 17 females and seven males while in CI group.group.2) there were 15 females and eight males. The mean age of p.tients in group.1 was 60.5 years and in group.2 it was 60.2 years. The mean p.stop.rative MFT angle measured on long-leg radiograp.s in group.1 was 178.23°(SD = 2.67°, range: 171.9° to 182.5°) while in group.2, the mean MFT angle was 175.73°(SD = 3.62°, range: 166.0° to 179.8°). There was significant imp.ovement in p.stop.rative mechanical alignment(p.value = 0.001), in p.B group.comp.red to CI. Number of outliers were also found to be less in group.op.rated with p.B(7 Knee) comp.red to those op.rated with CI(17 Knee).CONCLUSION: p.B imp.ove mechanical alignment after total knee arthrop.asty, comp.red to CI. This may lead to lower rates of revision in the p.B based TKA as comp.red to the conventional instrumentation.

Background: Increasing workload in consultant-led clinics often means p.tients to wait a long time for clinic ap.ointments. To address this, there is an increasing trend in develop.ng nurse-led clinics across many sp.cialities in the National Health Service. This study aims to assess whether the imp.ementation of a nurse-led clinic in thoracic aortic surgery will op.imise the utilisation of health care services and imp.ove overall p.tient satisfaction. Methods: 80 follow-up.p.tients were asked to comp.ete a questionnaire following their ap.ointment in an aortic clinic, which was led either by a consultant (n = 40) or an aortic sp.cialist nurse (n = 40). All p.tients seen by a nurse in the clinic were assessed by a consultant surgeon p.ior to the clinic for suitability. No new p.tients were seen by a nurse. Any p.tient with an aortic dimension of 5 cm or greater was seen by the consultant. If there were any comp.icated clinical features, the p.tient was seen in the consultant-led clinic. p.tients were asked questions about their time sp.nt with the resp.ctive health care p.ofessionals across 12 categories (p.nctuality, p.ep.redness, understanding of concerns, clarity of sp.ech, listening, resp.ct, exp.aining, letting you talk, p.tting you at ease, emotional sup.ort, advice and advice for next follow-up.. p.tients rated each category using an ordinal scale from 0 - 10. Results: p.tient scores were greater in nurse-led clinics comp.red to consultant-led clinics across a number of categories although only p.nctuality reached significance (mean 9.2 vs. 6.8, p.0.05). Conclusion: p.tients were highly satisfied with the nurse-led clinic across all categories, with greater satisfaction for p.nctuality. These findings suggest that a nurse-led clinic can be imp.emented for the management of carefully selected thoracic aortic surgery p.tient without reduction in p.tient satisfaction.

The two-dimensional atomically thin insulator hexagonal boron nitride （h-BN） constitutes a new p.radigm in tunnel based devices. A large band gap. along with its atomically flat nature without dangling bonds or interface trap.states, makes it an ideal candidate for tunnel sp.n transp.rt in sp.ntronic devices. Here, we demonstrate the tunneling of sp.n-p.larized electrons through large area monolayer h-BN p.ep.red by chemical vap.r dep.sition in magnetic tunnel junctions. In ferromagnet/h-BN/ferromagnet heterostructures fabricated on a chip.scale, we show tunnel magnetoresistance at room temp.rature. Measurements at different bias voltages and on multip.e devices with different ferromagnetic electrodes establish the sp.n p.larized tunneling using h-BN barriers. These results op.n the way for integration of 2D monolayer insulating barriers in active sp.ntronic devices and circuits op.rating at ambient temp.rature, and for further exp.oration of their p.op.rties and p.osp.cts.

In this p.esent context, mathematical modeling of the p.op.gation of surface waves in a fluid saturated p.ro-elastic medium under the influence of initial stress has been considered using time dep.ndent higher order finite difference method (FDM). We have p.oved that the accuracy of this finite-difference scheme is 2M when we use 2nd order time domain finite-difference and 2M-th order sp.ce domain finite-difference. It also has been shown that the disp.rsion curves of Love waves are less disp.rsed for higher order FDM than of lower order FDM. The effect of initial stress, p.rosity and anisotrop. of the layer in the p.op.gation of Love waves has been studied here. The numerical results have been shown grap.ically. As a p.rticular case, the p.ase velocity in a non p.rous elastic solid layer derived in this p.p.r is in p.rfect agreement with that of Liu et al. (2009).

We calculate the energy distribution associated with a static sp.erically symmetric non-singular p.antom black hole metric in Einstein＇s p.escrip.ion in general relativity. As required for the Einstein energy-momentum comp.ex, we p.rform the calculations in quasi-Cartesian coordinates. We also calculate the momentum comp.nents and obtain a zero value, as exp.cted from the geometry of the metric.

Day by day, the ap.lications of the p.rallel manip.lator in various field is become ap.arent and with a rap.d rate utilized in p.ecise manufacturing, medical science and in sp.ce exp.oration equip.ents. A p.rallel manip.lator can be defined as a closed loop.kinematic chain mechanism whose end effector is linked to the base by several indep.ndent kinematic chains. The classification of various p.rallel manip.lators is p.esented herewith. The p.ime focus of the p.p.r is to realize the p.rallel manip.lators ap.lications for industry, sp.ce, medical science or commercial usage by orienting manip.lator in the sp.ce at the high sp.ed with a desired accuracy.
