目的采用串联质谱标签(TMT)联合液相色谱串联质谱(LC-MS/MS)筛选免疫介导性脱髓鞘疾病诊断与鉴别诊断的潜在生物标志物。方法选择首都医科大学附属北京天坛医院2020年1月至2021年1月收治的20例脱髓鞘疾病患者(脱髓鞘组),包括10例吉兰-巴雷综合征(GBS)患者(GBS亚组)与10例多发性硬化(MS)患者(MS亚组)。以及10例神经系统非炎性疾病(NND)患者(NND组),通过TMT蛋白质组学技术筛选出脱髓鞘组与NND组、GBS亚组与MS亚组间具有表达差异的蛋白质(差异倍数>2或<0.5且差异有统计学意义),借助String数据库对组间差异蛋白质所参与的通路进行基因本体(GO)富集分析和京都基因和基因组百科全书(KEGG)富集分析。另选择同期就诊的80例脱髓鞘疾病患者(脱髓鞘病验证组)以及40名健康体检者(健康对照组)用于血脂一般指标的回顾性分析,脱髓鞘病验证组包括40例GBS患者(GBS验证组)及40例MS患者(MS验证组)。绘制受试者工作特征(ROC)曲线评价血脂一般指标对脱髓鞘病诊断及GBS与MS间鉴别诊断的价值。结果经TMT蛋白质组学技术检测出362种蛋白质,脱髓鞘组与NND组比较共有101个差异蛋白,GBS亚组与MS亚组比较共有45个差异蛋白。GO富集分析结果显示,脱髓鞘组与NND组相比,富集度前5条通路为巨噬细胞集落刺激因子及受体复合物、胆固醇输入负调控、极低密度脂蛋白颗粒清除负调控、含甘油三酯丰富的脂蛋白颗粒重构、胆固醇逆向转运。GBS亚组与MS组相比,富集度前5条通路为高密度脂蛋白颗粒受体结合、极低密度脂蛋白颗粒重塑负调控、胆固醇输入负调控、极低密度脂蛋白颗粒清除负调控、中等密度脂蛋白颗粒。KEGG富集分析显示,脱髓鞘组与NND组的差异蛋白富集到8条通路,为磷脂酰肌醇3激酶-蛋白激酶B信号通路、补体和凝固级联反应、细胞外基质及其受体交互、金黄色葡萄球菌感染、胆固醇代谢、Ras信号通路、吞噬体、丝裂原活化蛋白激酶信号通路。GBS亚组与MS亚组的差异蛋白富集到9条通路,为胆固醇代谢、补体和凝固级联反应、血小板激活、过氧化物酶体增殖物激活受体信号通路、维生素消化吸收、新型冠状病毒感染、脂肪消化吸收、轴突引导、中性粒细胞胞外陷阱形成通路。脱髓鞘病验证组与健康对照组比较,甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及载脂蛋白(apo)B水平较高(P均<0.01),高密度脂蛋白胆固醇(HDL-C)及apoA1水平较低(P均<0.01)。GBS验证组与MS验证组比较,LDL-C及apoB水平较高,HDL-C及apoA1水平较低(P均<0.05)。TG、TC、HDL-C、LDL-C、apoA1与apoB单独及联合检测对免疫介导性脱髓鞘病诊断的ROC曲线下面积分别为0.746、0.643、0.798、0.703、0.806、0.708和0.868。HDL-C、apoA1、LDL-C及apoB单独及联合检测对与GBS与MS鉴别诊断的ROC曲线下面积分别为0.692、0.653、0.632、0.695和0.718。结论免疫介导性脱髓鞘疾病患者与NND、GBS与MS患者脑脊液蛋白质组学存在差异,且差异蛋白主要存在于血脂代谢相关通路,血脂相关指标或可作为今后免疫介导性脱髓鞘疾病诊断及鉴别诊断的生物标志物。

Studies on Enterotoxins and Antimicrobial Resistance in Staphylococcus aureus Isolated from Various Sources

Staphylococcus aureus represents a public health challenge all over the world. Therefore, this study aims to analyze the prevalence of five genes (sea, seb, sec, see and seg) encoding the staphylococcal enterotoxins in S. aureus isolated from different sources and to evaluate the association of these toxins in comparison to susceptibility towards 12 antimicrobials;antimicrobial susceptibility was conducted by disc diffusion method. Detection of staphylococcal enterotoxins was performed by PCR and the ability to express these genes was assessed among isolates by RT-PCR. The most common enterotoxin gene was sea gene (66%), followed by seb, sec, see and seg (38%, 23%, 19% and 5%) respectively. Expression of sea, seb and seg genes was variable. However, sec and see genes were not expressed by any of the tested isolates. No statistically significant association exists between (seb, sec and see) and isolation sources, while the sea was significantly associated with clinical isolates. High significant correlation was found between elevated sea expression and multidrug-resistance. Our findings indicate that the pathogenic potential of S. aureus may be greater than previously thought. This emphasizes the utmost need to implement proactive measures and more emphasis will be placed on the application of hygiene practices in hospitals to control S. aureus infection and enterotoxins production.

Which Gastric Microbiota Associated with <i>H. pylori</i>in Dyspeptic Patients with Gastritis

Introduction: H. pylori is the comment infection worldwide. Its relation to dyspepsia has long been investigated. However, the association between H. pylori and other microbiota in the context of dyspepsia is less understood. The aim of this study was to determine different bacterial species isolated from the stomachs of patients with H. pylori infection and dyspepsia. Methods: A total of 81 patients were included and divided into: group I (N = 50) patients complaining of chronic dyspepsia and group II (N = 31) are patients with non-dyspeptic symptoms endoscoped for reasons other than dyspepsia. All patients were endoscoped and 4 gastric biopsies were obtained from each patient. All patients were examined initially by rapid urease test then histopathology to confirm H. pylori infection and determine the degree of gastric inflammation and finally tissue cultures for H. pylori and other bacterial species using matrix-assisted laser desorption ionization-time-of-flight mass spectroscopy (MALDI-TOF MS). Results: Regarding the demographics;patients of dyspepsia in group I were more consumers of spicy food and smoking than non-dyspeptic patients. Almost, all dyspeptic patients (98%) who had underlying gastric pathology with active gastritis and erosions were the most frequent reported pathological findings. Culture results showed significant association of Staphyloccus and Lactobacillus with dyspepsia while Streptoccous and Klebsilla were more frequent among non-dyspeptic patients. Conclusion: Dyspeptic patients in this study had different grades of gastric pathology and different species of microbiota were isolated, which seems to have concomitant interaction with H. pylori in pathogenicity of gastric mucosa and cause symptoms of chronic dyspepsia.

The starting 1-phenylbutane-1,3-dione (1) was used as key intermediate for the synthesis of several new thiophene and pyrazole derivatives. The newly synthesized compounds were evaluated for in vitro cytotoxicity against an Ehrlich ascites cells and in vivo cytotoxicity for compound 10d using EAC assay and 5-fluorouracil is used as reference drug. Compounds 7c, e and 10c, d showed significant activity in certain cancer cell and have been targeted for further studies, compound 10d is more effective and showed the highest activity. Structures of the newly prepared compounds were confirmed by both spectral, analytical data and molecular calculations.

Background: Negative and stigmatizing attitudes and behaviours towards people with mental health problems have the potential to lead to a lack of access to care, under-treatment, social marginalization, and can undermine the relationship between the patient and provider. The study aimed to measure attitude of healthcare professionals towards mental illness. Methods: Across sectional descriptive study was conducted over one month from April to May 2019 at Ras Al khaimah medical and health Science University and affiliated health care facilities. Mental Illness Clinicians’ Attitudes Scale version 4 Questionnaire was used to collect data from participants after obtaining their informed consent with assured confidentiality. Results: The study included 113 health care professionals. The mean score of attitude of health care professionals towards mental illness was 57.8 ± 10.8 out of 96. Negative attitudes were reported about the dangerousness, respect and suspicion over recovery of people with mental health problems, interactions with them in clinical practice, and fear of disclosure to colleagues or friends about mental health problems and confidence in capabilities of assessing mental health problems in primary care. The significant positive attitude was found among participants who were university faculty than those who worked in hospitals and Primary care centres (P = 0.04). Health care professionals who were university faculty and have reported more than 10 years of experience as health professionals reported significantly higher mean scores (P Conclusion: Educational programs might be useful to raise awareness towards the stigmatizing attitudes towards mental health and mental illnesses.

Phantom tumor of the lung is an infrequent presentation of volume overload in congestive heart failure. This finding is often mistaken for a lung mass that leads to extensive workup and unnecessary treatments. A 75-year-old male was evaluated for a right lower lobe rounded opacity. A transthoracic echocardiogram showed normal left ventricular function. Biopsy of the mass was unremarkable. Patient was managed with diuretics, and subsequent lung imaging revealed resolution of the opacity.

Role of Complement Regulatory Proteins (CD55, CD59, and CD35) on Red Blood Cells of <i>β</i>-Thalassaemia Patients

Background: β-thalassaemia (β-Thal) is an inherited chronic haemolytic anaemia resulting from absent or low level of synthesis of β-globin chains of haemoglobin A in erythropoietic cells. The complement system is an important part of innate immune response that may be implicated in red blood cell (RBC) lysis. Mammalian cells are provided with surface bound complement regulatory proteins (MCRPs) that regulate the activation of complement cascade, thus protecting them from uncontrolled complement-mediated lysis. Objective is to evaluate the role of complement regulatory proteins (CD55, CD59, and CD35) on red blood cells, and to explain the pathogenesis of anaemia in β-thalassemia major. Methods: This case-control study enrolled 74 β thalassemia major patients who were compared with 40 age and sex matched controls. We performed expression of CD55, CD59, and CD35 on RBCs using flow cytometry. Results: CD55 levels of β-thalassemia major patients (79.78% ± 18.54%) were significantly decreased compared to healthy controls (99.45% ± 0.59%)
