Relationship of Cognition, Depression and Anxiety to Glycemic Control in Older Adults with Diabetes

Objective: This study aimed to assess the relationship of cognition, depression and anxiety to glycemic control in elders with diabetes. DM is a chronic medical condition. Its control depends on adherence to medical therapy and making decisions related to lifestyle changes. This decision making capacity is affected by many factors including cognition and psychological status. Design: It was a case control study. Setting: It was done in Ain Shams University Hospital inpatients and DM outpatient clinic, Cairo, Egypt. Participants: Of the one hundred diabetic patients aged ≥ 60 years, 50 had Hemoglobin A1c (HbA1c) ≥ 7.5 (cases) and 50 had Hb A1c 7.5 (controls). Measurements: Cognition was assessed using minimental status examination (MMSE) test, Mattis Organic Mental Syndrome Screening Examination (MOMSSE) and Cambridge Cognitive Examination (CAMCOG) test. Geriatric depression scale-15 (GDS-15) was performed for depression assessment, while anxiety was assessed by DSM IV criteria. Laboratory investigations included: fasting blood sugar (FBS), post-prandial blood sugar (PPBS), glycated haemoglobin (Hb A1c), low density lipoprotein (LDL), high-density lipoprotein (HDL), total cholesterol, and triglycerides (TG). Results: Significant difference was found between the two groups regarding scores of cognitive tests: MMSE score (p = 0.004);below average (p = 0.02) and average scores (p = 0.05) of MOMSSE;CAMCOG score (p = 0.015);and CAMCOG divided items score including orientation (p = 0.003), comprehension (p = 0.005), expression (p = 0.020), attention (p = 0.002), and abstraction (p = 0.008) as well as depression screening scores (P = 0.002). Using Receiver Operating Characteristic, CAMCOG had better sensitivity and MOMSSE had better specificity. Conclusion: Cognitive impairment was associated with poor glycemic control, and impairment in attention and abstraction, related to executive function, functions were found to be associated with poor glycemic control. Anterior and posterior urethral valves: a rare association

Association of anterior and posterior urethral valve is a rare congenital anomaly causing lower urinary tract obstruction in children. This paper highlights our experience with a two-month-old boy with double urethral valves. Antenatal ultrasonography failed to detect any abnormality. At the age of 1 month, he presented with urinary dribbling, straining at micturition, and febrile urinary tract infection. Clinically, the urethra was dilated at micturition. Diagnosis was confirmed by retrograde urethrogram and urethroscopy. Endoscopic ablation of both valves was done by electrocautery hook.

A Secure Visual Cryptography Scheme Using Private Key with Invariant Share Sizes

Visual cryptography (VC) is one of the best techniques used to secure information. It uses the human vision to decrypt the encrypted images without any cryptographic computations. The basic concept of visual cryptography is splitting the secret image into shares such that when the shares are stacked, the secret image is revealed. In this paper we proposed a method that is based on the concept of visual cryptography for color images and without any pixel expansion which requires less space. The proposed method is used to encrypt halftone color images by generating two shares, random and key shares which are the same size as the secret color image. The two shares are generated based on a private key. At the receiving side, the secret color image is revealed by stacking the two shares and exploiting the human vision system. In this paper, we produce an enhanced form of the proposed method by modifying the encryption technique used to generate the random and the key shares. Experimental results have shown that the proposed and the enhanced methods suggest an efficient way to encrypt a secret color image with better level of security, less storage space, less time of computation and with a better value of PSNR.

The Golden Jackal (Canis aureus Linnaeus, 1758), which belongs to the Canidae family, is an opportunist carnivore in the Gaza Strip (365 square kilometers). The current study aims at giving notes on the occurrence and some ecological aspects of the species in the Gaza Strip, Palestine. The study, which lasted 14 years (2007-2020), is descriptive and cumulative in its style. It was based on frequent field visits, direct observations and meetings and discussions with wildlife hunters, farmers and other stakeholders. The findings of the study show that Gazans are familiar with the Golden Jackal to the extent that a Gazan family holds the Arabic name of the animal, which is “Wawi”. The Golden Jackal was sometimes encountered and hunted in the eastern parts of the Gaza Strip, which are characterized by the presence of wilderness areas, intensive agriculture, poultry pens and solid waste landfills. Like other a few mammalian faunas, the adult Golden Jackals enter the Gaza Strip through gaps in or burrows beneath the metal borders separating the Gaza Strip from the rest of the Palestinian Territories and Egypt. Gaza zoos were found to harbor tens of Golden Jackals trapped or hunted by clever wildlife hunters using different means such as wire cages known locally as “maltash” and foothold traps with metal jaws that may cause lesions to the trapped animals. Poisoning and shooting were also common methods used to control the jackals and other carnivores causing harm to agriculture and livestock. The animal was known among the Gazans as an omnivore, feeding on wild and domestic animals in addition to plant materials, garbage and carrions. In conclusion, the study recommends the need to raise ecological awareness to preserve the Golden jackal and to adopt safe control measures for jackals and other carnivores, including the construction of protective fences for agricultural fields and animal pens.

Ability of Comprehensive Geriatric Assessment to Detect Frailty

Background and Aim of the Work: Frailty is a state of reduced physiological reserve associated with increased susceptibility to disability. It is associated with a high morbidity and mortality. This work assessed the ability of comprehensive geriatric assessment to detect frailty in elderly patients. Subjects and Methods: A total number of 104 elderly patients (above 60 years old) were included in this study and they were subjected to a comprehensive geriatric assessment (CGA) including: (history and full clinical examination, Mini mental status examination (MMSE), Geriatric depression scale (GDS), Activities of daily living (ADL), Instrumental activities of daily living (IADL). The patients were divided into frail and non-frail groups using Fried’s criteria as applied by Avila-Funes et al., 2008, each group included 52 patients. Results: There were no significant differences between the two groups regarding age gender or smoking habits. But there was higher percentage of ADL, IADL dependence in addition to higher incidence of depression and cognitive impairment among frail cases. Conclusion: Comprehensive geriatric assessment can be used to detect frailty and its associated impairment and comorbidities.

Homocystiene and C-Reactive Protein in Detection of Frailty

Background and Aim of the Work: Frailty is a state of reduced physiological reserve and is associated with increased susceptibility to disability. It is associated with a high morbidity and mortality. The aim of this work is to assess the association between the levels of homocystiene and C-reactive protein and frailty and to examine the ability of homocystiene as a new marker to detect frailty. Subjects and Methods: A total number of 104 elderly subjects (above 60 years old) were included in this study and they were subjected to history, examination, comprehensive geriatric assessment, and laboratory investigations including: C-reactive protein (CRP) and Homocystiene (Hcy). The patients were divided into frail and non-frail groups using Fried’s criteria as applied by Avila-Funes et al., 2008 and each group included 52 patients. Results: There was no significant difference between the two groups as regards age gender or smoking habits. But there were higher levels of CRP (>10 mg/dl) and homocystiene (>12 mmol/l) among frail cases and by comparing both homocystiene was more sensitive in detection of frailty. Conclusion: Frailty was associated with eleveted homocystiene and CRP and homocystiene is more specific than CRP in detection of frailty.

Geochemical Nature of the Surface Water of the Area of Gafsa

Water presents an essential source of any durable development. The area of Gafsa which is geographically located in the zone of transition between central Tunisia and the Saharan platform knew these last years a dryness which threatening the reserves of water under land. Opposite this problem and because of the high cost of the subsoil waters exploitation, the recourse to surface water becomes an inevitable solution. The surface water of the area of Gafsa is of two types: spring waters and water of tap coming from the ground water. The good exploitation of this water passes first of all by a very good geochemical knowledge of their nature. The objective of this study is to seek through some diagrams the geochemical classification of this water by various tests at end to choose the most suitable processes to treat them. The analysis of these diagrams showed interressants results preparing a whole installation study of a water treatments station in the area of Gafsa.

Context: The prognostic significance of expression of V-Crk avian sarcoma virus CT10 oncogene homolog-like (CRKL) and Protein kinase CK2 β in endometroid carcinoma is not fully clarified. Aim: To assess the prognostic values and clinicopathological roles of CRKL and CK2 β expression in endometroid carcinoma by correlation their expression levels with clinicopathological parameters, response to therapy, and recurrence of the tumor and patients survival. Settings: Faculty of Medicine, Zagazig University, and Mansoura University. Methods: CRKL and CK2 β expressions were evaluated in 50 paraffin blocks EC patients that were followed up for 5 years. The relationship between their level of expressions, clinicopathological criteria and prognosis of patients was analyzed. Statistical analysis used: SPSS 22.0 for windows (SPSS Inc., Chicago, IL, USA) and MedCalc windows (Software bvba 13, Ostend, Belgium). Results: High expression of CRKL and CK was positively correlated with higher grade of the tumor (p = 0.004 and 0.006 respectively), presence of L.N metastases (p = 0.009 and 0.003 respectively), presence of distant metastases (p = 0.029 and 0.003 respectively) and advanced FIGO stage (p 0.001), poor response to therapy (p = 0.046 and 0.005 respectively), higher incidence of recurrence of the tumor after therapy (p = 0.004 and 0.048 respectively), higher incidence of cancer progression p = 0.018 and 0.044 respectively), poor PFS (p = 0.008 and 0.013 respectively), and OS rates

Ornaments as Means of Efficient Reinforcement in Heritage Buildings

Heritage consists of those items people have inherited and want to keep, as they give them the sense of the past of their cultural identity, but these items, especially the buildings, could not face deteriorations during their period of life, such as the change of loads, the act of the environmental conditions, the effect of the duration time of the materials and the misuse of the occupants. They need special treatments and reinforcements to keep the structural state or to gain extra forces to receive the new loads. But the traditional ways of reinforcement do not prove endless solutions as the most means of the deteriorations from the nature. So, the liability issue is how to find the least reinforcement within the means of technology with the least deterioration and in a less time or by other ways efficiently. The hypothesis considers that the ornaments in some heritage buildings are found in critical levels of the construction which can share in an efficient way in the reinforcement processes of such buildings.
