Spectrum of cardiac manifestations and its relationship to outcomes in patients admitted with scrub typhus infection

AIM To study the spectrum of cardiac manifestations in scrub typhus infection and assess its relationship to *** Demographic data, electrocardiographic(ECG) changes, left ventricular (LV) systolic and diastolic function, myocardial injury (defined as troponin T > 14 pg/mL), and pericardial effusion were documented. Myocarditis was diagnosed when myocardial injury was associated with global LV systolic dysfunction. The relationship between myocarditis and outcomes was assessed using logistic regression analysis and expressed as odds ratio(OR) with 95%*** The cohort(n = 81; 35 males) aged 49.4 ± 16.1 years(mean, SD) presented 8.1 ± 3.1 d after symptom onset. The APACHE-Ⅱ score was 15.7 ± 7.0. Fortyeight (59%) patients were ventilated, and 46 (56%) required vasoactive agents. Mortality was 9.9%. ECG changes were non-specific; sinus tachycardia was the most common finding. Myocardial injury was evident in 61.7% of patients and LV systolic dysfunction in 30.9%. A diagnosis of myocarditis was made in 12.3%. In addition, seven patients with regional wall motion abnormalities had LV systolic dysfunction and elevated cardiac enzymes. Mild diastolic dysfunction was observed in 18 (22%) patients. Mild to moderate pericardial effusion was seen in 51%. On multivariate logistic regression analysis, patients with myocarditis tended to be older (OR = 1.04, 95%CI: 0.99-1.09), had shorter symptom duration (OR = 0.69, 95%CI: 0.49-0.98), and tended to stay longer in hospital(OR = 1.17, 95%CI: 0.98-1.40). Myocarditis was not associated with increased *** In scrub typhus infection, cardiac manifestations are frequent and associated with increased morbidity but not mortality.

Valorization of agro-industrial fruit peel waste to fluorescent nanocarbon sensor:Ultrasensitive detection of potentially hazardous tropane alkaloid

Millions of tonnes of agro-industrial waste are generated each year globally,with the vast majority of it going untreated,underutilized,and disposed of by burning or landfilling,causing severe environmental distress and economic downturn.A practical solution to this global issue is to use green chemistry to convert this waste into value-added ***,in the present study,agro-industrial orange peel waste was valorized into fluorescent nanodiamond-like carbon sensor via a green route involving hydrothermal treatment of microwave carbonized orange peel *** developed sensor,used for the fluorescence detection of potentially hazardous drug atropine sulfate,exhibits unique dual linearity over concentration ranges of 300 nM to 1 M and from 1 M to 10 M,as well as ultra-low sensitivity of 34.42 nM and 356.46 nM,***,the sensor demonstrates excellent reproducibility,high stability,and satisfactory recovery when used to identify and quantify atropine sulfate in biological samples and commercially available pharmaceuticals,indicating promising multidisciplinary applications.

Hybrid Prairie Dog and Beluga Whale Optimization Algorithm for Multi-Objective Load Balanced-Task Scheduling in Cloud Computing Environments

The cloud computing technology is utilized for achieving resource utilization of remotebased virtual computer to facilitate the consumers with rapid and accurate massive data *** utilizes on-demand resource provisioning,but the necessitated constraints of rapid turnaround time,minimal execution cost,high rate of resource utilization and limited makespan transforms the Load Balancing(LB)process-based Task Scheduling(TS)problem into an NP-hard optimization *** this paper,Hybrid Prairie Dog and Beluga Whale Optimization Algorithm(HPDBWOA)is propounded for precise mapping of tasks to virtual machines with the due objective of addressing the dynamic nature of cloud *** capability of HPDBWOA helps in decreasing the SLA violations and Makespan with optimal resource *** is modelled as a scheduling strategy which utilizes the merits of PDOA and BWOA for attaining reactive decisions making with respect to the process of assigning the tasks to virtual resources by considering their priorities into *** addresses the problem of pre-convergence with wellbalanced exploration and exploitation to attain necessitated Quality of Service(QoS)for minimizing the waiting time incurred during TS *** further balanced exploration and exploitation rates for reducing the makespan during the task allocation with complete awareness of VM *** results of the proposed HPDBWOA confirmed minimized energy utilization of 32.18% and reduced cost of 28.94% better than approaches used for *** statistical investigation of the proposed HPDBWOA conducted using ANOVA confirmed its efficacy over the benchmarked systems in terms of throughput,system,and response time.

In this paper,the investigation of a novel compact 2×2,2×1,and 1×1 Ultra-Wide Band(UWB)based Multiple-Input Multiple-Output(MIMO)antenna with Defected Ground Structure(DGS)is *** proposed Electromagnetic Radiation Structures(ERS)is composed of multiple radiating *** MIMO antennas are designed and analyzed with and without *** feeding is introduced by a microstrip-fed line to significantly moderate the radiating structure’s overall size,which is 60×40×1 *** high directivity and divergence characteristics are attained by introducing the microstripfed lines perpendicular to each *** the projected MIMO antenna structures are compared with others by using parameters like Return Loss(RL),Voltage Standing Wave Ratio(VSWR),Radiation Pattern(RP),radiation efficiency,and *** same MIMO set-up is redesigned with DGS,and the resultant parameters are ***,the Multiple Input and Multiple Output Radiating Structures with and without DGS are compared for result considerations like RL,VSWR,RP,radiation efficiency,and *** projected antenna displays an omnidirectional RP with moderate gain,which is highly recommended for human healthcare *** introducing the defected ground structure in bottom layer the lower cut-off frequencies of 2.3,4.5 and 6.0 GHz are achieved with few biological effects on radio propagation in human body *** proposed design covers numerous well-known wireless standards,along with dual-function DGS slots,and it can be easily integrated into Wireless Body Area Networks(WBAN)in medical *** WBAN links the autonomous nodes that may be situated either in the clothes,on-body or beneath the skin of a *** system typically advances the complete human body and the inter-connected nodes through a wireless communication channel.

Self-assembled multifunctional nanotheranostics against circulating tumor clusters in metastatic breast cancer

Circulating tumor clusters(CTC)disseminating from the primary tumor are responsible for secondary tumor formation where the conventional treatments such as chemotherapy and radiotherapy does not prevent the metastasis at locally advanced stage of breast *** this study,a smart nanotheranostic system has been developed to track and eliminate the CTCs before it can colonize at a new site,which would reduce metastatic progression and increase the five-year survival rate of the breast cancer *** multiresponsive(magnetic hyperthermia and pH)nanomicelles incorporated with NIR fluorescent superparamagnetic iron oxide nanoparticles were developed based on self-assembly for dual modal imaging and dual toxicity for spontaneous killing of CTCs in blood stream.A heterogenous tumor clusters model was developed to mimic the CTCs isolated from breast cancer *** nanotheranostic system was further evaluated for the targeting property,drug release kinetics,hyperthermia and cytotoxicity against developed CTC model in *** vivo model in BALB/c mice equivalent to stageⅢandⅣhuman metastatic breast cancer was developed to evaluate the biodistribution and therapeutic efficacy of micellar nanotheranostic *** CTCs in blood stream and low distant organ metastasis after treatment with the nanotheranostic system demonstrates its potential to capture and kill the CTCs that minimize the secondary tumor formation at distant sites.

In Underwater Acoustic Sensor Network(UASN),routing and propagation delay is affected in each node by various water column environmental factors such as temperature,salinity,depth,gases,divergent and rotational *** sound velocity increases the transmission rate of the packets and the high dissolved gases in the water increases the sound *** dissolved gases and sound velocity environment in the water column provides high transmission rates among UASN *** this paper,the Modified Mackenzie Sound equation calculates the sound velocity in each node for energy-efficient *** Ratio Optimization Method(GROM)and Gaussian Process Regression(GPR)predicts propagation delay of each node in UASN using temperature,salinity,depth,dissolved gases *** gases,rotational and divergent winds,and stress plays a major problem in UASN,which increases propagation delay and energy *** values from GPR and GROM leads to node selection and Corona Virus Optimization Algorithm(CVOA)routing is performed on the selected *** proposed GPR-CVOA and GROM-CVOA algorithm solves the problem of propagation delay and consumes less energy in nodes,based on appropriate tolerant delays in transmitting packets among nodes during high rotational and divergent *** simulation results,CVOA Algorithm performs better than traditional DF and LION algorithms.
