Brand versus Generic Rosuvastatin in Egyptian Patients with Hyperlipidemia;Cost-Minimization Analysis

Background: Serum level of cholesterol is one of the most vital risk factors for cardiovascular diseases (CVD). Statins are highly effective drugs for reducing serum cholesterol;hence, preventing coronary heart disease (CHD). Rosuvastatin (Crestor) is one of the most potent and widely prescribed statins. Even though generic statins have been approved based on their bioequivalence with brand-name drugs, there remains considerable concern as regards their effectiveness and safety. Most clinicians and patients welcome the generic drug decreased costs;however, it is indispensable for them that effectiveness and safety are not compromised. Thus, the rationale intended for this study is to compare brand rosuvastatin and generic rosuvastatin as regard their economic impact using a cost-minimization analysis. Methods: This cost-minimization model estimates potential impact of rosuvastatin brand versus generic on the healthcare resource utilization for one-year frame from the payer perspective. The model conforms to real practice of management of hyperlipidemia in Egypt and was validated by experts. Results: The drug costs in the rosuvastatin brand group were 3,155,250 EGP while in the generic group were 2,299,030 EGP. The costs of CVD events in the rosuvastatin brand group were 5,863,558 EGP, while in the generic group were 6,810,180 EGP. The total costs in the rosuvastatin brand group were 9,018,808 EGP, while in the generic group were 9,109,210 EGP with a difference of −100,047 EGP. Conclusions: In conclusion, the real cost of generic treatment is more than that of the brand statin when taking into consideration the cardiovascular events.

Tako-tsubo cardiomyopathy in a male patient after administration of isoproterenol and implantation of a pacemaker

Background: Tako-tsubo cardiomyopathy occurs typicallly after an intrinsic adrenergic hyperstimulation triggered by a psychological or physically stressful event. Exceptionally, it may be caused by an exogenous hyperadrenergism. We report the case of an 85 year old man, hypertensive, with a history of ischemic stroke. He consulted for signs of heart failure with recurrent dizziness two weeks which was explained by an atrioventricular block. Initial echocardiography showed left ventricular ejection fraction to 60% with no wall motion abnormalities. The patient received iso-prenaline (0.02 mcg/kg/mn) for 20 hours before the implantation of a single chamber pacemaker. At 24 hours of admission an acute pulmonary edema occurred. The control echocardiography showed impaired left ventricular ejection fraction of 25% with apical ballooning and akinesis was also found on ventricu-lography. Coronary angiography showed no significant coronary lesions. Troponin level was elevated to 2 ng/ml. The pulmonary edema was then controlled. Subsequent clinical and echocardiographic were favo- rable after two weeks which was consistent with the diagnosis of tako-tsubo cardiomyopathy. Conclusion: This case illustrates an example of tako-tsubo syndrome induced by an exogenous catecholergic stimulation (isoprenaline) combined with an endogenous release after a pacemaker implantation which confirmed the physiopathological hypothesis of a catecholamine-mediated stunning in tako-tsubo cardiomyopathy.

Gemcitabine combined with cisplatin in the treatment of advanced malignant pleural mesothelioma (MPM): Institutional experience

Objective： The aim of our study was to evaluate the efficacy and safety of gemcitabine and cisplatin in patients with previously untreated advanced malignant pleural mesothelioma （MPM）. Methods： Thirty-three eligible patients with histologically proven advanced MPM with ECOG PS of 〈 2 and had adequate liver and kidney functions received gemcitabine （1000 mg/m2） on days 1 and 8 combined with cisplatin （80 mg/m^2） on day 1. Results： The majority of the study group were males and constituted 87.9% （n = 29）, while females constituted 12.1% （n = 4）. The median age was 52 years and ranged from 37 to 69 years. Regarding SWOG performance status, 5 patients were PS 0, 21 patients were PS 1 and 7 patients were PS 2. Regarding pathological subtypes, epithelial histology represented 84.8% of the study （n = 28） while sarcomatoid type represented 6% （n = 2） and biphasic type accounted for 9% （n = 3）. Regarding staging according to IMIG staging system, 12.1% of patients were stage Ⅱ, 54.5% were stage Ⅲand 33.3% were stage Ⅳ. Only one patient （3%） had attained a complete response, 18 patients （54.5%） showed partial response, 12 patients （36.4%） showed stationary disease, while 2 patients （6.1%） showed progressive disease. The correlation between response rate obtained and PS was statistically significant （P = 0.029）. After a median follow-up of 14.4 months, the median survival time was 20.3 months （95% Cl： 14.58-26.01 months） and the progression free survival time was 11.8 months （95% CI： 10.76-12.83 months） Conclusion： The combination of gemcitabine and cisplatin is an active and safe treatment in patients with MPM. Further larger comparative studies are warranted to document this finding.

目的 k最近邻节点算法(k-nearest neighbor algorithm,kNN)可以将复杂的医疗信息集合成临床诊断信息(比如确定良性或恶性病变)。该研究旨在分析kNN算法应用于大量临床数据集时的AUC(ROC曲线下面积)。材料与方法该研究经IRB批准,且实验选取了543例经病理证实为乳腺病灶的MR图像进行分析,所有的病灶由两名经验丰富的放射科医师用现有的描述方法进行前瞻性评估。kNN算法应用于诊断恶性与良性病变的步骤如下:首先,用递归特征消除来确定单个特征描述的重要性,将其按照重要性排列。然后,采取多类别描述方法的策略,将对照组分为4组:top-3、top-7、top-12和top-18组,相应的特征描述作为kNN算法的输入向量。最后,用kNN算法对四组数据处理,对结果进行量化,比较各组数据的AUC(为了尽量消除数据模型和测试数据的偏差,运用了4倍交叉验证)。病理组织学显示,实验数据组共有196个良性病变和347个恶性病变。结果测得最高的AUC为0.940(用top-18描述)。如果用top-12来描述,AUC降为0.928(P=0.23)。减少特征描述输入向量的维数会显著降低(P<0.05)kNN算法的AUC("top-7":AUC=0.895;"top-3":AUC=0.816)。结论 kNN对预测恶性肿瘤的精确度较高(AUC为0.940),由于这种描述方法对n≥12是有效的,说明kNN算法对多维数据的评估更加有效。

Recent biological trends in management of fracture non-union

Bone regeneration is a complex, well-orchestrated physiological process of bone formation, which can be seen during normal fracture healing, and is involved in continuous remodelling throughout adult life. Currently,there is a plethora of different strategies to augment the impaired or "insufficient" bone-regeneration process, including the "gold standard" autologous bone graft, free fibula vascularised graft, allograft implantation, and use of growth factors, osteoconductive scaffolds, osteoprogenitor cells and distraction osteogenesis. Improved "local" strategies in terms of tissue engineering and gene therapy, or even "systemic" enhancement of bone repair, are under intense investigation, in an effort to overcome the limitations of the current methods, to produce bone-graft substitutes with biomechanical properties that are as identical to normal bone as possible, to accelerate the overall regeneration process, or even to address systemic conditions, such as skeletal disorders and osteoporosis. An improved understanding of the molecular and cellular events that occur during bone repair and remodeling has led to the development of biologic agents that can augment the biological microenvironment and enhance bone repair. Orthobiologics, including stem cells, osteoinductive growth factors, osteoconductive matrices, and anabolic agents, are available clinically for accelerating fracture repair and treatment of compromised bone repair situations like delayed unions and nonunions. A lack of standardized outcome measures for comparison of biologic agents in clinical fracture repair trials, frequent off-label use, and a limited understanding of the biological activity of these agents at the bone repair site have limited their efficacy in clinical applications.

The Gaza Strip(365 km^(2))of Palestine(27,000 km^(2))is home to a wealth of terrestrial vertebrate *** of these faunistic species find their ways to preservation at the local ***,the current study comes to document the Palestinian terrestrial vertebrate fauna acquired by the biology exhibitions(BEs)of Al-Azhar University,Islamic University of Gaza and Al-Aqsa University that are located at the Gaza City of the Gaza *** amphibians,reptiles,birds and mammals preserved at BEs of the universities in question were surveyed and scientifically classified during a three-month period extending from January to March,*** study showed that all BEs of local universities are underdeveloped,lacking attention and suffer from specimen scarcity and good *** BE at Al-Azhar University is the best in the arrangement and preservation of bird specimens.A total number of 200 specimens belonging to 54 terrestrial vertebrate fauna species,39 families and 17 orders was recorded at *** constituted 40.7%of the total species recorded,followed by birds(38.9%),mammals(14.8%)and amphibians(5.6%).The Islamic University of Gaza was considered the best in terms of the number of preserved species(39.8%),followed by Al-Azhar University(36.3%)and Al-Aqsa University(23.9%).The Common Toad(Bufo viridis)was the most preserved among the amphibian species *** was the biggest reptilian order,comprising 20 species(8 lizards and 12 snakes),with the Syrian Black Snake(Coluber jugularis asianus)was the *** Palestine Viper(Vipera palaestinae)is endemic to Palestine and most venomous and dangerous to human *** Great White Pelican(Pelecanus onocrotalus)was the largest Palestinian bird preserved at BE of Al-Azhar *** Egyptian Mongoose(Herpestes ichneumon)and the Common Badger(Meles meles)were the biggest mammalian specimens preserved,while the Palestine Mole-rat(Spalax leucodon ehrenbergi)was the only Palestine endemic spe
