Transcatheter aortic valve implantation in very elderly patients： immediate results and medium term follow-up

Objective To evaluate immediate transcatheter aortic valve implantation （TAVI） results and medium-term follow-up in very elderly pa- tients with severe and symptomatic aortic stenosis （AS）. Methods This multicenter, observational and prospective study was carried out in three hospitals. We included consecutive very elderly （〉 85 years） patients with severe AS treated by TAVI. The primary endpoint was to evaluate death rotes from any cause at two years. Results The study included 160 consecutive patients with a mean age of 87 ± 2.1 years （range from 85 to 94 years） and a mean logistic EuroSCORE of 18.8% ± 11.2% with 57 （35.6%） patients scoring 〉 20%. Procedural success rate was 97.5%, with 25 （15.6%） patients experiencing acute complications with major bleeding （the most frequent）. Global mortality rate during hospitalization was 8.8% （n = 14） and 30-day mortality rate was 10% （n = 16）. Median follow up period was 252.24± 232.17 days. During the follow-up period, 28 （17.5%） patients died （17 of them due to cardiac causes）. The estimated two year overall and cardiac survival rates using the Kaplan-Meier method were 71% and 86.4%, respectively. Cox proportional hazard regression showed that the variable EuroSCORE ≥ 20 was the unique vari- able associated with overall mortality. Conclusions TAVI is safe and effective in a selected population of very elderly patients. Our findings support the adoption of this new procedure in this complex group of patients.

Functional recovery from central nervous system(CNS)trauma depends not only on axon regeneration or compensatory sprouting of uninjured fibers but also on the ability of newly grown axons to establish functional synapses with appropriate *** several studies have successfully promoted long-distance axonal regeneration in distinct CNS injury models,none of them have resulted in a viable therapeutic approach for patient recovery.A possible reason may be the lack of new synaptogenesis for reestablishing the circuitry lost after ***,we discuss how our understanding of the mechanisms that instruct synapse formation in the injured nervous system may contribute to the design of new strategies to promote functional restoration in traumatic CNS disorders.

Traumatic spinal cord injuries(SCI) are characterized by damage in the integrity of the spinal cord,which results in either temporary or permanent alterations in the locomotor,sensory and/or autonomic functions(Yezierski,2009).The traumatic event leads to impairments in voluntary control of movement below the injury by affecting the connection between the brain and the neurons localized in the spinal cord.

Effect of losing a fight on later agonistic behavior toward unfamiliar conspecifics in male Syrian hamsters

In many species, agonistic interactions result in social relationships that are stable over time. In Syrian hamsters, two unfamiliar males that are placed together will fight vigorously and a clear winner/loser relationship is usually established. In subsequent interactions, the loser will flee soon after detecting the familiar winner. Here we tested the hypothesis that losing a fight with a conspecific will affect future agonistic interactions not only toward that individual （i.e., the familiar winner） but also toward unfamiliar conspecifics. To test this hypothesis we paired two Syrian hamster males in three trials on one day in which the loser had tile opportunity to escape the winner. The next day the loser was paired with an unfarniliar male, also for three trials. If he lost again, he was tested on a third day with a third unfamiliar male. Subjects were those males that were losers on all three days. The latency to escape on the first trial on Days 2 and 3 was significantly shorter than on the first trial on Day l, indicating that losing against the first male affected the response toward unfamiliar males. However, the latency to escape on the first trial on Days 2 and 3 was significantly longer than that on the third trial on the preceding day, indicating that a loser treats unfamiliar males differently than a familiar winner. These results suggest that a defeat during an interaction with one male affects later agonistic behavior towards other, unfamiliar males [Current Zoology 57 （4）： 449-452, 2011].

Applying design science approach to architectural design development

Unlike research methods for social and positivist sciences,those for architectural design lack a discipline-specific conceptual *** science aims at producing outcomes for future use and therefore needs a robust methodological approach that encompasses different techniques and methods supporting an evidence-based architectural design *** study suggests that design science can be successfully applied to architectural design development and provides architects and designers with a powerful tool bridging the gap between research and *** so doing,this study explores the application of design science to implement a user-centered design approach.A design challenge is reframed within the robust framework of design science by referring to a case study on refugee *** traditional method by which shelter optimization is pursued by designers is questioned by involving the final users through an ethnographic approach within the framework of design *** design outcome produced through this process is a list of specifications allowing designers to create different architectural solutions and matching the requirements expressed by future *** authors argue that a user-centered design outcome can be achieved and validated through design science.

Mitochondrial dysfunction as a target in spinal cord injury:intimate correlation between pathological processes and therapeutic approaches

Traumatic spinal cord injuries interrupt the connection of all axonal projections with their neuronal targets below and above the lesion site. This interruption results in either temporary or permanent alterations in the locomotor, sensory, and autonomic functions. Damage in the spinal tissue prevents the re-growth of severed axons across the lesion and their reconnection with neuronal targets. Therefore, the absence of spontaneous repair leads to sustained impairment in voluntary control of movement below the injury. For decades, axonal regeneration and reconnection have been considered the opitome of spinal cord injury repair with the goal being the repair of the damaged long motor and sensory tracts in a complex process that involves:(1) resealing injured axons;(2) reconstructing the cytoskeletal structure inside axons;(3) re-establishing healthy growth cones;and(4) assembling axonal cargos. These biological processes require an efficient production of adenosine triphosphate, which is affected by mitochondrial dysfunction after spinal cord injury. From a pathological standpoint, during the secondary stage of spinal cord injury, mitochondrial homeostasis is disrupted, mainly in the distal segments of severed axons. This result in a reduction of adenosine triphosphate levels and subsequent inactivation of adenosine triphosphate-dependent ion pumps required for the regulation of ion concentrations and reuptake of neurotransmitters, such as glutamate. The consequences are calcium overload, reactive oxygen species formation, and excitotoxicity. These events are intimately related to the activation of necrotic and apoptotic cell death programs, and further exacerbate the secondary stage of the injury, being a hallmark of spinal cord injury. This is why restoring mitochondrial function during the early stage of secondary injury could represent a potentially effective therapeutic intervention to overcome the motor and sensory failure produced by spinal cord injury. The duration of sociosexual behaviors in male meadow voles Microtus pennsylvanicus varies before, during, and after copulation

The behaviors that surround copulation are characterized as sociosexual behaviors. These behaviors displayed by males that are directed at females may include allogrooming, wrestling, chasing, approach, and time spent together. The data supported the hypothesis that the duration of sociosexual behaviors differs during the pre-copulatory, peri-copulatory, and post-copulatory phases of the mating bout in meadow voles. Voles spent more time approaching conspecifics during the pre- and peri-copulatory phases than during the post-copulatory phase. Voles spent more time allogrooming, wrestling, and chasing during the pre-copulatory phase than during the peri- and post-copulatory phases. Voles spent similar amounts of time together during the pre-, peri-, and post-copulatory phases. The data suggest that sociosexual behaviors displayed by males may be involved in set- ring the pace and temporal components of the mating bout. During the pre-copulatory phase particular behaviors by male voles may attract females, during the peri-copulatory phase some of these behaviors may stimulate or motivate the female to mate, and during the post-copulatory phase certain behaviors may prepare the male to mate again
