Objective.:to retrospectively evaluate the feasibility and morbidity of total laparoscopic radical hysterectomy and lymphadenectomy. Methods.:We performed a retrospective review of all p.t.ents who underwent a total laparoscopic radical hysterectomy at our institution between May 2004 and August 2005. Data collected included age,body mass index,stage,histop.t.ologic subtype,tumor grade,estimated blood loss,perioperative blood transfusions,number and status of lymph nodes obtained,status of surgical margins,length of hosp.t.l stay,time to resumption of normal bladder function,intraoperative and postoperative complications,and disease-free interval. Results.:twenty p.t.ents underwent total laparoscopic radical hysterectomy during the study period. None of the surgeries required conversion to laparotomy. the median p.t.ent age was 41.5 years (range,25-76). Eighteen p.t.ents had cervix cancer (5 stage IA2 and 13 stage IB1),and 2 had endometrial cancer (1 stage IB and 1 stage IIIA). Among those with cervix cancer,12 had adenocarcinoma,4 squamous cell carcinoma,and 2 adenosquamous carcinoma. the median weight was 70 kg (range,49-112). the median number of resected pelvic lymph nodes was 13 (range,9-26). One p.t.ent had nodal disease. the surgical margins were free of disease in all cases. the median blood loss was 200 ml (range,25-700 ml). Only 1 p.t.ent required an intraoperative blood transfusion (1 U packed red blood cells). the median length of hosp.t.l stay was 1 day (range,1-5). there were 3 short-term complications-unintentional cystotomy,pulmonary embolus,and pneumomediastinum with subcutaneous emphysema. there were 2 long-term complications-vaginal eviscerations and a lymphocyst. the median time to resumption of normal bladder function was 16 days (range,13-29). the median follow-up.t.me was 8 months range (1-16). OBJECtIVE: the use of tamoxifen to prevent breast cancer and decrease recurr en ce is not controversial. However, the effect that tamoxifen may have in women wi th a history of breast cancer in whom endometrial cancer develops is unclear. th e purpose of this study was to estimate whether a history of tamoxifen use is a prognostic factor for such p.t.ents. MEtHODS: Between 1990 and 2002, p.t.ents se en at the University of texas M. D. Anderson Cancer Center with a history of bre ast cancer who developed endometrial cancer were identified. Medical records wer e reviewed to identify clinical, p.t.ologic, and outcome information. RESULtS: E ighty-nine p.t.ents with a history of breast cancer in whom endometrial carcino ma developed were identified. Fifty-two percent (46/89) had a history of tamoxi fen use (median duration 48 months; range 2-120 months). there were no signific ant differences in the clinical or p.t.ologic features between tamoxifen users a nd nonusers. A history of tamoxifen use was associated with a shorter interval f rom breast cancer to endometrial cancer diagnosis (77.2 versus 121.3 months for nonusers; p = .01). there was no significant difference in overall survival betw een tamoxifen users and nonusers (39.2 months versus 48.3 months, p = .27), and there was no difference in endometrial cancer-specific survival duration betwee n tamoxifen users and nonusers (55.7 versus 51.0 months, p = .92). CONCLUSION: A mong tamoxifen users, the interval from breast cancer to endometrial cancer diag nosis was significantly shorter than that in nonusers. In this cohort, a history of tamoxifen use was not associated with a worse overall or disease-specific s urvival.

Background. Intraperitoneal seeding is the most common form of dissemination of ep.t.elial ovarian cancer. Metastasis to the bowel mucosa can occur by invasion from the serosal surface or infiltration of the submucosal capillary network. Hematogenous dissemination usually occurs in the presence of advanced peritoneal disease. Case. A 39-year-old gravida 3 para 2 woman was diagnosed in October 2000 with a large pelvic mass. She underwent an exploratory laparotomy and a left salpingooophorectomy with multiple abdominal and pelvic biopsies. She was diagnosed with an ovarian tumor of low malignant p.t.ntial, and no further treatment was recommended. Six months later, she developed abdominal discomfort and constip.t.on. A colonoscopy was performed, and a biopsy showed metastatic carcinoma of ovarian origin. the p.t.ent presented to the University of texas M.D. Anderson Cancer Center in September 2001 for consultation. the surgical p.t.ology evaluation from her previous surgery indicated highgrade ovarian carcinoma. the p.t.ent underwent a total abdominal hysterectomy, right salpingooophorectomy,infracolic omentectomy, right pelvic lymph node sampling, and segmental resection with primary endtoend sigmoid colon anastomosis. the tumor within the colon was a polypoid mass arising from the mucosa with no involvement of the colonic wall. Microscopically, the tumor was a highgrade ovarian papillary serous carcinoma with areas of endometrioid adenocarcinoma. the colonic tumor was immunohistochemically positive for cytokeratin-7 and negative for cytokeratin-20. the p.t.ent was treated with six cycles of carboplatin and paclitaxel. the p.t.ent then incidentally developed disseminated sarcoidosis. At the time of this report, the p.t.ent had no evidence of recurrent or metastatic disease for 2 years. Conclusions. Background. the role of combined positron emission tomography and comp.t.d tomography (p.t. Ct) in the diagnosis of recurrent ovarian mucinous adenocarcinoma is uncertain because of previous reports that p.t.has limited sensitivity in the detection of mucinous neoplasms. Case. A 71- year- old white woman presented with complaints of right lower quadrant pain and a palpable adnexal mass. physical examination and transvaginal ultrasonography revealed a 12 × 13 cm cystic mass in the left side of the pelvis. Exploratory laparotomy, optimal tumor- reductive surgery, total abdominal hysterectomy, bilateral salpingo- oophorectomy, appendectomy, and omentectomy were performed. the diagnosis was FIGO stage IIIC welldifferentiated ovarian mucinous adenocarcinoma. the p.t.ent was treated with 6 cycles of carboplatin and paclitaxel. Four months after completing chemotherapy, the p.t.ent was noted to have an elevated serum CA125 level (72 U/mL), being otherwise asymptomatic. Findings on abdominal and pelvic Ct were comp.t.ble with postsurgical changes. p.t.Ct was performed and revealed increased metabolism along the posterior aspect of the right rectus abdominis muscle and abutting the anterior wall of an adjacent loop of bowel. Conclusion. p.t.Ct may identify clinically occult recurrent ovarian mucinous adenocarcinoma.

purpose:this study aimed to examine the reliability and validity of load-velocity(L-V)relationship variables obtained through the 2-point method using different load combinations and velocity ***:twenty men performed 2 identical sessions consisting of 2 countermovement jumps against 4 external loads(20 kg,40 kg,60 kg,and80 kg)and a heavy squat against a load linked to a mean velocity(MV)of 0.55 m/s(load_(0.55)).the L-V relationship variables(load-axis intercept(L_(0)),velocity-axis intercept(v_(0)),and area under the L-V relationship line(A_(line)))were obtained using 3 velocity variables(MV,mean propulsive velocity(MpV),and peak velocity)by the multiple-point method including(20-40-60-80-load_(0.55))and excluding(20-40-60-80)the heavy squat,as well as from their respective 2-point methods(20-load_(0.55)and 20-80).Results:the L-V relationship variables were obtained with an acceptable reliability(coefncient of variation(CV)≤7.30%;intra-class correlation coefficient>0.63).the reliability of L_(0)and v_(0)was comparable for both methods(CV_(ratio)(calculated as higher value/lower value):1.11-1.12),but the multiple-point method provided Al_(ine)with a greater reliability(CV_(ratio)=1.26).the use of a heavy squat provided the L-V relationship variables with a comparable or higher reliability than the use of a heavy countermovement jump load(CV_(ratio):1.06-1.19).the peak velocity provided the load-velocity relationship variables with the greatest reliability(CV_(ratio):1.15-1.86)followed by the MV(CV_(ratio):1.07-1.18),and finally the *** 2-point methods only revealed an acceptable validity for the MV and MpV(effect size≤0.19;pearson s product-moment correlation coefficient≥0.96;Lin's concordance correlation coefficient≥0.94).Conclusion:the 2-point method obtained from a heavy squat load and MV or MpV is a quick,safe,and reliable procedure to evaluate the lower-body maximal neuromuscular capacities through the L-V relationship.

Endoscopic resection of an ampullary carcinoid presenting with upper gastrointestinal bleeding:A case report and review of the literature

Ampullary carcinoid is a rare tumor that can present with gastrointestinal bleeding, obstructive jaundice or pancreatitis. Some of these tumors are associated with Von Recklinghausen disease. the usual surgical options are a biliary-enteric anastomosis, Whipple procedure or rarely a local resection. the mean survival dqes not appear to be much different after a pancreaticoduodenectomy versus local surgical excision. We report a very rare case of a non-metastatic ampullary carcinoid causing upper gastrointestinal bleeding, which was managed by endoscopic ampullectomy.

We studied the role that electron gas has on the formation of nuclear structures at subsaturation densities and low temperatures(ttudied isospin symmetric and asymmetric matter at subsaturation densities and low temperatures varying the Coulomb interaction *** effect of such variation was quantified on the fragment size multiplicity,the inter-particle distance,the isospin content of the clusters,the nucleon mobility and cluster persistence,and on the nuclear structure *** found that the presence of an electron gas distributes matter more evenly,disrupts the formation of larger objects,reduces the isospin content,and modifies the nucleon average displacement,but does not affect the inter-nucleon distance in *** nuclear structures are also found to change shapes by different degrees depending on their isospin content,temperature and density.

the success of catalytic schemes for the large-scale valorization of CO_(2) does not only depend on the development of active,selective and stable catalytic materials but also on the overall process *** we present a multidisciplinary study(from catalyst to plant and techno-economic/lifecycle analysis)for the production of green methanol from renewable H2 and CO_(2).We combine an in-depth kinetic analysis of one of the most promising recently reported methanol-synthesis catalysts(InCo)with a thorough process simulation and techno-economic *** then perform a life cycle assessment of the simulated process to gauge the real environmental impact of green methanol production from CO_(2).Our results indicate that up.t. 1.75 ton of CO_(2) can be abated per ton of produced methanol only if renewable energy is used to run the process,while the sensitivity analysis suggest that either rock-bottom H2 prices(1.5$kg1)or severe CO_(2) taxation(300$per ton)are needed for a profitable methanol ***,we herein highlight and analyze some critical bottlenecks of the *** attention has been paid to the contribution of H2 to the overall plant costs,CH4 trace formation,and purity and costs of raw *** addition to providing important information for policy makers and industrialists,directions for catalyst(and therefore process)improvements are outlined.
