Transketolase, the most critical enzyme of the non-oxidative branch of the pentose phosphate pathway, has been reported as a novel target in Plasmodium falciparum as it has least homology with the human host. Homology model of P. falciparum transketolase (PfTk) was constructed using the crystal structure of S. cervisiae transketolase as a template, and used for the identification and prioritization of potential compounds targeted against Plasmodium falciparum transketolase. The docking studies with fructose-6-phosphate and thiamine pyrophosphate showed that His31, Asp473, Ser388, Arg361 and His465 formed hydrogen bonds with fructose-6-phosphate while pyrimidine ring of coenzyme interacted with conserved residues of protein viz., Leu121, Glu415, Gly119. The major interacting residues involved in binding of oxythiamine pyrophosphate were similar to cofactor binding site of PfTk. An integrated pharmacophore, co-factor ThDP and substrate fructose-6-pho- sphate, based virtual screening of a small mo- lecule database retrieved eight and thirteen compounds respectively. When screened for their activity against P. falciparum transketolase, one compound in case of ThDP and three compounds in case of fructose-6-phosphate based screening were found active against PfTk. Identification of these novel and chemically diverse inhibitors provides initial leads for optimization of more potent and efficacious drug candidates to treat malarial infection.

Aim: Stroke is one of the leading causes of death and disability. The proportion of patients receiving recombinant tissue plasminogen activator is low in our country. Biomarkers to identify patients at risk of severe disease, and guide treatment and prognosis would be valuable. This article aims to identify the factors that can independently prognosticate the acute phase of ischemic stroke. Methods: All patients with the first episode of ischemic stroke admitted to the Neurology Department between 1st December 2017 to 31st March 2018 were included in this pilot study. Stroke severity was evaluated using the National Institute of Health Stroke Scale (NIHSS). Patients being admitted within 4.5 h of onset of symptoms were thrombolysed with injection alteplase. For each patient, 4 serum biomarkers (D-dimer, fibrinogen, C-reactive pro-tein and neuron specific enolase) were evaluated at admission and 24 h later. Discharged patients were assessed on an outpatient basis using the modified Rankin scale. The study primarily aimed to identify the factors predicting the severity and outcome of stroke, and to evaluate the effect of thrombolysis on the outcome. The secondary aim was to evaluate the role of biomarkers to predict the unfavorable outcome and the chance of post thrombolysis hemor-rhage. Results: Out of 30 patients included in the study, 10 had NIHSS 0-4, 12 had NIHSS 5-15 and 8 had NIHSS 16-42. Sixteen patients had unfavorable outcome (mRS score ≥ 2), and 5 patients expired. Old age, history of diabetes, CHADS2 score ≥ 2, and total anterior circulation stroke (TACS) independently affected stroke severity, whereas lowejection fraction < 35%, and TACS, independently predicted unfavorable outcome and mortality. High mean arte-rial blood pressure (MABP) and capillary blood glucose (CBG) at admission were significant predictors of stroke severity, unfavorable outcome, and mortality. We study the relation between monthly average counting rates of the cosmic ray intensity (CRI) observed at Moscow Neutron Monitoring Station, solar flare index (SFI) and coronal index during the solar cycles 22 and 23, for the period 1986-2008. The long-term behaviour of various solar activity parameters: sunspot numbers (SSN), solar flare index (Hα flare index), coronal index (CI) in relation to the duration of solar cycles 22 and 23 is examined. We find that the correlation coefficient of CRI with the coronal index as well as Hα flare index is relatively large anti-correlation during solar cycle 22. However, the monthly mean values of sunspot number, Hα flare index, and coronal index are well positively correlated with each other. We have analyzed the statistical analysis of the above parameters using of linear model and second order polynomial fits model.

Background: Studies have revealed a strong link between sleep-related breathing disorders and essential hypertension. However, the trends of sleep-disordered breathing in hypertensive patients have not been analyzed frequently. Methods: In this prospective, single-center and observational study, random patients with systemic essential hypertension, who were visiting GSVM Medical College, Kanpur, India between December 2012 and July 2014, were selected. After obtaining informed consent, patients were analyzed for symptoms of sleep-disordered breathing and Epworth Sleepiness Scale. Subsequently, all patients underwent overnight polysomnography to estimate the events of apnea and hypopnea. Observed data were compared between stage I and stage II hypertensive patients. Results: A total of 51 patients (age: 45.7 ± 13.43 years), comprising 36 stage I hypertensives and 15 stage II hypertensives, were enrolled in the study. About 47% of these patients displayed no symptoms of sleep-disordered breathing. Compared to stage I hypertensives, higher percentage of stage II hypertensives reported excessive day time sleepiness. Sleep latency time and sleep architecture were comparable between stage I and stage II hypertensives. Stage II hypertensives displayed shorter total sleep time and higher snore time than stage I hyper-tensives. Apnea Hypopnea Index (AHI) values increased with the severity of hypertension. Nocturnal oxygen desaturation was present in 25.0% and 53.3% of stage I and II hypertensives respectively. Mean lowest oxygen saturation was lower and the mean C-reactive protein levels were higher in stage II hypertensives as compared to stage I hypertensives. The mean C-reactive protein levels increased with increasing AHI scores, while the mean lowest oxygen saturation decreased with increasing AHI scores. Objective:To assess the efficacy of gabapentin in the treatment of low back pain ***:This prospective observational study was conducted over 6 months to assess the efficacy of gabapentin in patients suffering from low back *** medical history,pain severity by Visual Analogue scale(VAS)and sleep quality by Pittsburgh Sleep Quality Index(PQSI)were *** scores and PQSI scores before and after gabapentin treatment were compared,and gabapentin satisfaction post treatment were ***:This study included 100 low back pain patients with 65 males and 35 females,and the mean age was(39.0±10.5)*** commonest presentation was non-radiating low back pain(40%).The mean VAS score and the mean PQSI score in the study before treatment were 7.70±1.91 and 10.95±5.02,*** treatment with gabapentin,the mean VAS score and the mean PQSI score decreased to 2.75±1.79 and 4.90±2.20,respectively,and the differences before and after the treatment were significantly different(both P=0.001).Overall,62%of the patients were extremely satisfied with gabapentin because they reported no adverse drug ***,31%of the patients were satisfied and 7%were strongly dissatisfied with the ***:Gabapentin can improve sleep quality and reduce lower back pain as measured by the VAS and *** efficacy of this drug is relatively good,but further improvement is required.

In general, any stress inducing substance will affect the respiratory metabolism of an animal. Any alteration in the intermediary metabolism due to stress is bound to affect the activity of oxidative enzymes like lactic dehydrogenase (LDH) and succinic dehydrogenase (SDH). Both enzymes are involved in carbohydrate metabolism and have been used as an indicative criterion of exposure to chemical stress. Several authors have reported that the disturbance in the oxidative metabolism which leads to an alteration in whole oxygen consumption in different species of fishes exposed to pesticides. Carbohydrates are the primary and immediate source of energy. In stressed condition, the carbohydrate reserve (glycogen) is depleted to meet the energy demand. Depletion of glycogen may be due to the direct utilization for energy generation, a demand caused by the pesticide induced hypoxia. Similar findings have been reported in frog Rana tigrina. Glycogenolysis seems to be the result of increased secretion of catecholamines due to stress. Pesticides also inhibit energy production by suppressing aerobic oxidation of carbohydrates leading to energy crisis in animals. LDH and SDH are widely used in toxicology and clinical chemistry to diagnose the cell, tissue and organ damage. In the present study, the toxicological effect of methyl parathion on the LDH and SDH activity has been made in the fish, Cirrihinus mrigala. The study revealed that the acute toxicity (TUa) of methyl parathion was 14 ppm.

Plantar lichen planus masquerading eumycetoma of dark grains

Lichen planus is a common inflammatory disease but its perforating variety is not so common and it has been described in small number of text and articles. Here we reported a case of plantar lichen planus where there was a history of discharge of dark grains from the sole of foot and diagnosing the disease as eumycetoma of dark grains repeated antifungal therapy could not resolve the lesions and histopathologically it showed the classical pictures of lichen planus. Collaborating the clinical and histological features we have diagnosed the case as perforating lichen planus but Verhoeff-Van Gieson stain could not elucidate the perforating channel which ot difficult to delineate and often missed. So, we have put the diagnosis of plantar lichen planus and treated with intramuscular triamcinolone and the lesions resolved.
