Hindi translation and validation of the English version of the gastrointestinal symptom rating scale questionnaire:An observational study

BACKGROUND The gastrointestinal symptom rating scale(GSRS)is a questionnaire in English language which is designed to assess the clinical symptoms in patients with irritable bowel syndrome(IBS)and peptic ulcer *** validated scale has questions on around 15 items and has been validated in patients with dyspepsia and *** To translate and validate the English version of the GSRS questionnaire to the Hindi *** The purpose of the present work was to create a Hindi version of this questionnaire for use in the Indian *** process involved various steps as per the World Health Organization methodology including initial forward translation,backward translation,and assessment by an expert *** pilot testing was followed by testing in healthy and diseased *** The Hindi translation was pilot tested in 20 individuals and further validated in healthy controls(n=30,15 females)and diseased individuals(n=72,27 females).The diseased group included patients with functional dyspepsia and ***'s alpha for internal consistency on the final translated GSRS questionnaire was 0.715 which is considered *** questions significantly differentiated the diseased population from the healthy population(P value<0.05)in the translated Hindi version of the *** The translated Hindi GSRS can be used to evaluate gastrointestinal function in clinical trials and community surveys in Hindi speaking populations.

Optimal Eighth Order Convergent Iteration Scheme Based on Lagrange Interpolation

In this paper, based on fourth order Ostrowski method, we derive an optimal eighth order iteration scheme for obtaining simple roots of nonlinear equations using Lagrange interpolation and suitable weight functions. The scheme requires three evaluations of the function and one evaluation of the first derivative per iteration. Numerical examples are included to confirm the theoretical results and to show the competitive performance of the proposed iteration scheme.

Renal gluconeogenesis in insulin resistance:A culprit for hyperglycemia in diabetes

Renal gluconeogenesis is one of the major pathways for endogenous glucose *** in this process may contribute to hyperglycemia in cases with insulin resistance and *** reviewed pertinent studies to elucidate the role of renal gluconeogenesis regulation in insulin resistance and diabetes.A consensus on the suppressive effect of insulin on kidney gluconeogenesis has started to build ***-resistant models exhibit reduced insulin receptor(IR)expression and/or post-receptor signaling in their kidney *** IR expression or post-receptor signaling can cause impairment in insulin’s action on kidneys,which may increase renal gluconeogenesis in the state of insulin *** is now established that the kidney contributes up to 20%of all glucose production via gluconeogenesis in the post-absorptive ***,the rate of renal glucose release excessively increases in *** rise in renal glucose release in diabetes may contribute to fasting hyperglycemia and increased postprandial glucose *** glucose release by the kidneys and renal expression of the gluconeogenic-enzyme in diabetic rodents and humans further point towards the significance of renal ***,the available literature suggests that impairment in renal gluconeogenesis in an insulinresistant state may contribute to hyperglycemia in type 2 diabetes.

Insulin receptors in the kidneys in health and disease

Insulin is an important hormone that affects various metabolic processes,including kidney *** in insulin's action leads to insulin resistance in the target *** defects in post-receptor insulin signaling,impairment at the receptor level could significantly affect insulin sensitivity of the target *** kidney is a known target of insulin;however,whether the kidney develops "insulin resistance" is *** of the insulin receptor(IR) expression and its function is very well studied in major metabolic tissues like liver,skeletal muscles,and adipose *** physiological relevance of IRs in the kidney has recently begun to be *** credit goes to studies that showed a wide distribution of IR throughout the nephron segments and their reduced expression in the insulin resistance ***,altered renal and systemic metabolism observed in mice with targeted deletion of the IR from various epithelial cells of the kidney has strengthened this *** this review,we recapitulate the crucial findings from literature that have expanded our knowledge regarding the significance of the renal IR in normal-and insulin-resistance states.

Improved Ostrowski-Like Methods Based on Cubic Curve Interpolation

In this paper, we derive two higher order multipoint methods for solving nonlinear equations. The methodology is based on Ostrowski’s method and further developed by using cubic interpolation process. The adaptation of this strategy increases the order of Ostrowski’s method from four to eight and its efficiency index from 1.587 to 1.682. The methods are compared with closest competitors in a series of numerical examples. Moreover, theoretical order of convergence is verified on the examples.

Association of TAP1 Gene Polymorphism in Human Papilloma Virus Related Oropharyngeal Cancer

Background: Polymorphisms of TAP1 gene might be pertinent in development of cancer by altering the immune response. We studied the association of TAP1 gene polymorphism with HPV related oropharyngeal cancer. Methods: This prospective study consisted of 200 subjects divided into three groups;Group A—HPV positive oropharyngeal cancer (17/100), Group B—HPV negative oropharyngeal cancer (83/100) and Group C—Controls. TAP1ile333val polymorphism genotyping was performed by ARMS-PCR. Results: No significant difference was observed in the distribution of Val/Val genotype of Group A in comparison to Group C (OR = 0.663, 95% CI = 0.164 - 2.688, p = 0.742) and Group B (OR = 1.725, 95% CI = 0.388 - 7.675, p = 0.677) and thus was not associated with HPV associated cancer. But the frequency of Val/Val genotype was found to be significantly decreased in Group B as compared to controls and was linked with increased risk of oropharyngeal cancers (OR = 0.38, 95% CI = 0.15 - 0.97, p = 0.048). Conclusion: TAP1i333v gene polymorphism was not associated with HPV positive oropharyngeal cancer;however decreased frequency of Val/Val genotype raises the risk of oropharyngeal cancer.

Patch testing and cross sensitivity study of adverse cutaneous drug reactions due to anticonvulsants: A preliminary report

AIM To evaluate the utility of patch test and cross-sensitivity patterns in patients with adverse cutaneous drug reactions(ACDR) from common anticonvulsants. METHODS Twenty-four(M:F = 13:11) patients aged 18-75 years with ACDR from anticonvulsants were patch tested 3-27 mo after complete recovery using carbamazepine, phenytoin, phenobarbitone, lamotrigine, and sodium valproate in 10%, 20% and 30% conc. in pet. after informed consent. Positive reactions persisting on D3 and D4 were considered significant. RESULTS Clinical patterns were exanthematous drug rash with or without systemic involvement(DRESS) in 18(75%), Stevens-Johnsons syndrome/toxic epidermal necrolysis(SJS/TEN) overlap and TEN in 2(8.3%) patients each, SJS and lichenoid drug eruption in 1(4.2%) patient each, respectively. The implicated drugs were phenytoin in 14(58.3%), carbamazepine in 9(37.5%), phenobarbitone in 2(8.3%), and lamotrigine in 1(4.7%) patients,respectively. Twelve(50%) patients elicited positive reactions to implicated drugs; carbamazepine in 6(50%), phenytoin alone in 4(33.3%), phenobarbitone alone in 1(8.3%), and both phenytoin and phenobarbitone in 1(8.33%) patients, respectively. Cross-reactions occurred in 11(92%) patients. Six patients with carbamazepine positive patch test reaction showed cross sensitivity with phenobarbitone, sodium valproate and/or lamotrigine. Three(75%) patients among positive phenytoin patch test reactions had cross reactions with phenobarbitone, lamotrigine, and/or valproate. CONCLUSION Carbamazepine remains the commonest anticonvulsant causing ACDRs and cross-reactions with other anticonvulsants are possible. Drug patch testing appears useful in DRESS for drug imputability and cross-reactions established clinically.
