Purpose： To evaluate the efficacy of intramedullary Kirschner wires for the treatment of unstable tibial shaft fractures in children. Methods： This prospective study was conducted at the Department of Orthopaedic Surgery in Maharishi Markandeshwar Medical College from June 2005 to June 2010. Sixty-six children having closed fracture of the tibial shaft with a mean age of 7.7 years （range, 2-14 years） were recruited from emergency and outpatient department. They were treated with percutaneous intramedullary Kirschner wires. The clinical results of our study were rated on the basis of the criteria of union, nonunion, delayed union or malunion. All children were followed for one year. Results： Children achieved union in a mean time of 8 weeks （range, 6-10 weeks）. Postoperatively, three children （4.55%） had delayed union, one （1.52%） valgus deformity of lower leg, three （4.55%） post- operative knee pain and twelve （18.18%） skin irritation at pin site. Wires were removed after 8-22 weeks without any complications. No patient was lost to follow-up. The results were excellent in 95.45% and good in 4.55% children. Conclusion： This technique is cost-effective, simple, quick to perform, safe and reliable and avoids pro- longed hospitalization with good results.

Radiofrequency ablation(RFA)is the most common approach to thermal ablation for cancer ***,its efficacy is limited by incomplete ablation,and further optimization of RFA is ***,we demonstrate that incubation at 65°C triggers more EG7 tumor cell death by necrosis than treatment at 45°C,and the 65°C-treated cells are more effective at inducing antigen-specific CD8^(+)cytotoxic T lymphocyte(CTL)responses after injection in mice than the 45°C-treated *** cells(DCs)that phagocytose 65°C-treated EG7 cells become mature with upregulated MHCII and CD80 expression and are capable of efficiently inducing effector CTLs in mouse tumor ***(65°C)therapy of EG7 tumors induces large areas of tumor necrosis and stimulates CTL *** leads to complete regression of small(~100 mm^(3))tumors but fails to suppress the growth of larger(~350 mm^(3))*** administration of the Toll-like receptor-9(TLR9)agonist unmethylated cytosine-phosphorothioate-guanine oligonucleotide(CpG)to DCs phagocytosing 65°C-treated EG7 cells enhances the expression of MHCII and CD40 on DCs as well as DC-induced stimulation of CTL ***,the intratumoral administration of CpG following RFA also increases the frequencies of tumor-associated immunogenic CD11b−CD11c^(+)CD103^(+)DC2 and CD11b+F4/80+MHCII+M1 macrophages and increases CD4^(+)and CD8^(+)T-cell tumor infiltration,leading to enhanced CD4^(+)T cell-dependent CTL responses and potent inhibition of primary RFA-treated or distant untreated tumor growth as well as tumor lung metastasis in mice bearing larger ***,our data indicate that CpG administration,which enhances RFA-induced CTL responses and ultimately potentiates the inhibition of primary tumor growth and lung metastasis,is a promising strategy for improving RFA treatment,which may assist in optimizing this important cancer therapy.

Background:Helicobacter pylori infection has been associated with insulin resistance and non-alcoholic fatty liver disease(NAFLD).This study was done to evaluate the effect of ***-eradication therapy(HPET)in patients with NAFLD compared to standard management therapy(SMT).Methods:Eighty NAFLD patients with *** co-infection were randomized into SMT(diet and exercise,n=36)and HPET(SMT plus amoxicillin,clarithromycin,and pantoprazole,n=44)*** controlled attenuation parameter(CAP),anthropometric parameters,liver enzymes,lipid profile,and glycemic parameters including homeostasis model assessment-insulin resistance(HOMA-IR)were measured and compared between two groups at the baseline and 24 ***:Sixty-four participants(SMT group[n=28]and HPET group[n=36])were included in amodified intention-to-treat *** the SMT group and the HPET group had a significant reduction in CAP scores at 24weeks(P=0.002 and P<0.001,respectively),but the change between the groups was insignificant(P=0.213).Successful eradication of *** occurred in 68%of the HPET group and led to greater improvement in HOMA-IR at 24weeks compared to SMT or non-responder patients(P=0.007).The liver enzymes reduced significantly at 24weeks in both groups,but the changes between the groups were *** lipid parameters remained unchanged within the groups and between the groups at 24weeks.A significant increase in the levels of reduced glutathione was noted in the HPET group,but the change between the two groups was not statistically ***:HPET was found to be comparable to SMT alone in reducing hepatic steatosis and liver enzymes at 24 weeks in NAFLD ***,successful eradication of *** led to greater improvement in HOMA-IR(Trial registration CTRI/2017/05/008608).

Objective: To describe the primary carcinoma of the retroperitoneum that was positive for human papillomavirus (HPV), a surrogate molecular marker for high-risk HPV (p16) and p53. Methods: This is a case report. Results: We report a case of 50-year-old woman with a large retroperitoneal mass (squamous cell carcinoma on histology) that was treated in September of 2014 with radical concurrent chemotherapy with weekly cisplatin and pelvic radiation (4500 cGY) and additional right pelvic boost of 900 cGY. It was assumed she had FIGO stage IIIB local advanced cervical cancer. She initially presented with the right leg swelling, severe pain and difficulty in walking and was treated with antibiotics for presumed cellulitis. Several months later right leg extensive deep vein thrombosis (DVT) was diagnosed and was started on anticoagulation. CT scan and MRI reported a large right-sided retroperitoneal mass occluding the illiac vessels and ureter. The mass had invaded into the adjacent pelvic sidewall, with destruction of the iliacus muscle and underlying iliac wing. MRI showed that the cervix and uterus were both normal. On examination cervix appeared grossly normal and cervical punch biopsy revealed HSIL only. CT-guided biopsy of the mass revealed a moderately differentiated squamous cell carcinoma positive for p16 and p53. Conclusion: Primary squamous cell carcinoma of the retroperitoneum is a real entity that is unknown. It is unclear if primary neoplasia in the cervix was small from which a cell clone arose and migrated to the retroperitoneal space.

Locations that connect tracks from different rail-road companies—referred to as interchange points—ex- change crew, locomotives, and their associated consists. Because trains have a single degree of freedom in movement, that is, they can only operate along the tracks, any delay occurring at an interchange point causes cascading delays in connecting tracks. In addition, authentication and authorization that is expected to take place at interchanges in PTC controlled train movement may add extra delays due to mutual authentication between two security domains. In this paper we propose a model that can address safety and security con- cerns and their interrelationships that govern train movement through an interchange point. We show how a profile of safe operations can be computed for operating an interchange point.

Irreversible electroporation(IRE)is a new cancer ablation technology,but methods to improve IRE-induced therapeutic immunity are only beginning to be *** developed a mouse model bearing large primary(300 mm^(3))and medium distant(100 mm^(3))EG7 lymphomas engineered to express ovalbumin(OVA)as a nominal tumor *** established experimental protocols including IRE alone and IRE combined with Toll-like receptor(TLR)3/9 agonists(poly I:C/CpG)(IRE+pIC/CpG),PD-1 blockade(IRE+PD-1 blockade),or both(IRE+Combo)to investigate therapeutic effects on primary and distant EG7 tumors and conversion-promoting effects on the immunotolerant tumor microenvironment(TME).We demonstrated that IRE alone simulated very weak OVA-specific CD8^(+)T cell responses and did not inhibit primary tumor ***+pIC/CpG synergistically stimulated more efficient OVA-specific CD8^(+)T cell responses and primary tumor growth inhibition than IRE+PD-1 ***+pIC/CpG played a major role in the modulation of immune cell profiles but a minor role in the downregulation of PD-L1 expression in the TME and vice versa for IRE+PD-1 ***+Combo cooperatively induced potent OVA-specific CD8^(+)T cell immunity and rescued exhausted intratumoral CD8^(+)T cells,leading to eradication of not only primary tumors but also untreated concomitant distant tumors and lung ***+Combo efficiently modulated immune cell profiles,as evidenced by reductions in immunotolerant type-2(M2)macrophages,myeloid-derived suppressor-cells,plasmacytoid dendritic cells,and regulatory T cells and by increases in immunogenic M1 macrophages,CD169^(+)macrophages,type-1 conventional dendritic cells,and CD8^(+)T cells,leading to conversion of immunotolerance in not only primary TMEs but also untreated distant ***+Combo also showed effective therapeutic effects in two breast cancer ***,our results suggest that IRE+Combo is a promising strategy to improve IRE ablation therapy in cancer.

Improved Ostrowski-Like Methods Based on Cubic Curve Interpolation

In this paper, we derive two higher order multipoint methods for solving nonlinear equations. The methodology is based on Ostrowski’s method and further developed by using cubic interpolation process. The adaptation of this strategy increases the order of Ostrowski’s method from four to eight and its efficiency index from 1.587 to 1.682. The methods are compared with closest competitors in a series of numerical examples. Moreover, theoretical order of convergence is verified on the examples.
