Background:Acute-on-chronic liver failure(ACLF)is characterised by a sudden deterioration of underlying chronic liver disease,resulting in increased rates of mortality and liver *** prognostication can benefit optimal allocation of ***:ACLF was defined as per the disease criteria of the Asian Pacific Association for the Study of the *** discharge summaries from between January 2001 and April 2013 were *** primary outcome was mortality or liver transplantation within 60 days from onset of ***‘model for end-stage liver disease’(MELD)score and change in MELD at Weeks 1,2 and 4 were reviewed in order to identify the earliest point for prediction of mortality or liver ***:Clinical data were collected on 53 subjects who fulfilled the inclusion and exclusion *** 60 days from presentation,20 patients(37.7%)died and 4(7.5%)underwent liver *** MELD of-2 after 2 weeks was 75.0%sensitive and 75.9%specific for predicting mortality or liver *** the MELD score did not increase at 2 weeks,predictive chance of survival was 93.8%over the next 60 *** change at 1 week showed poor sensitivity and *** at 4 weeks was too late for ***:Change in MELD score at 2 weeks provides an early opportunity for prognostication in ACLF.A MELD score that does not deteriorate by Week 2 would predict 93.8%chance of survival for the next 60 *** finding warrants further validation in larger cohort studies.

In the present study, we have theoretically modelled a surface plasmon resonance （SPR） based sensing chip utilizing a prism made up of gallium phosphidee. It has been found in the study that a large range of refractive index starting from the gaseous medium to highly concentrated liquids can be sensed by using a single chip in the visible region of the spectrum. The variation of the sensitivity as well as detection accuracy with sensing region refractive index has been analyzed in detail. The large value of the sensitivity along with the large dynamic range is the advantageous feature of the present sensing probe.

AIM:To determine the incidence and factors responsible for anastomotic leaks and stricture following anterior resection(AR)and its subsequent ***:Retrospective analysis of data from 108 patients with rectal carcinoma who underwent AR or low anterior resection(LAR)to identify the various preoperative,operative,and post operative factors that might have influence on anastomotic leaks and ***:There were 68 males and 40 females with an average of 47 years(range 21-75 years).The median distance of the tumor from the anal verge was 8 cm(range 3-15 cm).Sixty(55.6%)patients underwent handsewn anastomosis and 48(44.4%)were *** median operating time was 3.5 h(range2.0-7.5 h).Sixteen(14.6%)patients had an anastomotic *** these,11 patients required reexploration and five were managed *** anastomotic leak rate was similar in patients with and without diverting stoma(8/60,13.4%with stoma and 8/48;16.7%without stoma).In 15(13.9%)patients,resection margins were positive for ***(17.6%)patients developed anastomotic strictures at a median duration of 8 mo(range 3-20 mo).Among these,15 patients were successfully managed with per-anal *** multivariate analysis,advance age(>60 years)was the only risk factor for anastomotic leak(P=0.004).On the other hand,anastomotic leak(P=0.00),mucin positive tumor(P =0.021),and lower rectal growth(P=0.011)were found as risk factors for the development of an anastomotic ***:Advance age is a risk factor for an anastomotic *** anastomotic leak,a mucin-secreting tumor,and lower rectal growth predispose patients to develop anastomotic strictures.

Multi and single carrier waveforms are utilized in cellular systems for high-speed data *** The Fifth Generation(5G)system,several waveform techniques based on multi carrier waveforms are ***,the Peak to Average Power Ratio(PAPR)is seen as one of the significant concerns in advanced waveforms as it degrades the efficiency of the *** proposed article documents the study,progress,and implementation of PAPR reduction algorithms for the 5G radio *** compare the PAPR algorithm performance for advanced and conventional *** simulation results reveal that the advanced Partial Transmission Sequence(PTS)and Selective Mapping(SLM)methods enhanced the throughput and gain of the 5G ***,we have also analyzed the performance of Orthogonal Time Frequency Space Modulation(OTFSM)based on a single carrier system and found that PAPR is significantly low and is best suited to fading *** is seen that the conventional algorithms lower the PAPR but increase the *** proposed PTS and SLM have shown good performance with low computational complexity.

We consider the extension of network lifetime of battery driven wireless sensor networks by splitting the sensing area into uniform clusters and implementing heterogeneous modulation schemes at different members of the clusters. A cross-layer optimization has been proposed to reduce total energy expenditure of the network;at network layer, routing is done through uniform clusters;at MAC layer, each sensor node of the cluster is assigned fixed or variable time slots and at physical layer different member of the clusters is assigned different modulation techniques. MATLAB simulation proved substantial network lifetime gains.

Massive-Multiple Inputs and Multiple Outputs(M-MIMO)is considered as one of the standard techniques in improving the performance of Fifth Generation(5G)radio.5G signal detection with low propagation delay and high throughput with minimum computational intricacy are some of the serious concerns in the deployment of *** evaluation of 5G promises a high quality of service(QoS),a high data rate,low latency,and spectral efficiency,ensuring several applications that will improve the services in every *** existing detection techniques cannot be utilised in 5G and beyond 5G due to the high complexity issues in their *** the proposed article,the Approximation Message Passing(AMP)is implemented and compared with the existing Minimum Mean Square Error(MMSE)and Message Passing Detector(MPD)*** outcomes of the work show that the performance of Bit Error Rate(BER)is improved with minimal complexity.

The collision between and Indian and Asian plate has not only give rise to some of the highest mountains of the world but also is responsible for the various global phenomena associated with paleoclimate and paleo\|*** indirect approaches have been employed in order to constrain the age of initiation of this collision (Beck et al 1995,*** 1996,Dewey 1989).Still it is being quoted anywhere between ca.60 to 40Ma in the *** main reason for the poor constraints on the age of the initiation of the collision is unavailability of a direct method to date *** this paper we present the Ar\|Ar ages of the ophiolites from the Indus Suture,Ladakh and discuss the scope of this approach in constraining the age of the collision more narrowly.1\ Samples and methodology\;Several ophiolitic melange occurrences have been reported belonging to the Indus Suture in the Ladakh *** are characterized by the typical oceanic floor assemblages like chert,limestone etc mixed with the peridotites,serpentinites,dolerites,basalts,and pillow lavas (*** 1990).We present here the 40 Ar\| 39 Ar age spectra of two volcanics from the Sumdo Nala section of central Ladakh and one pillow lava from Chiktan,which is about 100km west of the Sumdo *** samples were analysed using Ar\|Ar step heating method following the procedures described by Venketesan et al.1993.

In this paper, an energy efficient inter cluster coordination protocol developed for the wireless sensor networks has been proposed. By controlling the topology, longevity and the scalability of the network can be increased. Clustering sensor node is an effective topology for the energy constrained networks. So cluster based algorithm has been developed in which different levels of clusters are considered on the basis of received signal strength to recognize the distance of the clusters from the BS (base station) and to determine the number of cluster coordinators to make routes for the CHs to transmit the data. Based on the investigation of existing protocols in which cluster heads send data directly to the base station, it is found that direct transmission by the CHs is not an optimal solution and dissipates a lot of energy, so in this paper a novel EEICCP (Energy efficient inter cluster coordination) protocol has been proposed which evenly distributes the energy load among the sensor nodes and use the multi hop approach for the CHs. Analytical model of new protocol is projected and the algorithm is implemented in MATLAB. Moreover, EEICCP has shown remarkable improvement over already existing LEACH and HCR protocols in terms of reliability and stability. Our work has also been validated through the simulation results.

Hepatitis B viral(HBV)reactivation in the immunosuppressed is a significant problem even in patients who have achieved serological clearance due to the persistence of HBV as *** reactivation will continue to pose a significant healthcare burden given the high prevalence of HBV and increasing use of *** of hepatitis B surface antigen,antibody to Hepatitis B core antigen antibody and HBV DNA levels should be done routinely in all patients planned for significant immunosuppressant *** aimed to examine the factors affecting reactivation *** depended on HBV disease status,the underlying disease requiring immunosuppression,and the specific immunosuppressive *** antiviral prophylaxis can prevent reactivation,it increases cost and still has risk of delayed reactivation after stopping antivirals and close follow-up and on-demand treatment is a good alternative for patients at risk of reactivation.
