We have examined primary tumor sections from melanoma patients by immunohistochmistry (IHC) for the presence of the odontogenic ameloblast-associated protein (ODAM). Within these patient tissues we have observed a correlation of nuclear ODAM staining in the primary tumors with sentinel lymph node (sLN) metastasis. surgically, sLN invasion in melanoma is considered an important indicator of more aggressive, invasive melanoma and to date there are limited biomarkers which strongly correlate with metastatic disease. The observation that ODAM staining in melanoma associates with sLN invasion may have important prognostic implications which could assist in the management of melanoma. Notably, ODAM expression may correlate with pathway-signaling we have previously reported to be affected by ectopic ODAM expression in cultured melanoma and breast cancer cell lines.

Patients with nonerosive gastroesophageal reflux disease often have relatively low esophageal acid exposure and respond suboptimally to gastric acid suppressi on. In these patients, other constituents of gastric contents may induce esophag eal *** have demonstrated that gastric contents can cause heartburn when the gastric pH > 4. (Aliment Pharm Ther 14:129-134, 2000). The aim of this stu dy was to determine relative sensitivities to chenodeoxycholic and ursodeoxychol ic acids, and 0.1 N HCl, administered as provocative perfusion tests. Patients w ith functional heartburn and healthy control subjects were evaluated. Patients u nderwent a modified Bernstein acid infusion test and esophageal Barostat balloon distention. Time and volume to pain were recorded. Barostat balloon distention was performed using our standard ***- wise distentions were performed and pain was *** to chenodeox ycholic acid (Cheno) and Ursodeoxycholic acid (Urso) were assessed similarly to the Bernstein test using 2 mM concentrations of each, followed immediately by 5 mM if no pain was reported with 2 mM. Volume of bile acid infusion and length of time until pain was induced were assessed and compared to the same endpoints fo r acid sensitivity. “Total"time and “total" volume to induce pain were calcula ted for Cheno and Urso. Least-squares means were generated and twotailed t-tes ts and regression analyses were performed (Psignificance). Ten functional heartburn patients and six healthy contr ols were evaluated (3M, 13 F; age range, 19 to 56 years). since five of six cont rols had pain with ac- id infusion(hypersensitive), all subjects were analyzed as one group. Only thr ee subjects (all controls) had no pain with infusion of 2 mM Cheno and received the follow-up infusion of 5 mM. Objective:To better evaluate tertiary Gleason pattern reporting and to evaluate the impact of tertiary Gleason pattern 5(TP5)on prostate cancer pathological features and biochemical recurrence at our large single ***:We retrospectively reviewed 1962 patients who underwent radical prostatectomy(RP)for prostate cancer;TP5 was reported in 159 cases(8.1%).Men with Gleason score(Gs)7 and Gs 8 disease were divided into subgroups with and without TP5,and histopathological features were *** analyses were conducted to assess the impact on TP5 on biochemical-free survival(BFs).Results:Tumors possessing Gs 3+4 with TP5 were more likely to exhibit extraprostatic extension(EPE)and had a larger tumor diameter(TD)than Gs 3+4 *** 3+4 with TP5 was also associated with positive surgical margins(sM),seminal vesicle involvement(sVI),and higher pre-operative prostate-specific antigen(PsA)values,but without statistical *** 4+3 with TP5 more commonly presented with EPE,positive sM,sVI,and greater TD and pre-operative PsA level than Gs 4+3 *** multivariate analysis,Gleason score,EPE,and TP5 were overall independent risk factors for PsA recurrence in this ***,Gs 4+3 with TP5 was associated with shorter time to recurrence versus Gs 4+3 ***:Our results emphasize the importance of TP5 and suggest that criteria for tertiary pattern reporting in prostate cancer should be *** studies are needed to evaluate the role of tertiary patterns in prognostic models.

Crohn's disease(CD) is a chronic idiopathic inflammatory disease of gastrointestinal tract characterizedby segmental and transmural involvement of gastrointestinal tract. Ileocolonic and colonic/anorectal is a most common and account for 40% of cases and involvement of small intestine is about 30%. Isolated involvement of stomach is an extremely unusual presentation of the disease accounting for less than 0.07% of all gastrointestinal CD. To date there are only a few documented case reports of adults with isolated gastric CD and no reports in the pediatric population. The diagnosis is difficult to establish in such cases with atypical presentation. In the absence of any other source of disease and in the presence of nonspecific upper gastrointestinal endoscopy and histological findings, serological testing can play a vital role in the diagnosis of atypical CD. Recent studies have suggested that perinuclear anti-neutrophil cytoplasmic antibody and anti-saccharomycescervisia antibody may be used as additional diagnostic tools. The effectiveness of infliximab in isolated gastric CD is limited to only a few case reports of adult patients and the long-term outcome is unknown.

目的磺胺、磺胺嘧啶和氨苯砜是第一批通过破坏叶酸生物合成过程来治疗疟疾的磺胺类药物,叶酸生物合成过程是寄生虫存活的关键。因此,我们旨在通过合理的药物设计方法合成新型N-(2-芳基氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺衍生物。方法按常规方法合成所有化合物,并用光谱分析(1H核磁共振和质谱)对产物进行表征。使用薄层色谱法(TLC)监测反应进程。分析了所有衍生物在疟原虫半胱氨酸蛋白酶falcipain-2变构位点的有效结合模式。采用肉汤稀释法测定抗菌和抗真菌活性。结果s6(N-(2-噻唑-4-基)-乙酰基-氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺与s9(N-(1H-苯并[d]咪唑-2-基)氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺形成5个氢键;s8(N-(2-1H-咪唑-2-基)氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺和s10(N-(1-苯并[d]咪唑-5-基)氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺与酶的变构位点形成四个氢键。考虑到与目标酶的对接分数和氢键的形成,新的衍生物被加工用于湿实验室合成。所有新合成的衍生物都具有体外抗疟疾、抗真菌和抗菌活性。与标准品相比,所有衍生物对恶性疟原虫株表现出足够的敏感性。此外,化合物s9和s10显示出最有效的双重抗微生物和抗疟疾活性。它们在分子对接研究中也显示出强大的分子相互作用。结论基于以上结果,我们认为N-(2-芳基氨基苯基)-2,3-二苯基喹喔啉-6-磺酰胺衍生物具有作为抗疟药、抗真菌药和抗菌剂的优异生物潜力。

Does music reduce anxiety and discomfort during flexible sigmoidoscopy?A systematic review and meta-analysis

AIM To investigate the role of music in reducing anxiety and discomfort during flexible sigmoidoscopy. METHODs A systematic review of all comparative studies up to November 2016, without language restriction that were identified from MEDLINE and the Cochrane Controlled Trials Register(1960-2016), and EMBAsE(1991-2016). Further searches were performed using the bibliographies of articles and abstracts from major conferences such as the EsCP, NCRI, AsGBI and AsCRs. Me sH and text word terms used included "sigmoidoscopy", "music" and "endoscopy" and "anxiety". All comparative studies reporting on the effect of music on anxiety or pain during flexible sigmoidoscopy, in adults, were included. Outcome data was extracted by 2 authors independently using outcome measures defined a priori. Quality assessment was *** A total of 4 articles published between 1994 and 2010, fulfilled the selection criteria. Data were extracted and analysed using OpenM etaA nalyst. Patients who listened to music during their flexible sigmoidoscopy had less anxiety compared to control groups [Random effects; sMD: 0.851(0.467, 1.235), s.E = 0.196, P s no statistically significant heterogeneity(Q = 0.085, df = 1, P = 0.77, I2 = 0). Patients who listened to music during their flexible sigmoidoscopy had less pain compared to those who did not, but this difference did not reach statistical significance [Randomeffects; sMD: 0.345(-0.014, 0.705), s.E = 0.183, P = 0.06]. Patients who listened to music during their flexible sigmoidoscopy felt it was a useful intervention, compared to those who did not(P s no statistically significant heterogeneity(P = 0.528, I2 = 0). CONCLUsION Music appeared to benefit patients undergoing flexible sigmoidoscopies in relation to anxiety and was deemed a helpful intervention. Pain may also be reduced however further investigation is required to ascertain this.

Lepidolite pegmatite occurs as intrusive within biotite gneiss and amphibolite of Assam Meghalaya Gneissic Complex (AMGC) or Precambrian Gneissic Complex in the Dhubri district, Assam. AMGC is the north western extension of the Proterozoic rocks of Meghalaya Plateau or shillong plateau. In the field it occurs as small to large veins and scattered boulders. Lepidolite pegmatite is later intruded by non lepidolite pegmatite. Pegmatites are medium to coarse grained with quartz and K-feldspar. It also contains lepidolite, which occurs in the form of flakes and clusters varying from pink to purple in colour. Petrography of lepidolite pegmatite reveals lepidolite as major constituents with quartz, K-feldspar and muscovite as minor constituents. XRD analysis reveals lepidolite (muscovite) is major mineral phase with kaliophilite in minor amount. Geochemically, they are calc-alkaline to high calc-alkaline and per-aluminous in nature. On the basis of Alumina saturation Index (AsI), these pegmatites resemble Lithium-Cesium-Tantalum (LCT) family and compositional affinity with s-type granites of orogenic environments. Trace element compositions (Rb, sr, Ba) indicate crystal fractionations, variable degrees of fractionation, highly evolved nature of pegmatite’s and strongly differentiated granites protoliths as source. The different tectonic discrimination diagrams indicate s-type and I-type melt for pegmatite derivations. Therefore, both the studied pegmatites could be an evolved variety of granitic rocks that originated from the same magma. The REE is relatively low to moderate.
