珍珠岩对蚯蚓同步处理污泥-狐尾藻的研究

本研究利用太平二号蚯蚓构建生物反应器,以污水处理厂脱水污泥和粉绿狐尾藻作为研究对象,探究添加不同比例的珍珠岩对蚯蚓动物反应器处理混合物的减量化、无害化和资源化效果.结果表明,添加珍珠岩能够显著提高蚯蚓动物反应器同步处理污泥-狐尾藻的效率,经反应器处理后,混合物减量化效果明显,处理后产物的pH、有机质、碱解氮和重金属含量(Cu、Pb、Cr)均表现为明显下降,电导率(EC)和速效磷显著上升.经反应器处理后,产物品质符合在pH>6.5的中性和碱性土壤上的施用标准.结果显示,当珍珠岩添加比例为4%时,对动物反应器的实用性和经济性效果最佳.

The local injection of triamcinolone acetonide(TA) is effective in preventing pyloric stenosis and deformity following large endoscopic submucosal dissection(ESD).However,because of its long-acting nature,TA can induce long-term local immunosuppression and subsequent adverse *** report a case of a cytomegalovirus(CMV) ulcer that formed only at the TA local injection site.A 68-year-old man underwent ESD to treat early gastric cancer that formed over the *** lesion extended to the duodenum,and an artificial ulcer covered more than two-thirds of the circumference of the *** prevent pyloric stenosis,TA was locally injected into the ulcer *** day 12,a deeper ulcer 10 mm in diameter was discovered in the center of the post-ESD *** revealed large cells with intranuclear inclusion bodies,which stained positive for the anti-CMV *** TA injections are useful,however,CMV ulcer might occur as adverse events.

Malignant peripheral nerve sheath tumor is a rare soft tissue sarcoma. Breast is an extremely rare location of this lesion and presentation as a breast lump in the absence of pain or previous benign neural tumor is even rarer. We report such a lesion in a 60 year-old female who presented with hard and painless breast lump for 2 years. Histopathology revealed a malignant spindle cell tumor of low grade potential. It was subsequently confirmed to be malignant peripheral nerve sheath tumor (MPNST) on the basis of immunopositivity for vimentin, neurone specific enolase and S-100.

目的:探讨早期血运重建及体外膜肺氧合(ECMO)治疗急性心肌梗死合并心原性休克患者的疗效,并分析影响其预后的因素。方法:回顾2018年1月至2019年12月郑州大学人民医院早期血运重建及ECMO救治的急性心肌梗死合并心原性休克66例患者的临床资料。收集并分析患者一般情况、临床特征、合并症、冠状动脉病变特点,ECMO前后血流动力学变化及应用时长、并发症及疗效,是否应用主动脉球囊反搏(IABP)及院内临床转归等。结果:66例患者平均年龄(56.6±11.7)岁,其中男性46例。患者入院均行早期血运重建:5例行冠状动脉旁路移植术(CABG);61例行经皮冠状动脉介入治疗(PCI),其中44例行入院即刻早期血运重建,17例行院内延迟早期血运重建;其中42例患者为多支冠状动脉病变。所有患者均应用ECMO辅助,其中39例使用了IABP。ECMO治疗后患者平均动脉压、中心静脉压、血乳酸水平及pH值均得到显著改善(P均<0.05)。66例患者中38例成功撤机,30例存活出院,36例死亡;并发症分别为20例肾功能不全或急性肾功能衰竭、11例消化道出血、5例脑出血、1例肺出血、9例下肢血栓及3例下肢缺血。多因素Logistic回归分析显示高血压,多脏器功能衰竭,发生至少2次呼吸心脏骤停、意识丧失及恶性心律失常事件为患者院内死亡的高危因素(P<0.05),应用IABP对院内结局无显著影响。结论:早期血运重建及ECMO用于救治急性心肌梗死合并心原性休克的患者是可行的。

AIM: To retrospectively review the results of over-thescope clip (OTSC) use in our hospital and to examine the feasibility of using the OTSC to treat perforations after endoscopic submucosal dissection (ESD). METHODS: We enrolled 23 patients who presented with gastrointestinal (GI) bleeding, fistulae and perforations and were treated with OTSCs (Ovesco Endoscopy GmbH, Tuebingen, Germany) between November 2011 and September 2012. Maximum lesion size was defined as lesion diameter. The number of OTSCs to be used per patient was not decided until the lesion was completely closed. We used a twin grasper (Ovesco Endoscopy GmbH, Tuebingen, Germany) as a grasping device for all the patients. A 9 mm OTSC was chosen for use in the esophagus and colon, and a 10 mm device was used for the stomach, duodenum and rectum. The overall success rate and complications were evaluated, with a particular emphasis on patients who had undergone ESD due to adenocarcinoma. In technical successful cases we included not only complete closing by using OTSCs, but also partial closing where complete closure with OTSCs is almost difficult. In overall clinical successful cases we included only complete closing by using only OTSCs perfectly. All the OTSCs were placed by 2 experienced endoscopists. The sites closed after ESD included not only the perforation site but also all defective ulcers ***: A total of 23 patients [mean age 77 years (range 64-98 years)] underwent OTSC placement during the study period. The indications for OTSC placement were GI bleeding (n = 9), perforation (n = 10), fistula (n = 4) and the prevention of post-ESD duodenal artificial ulcer perforation (n = 1). One patient had a perforation caused by a glycerin enema, after which a fistula formed. Lesion closure using the OTSC alone was successful in 19 out of 23 patients, and overall success rate was 82.6%. In health sciences, much efforts had been made in past years to explore the radio-protective agents from natural resources due to rapid exposure of radiations to environment such as space traveling, radiotherapy and largely growing telecommunication industry. It becomes crucial to find natural sources for radio-protection. In correspondence, dragon's blood (DB) is a traditional Chinese medicinal plant that possesses great medicinal values due to the presence of several phenolic compounds. For a long time, DB has been used in treatment of blood stasis, inflammation, oxidative stress, immune suppression and tumors, but recently it has been extensively used as radio-protective agent. There is no comprehensive review on radio-protective characterization of DB resin extract in literature. In our review, an attempt has been made to highlight unique and inherent radio-protection in liver, brain, kidney, lung, spleen and cerebrum. This review will help people in exploring the radioactive protectants from natural resources.

Colorectal cancer screening use among insured adults: Is out-of-pocket cost a barrier to routine screening?

AIM To describe the characteristics of adults who needed to see a doctor in the past year but could not due to the extra cost and assess the impact of limited financial resources on the receipt of routine fecal occult blood test, sigmoidoscopy, or colonoscopy for colon cancer screening among insured patients. METHODS Data obtained from the 2012 Behavioral Risk Factor Surveillance System included 215436 insured adults age 50-75 years. We computed frequencies, adjusted odds ratios(a ORs), and 95%CIs using SAS v9.3 software. RESULTS Nine percent of the study population needed to see a doctor in the past year but could not because of cost. The numbers were significantly higher among those aged 50-64(P < 0.0001), Non-Hispanic Whites(P < 0.0001), and those with a primary care physician(P < 0.0001) among other factors. Adjusting for possible confounders, a ORs for not seeing the doctor in the past year because of cost were: stool occult blood test within last year a OR = 0.88; 95%CI: 0.76-1.02, sigmoidoscopy within last year a OR = 0.72; 95%CI: 0.48-1.07, colonoscopy within the last year a OR = 0.91; 95%CI: 0.81-1.02. CONCLUSION We found that the limited financial resources within the past 12 mo were significantly associated with colorectal cancer(CRC) non-screening. Patients with risk factors identified in this study should adhere to CRC guidelines and should receive financial help if needed.

心源性休克的常见原因包括急性心肌梗死、心肌病、心包疾病、心肌炎、开胸手术,其主要表现为心输出量下降、平均动脉压下降、终末靶器官灌注不足,休克进入死亡瀑布。尽管介入、溶栓技术取得很大进步,但ST段抬高型心肌梗死合并心源性休克患者住院病死率仍高达50%以上。Impella是心脏血流动力学辅助装置轴流泵,可稳定心源性休克患者血流动力学,减轻左心室负荷,增加器官的有效灌注,促进患者康复。本文就目前治疗心源性休克面临的挑战及Impella心脏轴流泵在心源性休克救治中作用的研究进展作一综述。

Radical excision of Barrett's esophagus and complete recovery of normal squamous epithelium

To treat Barrett's esophagus(BE),radiofrequency ablation or cryotherapy are effective treatments for eradicating BE with dysplasia and intestinal metaplasia,and reduce the rates of Barrett's esophageal adenocarcinoma(BAC).However,patients with BE and dysplasia or early cancer who achieved complete eradication of intestinal metaplasia,BE recurred in 5% within a year,requiring expensive endoscopic *** performed endoscopic submucosal dissection as complete radically curable treatment procedure for BE with dysplasia,intestinal metaplasia and BAC.
