Spontaneous Spinal Epidural Hematoma Causing Paraplegia: A Case Report

A spontaneous spinal epidural hematoma without any identified etiology is a very rare entity. We report here a 44 years old woman admitted within our department for 04 days of severe back pain complicated rapidly of bilateral lower-limbs weakness and urinary retention. Neurological examination found a complete paraplegia (0/5 stergh), with incomplete sensory deficit below the T-10 level, bowel and bladder dysfunction, decreased deep and superficial reflex. MRI with different sequences showed a large epidural mass lesion with slightly high signal intensity on T1-weighted images and heterogeneous low signal intensity on T2-weighted images from T11 to L1 suggesting an epidural hematoma. She underwent an urgent total laminectomy from T11 to L1 following by a complete removal of a large dark and compact epidural hematoma compressing the spinal cord. Operatively, there’s no sign suggesting an AVM. Postoperative course was uneventful and she was discharged ten days after surgery with complete recovery.

The aim of the present study was to estimate the production cost of dates of cv. Majhoul (Phoenix dactylifera L.) as part of the Moroccan strategy to develop the date palm sector, and to compare it to that reported in the main exporting regions. Data were collected through surveys of farmers and farm owners carried out in the “Meski Boudnib” extension zone during 2018 and 2019, as well as from studies performed by some governmental organizations. The surveys were conducted on 30 modern and productive date palm farms, managed in monoculture and equipped with localized irrigation systems. All fixed and variable costs were calculated. It was found, through our calculations, that the subsidies from the Moroccan government reduce the average investment cost of a modern date palm farm by 56%. It was also shown that the production cost of one ton of date fruits of cv. Majhoul in the extension zone “Meski Boudnib” (Morocco) is estimated at about 6060 MAD (US$ 634.5), out of which 28% represents the labor cost and 23% represents mineral and organic fertilization costs. The findings of the present study revealed that the production cost of one ton of cv. Majhoul dates in “Meski Boudnib” is lower than those reported in the Coachella Valley (20,189 MAD/ton;US$ 2114) and the Jordan Valley (10,536 MAD/ton;US$ 1103).

The late Neoproterozoïc Toubkal inlier (Ancient Massif of the High-Atlas, Morocco) contains two igneous basaltic series (Tircht and Sidi Chamharouch). Investigated rocks display characteristics of within-plate continental tholeiitic and are similar to rocks originated in orogenic contexts. The geochemical results allow the assumption that subduction active processes are indirectly responsible for the genesis of theses rocks. The orogenic signature is linked probably to a Pan-African magmatic source previously metasomatized in the northern margin of the West African Craton.

Dermoid Cysts of the Conus Medullaris Associated with Sacral Meningocele. A Case Report

Spinal dermoid cysts are gradually progressive benign tumors that may be congenital or acquired. They account for 0.8% to 1.1% of all primary spinal tumors. MRI is the gold standard for the radiological assessment. Management is difficult for tumors involving highly eloquent areas such as the conus medullaris. In this report, we present a rare case of an adult woman with dermoid cysts in the conus medullaris associated with a sacral meningocele which was treated with a midline myelotomy that drained yellowish keratinous fluid and decompressed the cyst. No aggressive attempt at complete resection of the cyst wall was undertaken. The patient fully recovered, and her condition considerably improved after surgery and remains good at a 6-month follow-up.

Cervical Spondylodiscitis Revealed by Tracheostomy Infected: A Rare Case Report Management and Outcome

Background: Spondylodiscitis is a spinal infection rare affecting primarily the intervertebral disk and the adjacent vertebral bodies. His diagnosis is difficult and often delayed or missed due to the rarity of the disease. Cervical spondylodiscitis has quite rare findings regarding the common location of spinal abscesses in the lumbar and thoracic regions. To obtain the best patient outcomes, these spinal infections require prompt diagnosis and appropriate treatment. Case description: A 44-year-old boy was admitted to the neurosurgery department of our hospital for heaviness in 4 limbs without gait disorders in context infected tracheostomy after staying in intensive care. MRI spine showed a spine deformity with lyse C4C5 epiduritis and spinal cord compressed. Antibiotics intravenous were started for 1 month the patient benefited from a corpectomy with an iliac graft and anterior cervical plate. The anatomopathological examination revealed an inflammation not specific. He was therefore put on antibiotics for 6 weeks. Three months later the neck pain and limb pain resolved after treatment and a complete return of lower extremity strength. Conclusion: Cervical spondylodiscitis has increased and become more aggressive. While radical surgical debridement, stable reconstruction together with antibiotic therapy remained a reliable approach to achieve complete healing of the inflammation, anterior alone surgery became more applicable.

The sustainability of groundwater resources for drinking water supplies, agriculture, and industry a prime concern in countries dominated by arid and semi-arid climates such as Morocco. The growing demand for groundwater coupled with impacts from land use and climate change make sustainability an even more important water management goal. In order to make sound decisions about water use and protection of water quality, managers and policy makers must have a sound understanding of such factors as the location and amount of groundwater recharge and groundwater ages. Due to the population growth and climate change (causing long periods of drought) in the world, many countries have intensively increased their use of water sources for supplying potable water to population and for their agricultural (irrigation) and industrial developments. Due to the lack of surface waters, people exploit mainly underground water reservoirs. So, it is necessary to study and characterize these water reservoirs to avoid any excess of exploitation. The water resources of the Essaouira basin are characteristic of a semi-arid climate, and are severely impacted by the climate (quantity and quality). Considering the importance of the Essaouira aquifer in the groundwater supply of the region, a study was conducted in order to comprehend this aquifer groundwater evolution. It is an aquifer located on the Atlantic coastline, southern (Morocco), corresponding to a sedimentary basin with an area of near200 km2. Covering the Palaeozoic bedrock, the sedimentary series range from the Triassic to the Quaternary. The geological structures delineate a syncline bordered by the Tidzi diapir of Triassic age which outcrops to the East and South. In the Essaouira basin a multi-aquifer was identified constituted by detrital deposits of the Plioquaternary and dolomitic limestones of the Turonian. Adrenal myelolipoma is a benign, non-secreting tumor composed of adipose and myeloid tissue that can be complicated by retroperitoneal pain or hemorrhage. Its diagnosis is carried by the computed tomography (CT) and confirmed histologically. Surgery may be necessary in case of large volume, symptomatic mass or complication. We report the case of a 36-year-old patient who has a combination of myelolipoma and hereditary spherocytosis, which is extremely rare in the world literature. Computed tomography guided the diagnosis and surgical excision was performed because of the volume of the mass and its symptomatic nature. The evolution at two years was without recurrences.

The posterior meningeal artery (PMA) usually originates from the third segment of the vertebral artery. Many variations in its origin and course have been observed;however, as far as we know, the association of true aneurysm of the PMA and its anomalous origin from the internal carotid artery has not been reported previously. We reported the case of a 59-year-old woman who suddenly presented a loss of consciousness without head trauma, computed tomography (CT) revelated intracerebellous hematoma associated with a subarachnoid hemorrhage of the posterior cerebral fossa. Cerebral angiography demonstrated a true aneurysm of the PMA which originated from the internal carotid artery.

Mycoplasma synoviae （MS） is an economically important pathogenic agent of chickens, causing air sacculitis and synovitis. The diagnosis and monitoring of M. synoviae infection is usually made using serological assays, while confirmative diagnosis is made by isolation and identifcation of the organism, because of the cross reaction between M. gallisepticum and M. synoviae. This study was conducted from 2011 to 2013 during which l l broiler breeder flocks were sampled. These farms were located in all regions of morocco, The sampling was conducted as follows： Farms were visited on day one old chicks ＂day of importation from Europe＂. 20 to 60 ＂chicks＂ were randomly sampled. At the age of 8, 16, 32 and 56 weeks, 60 blood samples and 60 tracheal swabs were collected at each sampling. The serological screening was performed using Rapid Slide Agglutination （RSA） according the OIE protocol and Indirect el1SA （IDEXX） according the manufacturer＇s instructions. The molecular diagnosis was performed using a commercial kit of a duplex real time PCR （Life Biotechnology）. The results revealed that one day old chicks were negative to MS by RSA and PCR, however they have a variable stock of maternal antibodies （Ig-Y） detected by ielISA. The seroprevalence found by RSA is variable and increase with the age （Sth week： 55%, 16th week： 91%, 32tb and 56th week： 100%）, the same profile was traced by PCR （Sth week： 36%, 16th week： 64%, 32th week： 82%, 56th week： 100%）, however, all farms were positive by ielISA, from the first day to 56th weeks. These results show that MS infections are very frequent and very widespread among poultry breeder flocks, and showed a perfect agreement between serological tests and Real time PCR starting from 32th week of age.
