BACKGROUND: Gastric bypass is a widespread bariatric procedure that carries a high incidence of gallstone formation postoperatively. Controversy exists regarding the importance and consequences of gallstones in these patients. There are surgeons who consider gallstone-related complications after gastric bypass important enough to require routine removal of the gallbladder during gastric bypass (prophylactic cholecystectomy). However, this can lead to increased costs and risks. This study aimed to identify complications related to cholelithiasis after Roux-en-Y gastric bypass (RYGBP). METHODS: This is a prospective observational study of 40 morbidly obese patients free of gallbladder disease. The patients underwent open RYGBP at a public hospital in Brazil from February to October 2007. They were followed up clinically and ultrasonographically at 6 months and 1, 2, and 3 years after surgery. Of the patients, 38 patients were followed up for 3 years. RESULTS: Eleven patients (28.9%) developed cholelithiasis, four (10.5%) experienced biliary pain, and 2 suffered from acute biliary pancreatitis (5.3%). These patients had their gallbladders removed laparoscopically. No patient presented with acute cholecystitis, choledocholithiasis, or bile duct dilation during the follow-up period. There were no deaths. CONCLUSIONS: Gallstone-related complications after RYGBP were relatively common. Some of these complications, like acute pancreatitis, are known to have potentially severe outcomes. It seems reasonable to perform cholecystectomy during gastric bypass in the presence of cholelithiasis or after this procedure if gallstones develop.

Evaluation of hypoglycemic and hypolipidemic activities of aqueous extract of Cistus ladaniferus in streptozotocin-induced diabetic rats

Objective:To evaluate the effect of aqueous leaf extract of Cistus ladaniferus(***) on glycemic and lipidemic status in diabetic ***:Overnight fasted rats were injected intraperitoneally with streptozotocin(45 mg/kg) to induce experimental *** aqueous extract of the leaves of *** was administered orally at the dose of 500 mg/kg body weight to diabetic rats for a period of 28 *** effect,body weight,oral glucose tolerance,change in lipid parameters,urea,creatinine,aspartate aminotransferase and alanine aminotransferase levels of diabetic rats treated with aqueous extract were evaluated in experimental ***:Administration of 500 mg/kg of *** extract to diabetic rats for 28 days resulted in a significant reduction in the levels of blood glucose,alanine aminotransferase,aspartate aminotransferase,urea and ***,the extract of *** improved glucose tolerance in diabetic rats,and its antidiabetic effect was similar to the one obtained with *** hypolipidemic effect was demonstrated by an important decrease in plasma total cholesterol,triglycerides and low density lipoprotein-cholesterol ***:It is concluded that *** leaf extract showed an antidiabetic activity in experimental diabetes which was similar to the one obtained with glibenclamide.

This article implements maximum power point tracking(MPPT)based on the improved hill-climbing algorithm for photovoltaic(PV)systems feeding resistive loads.A direct current-to-direct current boost converter is inserted between the PV system and the load to achieve *** converter is managed using MPPT based on the hill-climbing *** objective of this paper is to optimize the code program to achieve the best compromise between accuracy and rapidity by implementing this algorithm using a *** PV systems are tested under identical meteorological *** the first,an improved hill-climbing MPPT controller is used whereas,in the second,the conventional version is *** experimental results obtained show a signifi-cant enhancement in terms of speed for the improved algorithm with a value of 0.4 s for the response time and 3%for the oscillation power;those values remain satisfactory in terms of precision of the algorithm compared with the conventional system studied and the compared algorithm from the literature.

One-stage revision in two cases of Salmonella prosthetic hip infection

We describe two cases of prosthetic joint infection(PJI) of the hip due to Salmonella. The first patient presented with an early infection 5 d after being discharged following a total hip replacement and the second patient presented at the emergency ward with a late infection, thirteen years following a total hip replacement. Both cases occurred within one month of each other at our institution and both were successfully treated with a one-stage revision. PJI caused by Salmonella species is very rare: so far only 20 Salmonella PJIs of the hip have been described. Therefore, full consensus on the best treatment approach has not yet been reached. An aggressive two-stage approach is advised because of the virulence of Salmonella, although a limited number of successful one-stage approaches have been describedas well. According to the latest guidelines, one-stage revision has comparable success rates and less morbidity compared to two-stage treatment, when selecting the right patients. In our opinion, PJI caused by Salmonella should be treated just as PJI caused by other bacteria, with consideration of the selection criteria as mentioned in several treatment guidelines. As illustrated by these two cases, one-stage revision can be successful in both early and late Salmonella PJI of the hip.

Performance Evaluation of an Evaporative Cooling Pad for Humidification-Dehumidification Desalination

The perfect combination of renewable energy and desalination technologies is the key to meeting water demands in a cost-effective,efficient and environmentally friendly *** desalination technique by humidificationdehumidification is non-conventional approach suitable for areas with low infrastructure(such as rural and decentralized regions)since it does not require permanent *** this study,this technology is implemented by using solar energy as a source of thermal power.A seawater desalination unit is considered,which consists of a chamber with two evaporators(humidifiers),a wetted porous material made of a corrugated cellulose cardboard and a condenser(dehumidifier).The evaporation system is tested with dry bulb temperature and relative air humidity *** results of numerical simulations indicate that higher inlet air velocities(from 0.75 to 3 m/s)lead to a decrease in theΔT,ΔRH,and *** the air remaining within the evaporator for 30 cm,the temperature differential increases to 5.7°C,accompanied by a 39%rise in relative humidity *** changes result in a significant enhancement in humidification efficiency,achieving a remarkable efficiency level of 78%.However,a wettability value of 630 m^(2)/m^(3)leads to a smaller reduction of these *** the pad thickness,particularly to 0.3 m,improves performance by boostingΔT,ΔRH,and effectiveness,especially for pads with a wettability of 630 m^(2)/m^(3),for which superior performances are predicted by the numerical tests.
