AIm:To provide a comprehensive and more representative national data on the disease,especially on treatment options and outcomes,and to determine access of retinoblastoma patients from Luzon,Visayas and mindanao to eye care,and determine if access is associated with delay in consultation,staging and ***:Cohort study of retinoblastoma patients seen in eleven institutions located in the three major areas of the Philippines namely Luzon,Vizayas and mindanao from ***:Totally 636 patients,involving 821 eyes,were ***(57%)were from Luzon and were seen in institutions in Luzon(72%).Annually,58±10 new cases were seen with 71%having unilateral *** delay of consultation remained long at 9(3,17)mo,longest in patients with unilateral disease(Pm the Visayas(Pma Staging System,only 35%of patients had Stage 1 while 47%already had extraocular *** was the most common treatment received by 484 patients while intravenous chemotherapy was received by *** were 250(39%)patients alive,195(31%)dead,85(13%)abandoned,17(3%)refused and 89(14%)with no ***:This study presents the largest cohort of retinoblastoma patients in the Philippines in terms of patients’and participating institutions’number and geographical location and type of institution(private and public).It also presents more comprehensive data on the treatments used and outcomes(survival,globe salvage,and vision retention rates).Delay in consultation was still long among patients leading to advanced disease stage and lower survival *** increasing capacity to diagnose and manage retinoblastoma in the country,the delay of consultation remains long primarily due to accessibility issues to eye care institutions especially in the Visayas and financial *** delay was still significant that overall survival rate remain low.

Prediction of delayed graft function using different scoring algorithms: A single-center experience

AIm To compare the performance of 3 published delayed graftfunction(DGF) calculators that compute the theoretical risk of DGF for each *** This single-center,retrospective study included 247 consecutive kidney transplants from a deceased *** kidney transplantations were performed at our institution between January 2003 and December *** compared the occurrence of observed DGF in our cohort with the predicted DGF according to three different published calculators. The accuracy of the calculators was evaluated by means of the c-index(receiver operating characteristic curve).RESULTS DGF occurred in 15.3% of the transplants under *** c index of the Irish calculator provided an area under the curve(AUC) of 0.69 indicating an acceptable level of prediction,in contrast to the poor performance of the Jeldres nomogram(AUC = 0.54) and the Chapal nomogram(AUC = 0.51). With the Irish algorithm the predicted DGF risk and the observed DGF probabilities were close. The mean calculated DGF risk was significantly different between DGF-positive and DGF-negative subjects(P m 10% to 51% for recipients with DGF and from 4% to 56% for those without *** sensitivity,specificity and positive predictive value of a calculated DGF risk ≥ 30% with the Irish nomogram were 32%,91% and 38%. CONCLUSION Predictive models for DGF after kidney transplantation are performant in the population in which they were derived,but less so in external validations.

Decreased Expression of TRPm3 and mAChRm3 in the Small Intestine in Chronic Fatigue Syndrome/myalgic Encephalomyelitis

Introduction: Chronic Fatigue Syndrome/myalgic Encephalomyelitis (CFS/mE) is often associated with gastrointestinal disturbance and inflammatory markers;however, there have been no histological studies performed in the small intestine from CFS/mE patients. The aim of this investigation was to assess the expression of certain inflammatory markers and inflammatory receptors, namely transient receptor potential melastin 3 (TRPm3) ion channels and muscarinic acetylcholine m3 (mAChRm3) receptors, in small intestinal tissues in a case controlled study comprising a CFS/mE patient and a healthy non-fatigued control. method: Immunohistochemistry was performed on a small intestinal biopsy from a CFS/mE patient (age = 50;female) with self-reported symptoms of gastrointestinal disturbance and a non-fatigued control (NFC), (age = 28;female). Semi-quantitative analysis of expression was undertaken for interferon-gamma (IFNy), interleukin-1 alpha (IL-1α), tumour necrosis factor-alpha (TNFα), TRPm3 ion channels and mAChRm3 acetylcholine receptors. Results: There was significantly decreased expression of TRPm3 in the CFS/mE patient (35% ±9%) and a significant decrease in mAChRm3 in the CFS/mE patient (54% ±9%). There was no difference in IL-1α between CFS/mE patient and NFC, however;there was an increase in IFNy (13% ±6%) in the CFS/mE patient compared to NFC. There was a difference observed in TNFα in CFS/mE compared to NFC. Conclusion: Differences were noted in the expression of specific TRP ion channels and cholinergic receptors in CFS/mE compared with NFC, with CFS/mE demonstrating decreased TRPm3 and mAChRm3. Further, IFNy was increased, and TNFα decreased, in the small intestine of the CFS/mE patient with reported gastrointestinal disturbance.

Determination of the Taxonomic Diversity of the Intertidal Fish Communities on the Pacific Coast of Baja California Sur, méxico from 2015-2019

The coastal areas of the Baja California peninsula are characterized mainly by rocky areas, where rock pools dominate as important coastal habitats because of the tide cycle, when the water is trapped during the low tide, forming the pools. Environmental parameters like temperature, salinity and dissolved oxygen of the water trapped in the pools are key factors for the flora and fauna that inhabit these areas, particularly during the warmer months when the mean values of these parameters are usually higher than the mean values of the sea surface. The aim of the present study was to determine the variation of the taxonomic diversity of the fish communities that inhabit the rock pools in the common land known as Conquista Agraria in the Pacific coast of Baja California Sur, méxico, from 2015 to 2019. A total of 59 visual censuses were carried out monthly during the second low tide of the full moon. Temperature, salinity, and dissolved oxygen showed variation throughout the study period, with two distinct seasons (warm and cold), which influenced the taxonomic diversity and taxonomic distinctness of the species recorded in the area. There was a higher diversity in the warmer months (summer) and years (2015 and 2016). In addition, according to the mDS analysis, 2017 and 2018 showed high similarity of species.

Background: The availability of an anesthesiologist is often a limiting factor in the number of operations that can be performed by International medical Surgical Response Teams (ImSuRT). Because emergency physicians (EPs) possess skills in airway control, management of moderate and deep sedation, and ventilator management, we propose that with proper training in general anesthesia, EPs can serve as anesthetists for ImSuRT with anesthesiologist supervision. methods: During a 10-week period, a board-certified EP administered general anesthesia to 60 patients prior to a surgical medical mission trip. The breakdown of surgical cases was: 11 orthopedic, 2 genitourinary, 20 ear, nose, and throat, 8 obstetrics and gynecological, 13 general surgery, and 6 vascular. A simplified protocol for induction, maintenance, and emergence was adhered to for all cases. Results: Fourteen orthopedic cases using general anesthesia were performed in a one-week period in Haiti. These cases involved open reduction and internal fixation (ORIF), hemiarthoplasty, hardware removal, tendon transfer and external fixation of fractured bone. Conclusion: We demonstrate the feasibility of a model curriculum to train EPs in the basics of anesthesia. The EP can safely and effectively deliver general anesthesia for major cases on surgical medical mission trips under the auspices of an anesthesiologist in an austere environment.

Variation of Density and Biomass of the Ichthyofauna Associated to Soft Bottoms of a Western Coastal Lagoon of B.C.S., mexico

An analysis of the variation of the density and biomass of the fish communities of the lagoon of La Paz, mexico was carried out from November 2016 to September 2017. A total of 2763 organisms were collected with a total biomass of 211,422.93 g in an area of 5022 m2, a density and biomass of 0.550 lnd/m2 and 44.091 g/m2. The site with the largest biomass was recorded in Zacatecas (11.179 g/m2) located north of the lagoon, while Grand Plaza recorded the lowest biomass (2.732 g/m2). Regarding the density, the maximum value was recorded in the site Yate Hundido, located northwest of the lagoon (0.129 lnd/m2) and the lowest was Las Palmas (0.036 lnd/m2) located north of the lagoon. The most abundant species was Diapterus peruvianus with 683 individuals and a biomass of 37,507.57 g. The physicochemical variables showed two seasons with significant change where we can observe higher temperatures in the summer (average of 27.45°C) and minimum in winter (average of 22.49°C). The Shannon-Weaver diversity index showed a difference (p = 0.050) between months, with the highest value recorded in April (H' = 2.133 bits/ind) and the lowest in June (H'mal;">mily:;" "=""> = 1.041 bits/ind). The biomass and density values recorded in the lagoon were found to be associated with the spatio-temporal variation of temperature of the lagoon, as well as the resident and transitory species located in the area of study. Emphysematous cholecystitis is a rare subtype of acute cholecystitis characterized by the presence of gas in the gallbladder wall secondary to ischemia. Typically, this is a result of cystic artery vascular compromise with a concomitant infection from gas-forming organisms such as Clostridium species, Klebsiella species, or Escherichia coli. The mortality rate of acute emphysematous cholecystitis is 15% - 20% compared with 1.4% in uncomplicated acute cholecystitis. The subsequent development of a cholecystocutaneous fistula, an abnormal connection between the gallbladder and the skin, is also a rare complication of gallbladder disease. We describe a case of a 77-year-old male who presented with right flank necrotizing fasciitis which developed from a cholecystocutaneous fistula secondary to emphysematous cholecystitis. Once the necrotic tissues were adequately debrided, the large open wound was treated with negative pressure wound therapy with instillation (NPWT-i) utilizing hypochlorous acid (HOCL). The wound was closed with a split-thickness skin graft.

Cultivated potato is a clonally propagated autotetraploid species with a highly heterogeneous *** assemblies of six cultivars including two chromosome-scale phased genome assemblies revealed extensive allelic diversity,including altered coding and transcript sequences,preferential allele expression,and structural variation that collectively result in a highly complex transcriptome and predicted proteome,which are distributed across the homologous *** species contribute to the extensive allelic diversity in tetraploid cultivars,demonstrating ancestral introgressions predating modern breeding *** a clonally propagated autotetraploid that undergoes limited meiosis,dysfunctional and deleterious alleles are not purged in tetraploid *** a quarter of the loci bore mutations are predicted to have a high negative impact on protein function,complicating breeder’s efforts to reduce genetic *** StCDF1 locus controls maturity,and analysis of six tetraploid genomes revealed that 12 allelic variants of StCDF1 are correlated with maturity in a dosage-dependent *** of the complexity of the tetraploid potato genome with its rampant structural variation and embedded deleterious and dysfunctional alleles will be key not only to implementing precision breeding of tetraploid cultivars but also to the construction of homozygous,diploid potato germplasm containing favorable alleles to capitalize on heterosis in F1 hybrids.
