BACkGrOUNd Acute epiploic appendagitis of the appendix (AEAA) is a rare self-limiting inflammatory disorder of the epiploic appendages (EA) close to the vermiform appendix, which often times mimicking the presentation of acute appendicitis (AA). To date, very few cases of AEAA have been reported. We report a case of a 52-year old man with the clinical suspicion of AA, but post-operative specimen examination confirmed AEAA as the final diagnosis. CASE SUMMArY A 52-year-old morbidly obese man presented to the emergency department with a 1-d history of the right lower quadrant (rLQ) abdominal pain. Physical examination revealed localized rLQ tenderness mimicking AA. The computed tomography abdomen was inconclusive, and a decision was made to perform laparoscopic appendectomy (LA). during the LA, an infarcted epiploic appendage at the tip of appendix and adherent to the abdominal wall was found, which was entirely excised. Final pathology showed congested and hemorrhagic epiploic appendage without any accompanied acute inflammatory changes in the wall of the appendix. Postoperative course was uneventful and he was doing well at seven months follow-up. CONCLUSION The possibility of AEAA should be considered in patients clinically suspected of having AA. Surgery is considered for those refractory to conservative management, with inconclusive diagnosis or develop complications at presentation.

Hydrometeorological assessments and suspended sediment delivery from a central Himalayan glacier in the upper Ganga basin

Integrated hydrometeorological investigations are not frequently available at a regional scale over a longer time period, especially near the terminus of Indian Himalayan glaciers. An integrated approach to the collection of hydrological data has major advantages for understanding the runoff generation mechanisms at basin scale, particularly when coupled with meteorological observations. The current study involves time series analysis of hydrometeorological records collected near the terminus of the Chorabari Glacier, for four consecutive ablation seasons(June-Sept.) 2009-2012. The analysis shows that variation in rainfall was higher(c= 0.9) at the same elevation over proximal sites, while the intensity of extreme rainfall events was 121-160 mm/d. The diurnal temperature range(dTr) has a tendency to reduce over the ablation season because of the onset of the Indian Summer Monsoon(ISM) and then further increases during the ISM withdrawal indicating humid-temperate conditions. The peak discharge(Qpeak) was found to be higher during July and August. Snow and glacier melt contributed 76% of the total suspended sediment transport during peak ISM months(July and August) reflecting seasonal evolution of the hydrologic conduits. The results indicate that karakoram and western Himalayan glaciers produce comparatively low sediment yield compared to central Himalayan glaciers. The hydrological variations are depicted through flow duration curves(FdC) for meltwater discharge and sediment load. The flow corresponding to Q, Q, and Q(where Qx is the discharge that is exceeded x percent of the time referred to as % dependability) are 4.2, 3.7, and 2.8 m~3/s; and the corresponding dependability for suspended sediment loads(SSLs) are 409.0, 266.0, and 157.2 t/d, respectively. Background:To develop and validate a deep learning-based approach to the fully-automated analysis of macaque corneal sub-basal nerves using in vivo confocal microscopy(IVCM).Methods:IVCM was used to collect 108 images from 35 macaques.58 of the images from 22 macaques were used to evaluate different deep convolutional neural network(CNN)architectures for the automatic analysis of sub-basal nerves relative to manual *** remaining images were used to independently assess correlations and interobserver performance relative to three ***:Correlation scores using the coefficient of determination between readers and the best CNN averaged *** inter-observer comparison,inter-correlation coefficients(ICCs)between the three expert readers and the automated approach were 0.75,0.85 and *** ICC between all four observers was 0.84,the same as the average between the CNN and individual ***:deep learning-based segmentation of sub-basal nerves in IVCM images shows high to very high correlation to manual segmentations in macaque data and is indistinguishable across *** quantitative measurements of corneal sub-basal nerves are important biomarkers for disease screening and management,the reported work offers utility to a variety of research and clinical studies using IVCM.

kantowski-Sachs plane symmetric models are investigated in bimetric theory of gravitation proposed by rosen [1] 08d0C9EA79F9BACE118C8200AA004BA90B02000000080000000E0000005F005200650066003400310035003100330038003900340034000000 in the context of bulk viscous fluid. Taking conservation law and the equation of state, two different models of the universe are obtained. It is observed that kantowski-Sachs vacuum model obtained in first case and bulk viscous fluid model obtained in second case. It is also observed that the bulk viscous cosmological model always represents an accelerated universe and consistent with the recent observations of type-1a supernovae. Some physical and geometrical features of the viscous fluid model are studied.

Folic acid supplementation inhibits recurrence of colorectal adenomas:A randomized chemoprevention trial

AIM: To determine whether folic acid supplementation will reduce the recurrence of colorectal adenomas, the precursors of colorectal cancer, we performed a double-blind placebo-controlled trial in patients with adenomatous polyps. METHOdS: In the current double-blind, placebo-controlled trial at this VA Medical Center, patients with colorectal adenomas were randomly assigned to receive either a daily 5 mg dose of folic acid or a matched identical placebo for 3 years. All polyps were removed at baseline colonoscopy and each patient had a follow up colonoscopy at 3 years. The primary endpoint was a reduction in the number of recurrent adenomas at 3 years. rESULTS: Of 137 subjects, who were eligible after confirmation of polyp histology and run-in period to conform compliance, 94 completed the study; 49 in folic acid group and 45 in placebo group. recurrence of adenomas at 3-year was compared between the two groups. The mean number of recurrent polyps at 3-year was 0.36 (Sd, 0.69) for folic acid treated patients compared to 0.82 (Sd, 1.17) for placebo treated subjects, resulting in a 3-fold increase in polyp recurrence in the placebo group. Patients below 70 years of age and those with left-sided colonicadenomas or advanced adenomas responded better to folic acid supplementation. CONCLUSION: High dose folic acid supplementation is associated with a signif icant reduction in the recurrence of colonic adenomas suggesting that folic acid may be an effective chemopreventive agent for colorectal neoplasia.

Currently,the industry is experiencing an exponential increase in dealing with binary-based combinatorial *** this sense,metaheuristics have been a common trend in the field in order to design approaches to solve them ***,a well-known strategy consists in the use of algorithms based on discrete swarms transformed to perform in binary *** the No Free Lunch theorem,we are interested in testing the performance of the Fruit Fly Algorithm,this is a bio-inspired metaheuristic for deducing global optimization in continuous spaces,based on the foraging behavior of the fruit fly,which usually has much better sensory perception of smell and vision than any other *** the other hand,the Set Coverage Problem is a well-known NP-hard problem with many practical applications,including production line balancing,utility installation,and crew scheduling in railroad and mass transit *** this paper,we propose different binarization methods for the Fruit Fly Algorithm,using Sshaped and V-shaped transfer functions and various discretization methods to make the algorithm work in a binary search *** are motivated with this approach,because in this way we can deliver to future researchers interested in this area,a way to be able to work with continuous metaheuristics in binary *** new approach was tested on benchmark instances of the Set Coverage Problem and the computational results show that the proposed algorithm is robust enough to produce good results with low computational cost.

Introduction: Low bodyweight type 2 dM is a distinct clinical entity having many inherent peculiarities seen in India and developing countries, constituting 11% to 25% of type 2 diabetic subjects. Our study aimed at assessing the prevalence of inflammatory markers like hsCrP, adiponectin and NF-κB expression in peripheral blood mononuclear cells in subjects with type 2 dM in relation to BMI. Materials and Methods: 57 consecutive type 2 diabetics were recruited for study, classified as Low Bodyweight (A = BMI dard weight (B = BMI 18.5 - 24.99) and Obese (C = BMI ≥ 25). Group d comprised 14 healthy controls. They were evaluated for clinical parameters, FBG, 2hrPPBG, HbA1c, lipid profile and above mentioned inflammatory markers. results: Serum hsCrP was significantly higher in all group of diabetics as compared to Group d but was lowest in Group A. Adiponectin levels were highest in Group d, similar in Groups B and C but lowest in Group A. NF-κB expression, though higher in diabetic subjects than controls (Od = 0.041 ± 0.006), was least in Group A (Od = 0.045 ± 0.005). discussion and Conclusion: Our study revealed that Indians with type 2dM are in a pro-inflamematory state. Low bodyweight type 2 diabetics had the least pro-inflammatory load. This further supported the earlier observation of lesser macrovascular disease load and testifying that Low Bodyweight type2dM constitutes a distinct entity.
