2004年12月26日苏门答腊特大地震是过去50年间在俯冲带发生的第三大地震。其破裂开始于锡默卢岛西北部30～40km深处(Lay et al,2005),传播1300km至安达曼群岛北部(Ammon et al,2005)。这次大地震是由俯冲的印度-澳大利亚板块与上部的巽他板块之间界面的突然滑动引起的(rhie et al,2007;Vigny et al,2005)。尽管俯冲界面结构的详细认识对于确定大型逆冲地震潜在源非常重要,但是,目前为止可用的数据(Henstock et al,2006;Sibuet et al,2007;Dewey et al,2007;Engdahlet al,2007)还无法提供这样的信息。本文展示了震源区一条高质量的地震剖面,其展布从深海平原向下延伸到弧前下面40km。地震数据显示,俯冲地壳和大洋莫霍面(地壳-地幔边界)被陆向倾斜的逆冲断层损坏并移位,意味着此大型逆冲断层目前位于大洋地幔内。我们对增生楔前缘的活动逆冲断层成像,其特点与陡峭倾斜面上的逆冲型余震一致。我们的观测结果表明,是壳幔间的强耦合导致地幔岩石脆性破裂,从而引发此次异常大的地震。

Left ventricular (LV) pseudoaneurysm is a late mechanical complication of myocardial infarction (MI). A giant LV pseudoaneurysm is a rare presentation. We report a case of a giant LV pseudoaneurysm in a post MI patient, who presented with hemoptysis. Hemoptysis is a rare clinical presentation of LV pseudoaneurysm. The patient had successful surgical repair of the aneurysm and had a favorable outcome in 9 mo’ follow-up. The imaging modalities and surgical treatment of a pseudoaneurysm is discussed.

大花菟丝子是一种金黄色、无叶、多年生寄生草本植物,它隶属于旋花科,俗称发菜、鬼肠等。经研究,大花菟丝子具有止痉、抗惊厥、抗类固醇生成、降血压、助肌肉松弛、强心、利尿、抗病毒、抗菌、抗氧化以及助毛发生长等一系列药用作用。目前从大花菟丝子中提取的有效化学成分有岩白菜素、阿马别林、β-谷甾醇、豆甾醇、山奈酚、半乳糖醇、杨梅酮、槲皮素、香豆素和齐墩果酸等。本文概述了生药学和植物化学文献中有关大花菟丝子的研究结果,并从传统医学和生物学角度评价了它的医药用途。

Schwannoma is a slow growing solitary tumor that preferentially involves spinal nerve roots, and sympathetic, cervical, and vagus nerves. There are several clinico-pathologic variants of schwannoma, including schwannoma with a degenerative change (ancient schwannoma), cellular schwannoma, plexiform schwannoma, epithelioid schwannoma, and melanotic schwannoma. About 10% of cases of schwannomas are associated with multi-system disorders such as neurofibromatosis, schwannomatosis, multiple meningiomas, and Carney complex. Schwannoma rarely present as an intraocular tumor and is often misdiagnosed as malignant melanoma. Immunohistochemical positivity with S-100 stain and demonstration of long-spaced collagen (Luse bodies) are helpful in establishing the diagnosis. In this article, we review the clinical and histopathological findings of a sporadic plexiform pigmented schwannoma involving the iris, ciliary body, and the choroid.

BACkGrOUND A diverse country like India may have variable intensive care units(ICUs)practices at state and city *** To gain insight into clinical services and processes of care in ICUs in India,this would help plan for potential educational and quality improvement *** The Indian ICU needs assessment research group of diverse-skilled individuals was formed.A pan-India survey"Indian National ICU Needs"assessment(ININ 2018-I)was designed on google forms and deployed from July 23rd-August 25th,*** survey was sent to select distribution lists of ICU providers from all 29 states and 7 union territories(UTs).In addition to emails and phone calls,social medial applications-WhatsApp™,Facebook™and LinkedIn™were used to remind and motivate *** completing and submitting the survey,providers gave their consent for research *** study was deemed eligible for category-2 Institutional review Board exempt *** There were total 134 adult/adult-pediatrics ICU responses from 24(83%out of 29)states,and two(28%out of 7)UTs in 61 *** had median(IQr)16(10-25)beds and most,were mixed medical-surgical,111(83%),with 108(81%)being adult-only *** responders were young,median(IQr),38(32-44)years age and majority,n=108(81%)were *** consultants were,n=101(75%).A total of 77(57%)reported to have 24 h in-house intensivist.A total of 68(51%)ICUs reported to have either 2:1 or 2≥:1 patient:nurse *** than 80%of the ICUs were open,and mixed *** followed regularly by the ICUs included sepsis care,ventilator-associated pneumonia(83%each);nutrition(82%),deep vein thrombosis prophylaxis(87%),stress ulcer prophylaxis(88%)and glycemic control(92%).Digital infrastructure was found to be poor,with only 46%of the ICUs reporting high-speed internet *** In this large,national,semi-structured,need-assessment survey,the need for improved manpower including;in-house intensivists,and decreasing

A simple, controlled, robust and scalable three-stage manufacturing process of Afatinib Dimaleate was assessed and optimized leading to improved yield and quality. The synthetic process involves sequence of reactions as nitro-reduction, amidation and salification. The developed and optimized route was demonstrated on 300 g scale with over all isolated yield of 84% for Afatinib free base. The developed process has the capability to control not only the process related impurities but also the degradation impurities. One new impurity was identified during the process development studies and characterized as acetamide impunity, chemically known as (S)-N-(4-((3-chloro-4-fluorophenyl) amino)-7-((tetrahydrofuran-3-yl) oxy) quinazolin-6-yl) acetamide. Other impurities were identified as degradation impurities, Process impurities and were labeled as 1-(4-((3-chloro-4-fluorophenyl) amino)-7-(((S)-tetrahydrofuran-3-yl) oxy) quinazoline-6-yl)-5-Hydroxypyrrolidin- 2-one (hydroxy impurity), Afatinib N-Oxide impurity and N4-(3-chloro-4-fluorophenyl)-7-[[(3S)-tetrahydro- 3-furanyl] oxy]-4,6-quinazolinediamine (Intermediate-1).
