Laparoscopic management of remnant gall bladder with stones: Lessons from a tertiary care centre's experience

Objective:Laparoscopic cholecystectomy is currently the gold standard for treating symptomatic gallstone *** its success,approximately 10%of patients may experience persistent biliary symptoms,leading to the post-cholecystectomy syndrome.A remnant gallbladder with cystic duct or bile duct stones is one of the causes of this *** objective of this study was to shed light on the clinical manifestations,evaluation,therapeutic strategies,and outcomes associated with laparoscopic management of symptomatic remnant ***:This was a retrospective study,conducted over a five-year period(January 2017 to December 2022)at Apollo Hospitals in south *** patients who underwent laparoscopic completion cholecystectomy for a remnant gall bladder were *** following data were collected:patient demographics,symptoms,preoperative investigations,intraoperative details and post operative ***:In total,36 patients were included and *** majority of patients were male(25,69.4%),with a mean age of 50.7±12.1 *** most common presentation was pain in the upper abdomen or right upper quadrant region(24,66.7%).The laparoscopic approach was attempted in all patients,with a success rate of 94.4%.Two patients required conversion to open *** fistula to the colon was observed in one *** was observed in 7 patients(19.4%),and stone clearance was successfully achieved using endoscopic retrograde cholangiopancreatography in all patients ***:Incomplete gall bladder removal either intentionally or unintentionally leaves a remnant gall bladder that is at risk for stone formation and *** who have this clinical entity with symptoms require a redo or complete cholecystectomy,a complex procedure associated with certain *** study highlights the feasibility and safety of laparoscopic completion cholecystectomy for the management of remnant gallbladder

Graphene derivatives as efficient hole transport materials for lead-free double perovskite(CssnI) solar cells: a numerical study

The double perovskite Cs2snI6has notable optical and electrical characteristics， rendering it a highly prospective candidate for deployment as the absorber layer in perovskite solar cells (PsCs). We simulated the performance of PsCs using lead-free Cs2snI6double perovskite absorber layer and graphene derivatives， namely graphene oxide (GO) and reduced graphene oxide (rGO)， as hole transport layers (HTLs). Our findings show that rGO offers an excellent hole extraction property with minimal interfacial recombination compared to GO. PsC utilizing rGO as the HTL material achieved a power conversion efficiency of 19.07%， while those employing GO HTL attained only 16.9%. The superior performance of rGO is attributed to its greater charge carrier mobility of rGO and its favorable energy level alignment with the double perovskite absorber layer. In contrast， the mismatch in the charge transport properties between the absorber layer and the GO HTL resulted in poor performance. Our study showcases the potential of using graphene derivative-based HTLs for realizing lead-free PsCs.

Novel succinoylated carboxymethyl guar gum nanocarriers of glimepiride for controlling type-2 diabetes

In this study,guar gum(GG)was chemically modified to its carboxymethyl derivative,which was then esterified with octenyl succinic anhydride(OsA)using a nucleophilic catalyst 4-dimethylaminopyridine(DMAP)to render the derivative amphiphilic *** carboxymethyl guar gum(CMGG)and succinoylated CMGG was characterized using Fourier transform infrared spectroscopy(FTIr)*** amphiphilic CMGG synthesized using DMAP/OsA ratio of 0.5:1(CMGGOsA-I),was found to be *** amphiphilic guar gum self-assembled in water to form nanocarriers with mean diameter of 430 nm and zeta potential of19.0 *** electron microscope(TEM)image showed spherical nature of the developed CMGGOsA-I nano*** presence of amphiphilic CMGG,the aqueous solubility of glimepiride was enhanced by about *** nanocarriers released glimepiride in simulated gastrointestinal fluids for a period of more than 24 h,following Higuchi's ***-Peppas modeling of the drug release data revealed that a combination of swelling and diffusion mechanism was operative in the event of drug *** streptozotocin-induced diabetic rat model,the nanocarriers outperformed pure drug suspensions in terms of anti-diabetic activity,which lasted up to 24 ***;the newly synthesized amphiphilic CMGG nanocarriers demonstrated controlled drug release properties and showed promise for controlling type-2 diabetes.

Hybrid-Vlasov simulation of soft X-ray emissions at the Earth’s dayside magnetospheric boundaries

solar wind charge exchange produces emissions in the soft X-ray energy range which can enable the study of near-Earth space regions such as the magnetopause,the magnetosheath and the polar cusps by remote sensing *** solar wind Magnetosphere Ionosphere Link Explorer(sMILE)and Lunar Environment heliospheric X-ray Imager(LEXI)missions aim to obtain soft Xray images of near-Earth space thanks to their soft X-ray Imager(sXI)*** earlier modeling works have already simulated soft X-ray images as might be obtained by sMILE sXI during its mission,the numerical models used so far are all based on the magnetohydrodynamics description of the space *** investigate the possible signatures of ion-kinetic-scale processes in soft Xray images,we use for the first time a global hybrid-Vlasov simulation of the geospace from the Vlasiator *** simulation is driven by fast and tenuous solar wind conditions and purely southward interplanetary magnetic *** first produce global X-ray images of the dayside near-Earth space by placing a virtual imaging satellite at two different locations,providing meridional and equatorial *** then analyze regional features present in the images and show that they correspond to signatures in soft X-ray emissions of mirrormode wave structures in the magnetosheath and flux transfer events(FTEs)at the *** results suggest that,although the time scales associated with the motion of those transient phenomena will likely be significantly smaller than the integration time of the sMILE and LEXI imagers,mirror-mode structures and FTEs can cumulatively produce detectable signatures in the soft X-ray *** instance,a local increase by 30%in the proton density at the dayside magnetopause resulting from the transit of multiple FTEs leads to a 12%enhancement in the line-of-sight-and time-integrated soft X-ray emissivity originating from this ***,a proton density increase by 14%in the magnetosheath

FEA-based Geometrical Modification of switched reluctance Motor for radial Force reduction

switched reluctance motors(srMs)are becoming increasingly popular in the automotive sector owing to their robust ***,srMs are preferred particularly for EV applications owing to their fault tolerance,magnet-free structure,and high power/torque *** main concerns of srM compared to other machines include torque ripple and *** primary cause of vibration is the radial force created by the srM.A geometry-based modification of the srM to reduce the radial force without significantly changing the average torque produced is *** primary goal is to design a 4-phase,8/6 srM with a lower radial *** possible geometrical alterations are proposed:one with square windows and the other with circular holes on the rotor *** windows are sized and positioned to avoid flux *** criteria are developed for the optimal window size and *** element analysis(FEA)modelling of the srM is used to validate its *** FEA results are compared with the performance parameters obtained using the analytical *** the multiphysics design tool ANsYs to obtain the natural frequencies and associated deformations through modal *** to the conventional geometry,the radial force is significantly reduced by providing windows.

While we eagerly anticipate sMILE’s(solar wind Magnetosphere Ionosphere Link Explorer)unprecedented X-ray observations of the Earth’s magnetosheath and the initiation of a new era of magnetospheric research,it seems appropriate to look ahead to the abilities of the next generation of astrophysics *** these,the Line Emission Mapper(LEM),a large aperture micro-calorimeter based mission,is currently planned to be able to observe the magnetosheath at high spectral resolution(~2 eV).With a field of view of~30′,LEM will allow higher spatial resolution and higher cadence measurement of the motion of a very small portion of the magnetopause over relatively short periods of time(multiple hours),complementing sMILE’s global ***’s strength is its spectral *** will be able to measure the abundance of a broad range of elements and ionization states,many of which are inaccessible to current in situ instruments,and will be able to separate the emission from the magnetosheath from the emission from the cosmic X-ray background using the difference in their relative velocities.

In recent years,nitric oxide(NO)has emerged as a key redox signaling molecule in plants functioning in the regulation of key developmental programs and the orchestration of responses to a plethora of environmental cues(kolbert et al.,2019).The predominant route for the transfer of NO bioactivity is through s-nitrosylation,the addition of an NO moiety to a protein cysteine thiol to form an s-nitrosothiol(Yun et al.,2011).specificity for this process is established by the structural constraints imposed by tertiary protein structure in gating access to given cysteine redox switches and associated proteins that can either facilitate the addition or removal of the NO moiety at these residues(Umbreen et al.,2018).

In this study,a calcium-phosphate coating was formed on a Mg-Mn-Ce alloy by the plasma electrolytic oxidation(PEO).The antibiotic vancomycin,widely used in the treatment of infections caused by staphylococcus aureus(***),was impregnated into the *** with vancomycin showed high bactericidal activity against *** mechanical and electrochemical properties of the formed coatings were studied,as well as in vitro cytotoxicity tests and in vivo tests on mature male rats were *** to sEM,EDs,XrD and XPs data,coatings had a developed morphology and contained hydroxyapatite,which indicates high *** analysis of roughness of coatings without and with vancomycin did not reveal any differences,confirming the high roughness of the *** electrochemical tests,an increase in corrosion resistance by more than two times after the application of PEO coatings was *** to the results of an in vivo study,after 28 days of the implantation of samples with calcium phosphate PEO coating and vancomycin,no signs of inflammation were observed,while an inflammatory reaction developed in the area of implantation of bare alloy,followed by *** release tests from the coatings show a sharp decrease in the concentration of the released antibiotic on day 7 and then a gradual decrease until day *** the experiment,no significant deviations in the condition and behavior of the animals were observed;clinical tests did not reveal a systemic toxic reaction.
