Co-existence of hepatocellular adenoma and focal nodular hyperplasia in a young female

Focal nodular hyperplasia (FNH) and hepatocellular adenoma (HA) are both benign hepatocellular lesions, presenting mainly in women of childbearing age in non-cirrhotic, non-fibrotic livers. Simultaneous occurrence of these two lesions is extremely rare. We herein report a case of a young female without any predisposing risk factors who presented to our emergency department complaining of acute abdominal pain. Imaging studies revealed a 6 cm lesion in the right hepatic lobe and a 2.5 cm lesion in the left hepatic lobe, respectively. In view of the patient’s symptoms and lack of a confirmed diagnosis based on imaging, we performed a bisegmentectomy V-VI and a wedge resection of the lesion in segment iii by laparotomy. Postoperative course was uneventful and the patient was discharged on the fourth postoperative day. The pathology report demonstrated an HA in segments V-VI and FNH in segment iii, respectively. Six months later, the patient remains asymptomatic with normal liver function tests, ultrasound and magnetic resonance imaging follow-up. To our best knowledge, this is the first case to describe simultaneous occurrence of HA and FNH without the presence of any known risk factors for these entities. The uncertainty in diagnosis and acuteness of presenting symptoms were established criteria for prompt surgical intervention.

Strain engineering as one of the most powerful techniques for tuning optical and electronic properties of iii-nitrides requires reliable methods for strain *** this work,we reveal,that the linear model based on the experimental data limited to within a small range of biaxial strains(r the non-destructive raman study of strain with nanometer-scale spatial resolution is not valid for the binary wurtzite-structure group-iii nitrides GaN and ***,we found that the discrepancy between the experimental values of strain and those calculated via raman spectroscopy increases as the strain in both GaN and AlN ***,a new model has been developed to describe the strain-induced raman frequency shift in GaN and AlN for a wide range of biaxial strains(up to 2.5%).Finally,we proposed a new approach to correlate the raman frequency shift and strain,which is based on the lattice coherency in the epitaxial layers of superlattice structures and can be used for a wide range of materials.

Incidence of Subclinical Hypothyroidism in Cardiac Surgery Patients. Comparison of Presentation Characteristics, Hospital and Medium-Term Outcomes with Euthyroid Patients

Subclinical hypothyroidism (SCHT) is common, with an occurrence of up to 10% of the adult population and defined biochemically only by elevated TSH and normal T4. SCHT affects negatively on lipid and carbohydrate metabolism increasing the risk of ischemic heart disease, affects negatively on cardiac performance and have a close correlation with renal function. The aim of this study is to compare presentation characteristics and outcomes between euthyroid pts and pts with SCHT who underwent cardiac surgery. Methods: 474 pts from June 2003 through September 2004 had TSH and T4 measured. 365 pts were euthyroid (Group 1), 41 pts had SCHT (Group 2). Groups were compared by demographics and EuroSCOrE (ES) risk profiles. Operative and hospital outcomes were compared as was follow-up mortality up to 96 months. results: There were more females in Group 2, p = 0.04, more pts with CHF and number of NYHA iii-IV pts (p re pts in Group 2 had elevated s-crea (p rial fibrillation (p = 0.007). Comparing the Groups by EuroSCOrE (ES) showed higher risk scores in Group 2 pts (Additive ES 6.8 vs 8.5 and Logistic ES 12.3% vs 18.1%, p = 0.01 and 0.03). Hospital mortality was higher in Group 2 (12.2% vs 4.1%, p = 0.04) and the number of pts needing extended care was higher in Group 2 (p = 0.01). Follow up mortality was doubled in Group 2 pts up to 96 months compared to Group 1 (p resentation characteristics and risk scores are different and worse in SCHT pts compared with euthyroid pts. Hospital and follow-up mortality are increased in SCHT pts.

Soil chemical fertility change over four decades in the Morvan Mountains and influence of tree species (Burgundy, France)

Background: Intensive silvicultural practices and the planting of monospecific forests of coniferous, that are more productive compared to hardwoods may threaten over the mid to long-term the sustainability of soil chemical fertility of forest ecosystems, and are a major concern for forest managers and ***: We investigated the tree species effect(Quercus sessiliflora Smith, Fagus sylvatica L., Picea abies Karst.,Pseudotsuga menziesii Mirb. Franco., Abies nordmanniana Spach. and Pinus nigra Arn. ssp. laricio Poiret var. corsicana) on the change over time of soil chemical properties and nutrient pool sizes in the mineral and organic layers of the soil during the 45 years after the plantation of the Breuil-Chenue common garden experiment(Burgundy,France). The organic and mineral soil layers down to 70-cm depth were sampled in the different monospecific plots in 1974, 2001 and ***: Exchangeable Ca and Mg pools and soil pH increased on average over time in the 0–70 cm soil profile in most stands. However, in the topsoil layers(0–15 cm), the decrease of pH, the increase of exchangeable acidity over time under the coniferous stands and the decrease of exchangeable K pools in most stands highlighted that soil acidification is still on-going at Breuil-Chenue site but the intensity of this process depends on the tree ***, three groups of species could be distinguished: i) Nordmann fir(Abies nordmanniana Spach.)/Norway spruce(Picea abies Karst.) where acidolysis and chelation occurred, resulting in the most pronounced pH decrease in the topsoil, ii) Douglas fir(Pseudotsuga menziesii Mirb. I-131 MIBG Therapy for Advanced Stage iii &IV Neuroblastoma

radio-labelled MIBG therapy has been tried in the treatment of advanced stage iii and IV neuroblastoma in an attempt to improve patients outcome. The use of radiolabelled MIBG to treat neuroblastoma has arisen from the high sensitivity and specificity of in vivo MIBG imaging for detection of primary and metastatic tumours. Although, clinical trials have been performed to determine the role of MIBG therapy, the overall therapeutic strategy of MIBG therapy is not precisely defined. Aim of the work: to determine the impact of MIBG therapy on patients’ outcome especially their quality of life. Patients & methods: 42 paediatri neuroblastoma patients are included in this study, 12 (28.5%) stage iii disease and 30 (71.5%) stage IV. I-131 MIBG doses ranged from 100 to 150 mCi with number of courses ranged from 1-8 according to response and toxicity. results: Group I (stage iii): 1/12 patients showed complete response, 5/12 patients had partial response and the other 6 patients remained stable. The 5 years event free survival (EFS) of stage iii patients was 46.6% and the overall survival was 75%. Group II (stage IV): 2/30 patients achieved Cr, 9/30 patients showed Pr to I-131 MIBG and the other 18 remained stable. The 5 years event free survival (EFS) of stage IV patients was 48.2% and the overall survival was 69%. Conclusion: We conclude that 131I-MIBG therapy has favourable therapeutic effects that are translated into an overall improved outcome with good quality of life.

The acquisition of chemical information is a critical need for medical diagnostics, food/environmental monitoring, and national security. Here, we report an electrochemical information processing approach that integrates（i） complex electrical inputs/outputs,（ii） mediators to transduce the electrical I/O into redox signals that can actively probe the chemical environment, and（iii） a redox capacitor based on chitosan and catechol that manipulates signals for information extraction. We demonstrate the capabilities of this chemical information processing strategy using biothiols because of the emerging importance of these molecules in medicine and because their distinct chemical properties allow evaluation of hypothesis-driven information probing. We show that input sequences can be tailored to probe for chemical information both qualitatively（step inputs probe for thiol-specific signatures） and quantitatively（oscillating inputs yield signals that can be correlated to concentrations over 5 orders-of-magnitude）. This approach allows the capabilities of signal processing to be extended for the rapid, robust and on-site analysis of chemical information.
