Spontaneous Spinal Epidural Hematoma Causing Paraplegia: A Case report

A spontaneous spinal epidural hematoma without any identified etiology is a very rare entity. We report here a 44 years old woman admitted within our department for 04 days of severe back pain complicated rapidly of bilateral lower-limbs weakness and urinary retention. Neurological examination found a complete paraplegia (0/5 stergh), with incomplete sensory deficit below the T-10 level, bowel and bladder dysfunction, decreased deep and superficial reflex. MrI with different sequences showed a large epidural mass lesion with slightly high signal intensity on T1-weighted images and heterogeneous low signal intensity on T2-weighted images from T11 to L1 suggesting an epidural hematoma. She underwent an urgent total laminectomy from T11 to L1 following by a complete removal of a large dark and compact epidural hematoma compressing the spinal cord. Operatively, there’s no sign suggesting an AVM. Postoperative course was uneventful and she was discharged ten days after surgery with complete recovery.

Purpose: To evaluate the accuracy and limits of a frozen section analysis in our department in order to develop a quality control. Materials and methods: all frozen sections reported at our department between 1st January 2010 and 31st December 2010, have been retrospectively evaluated. The frozen section results were compared to permanent paraffin section results. results: Frozen sections and final diagnosis agreed in 95% and disagreed in 5% (13 cases), 6.6% of the cases were deferred (17 cases). The most common pathological processes encountered were presence/typing of neoplasm (85.4%) and assessment of surgical margins (7.3%). Conclusion: The accuracy of frozen sections diagnosis at our department can be interpreted as comparable with the most international quality control series and is valuable to assist surgeons in their decision.

Background: Mycosis Fungoides (MF) is the most common and indolent form of Cutaneuse T-cell Lymphomas (CTCL), that usually occurs in old adults. Objectives: To determine epidemiologic features and patients characteristics of MF in Isfahan (Iran). Methods: We performed a retrospective study in MF clinic of alzahra hospital that is the main center for treatment of MF patients in Isfahan (Iran) and evaluated clinicopathologic features. results: In 3 years 25 patients were referred to Alzahra MF clinic. 18 patients diagnosed as MF. Seven (38.9%) patients were male and 11 (61.1%) were female with male to female ratio of 1:1.57. The mean age of patients was 41.06 years. 88.9% of our patients were in stages ΙA and ΙB. Conclusion: Most of our patients presented in early stages that were similar to other studies, while male: female ratio is different from other studies.

Anzali International wetland is one of the most important places for various organisms such as fishes. Zooplankton are the first consumers in the ecosystem, and they are perfect food for the larvae of fishes. The present study conducted monthly during January 2012 to December 2013 in 9 different stations with zooplankton population and chemical characteristics analysis such as water temperature ranged from 10°C - 22°C, pH determined alkaline nature of the wetland ranged from 7.05 to 9.47, dissolved oxygen was recorded in the range of 3.36 mg/l to 10.51 mg/l, nitrate was ranged between 0.48 - 4.36 mg/l, total phosphates was between 0.15 - 0.67 mg/l, salinity was recorded between 220 - 692 mg/l, TDS was determined between 246 - 1971 mg/l, BOD and COD was also recorded 2 - 36 mg/l and 4 - 74 mg/l respectively. Total 61 zooplankton species were found belonging to 4 groups: Protozoa (22 sp.), rotatoria (29 sp.), Copepoda (5 sp.) and Cladocera (4 sp.). rotatoria were found dominating other groups of zooplankton. Kruskal Wallis test showed that there was significant difference between density of zooplankton in different stations, months and seasons (P ≤ 0.05) and significant differences were found between densities of different zooplankton phylum (P ≤ 0.05). The water body is continuously receiving domestic discharge leading to large amount of nutrient inputs and high amount of phosphate and nitrate in the water body indicates that water is eutrophic in nature.

Introduction: Hyalinizing spindle cell tumor with giant rosettes (HSCT) is a very uncommon mesenchymal tumor that has similar morphological and biological features to the low-grade fibromyxoid sarcoma (LGFMS). Case report: reported herein is a case of primary tumour of buttock HSCT that had rare FUS-CrEB3L1 fusion transcripts, a product of characteristic chromosomal abnormality t (7;16) (q33;p11) of HSCT and LGFMS. The patient was a 48-year-old man who had a large solitary mass in the buttock. Histologically, it was composed of bland spindle cells with variable cellularity deposited in a densely hyalinized stroma alternating with myxoid areas. Characteristic collagen rosettes were scattered in the cellular areas. reverse transcription-polymerase chain reaction (rT-PCr) assay using formalin-fixed, paraffin-embedded tissue detected FUS-CrEB3L1 fusion transcripts. In our knowledge is the second case may display a variant FUS/CrEB3L1 fusion transcript in international literature. Conclusion: LGFMS and HSCT probably have a wider spectrum of morphologic features than previously thought, the awareness of which will help pathologists to avoid diagnostic pitfalls. Demonstration of the t (7;16) translocation will help to diagnose difficult cases with unusual histologic features.

Cervical Spondylodiscitis revealed by Tracheostomy Infected: A rare Case report Management and Outcome

Background: Spondylodiscitis is a spinal infection rare affecting primarily the intervertebral disk and the adjacent vertebral bodies. His diagnosis is difficult and often delayed or missed due to the rarity of the disease. Cervical spondylodiscitis has quite rare findings regarding the common location of spinal abscesses in the lumbar and thoracic regions. To obtain the best patient outcomes, these spinal infections require prompt diagnosis and appropriate treatment. Case description: A 44-year-old boy was admitted to the neurosurgery department of our hospital for heaviness in 4 limbs without gait disorders in context infected tracheostomy after staying in intensive care. MrI spine showed a spine deformity with lyse C4C5 epiduritis and spinal cord compressed. Antibiotics intravenous were started for 1 month the patient benefited from a corpectomy with an iliac graft and anterior cervical plate. The anatomopathological examination revealed an inflammation not specific. He was therefore put on antibiotics for 6 weeks. Three months later the neck pain and limb pain resolved after treatment and a complete return of lower extremity strength. Conclusion: Cervical spondylodiscitis has increased and become more aggressive. While radical surgical debridement, stable reconstruction together with antibiotic therapy remained a reliable approach to achieve complete healing of the inflammation, anterior alone surgery became more applicable.

Influence of Physico-Chemical Factors, Zooplankton Species Biodiversity and Seasonal Abundance in Anzali International Wetland, Iran

Anzali International wetland is located in the south coast of the Caspian Sea. Physicochemical analysis and zooplankton survey of the wetland were carried out on monthly basis for the period of one year from January 2012 to December 2013 at 9 different stations of Anzali wetland and its related channel to the Caspian Sea. Water temperature of the wetland followed more or less similar trend as that of air temperature. pH determined alkaline nature of the wetland ranging between 7.05 to 9.47, dissolved oxygen was recorded in the range of 3.36 mg/l to 10.51 mg/l while other parameters recorded were water temperature (10 to 23 C), Nitrate (0.48 to 4.36 mg/l), Total Phosphates (0.15 to 0.67 mg/l), Salinity (220 to 692 mg/l), Electrical Conductivity (235 to 1369 μs/cm), TDS (246 to 1971 mg/l), BOD (2 to 36 mg/l) and COD (4 to 74 mg/l). During study period, total 60 species of zooplankton were identified in four main groups such as Protozoa (22 sp.), rotifera (29 sp.), Copepoda (5 sp.) and Cladocera (4 sp.). The highest numbers of zooplankton were recorded in summer months and lowest in winter which is the second rainy season of this area. The water body is receiving domestic discharge, agricultural run-off and industrial wastes leading to large amount of nutrient inputs to the ecosystem which indicates the eutrophic statue of the wetland. The aim of present study was to investigate the interrelationship between physicochemical factors and zooplankton population in context of their seasonal abundance.

respiratory epithelial adenomatoid hamartoma (rEAH) is an uncommon lesion of the upper aerodigestive tract first described by Wenig and Heffner in 1995 as prominent glandular proliferations lined by ciliated respiratory epithelium originating from the surface epithelium. Case report: We report a case of 48-year-old women with nasal polyposis history, which consults for nasal obstruction, with suspicion of malignancy on CT. Surgical resection showed a respiratory epithelial adenomatoid hamartoma (rEAH) of the nasal cavity. Conclusion: rEAH is a recently described pathologic entity that can present rhinorrhea, epistaxis, hyposmia, and headaches. It is a rare lesion of nasal and paranasal sinuses, but should be considered in the differential diagnosis because it is a benign lesion and complete surgical resection is curative.

针对坐落于意大利帕维亚大学的TrIGA Mark Ⅱ反应堆热柱结构进行优化设计,从而满足面向硼中子俘获治疗(BNCT)的单光子发射计算机断层成像(SPECT)研究要求。为提高计算效率并减小统计误差,对比分析使用SSW/SSr方法与直接使用反应堆为源项时热柱内照射位置处中子能谱,其结果基本一致,从而验证了SSW/SSr方法的可靠性。为在该反应堆开展BNCT中SPECT实验,热柱中子束需准直为笔形束。对比分析四种热柱优化方案下束流口处及探测器处热中子和光子通量:40cm长石墨(射束口5cm×3cm);0.5cm厚硼包裹40cm长石墨(射束口5cm×3cm);30cm长天然锂聚乙烯(射束口直径4cm);30cm长天然锂聚乙烯(20cm长射束口直径5cm,5cm长射束口直径4cm,5cm长射束口直径2cm)。结果显示,射束口处热中子通量分别为1.05×108,2.52×107,6.08×107和5.10×107#/(cm2·s)。综合考虑中子准直效果及光子污染,方案三具有最优性能。为后续进行BNCT-SPECT理论和实验研究提供了基础,从而有效促进BNCT剂量准确评估方法的研究进程。
