Capsule endoscopy:Present status and future expectation

Video capsule endoscopy (CE) since its introduction 13 years back, has revolutionized our approach to small intestinal diseases. Obscure gastrointestinal bleed (OGIB) continues to be the most important indication for CE with a high sensitivity, specificity as well as positive and negative predictive values. It is best performed during ongoing bleed or immediately thereafter. Overt OGIB has a higher diagnostic yield than occult OGIB. However, even in iron deficiency anemia, CE is emerging as important investigation after initial negative work up. In suspected Crohn’s disease (CD), CE has been shown superior to traditional imaging and endoscopic technique and should be considered after a negative ileocolonoscopy. Although CE has also been used for evaluating established CD, a high capsule retention rate precludes its use ahead of cross-sectional imaging. Celiac disease, particularly where gastro-duodenoscopy cannot be performed or is normal, can also be investigated by CE. Small bowel tumor, hereditary polyposis syndrome, and non-steroidal anti-inflammatory drugs induced intestinal damage are other indications for CE. Capsule retention is the only significant adverse outcome of CE and occurs mostly in presence of intestinal obstruction. This can be prevented by use of Patency capsule prior to CE examination. Presence of cardiac pacemaker and intracardiac devices continue to be relative contraindications for CE, though data do not suggest interference of CE with these devices. Major limitations of CE today include failure to control its movement from outside, inability of CE to acquire tissue for diagnosis, and lack of therapeutic help. With ongoing interesting and exciting developments taking place in these areas, these issues would be solved in all probability in near future. CE has the potential to become one of the most important tools in diagnostic and possibly in the therapeutic field of gastrointestinal disorder.

Inflow and outflow permeability tests in a very soft clay under low stresses

In situ inflow and outflow permeability tests with the BAT probe at SarapuíII soft clay test site are presented.A description of the BAT permeability test is provided,discussing its advantages and shortcomings,especially in the case of very soft clays under low *** pressures were monitored during probe installation and were found to be slightly lower than piezocone u2 pore pressures,consistent with the position of the *** role of filter tip saturation was investigated after the usual saturation procedure provided an unsatisfactory pore pressure response during probe *** show that the vacuum saturation procedure provides adequate response during installation and increases the reliability of the coefficient of permeability determination in early *** inflow and outflow tests yielded similar results,indicating that careful execution of the test can lead to good test repeatability regardless of the loading *** sequences of alternated inflow and outflow tests have yielded similar results,indicating that soil reconsolidation and filter clogging were negligible in the tests *** are presented concerning the relationship between index parameters and the in situ coefficient of permeability for SarapuíII clay,which plot outside the range of existing databases.

Phosphate tailings are usually used as backfill material in order to recycle tailings *** study considers the effect of the mix proportions of clinker-free binders on the fluidity,compressive strength and other key performances of cementitious backfill materials based on phosphate *** particular,three solid wastes,phosphogypsum(PG),semi-aqueous phosphogypsum(HPG)and calcium carbide slag(CS),were selected to activate wet ground granulated blast furnace slag(WGGBS)and three different phosphate tailings backfill materials were ***,rheology,settling ratio,compressive strength,water resistance and ion leaching behavior of backfill materials were *** to the results,when either PG or HPG is used as the sole activator,the fluidity properties of the materials are *** tailings backfill material activated with PG present the largest fluidity and the lowest yield ***,the backfill material’s compressive strength is considerably increased to 2.9 MPa at 28 days after WGGBS activation using a mix of HPG and CS,all with a settling ratio of only 1.15 ***,all the three ratios of binder have obvious solidification effects on heavy metal ions Cu and Zn,and P in phosphate tailings.

The demands on conventional communication networks are increasing rapidly because of the exponential expansion of connected multimedia content. In light of the data-centric aspect of contemporary communication, the information-centric network (ICN) paradigm offers hope for a solution by emphasizing content retrieval by name instead of location. If 5G networks are to meet the expected data demand surge from expanded connectivity and Internet of Things (IoT) devices, then effective caching solutions will be required to maximize network throughput and minimize the use of resources. Hence, an ICN-based Cooperative Caching (ICN-CoC) technique has been used to select a cache by considering cache position, content attractiveness, and rate prediction. The findings show that utilizing our suggested approach improves caching regarding the Cache Hit ratio (CHr) of 84.3%, Average Hop Minimization ratio (AHMr) of 89.5%, and Mean Access Latency (MAL) of 0.4 s. Within a framework, it suggests improved caching strategies to handle the difficulty of effectively controlling data consumption in 5G networks. These improvements aim to make the network run more smoothly by enhancing content delivery, decreasing latency, and relieving congestion. By improving 5G communication systems’ capacity to manage the demands faced by modern data-centric applications, the research ultimately aids in advancement. Copyright © 2024 The Authors. Dear Editor,We present a case of ruptured renal angiomyolipoma(AML)and bilateral renal staghorn *** discuss about the management of this patient by angioembolization,partial nephrectomy,and pyelolithotomy,followed by contralateral interval percutaneous nephrolithotomy(PCNL).Our case is a 40-year-old male who presented with history of dull aching and dragging sensation in bilateral flanks of 6-month *** was admitted and evaluated at other hospital for dizziness,sweating,and loss of *** was found to have bilateral staghorn calculi and left AML and was referred to our institute for further management after initial *** proper written and informed consent was taken from the patient for management as well as publication of the report and images,maintaining the patient’s anonymity.

Polyelectrolyte Complexes between Hyperbranched and Linear Polysaccharides: Fucoidan/Chitosan

Polyelectrolyte complexes(PECs)of hyperbranched(HB)and linear polysaccharides are promising as more effective encapsulation agents compared to PECs formed by linear *** investigated the PECs between the HB anionic polysaccharide fucoidan(FUC)and the cationic linear polysaccharide chitosan(CS).The FUC had a molecular weight(MW)of 30×*** PECs were prepared in three solvents(water,0.01 and 0.1 mol/L acetic acid)with CS of MW of 15,110 and 170 kDa,and deacetylation degrees(DDA)of 70%and 97%.The structures of the PECs and the initial FUC were investigated by multi-angle static and dynamic light *** the FUC contained 18 wt%of—OSO3 groups and 5 wt%of uronic acid units,it was a“strong-weak”copolyanion,so the HB macromolecules of the FUC formed nanogel particles in 0.1 mol/L AcOH and open branched structures in water,as confirmed by the Kratky *** mixing the solutions of original components,the PEC structures underwent an equilibration period,the duration of which increased with the MW of *** the charge stoichiometry was approached,the PECs shrank;the fractal dimension approached unity,indicating the side-by-side packing of adjacent FUC branches with the help of *** aggregation in the vicinity of the charge compensation was hardly observed,as it occurred in a very narrow *** PEC content at theζ-potential inversion depended on solvents’pH and the DDA of *** the extreme case of core-shell PECs in 0.1 mol/L AcOH,obtained by mixing FUC nanogels with the solutions of high MW CS of 97%DDA,the protruding tails of CS formed a positively charged shell in the whole range of FUC content(10 wt%ron microscopy and atomic force microscopy images of dried samples were discussed in relation to the light scattering results.
