Less common etiologies of exocrine pancreatic insufficiency

Exocrine pancreatic insufficiency(EPI), an important cause of maldigestion and malabsorption, results from primary pancreatic diseases or secondarily impaired exocrine pancreatic function. Besides cystic fibrosis and chronic pancreatitis, the most common etiologies of EPI, other causes of EPI include unresectable pancreatic cancer, metabolic diseases(diabetes); impaired hormonal stimulation of exocrine pancreatic secretion by cholecystokinin(CCK); celiac or inflammatory bowel disease(IBD) due to loss of intestinal brush border proteins; and gastrointestinal surgery(asynchrony between motor and secretory functions, impaired enteropancreatic feedback, and inadequate mixing of pancreatic secretions with food). This paper reviews such conditions that have less straightforward associations with EPI and examines the role of pancreatic enzyme replacement therapy(PERT). Relevant literature was identified by database searches. most patients with inoperable pancreatic cancer develop EPI(66%-92%). EPI occurs in patients with type 1(26%-57%) or type 2 diabetes(20%-36%) and is typically mild to moderate; by definition, all patients with type 3 c(pancreatogenic) diabetes have EPI. EPI occurs in untreated celiac disease(4%-80%), but typically resolves on a gluten-free diet. EPI manifests in patients with IBD(14%-74%) and up to 100% of gastrointestinal surgery patients(47%-100%; dependent on surgical site). With the paucity of published studies on PERT use for these conditions, recommendations for or against PERT use remain ambiguous. The authors conclude that there is an urgent need to conduct robust clinical studies to understand the validity and nature of associations between EPI and medical conditions beyond those with proven mechanisms, and examine the potential role for PERT.

Approach to traumatic cardiac arrest in the emergency department: a narrative literature review for emergency providers

Development of a Water Quality Index (WQI) of an Artificial Aquatic Ecosystem in mexico

A Water Quality Index (WQI) is a simple numeric expression reflecting the quality of water in any ecosystem at a given time. The objective of this study was to develop a WQI for the man-made dam Francisco I. madero located in Chihuahua, mexico. Eight points were randomly selected in the dam area and at each point water samples were collected monthly from march 2011 to February 2012 at three depths;0.30 m, 5 m and 10 m. The following physical-chemical variables were measured: potential hydrogen (pH), electrical conductivity (EC), dissolved oxygen (DO), temperature (T), turbidity, total dissolved solids (TDS), total hardness (TH) and chlorides (Cl-). In a first step for data analysis, an analysis of variance (ANOVA) was performed for each variable considering a factorial treatment design 12 × 3 in which factor A was the month with 12 levels (sampling months) and factor B was the depth with three levels (0.30 m, 5 m and 10 m). In a second statistical step, the WQI was calculated for each month only for the surface sampling (0.30 m) and the resulting value was classified under three categories;2.5 as excellent water. The results showed the following ranges for single variables: pH of 7.63 - 10.65, EC of 190 - 320 μS·cm-1, DO of 1.30 - 12.1 mg·L-1, T of 11.30°C - 30°C, Turbidity of 0-1, 120 NTU, TDS of 170 - 220 mg·L-1, TH of 240 - 900 mg·L-1 and Cl- of 7.28 - 7034 mg·L-1. The calculated WQI demonstrated that water quality varies seasonally and was classified as poor in the rainy season to good in winter season. We conclude that in general the water from the dam is acceptable and suitable for ecological and a broad spectrum of other purposes.

In addition to clinical risk markers, indices of left ventricular(LV) systolic function are valuable prognostic markers after acute myocardial infarction(mI). Previous studies have also suggested that LV diastolic function may contribute with prognostic information. The present study assessed whether this assumption applies to a large population of patients with acute mI who underwent thrombolyt ic therapy.520 out of 608 patients participating in the ATTenuation by Adenosine of Cardiac Complications (ATTACC) study, with an ST-elevation acute mI underwe nt two-dimensional and Doppler echocardiographic examination at 4 (range 2-10) days after admission. During the follow-up period of 31 (S.D.±11) months, car diovascular death occurred in 57 (11%) patients, nonfatal acute mI occurred in 77 (15%), and 124 (24%) patients suffered a combined cardiovascular end-point (either nonfatal acute mI or cardiovascular death). Univariate regression analy sis showed that all indices of LV systolic function predicted cardiovascular dea th and combined cardiovascular end-points. Regarding LV diastolic function only a restrictive filling pattern predicted cardiovascular death. In a multistep mu ltivariate regression analysis in which the variables were introduced in a hiera rchic order age, history of systemic hypertension, wall motion score index (WmSi ), and history of previous mI and diabetes mellitus were independent predictors of cardiovascular death. A history of systemic hypertension or congestive heart failure were independent predictors of nonfatal acute mI, while a history of sys temic hypertension, wall motion score index and diabetes mellitus independently predicted combined cardiovascular *** results of this study confirme d that clinical risk indicators and LV systolic function were the most important independent predictors of cardiovascular death and combined cardiovascular end -points. The tremendous potential of triboelectric generators-TENGs for converting mechanical energy into electrical energy places them as one of the most promising energy harvesting technologies. In this work, the fabrication of enhanced performance TENGs using Ag octahedron nano-assemblies on ITO as electrodes significantly increases the electric charge collection of the induced tribocharges. Thereby, nanostructured electrical contacts coated with Ag macroscale nano-assemblies with octahedral features were obtained by the electrodeposition technique on flexible PET/ITO substrates. Consequently, the nanostructured triboelectric generator-TENG exhibited 65 times more maximum output power, and almost 10 times more open circuit output voltage than that of a TENG with non-nanostructured contacts passing from μW to m W capabilities, which was attributed to the increment of intrinsic interface states due to a higher effective contact area in the former. Likewise, output performances of TENGs also displayed an asymptotic behavior on the output voltage as the operating frequency of the mechanical oscillations increased, which is attributed to a decrement in the internal impedance of the device with frequency. Furthermore, it is shown that the resulting electrical output power can successfully drive low power consumption electronic devices. On that account, the present research establishes a promising platform which contributes in an original way to the development of the TENGs technology.

Ultrafine Grained HSLA Steels for Cold Forming

The industrial level production of ultrafine grained （or ultrafine ferrite） ferrous alloys was investigated through three examples of steels that complied with the EN 10149-2 Euronorm and were produced by advanced con- trolled hot rolling techniques. The steel samples were tension tested and chemically analyzed, and the microstructure was evaluated through quantitative metallographic techniques to determine parameters such as yield stress, amount of microalloying elements, strain hardening coefficient, grain size, and grain size distribution. These steels were micro- alloyed with Ti, Nb, and mn with ASTm grain sizes of approximately 13--15. The careful control of chemical com- position and deformation during production, giving a specific attention to the deformation sequences, austenite non- reerystallization temperatures and allotropic transformations during cooling, are indispensable to obtain steels with an adequate strain hardening coefficient that allows cold working operations such as bending, stretching or drawing.

Structural Ultrafine Grained Steels Obtained by Advanced Controlled Rolling

Steels with ultrafine grains （lower than 5 μm）, which usually known as ultrafine ferrite or ultrafine grained materials, are presently the object of intense research, because of the improvement in resistance and fracture toughness they may reach compared to conventional steels （with grain sizes above this value）. It is shown that the forenamed steels designated in the Euronorm EN 10149-2, which are manufactured by advanced techniques of controlled rolling and mainly used in automotive industry, have an ultrafine grain size in the range of 2.5 to 3.5 μm, and with elastic yield stresses higher than 400 mPa. Based on the morrison-miller criterion, it is shown that values of the strain-hardening coefficient lower than 0.08 would make the industrial application of these steels unfeasible.

Split-root systems applied to the study of the legume-rhizobial symbiosis: What have we learned?

Split-root system（SRS） approaches allow the differential treatment of separate and independent root systems, while sharing a common aerial part. As such, SRS is a useful tool for the discrimination of systemic（shoot origin）versus local（root/nodule origin） regulation mechanisms. This type of approach is particularly useful when studying the complex regulatory mechanisms governing the symbiosis established between legumes and Rhizobium bacteria. The current work provides an overview of the main insights gained from the application of SRS approaches to understand how nodule number（nodulation autoregulation） and nitrogen fixation are controlled both under non-stressful conditions and in response to a variety of stresses. Nodule number appears to be mainly controlled at the systemic level through a signal which is produced by nodule/root tissue, translocated to the shoot, and transmitted back to the root system, involving shoot Leu-rich repeat receptor-like kinases. In contrast, both local and systemic mechanisms have been shown to operate for the regulation of nitrogenase activity in nodules. Under drought and heavy metal stress, the regulation is mostly local,whereas the application of exogenous nitrogen seems to exert a regulation of nitrogen fixation both at the local and systemic levels.

The following paper reports on a comparative study of the effects of two types of carbon nanotubes,namely multiwall(mWCNT)and single-wall(SWCNT)carbon nanotube,on the properties of 3D-printed parts produced with acrylonitrile-butadiene-styrene(ABS)nanocomposites with various CNT loadings of5-10 wt.%.Quasi-static tensile properties and Vicat softening temperature of 3D-printed parts were enhanced with the increasing CNT *** highest enhancement in tensile properties was observed for the ABS/CNT nanocomposites at 10 wt.%filler loading.3D-printed ABS/SWCNT composites showed higher tensile modulus,better creep stability and higher Vicat ***,the strength of ABS/SWCNT 3D samples is relatively lower than that of ABS/*** addition,3D-printed parts exhibited anisotropic electrical conductive behaviour,which has a conductivity of through-layer of about2-3 orders of magnitude higher than *** highest conductivity of 3D-printed samples reached25.2 S/m,and 9.3 S/m for ABS/mWCNT and ABS/SWCNT composites at 10 wt.%,*** results obtained,*** successful fuse filament fabrication and the consequent electromechanical properties,confirm that these 3D printable nanocomposite could be properly utilized for the production,and application up to about 90°C,of thermoelectric devices and/or resistors for flexible circuits.
