Background Acinetobacter baumanii （A. baumanii ） remains an important microbial pathogen resulting in nosocomialacquired infections with significant morbidity and mortality. The mechanism by which nosocomial bacteria, like A. baumanii, attain multidrug resistance to antibiotics is of considerable interest. The aim in this study was to investigate the spread status of antibiotic resistance genes, such as multiple 13-1actamase genes and aminoglycoside-modifying enzyme genes, from A. baumanii strains isolated from patients with lower respiratory tract infections （LRTIs）. Methods Two thousand six hundred and ninety-eight sputum or the bronchoalveolar lavage samples from inpatients with LRTIs were collected in 21 hospitals in the mainland of China from November 2007 to February 2009. All samples were routinely inoculated. The isolated bacterial strains and their susceptibility were analyzed via VITEK-2 expert system. Several kinds of antibiotic resistant genes were further differentiated via polymerase chain reaction and sequencing methods. Results Totally, 39 A. baumanii strains were isolated from 2698 sputum or bronchoalveolar lavage samples. There was not only a high resistant rate of the isolated A. baumanfi strains to ampicillin and first- and second-generation cephalosporins （94.87%, 100% and 97.44%, respectively）, but also to the third-generation cephalosporins （ceftriaxone at 92.31%, ceftazidine at 51.28%） and imipenem （43.59%） as well. The lowest antibiotic resistance rate of 20.51% was found to amikacin. The OXA-23 gene was identified in 17 strains of A. baumanii, and the AmpC gene in 23 strains. The TEM-1 gene was carried in 15 strains. PER-1 and SHV-2 genes were detected in two different strains. Aminoglycoside-modifying enzyme gene aac-3-1a was found in 23 strains, and the aac-6＂lb gene in 19 strains, aac-3-1a and aac-6＂lb genes hibernated in three A. baumanfi strains that showed no drug-resistant phenotype. Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

The Large sky Area Multi-Object Fiber Spectroscopic Telescope（LAMOST） general survey is a spectroscopic survey that will eventually cover approximately half of the celestial sphere and collect 10 million spectra of stars, galaxies and QSOs. Objects in both the pilot survey and the first year regular survey are included in the LAMOST DR1. The pilot survey started in October 2011 and ended in june 2012, and the data have been released to the public as the LAMOST Pilot Data Release in August 2012. The regular survey started in September 2012, and completed its first year of operation in june 2013. The LAMOST DR1 includes a total of 1202 plates containing 2 955 336 spectra, of which 1 790 879 spectra have observed signalto-noise ratio（SNR） ≥ 10. All data with SNR ≥ 2 are formally released as LAMOST DR1 under the LAMOST data policy. This data release contains a total of 2 204 696 spectra, of which 1 944 329 are stellar spectra, 12 082 are galaxy spectra and 5017 are quasars. The DR1 not only includes spectra, but also three stellar catalogs with measured parameters： late A,FGK-type stars with high quality spectra（1 061 918 entries）, A-type stars（100 073 entries）, and M-type stars（121 522 entries）. This paper introduces the survey design, the observational and instrumental limitations, data reduction and analysis, and some caveats. A description of the FITS structure of spectral files and parameter catalogs is also provided.

目的:探索行急诊经皮冠状动脉介入治疗(PCI)的患者血浆大内皮素水平与术后出现对比剂诱导的急性肾损伤(CI-AKI)的相关性。方法:选取2013-01-01到2015-06-30于阜外医院行急诊PCI的患者共1 061例纳入研究。根据大内皮素水平将患者分为大内皮素正常组(n=236)和大内皮素升高组(n=825),分析两组患者基线资料和介入操作特征,以及CI-AKI发生率和术后6个月、12个月复合终点事件(包括非致死性心肌梗死、再次血运重建、脑卒中和全因死亡)发生率,并进行Logistic分析明确CI-AKI的危险因素。结果:行急诊PCI的患者术后CI-AKI发生率为22.7%(241/1 061)。大内皮素升高组患者CI-AKI发生率和术后6个月、12个月复合终点发生率均显著高于大内皮素正常组(P值分别为0.041、0.040和0.021)。校正其他变量后,大内皮素不论作为连续性变量还是分类变量均显著增加急诊PCI患者术后CI-AKI的风险。结论:在行急诊PCI的患者中,血浆大内皮素水平升高会显著增加患者术后CI-AKI的风险。

Background： Urine output （UO） is an essential criterion of the Kidney Disease Improving Global Outcomes （KD1GO） definition and classification system tbr acute kidney injury （AKI）, of which the diagnostic value has not been extensively studied. We aimed to determine whether AKI based on KDIGO UO criteria （KDtGOLro） could improve the diagnostic and prognostic accuracy, compared with KDIGO serum creatinine criteria （KDIGOscr）.Methods： We conducted a secondary analysis of the database of a previous study conducted by China Critical Care Clinical Trial Group （CCCCTG）, which was a 2-month prospective cohort study （July 1,2009 to August 31,2009） involving 3063 patients in 22 tertiary Intensive Care Units in Mainland of China. AKI was diagnosed and classified separately based on KDIGOt,o and KDlGOsc,. Hospital mortality of patients with more severe AKI classification based on KDIGOvo was compared with other patients by univariate and multivariate regression analyses. Results： The prevalence of AKl increased from 52.4% based on KDIGOscr to 55.4% based on KD1GOsc~ combined with KDIGOuo. KDIGOv~~ also restllted in an upgrade of AKI classification in 7.3% of patients, representing those with more severe AK1 classification based on KDIGOvo. Compared with non-AKI patients or those with maximum AKI classification by KDIGOscr, those with maximum AKI classification by KDIGOuo had a significantly higher hospital mortality of 58.4% （odds ratio [OR]： 7.580, 95% confidence interval [CI]： 4.141-13.873, P 〈 0.001）. In a multivariate logistic regression analysis, AKI based on KDIGOuo （OR： 2.891, 95% CI： 1.964-4.254, P 〈 0.001）, but not based on KDIGOscr （OR： 1.322, 95% CI： 0.902-1.939, P = 0.152）, was an independent risk factor for hospital mortality. Conclusion： UO was a criterion with additional value beyond creatinine criterion for AKI diagnosis and classification, which can help identify a group of patients with high risk of death.

目的研究培美曲塞联合铂类药物一线用药治疗晚期非小细胞肺癌（NSCLC）的近期及远期疗效,分析胸苷酸合酶（TS）表达与培美曲塞联合铂类药物疗效的关系.方法回顾性分析2008年1月至2009年10月在北京肿瘤医院接受培美曲塞联合铂类药物一线化疗的51例Ⅲb/Ⅳ期非鳞癌NSCLC患者.共35例患者接受培美曲塞联合顺铂方案化疗,16例接受培美曲塞联合卡铂方案化疗.对其中30例的组织标本采用免疫组织化学法检测TS的表达.主要研究终点为总生存期,次要终点为缓解率、无进展生存期及不良反应.采用SPSS 16.0软件进行统计学分析.运用卡方检验分析临床因素等与化疗疗效的相关性,采用Kaplan-Merier曲线法和log-rank检验进行生存分析.结果 51例患者中1例失访（但可评价疗效及无进展生存期）,余50例资料完整.客观有效率（完全缓解＋部分缓解）为37％（19/51）,疾病控制率（完全缓解＋部分缓解＋稳定）为71％（36/51）.中位无进展生存（PFS）为5.3个月（95％ CI为3.9 ～6.7）,中位总生存期为19.0个月（95％ CI为11.6～26.4）.单因素分析结果显示,性别、病理类型、吸烟状态和疗效与生存期相关.多因素分析结果显示,病理类型是影响患者总生存期的独立预后因素.根据美国国立癌症研究所常见毒性分级标准,3～4级不良反应的发生率低,主要包括白细胞减少（5/51）、中性粒细胞减少（10/51）及消化道反应（2/51）.30例患者的标本中TS表达阳性10例.卡方分析结果显示,TS表达与疗效无关.多因素分析结果显示,病理类型、疗效及TS表达水平与总生存期相关（均P＜0.01）.结论培美曲塞联合铂类药物一线用药治疗晚期非鳞癌NSCLC疗效确切,患者耐受性好.TS表达水平与患者生存期相关.

利用OMA光学多道分析仪测量了激光与薄膜靶相互作用中产生的辐射光谱,在靶后观察到红移的二次谐波发射.这种二次谐波是v×B加热产生的、具有微脉冲结构的超热电子束在等离子体-真空边界产生的相干渡越辐射(CTR).随着激光能量的增大,红移峰向长波方向移动,光谱同时发生展宽.分析认为,等离子体临界面的迅速膨胀是导致二次谐波红移的主要原因.随着预脉冲能量的增大,临界面膨胀速度增大,导致了发射峰更大的红移.实验还测量了靶面法线方向的辐射光谱,观察到基频辐射的红移和展宽.CTR为诊断临界面的运动方向和速度提供了一种新的方法.

目的对2个白纹伊蚊野外种群的kdr抗性突变进行检测分析,评估其对拟除虫菊酯类杀虫剂的抗性风险。方法利用PCR扩增白纹伊蚊钠离子通道基因(voltage-gated sodium channel, VGSC)的两个结构域并进行测序,分析与击倒抗性(knockdown resistance,kdr)有关位点(1 016、1 532和1 534位点)的突变与系统进化关系。结果在2个种群的60只白纹伊蚊中,检测到位点1 016发生突变的有3只,占5.00%;位点1 534发生突变的有2只,占3.33%;位点1 532发生突变的有17只,占28.33%。对应的序列单倍型分析显示种群内kdr突变具有多种进化起源。结论白纹伊蚊野外种群在拟除虫菊酯类杀虫剂选择压力下,其靶标发生了多位点kdr突变,今后伊蚊防治应加强环境治理和蚊幼控制,合理使用拟除虫菊酯类杀虫剂控制成蚊进而延缓抗性产生。
