目的研究宫颈混合癌中腺样囊性癌(ACC)和腺样基底细胞癌(ABC)的临床特点、病理学特征、分子机制、治疗及预后。方法回顾分析2020年1例宫颈ACC伴ABC和CIN的病例,分析免疫组化、人乳头瘤病毒(HPV)感染状态及荧光免疫原位杂交(FISH)检测MYB-NFIB基因融合情况,随访预后。结果老年女性因阴道出血入院,查体发现宫颈口有一菜花状肿物,触血阳性。高危型HPV 16、33、52型阳性,组织病理学结果为ACC伴ABC及CIN3级,ACC呈小管状、筛状及实性片状生长,免疫标记CK7部分(+)、CD117(+),p40(-)、p63(-)。ABC呈小巢状或条索状生长,免疫标记:CD117(-)、p63(+)、p40(+),两者p16都弥漫强阳性。MYB-NFIB基因融合阴性,结果同国内外文献一致。结论ACC与ABC常混合存在,是高危型HPV相关性癌。

目的探讨椎旁肌群磁共振成像(MRI)影像学评估理筋手法治疗慢性非特异性下腰痛疗效的价值。方法选取2019年3月—2021年3月济南市中西医结合医院收治的114例慢性非特异性下腰痛患者,均给予理筋手法治疗,采用椎旁肌群MRI影像学测定椎旁肌相对横截面积(RCSA)和脂肪信号分数(FSF),绘制受试者工作特征(ROC)曲线分析总RCSA和总FSF在理筋手法治疗效果中的评估价值。结果治疗后视觉模拟评分法(VAS)和Roland-Morris功能障碍评分(RMDQ)均低于治疗前(P<0.05);治疗后共有34例治疗显效,52例治疗有效,总有效率为75.44%;无效患者治疗前后多裂肌总RCSA差值和总FSF差值均低于有效患者(P<0.05),无效患者治疗前后竖脊肌总RCSA差值和总RCSA差值均低于有效患者(P<0.05);多裂肌总RCSA、多裂肌总FSF、竖脊肌总RCSA、竖脊肌总FSF评估理筋手法治疗慢性非特异性下腰痛患者的ROC曲线下面积分别为0.827、0.834、0.804和0.760,敏感性分别为79.36%(95%CI:0.512,0.897)、82.69%(95%CI:0.517,0.952)、75.64%(95%CI:0.457,0.890)和70.21%(95%CI:0.403,0.847);特异性分别为71.25%(95%CI:0.413,0.857)、78.69%(95%CI:0.563,0.942)、70.25%(95%CI:0.410,0.820)和59.36%(95%CI:0.393,0.806)。结论椎旁肌群MRI影像学可用于定量评估理筋手法治疗慢性非特异性下腰痛患者的疗效,对于指导临床医师调整治疗方案具有重要价值。

Efficient and recyclable Rh-catalytic system with involvement of phosphine-functionalized phosphonium-based ionic liquids for tandem hydroformylation—acetalization

The phosphine-functionalized phosphonium-based ionic liquids(dppm-Q, dppe-Q, dppp-Q and dppb-Q) as the bi-functional ligands enable the efficient one-pot tandem hydroformylationeacetalization. It was found that, in dppm-Q, dppe-Q, dppp-Q and dppb-Q, the incorporated phosphino-fragments were responsible for Rh-catalyzed hydroformylation and the phosphoniums were in charge of the subsequent acetalization as the Lewis acid catalysts. Moreover, the diphosphonium-based ionic liquid of dppb-DQ could be applied as a co-solvent to immobilize the Rh/dppb-Q catalytic system with the advantages of the improved catalytic performance, the available catalyst recyclability, and the wide generality for the substrates.

Effect of c-fos antisense probe on prostaglandin E_2-induced upregulation of vascular endothelial growth factor mRNA in human liver cancer cells

AIM: To examine the effect of prostaglandin E2 (PGE2) on the expression of vascular endothelial growth factor (VEGF) mRNA in the human hepatocellular carcinoma (HCC) HepG2 cells and the possible involvement of c-fos protein in this ***: Human HCC HepG2 cells were divided into three groups treated respectively with PGE2, a combination of PGE2 and c-fos antisense oligodeoxynucleotide (ASO),and PGE2 plus c-fos sense oligodeoxynudeotide (SO). The expression of VEGF mRNA in HepG2 cells after different treatments was detected by reverse transcriptase-polymerase chain reaction (RT-PCR). The relative expression level of VEGF mRNA in HepG2 cells in each group was ***: Administration of PGE2 resulted in an increased expression of c-fosand VEGF mRNA in HepG2 cells. The relative expression level of c-fos mRNA reached the peak at 3 h (68.4±4.7%) after PGE2 treatment, which was significantly higher than that at 0 h (20.6±1.7%, P＜0.01).Whereas, the highest expression level of VEGF mRNA was observed at 6 h (100.5±6.1%) after PGE2 treatment, which was significantly higher than that at 0 h (33.2±2.4%,P＜0.01). C-fos ASO significantly reduced PGE2-induced VEGF mRNA expression in HepG2 ***: PGE2 increases the expression and secretion of VEGF in HCC cells by activating the transcription factor c-fos, promotes the angiogenesis of HCC and plays an important role in the pathogenesis of liver cancer.

Design,Synthesis and Biological Activity of Ethyl 2-(N-Substituted-arylsulfonamido)-2-oxoacetate

Thirteen new ethyl 2-（N-substituted-arylsulfonamido）-2-oxoacetates（3a-3m）, based on the structure of Ketol-acid reductoisomerase（KARI） inhibitor IpOHA, were designed and synthesized. Their structures were established on the basis of ^1H NMR, IR, MS, and elemental analyses. The bioassay result reveals that the structural changes from hydroxyl group on the N atom of IpOHA to arylsulfonyl groups does not enhance the inhibitory activity of the compounds to KARI in vitro. Compounds 3c, 3h, 3k, and 3m are more effective than IpOHA against the monocotyledonous bamygrass at 100 ug/mL in herbicidal tests.

The “No-touch” technique improves the survival of patients with advanced hepatocellular carcinomas treated by liver transplantation: A single-center prospective randomized controlled trial

Background:liver transplantation(LT)is the best treatment for patients with hepatocellular carcinoma(HCC).However,the surgical technique needs to be *** present study aimed to evaluate the“no-touch”technique in ***:From January 2018 to December 2019,we performed a prospective randomized controlled trial on HCC patients who underwent *** patients were randomized into two groups:a no-touch technique LT group(NT group,n=38)and a conventional LT technique group(CT group,n=46).Operative outcomes and survival in the two groups were ***:The perioperative parameters were comparable between the two groups(P>0.05).There was no significant difference between the two groups in disease-free survival(DFS)(P=0.732)or overall survival(OS)(P=0.891).Of 36 patients who were beyond the Hangzhou criteria for LT,the DFS of the patients in the NT group was significantly longer than that in the CT group(median 402 vs.126 days,P=0.025).In 31 patients who had portal vein tumor thrombosis(PVTT),DFS and OS in the NT group were significantly better than those in the CT group(median DFS 420 vs.167 days,P=0.022;2-year OS rate 93.8%vs.66.7%,P=0.043).In 14 patients who had diffuse-type HCCs,DFS and OS were significantly better in the NT group than those in the CT group(median DFS 141 vs.56 days,P=0.008;2-year OS rate 75.0%vs.33.3%,P=0.034).Multivariate analysis showed that for patients with PVTT and diffusetype HCCs,the no-touch technique was an independent favorable factor for OS(PVTT:HR=0.018,95%CI:0.001-0.408,P=0.012;diffuse-type HCCs:HR=0.034,95%CI:0.002-0.634,P=0.024).Conclusions:The no-touch technique improved the survival of patients with advanced HCC compared with the conventional *** no-touch technique may provide a new and effective LT technique for advanced HCCs.

Background:Portal vein tumor thrombosis(PVTT)is regarded as a contraindication for liver transplantation(LT)in hepatocellular carcinoma(HCC).However,some of these patients may have a favorable prognosis after *** this study,we evaluated the biological behavior of HCC with PVTT using tumor biomarker(alpha-fetoprotein,AFP)and 18 F-FDG positron emission tomography(tumor standard uptake value)to identify a subset of patients who may be suitable for ***:Seventy-five HCC-PVTT liver recipients transplanted during February 2016 and June 2018 were *** pre-transplant prognostic factors were identified by univariate and multivariate *** status was identified following Vp classification(Vp1-Vp4).Results:Three-year recurrence-free survival and overall survival rates were 40%and 65.4%in Vp2-Vp3 PVTT patients,21.4%and 30.6%in Vp4 PVTT patients(P8 cm,pretransplant AFP level>1000 ng/m L and intrahepatic tumor maximal standard uptake value(SUVmaxtumor>5)were independent risk factors for HCC recurrence and overall survival after LT in Vp2-3 PVTT *** risk patients were defined as total tumor diameter≤8 cm;or if total tumor diameter more than 8 cm,with both pre-transplant AFP level less than 1000 ng/m L and intrahepatic tumor SUVmax less than 5,***-two Vp2-3 PVTT HCC patients(46.8%)were identified as low risk patients,and their 3-year recurrence-free and overall survival rates were 67.6%and 95.2%,***:Patients with segmental or lobar PVTT and biologically favorable tumors defined by AFP and 18 F-FDG SUVmax might be suitable for LT.

Impact of treatment modalities on patients with recurrent hepatocellular carcinoma after liver transplantation:Preliminary experience

Background:Post-liver transplantation(LT)hepatocellular carcinoma(HCC)recurrence still occurs in approximately 20%of patients and drastically affects their *** study aimed to evaluate the efficacy of various treatments for recurrent HCC after LT in a Chinese ***:A total of 64 HCC patients with tumor recurrence after LT were enrolled in this *** and multivariate analyses were performed to identify factors affecting post-recurrence ***:Of the 64 patients with recurrent HCC after LT,those who received radical resection followed by nonsurgical therapy had a median overall survival(OS)of 20.9 months after HCC recurrence,significantly superior to patients who received only nonsurgical therapy(9.4 months)or best supportive care(2.4 months).The one-and two-year OS following recurrence was favorable for patients receiving radical resection followed by nonsurgical therapy(93.8%,52.6%),poor for patients receiving only nonsurgical therapy(30.8%,10.8%),and dismal for patients receiving best supportive care(0%,0%;overall Plimus-based immunosuppressive therapy,OS was significantly increased with sirolimus-based therapy at one and two years after HCC recurrence(P=0.035).Multivariate analysis showed radical resection combined with nonsurgical therapy for recurrent HCC and sorafenib-lenvatinib sequential therapy were independent favorable factors for post-recurrence ***:Aggressive surgical intervention in well-selected patients significantly improves OS after recurrence.A multidisciplinary treatment approach is required to slow down disease progression for patients with unresectable recurrent HCC.
