Association ofβ-lactam antimicrobial's exposure with carbapenem-resistant Pseudomonas aeruginosa infection:a cumulative meta-analysis

Background:Carbapenems are effective against severe Pseudomonas aeruginosa nosocomial ***,carbapenem-resistant Pseudomonas aeruginosa is a serious public health *** understanding of the risk of inappropriate exposure to different antimicrobials in resistant Pseudomonas aeruginosa infection could help in elucidating the effective approach towards using antimicrobials in vulnerable patients with CRPA ***:To investigate the association between exposure ofβ-lactam antimicrobials and CRPA infection relative to control ***:The MEDLINE/PubMed and OVID/Embase databases were used to search case-control and cohort studies in English language which reported antimicrobial exposure as risk factors for CRPA *** pooled odds ratios(OR)were calculated using a random-effect and fixed-effect model,and forest plots from a cumulative meta-analysis method were used to better show how pooled OR changed as updated evidence ***:A total of 24 studies comprising 7039 participants were included for cumulative meta-analysis.A positive correlation was found between development of CRPA infection and exposure of beta-lactam antimicrobials:carbapenems(OR=7.60,95%CI:3.95 to 14.62,P<0.0001),imipenem(OR=9.81,95%CI:5.56 to 17.33),ampicillin(OR=1.86,95%CI:1.14 to 2.41),piperacillin(OR=2.82,95%CI:1.46 to 2.43),penicillins(OR=1.42,95%CI:0.90 to 2.24),cephalosporins(OR=1.88,95%CI:1.46 to 2.43)andβlactamase inhibitors(OR=1.96,95%CI:1.44 to 2.67).Further,exposure of other antimicrobial agents like quinolone(OR=2.35,95%CI:1.78 to 3.10),ciprofloxacin(OR=2.35,95%CI:1.66 to 3.95),aminoglycoside(OR=2.17,95%CI:1.60 to 2.95),amikacin(OR=3.11,95%CI:2.10 to 4.61),glycopeptides(OR=3.02,95%CI:1.92 to 4.75)and vancomycin(OR=3.26,95%CI:1.48 to 7.18),were also found to be positively associated with development of CRPA ***:Exposure of all kinds ofβ-lactams is significantly associated with development of carbapenemresistant Pseudomo

Pulmonary Resection for Metachronous Lung Cancer Following Contralateral Pneumonectomy

Patients who undergo pneumonectomy for lung cancer are at risk of recurrent disease and metachronous tumours in the remaining lung. Establishing a diagnosis can be difficult as biopsy may induce a pneumothorax, whilst resection is limited by remaining lung function. However, pneumonectomy should not be a contraindication to further lung resection. We report two cases of lung cancer resection in the residual lung 6 and 11 years following pneumonectomy respectively. A limited resection can be safely performed for a new pulmonary parenchymal lesion on a background of a previous pneumonectomy.

Aspect Ratio: A Major Controlling Factor of Radiation Characteristics of Microstrip Antenna

The effect of aspect ratio (width-to-length ratio) on overall radiation characteristics of a rectangular microstrip patch antenna is theoretically investigated with a view to develop a concrete physical insight in to the phenomenon. The detailed variation of co polarized and cross polarized radiation performance as a function of aspect ratio is thoroughly presented. The gain enhancement of a microstrip patch antenna along with a radical variation of cross polarized field radiation with aspect ratio is obtained. Indeed the conjecture of occurring different fringing fields with different aspect ratio is employed to theoretically estimate the effective size of a rectangular patch, and hence to determine the quantitative change in the gain value. Close agreement amongst the theory and simulations justifies the conjecture indicating a concrete physical insight in to the fact. The aspect ratio for which the cross polarized field radiation is minimum for a particular patch is also presented and justified quantitatively.

Background:The prevalence rates of freezing of gait(FOG)in Parkinson’s disease(PD)vary widely,ranging from 14.0 to 55.1%.Our aim is to calculate the overall prevalence of FOG in all PD patients with different disease durations and ***:Using Medline/PubMed/Embase,we carried out a systematic literature search for studies reporting the PD and clinically relevant ***:After primary screening,a total of 35 studies were identified and further analyzed for inclusion into the analysis,and 29 studies fulfilled the quality criteria and included in this *** overall prevalence of FOG in PD was 39.9%(95%CI 35.3-44.5%).The FOG identified by the freezing of gait questionnaire item 3 may be more prevalent(43.8%,95%CI 38.5-49.1%)than the FOG identified by the Unified Parkinson’s Disease Rating Scale item 14(36.0%,95%CI 29.0-43.1%).Disease duration and severity are both the clinical features associated with the *** highest FOG prevalence rate in PD patients was seen in patients with disease durations≥10 years,at 70.8%,followed that of PD patients with disease durations≥5 years(53.3%),and PD patients with disease durations<5 years(22.4%).FOG presented in 28.4%of PD patients with Hoehn and Yahr staging(H&Y)score≤2.5,and in 68.4%of PD patients with H&Y score≥***:This meta-analysis confirms that the prevalence of FOG in PD is considerable,and highlights the need for accurate identification of FOG in PD.
