The study of thermal comfort in the built environment is of great relevance since it stimulates the development of more sustainable buildings suited to the local climate and able to meet the human need for *** objective of this research was to develop,construct and test a passive heating system adaptable to existing buildings,reducing the need for major interventions and increasing thermal comfort in the indoor *** adopted methodological approach was a case study in a single-family residence located in the Brazilian city of petrópolis,a mountainous region with a humid subtropical climate and a rigorous *** proposed passive heating system is totally isolated,thus mitigating air infiltration and promoting increase of temperature in the internal environment through the absorption of solar energy and greenhouse *** kind of solution is especially interesting for residents of this region,since most of the city buildings are not adequately prepared to handle low ***,given local climatic conditions,residents need to spend a lot of money on the acquisition and operation of electric or gas heating *** results indicated that the developed system,in fact,increased the temperature of the studied room when compared to an adjacent room,which did not receive the *** findings of this paper,therefore,provide a valuable reference for experts and practitioners in the selection of heating systems to be used in cold regions,and proved that passive systems can provide thermal comfort at the same time that optimize the interaction of the building with the local ecosystem.

AIM:To evaluate the effects of soy supplementation on insulin resistance,fatty liver and alanine aminotransferase(ALT) levels in non-diabetic patients with chronic hepatitis c(cHc).METHODS:In a prospective,randomized and singleblinded clinical trial,we compared patients with cHc who had casein as a supplement(n = 80)(control group),with patients who consumed a soy supplement diet(n = 80) [intervention group(IG)].Both groups received 32 g/d of protein for 12 ***:patients' baseline features showed that 48.1% were overweight,43.7% had abdominal fat accumulation,34.7% had hepatic steatosis and 36.3% had an homeostasis model assessment index of insulin resistance(HOMA-IR) ≥ *** analysis showed that protein supplementation diet reduced hepatic steatosis in both groups;however,significant reductions in ALT levels occurred in the soy *** regression modeling indicated that in the presence of severe fibrosis(f3/f4),g glutamyl transferase elevation and high density lipoprotein(HDL) reduction,the intervention group had 75% less chance of developing hepatic steatosis(OR= 0.25;95% cI:0.06-0.82) and 55% less chance of presenting with an ALT level ≥ 1.5 × the upper limit of normal(ULN)(OR = 0.45,95% cI:0.22-0.89).Soy treatment did not have any effect on insulin resistance(OR = 1.92;95% cI:0.80-4.83),which might be attributed to the fact that the HOMAIR values at baseline in most of our patients were in the normal *** hepatic fibrosis,an ALT level > 1.5 × ULN and visceral fat were predictors of an HOMA-IR ≥ *** IG group had a reduced risk of an ALT level > 1.5 × *** HOMA-IR ≥ 3.0 and HDL factors for increased ***:Soy supplementation decreased ALT levels and thus may improve liver inflammation in hepatitis c virus(HcV) patients;it also reduced hepatic steatosis in a subgroup of patients but did not change insulin *** should be considered in the nutritional care of HcV patients.

Aliphatic c–H groups are among the most abundant yet least reactive functional groups in organic molecules. Nature has evolved metalloenzymes such as those of the cytochrome p450 superfamily that efficiently activate individual tertiary（3） and secondary （2） c（sp3）–H bonds in complex molecules with the highest level of regio-and stereoselectivity [1], thus encouraging studies aimed at engineering artificial metalloenzymes for synthetic applications [2].

Traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease are three distinct neurological disorders that share common pathophysiological mechanisms involving neuroinflammation. One sequela of neuroinflammation includes the pathologic hyperphosphorylation of tau protein, an endogenous microtubule-associated protein that protects the integrity of neuronal cytoskeletons. Tau hyperphosphorylation results in protein misfolding and subsequent accumulation of tau tangles forming neurotoxic aggregates. These misfolded proteins are characteristic of traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease and can lead to downstream neuroinflammatory processes, including assembly and activation of the inflammasome complex. Inflammasomes refer to a family of multimeric protein units that, upon activation, release a cascade of signaling molecules resulting in caspase-induced cell death and inflammation mediated by the release of interleukin-1β cytokine. One specific inflammasome, the NOD-like receptor protein 3, has been proposed to be a key regulator of tau phosphorylation where it has been shown that prolonged NOD-like receptor protein 3 activation acts as a causal factor in pathological tau accumulation and spreading. This review begins by describing the epidemiology and pathophysiology of traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease. Next, we highlight neuroinflammation as an overriding theme and discuss the role of the NOD-like receptor protein 3 inflammasome in the formation of tau deposits and how such tauopathic entities spread throughout the brain. We then propose a novel framework linking traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease as inflammasomedependent pathologies that exist along a temporal continuum. finally, we discuss potential therapeutic targets that may intercept this pathway and ultimately minimize long-term neurological decline.

Grade 4 febrile neutropenia and fournier's Syndrome associated with triple therapy for hepatitis c virus:A case report

The use of triple therapy for hepatitis c not only increases the rate of sustained virological responses compared with the use of only interferon and ribavirin(RBV) but also leads to an increased number of side effects. The subject of this study was a 53-year-old male who was cirrhotic with hepatitis c virus genotype 1 A and was a previous null non-responder. We initially attempted retreatment with boceprevir(BOc), peginterferon and RBV, and a decrease in viral load was observed in the 8th week. In week 12, he presented with disorientation, flapping, fever, tachypnea, arterial hypotension and tachycardia. He also exhibitedleucopenia with neutropenia. cefepime and filgrastim were initiated, and treatment for hepatitis c was suspended. A myelogram revealed hypoplasia, cytotoxicity and maturational retardation. After 48 h, he developed bilateral inguinal erythema that evolved throughout the perineal area to the root of the thighs, with exulcerations and an outflow of seropurulent secretions. Because we hypothesized that he was suffering from fournier's Syndrome, treatment was replaced with the antibiotics imipenem, linezolid and clindamycin. After this new treatment paradigm was initiated, his lesions regressed without requiring surgical debridement. Triple therapy requires knowledge regarding the management of adverse effects and drug interactions; it also requires an understanding of the importance of respecting the guidelines for the withdrawal of treatment. In this case report, we observed an adverse event that had not been previously reported in the literature with the use of BOc.
