Monteggia fracture dislocation equivalents analysis of eighteen cases treated by open reduction and internal fixation

Objective： Monteggia fracture disloca- tion equivalent, though already described by Bado, is still an unclassified entity. We aimed to retrospectively analyze 18 cases of Monteggia variants and discuss the injury mechanisms, management, and outcome along with a re- view of the literature. Methods： A retrospective record of Monteggia frac- ture dislocation （2003-2008） was reviewed from medical record department of our institute. Classic Monteggia frac- ture dislocation, children below 12 years or adults over 50 years, as well as open grade iI ＆ Ill cases were excluded from this study. Monteggia variant inclusion criteria in- cluded fracture of the proximal ulna together with a fracture of the radial head or neck and skeletal maturity. Totally 26 patients were identified with Monteggia variants and 18 were available for follow-up, including 11 males and 7 fe- males with the mean age of 35 years. The ulna fracture was treated by compression plating along with tension band wiring. Radial head/neck was reconstructed in 12 patients while excised in 6 patients. Results： Follow-up ranged from 1-4 years, mean 2.6 years. Patients were assessed clinicoradiologically. MayoElbow Performance Score outcomes. At final follow-up, was employed to assess the the results were excellent in 10 patients, good in 4, fair in 2 and poor in 2. Mean range of motion of the elbow was 20°, 116°, 50° and 55° for extension, flexion, pronation and supination, respectively. Two patients had complications in the form of heterotopic ossification and stiffness of the elbow. One nonunion ulna, primarily treated by tension band wiring, was managed by refixation with locking reconstruction plate and bone grafting. Bone grafting was only required in this patient for nonunion. Another patient had implants removed on his request. The results in our series closely correlated with extent of intraarticular damage, coronoid fracture and comminuted fractures. Conclusions： Monteggia fracture dislocation equiva-

BACKGROUND Sepsis is a severe clinical syndrome related to the host response to *** severity of infections is due to an activation cascade that will lead to an auto amplifying cytokine production:The cytokine *** by CytoSorb®therapy is a new technology that helps to address the cytokine storm and to regain control over various inflammatory *** To evaluate prospectively CytoSorb®therapy used as an adjunctive therapy along with standard of care in septic patients admitted to intensive care unit(ICU).METHODS This was a prospective,real time,investigator initiated,observational multicenter study conducted in patients admitted to the ICU with sepsis and septic *** improvement of mean arterial pressure and reduction of vasopressor needs were evaluated as primary *** change in laboratory parameters,sepsis scores[acute physiology and chronic health evaluation(APACHE II)and sequential organ failure assessment(SOFA)]and vital parameters were considered as secondary *** outcomes were also evaluated in the survivor and nonsurvivor *** statistics were used;a P value<0.05 was considered RESULTS Overall,45 patients aged≥18 and≤80 years were included;the majority were men(n=31;69.0%),with mean age 47.16±14.11 *** CytoSorb®therapy,26 patients survived and 3 patients were lost to *** the survivor group,the percentage dose reduction in vasopressor was norepinephrine(51.4%),epinephrine(69.4%)and vasopressin(13.9%).A reduction in interleukin-6 levels(52.3%)was observed in the survivor *** count improved to 30.1%(P=0.2938),and total lung capacity count significantly reduced by 33%(P<0.0001).Serum creatinine and serum lactate were reduced by 33.3%(P=0.0190)and 39.4%(P=0.0120),*** mean APACHE II score was 25.46±2.91 and SOFA scores was 12.90±4.02 before initiation of CytoSorb®therapy,and they were reduced significantly post therapy(APACHE II 20.1±2.47;P<0.0001 and SOFA 9.04±3.0

Elbow dislocation with concomitant dia physeal fractures of radius and ulna has been reported rarely. This injury could be included in Monteggia equivalent le sions based on the mechanism of injury, radiographic pat tern and method of treatment as described by Bado. We report a rare case of Monteggia equivalent lesion in an adult with unclear mechanism of injury. The possible mechanismof injury, its management and the followup results were described. An attempt to solve the controversy regarding whether labeling it as type 1 or type 2 was made.

Body mass index and COVID-19 outcomes:A retrospective cross-sectional study at a tertiary care center in India

Objective:To determine the association between body weight and COVID-19 ***:This is a retrospective cohort study of COVID-19 patients admitted in a dedicated COVID-19 hospital,a tertiary health care center,between May and June *** data and baseline variables,including age,sex,body mass index(BMI),and comorbidities were ***(death or mechanical ventilation)of the patients with different BMI,age,comorbidities,and qSOFA scores were ***,the risk factors for death or mechanical ventilation were ***:The mean age of the subjects was(51.8±14.7)years old,and 233(74.2%)were *** were 103(32.8%)patients with normal weight,143(45.5%)patients were overweight,and 68(21.7%)patients were ***-hospital deaths and need of mechanical ventilations were significantly higher in the obese and the overweight group compared to the normal weight group,in age group≥65 years compared to<65 years,in patients with≥1 comorbidities compared to patients without comorbidities,in patients with qSOFA scores≥2 compared to patients with qSOFA scores<*** was a significantly increased risk of death(RR:4.1,95%CI 1.0-17.4,P=0.04)and significantly increased need of mechanical ventilation(RR:5.2,95%CI 1.8-15.2,P=0.002)in the obese patients compared with those with normal weight after controlling other ***:Obesity is one of the significant risk factors for adverse outcomes in COVID-19 patients and should be considered during management.

An in vitro infection model system to study proteins expressed during interaction of mycobacterium with murine macrophages

Resurgence of mycobacterial diseases particularly tuberculosis has caused a renewed interest to unravel the strategies employed by mycobacteria for intracellular survival. In spite of advancement in mycobacterial research, our knowledge about genes and their corresponding functional proteins involved during the interaction of mycobacterium with host’s macrophages is fragmentary. This study pertains to development of a suitable in vitro model using murine macrophages and Mycobacterium bovis BCG to study proteins expressed during macrophage-myco bacterium interactions. Peritoneal macrophages from BALB/c mice were infected with M. bovis BCG and intracellular replication was assessed by {3H} thymi- dine uptake assay which was maximal when macrophage to mycobacterium ratio was 1:10. SDS-PAGE was employed to study the proteins expressed and selected proteins were subjected to mass spectrometry. Seven proteins found to be upregulated during macrophage-mycobacterium interaction were identified by MALDI-TOF. The results indicate that the present in vitro infection model was able to support the growth of M. bovis BCG in murine macrophages and is an ideal model to determine the pattern of functions of gene expression during the interaction of mycobacterium with macrophages. The differentially expressed proteins will help in understanding the mycobacterial molecular basis of adaptation to intracellular macrophage environment.

In vitro gene silencing of independent phosphoglycerate mutase (iPGM) in the filarial parasite Brugia malayi

Background:The phosphoglycerate mutase(PGM)enzyme catalyzes the interconversion of 2-and 3-phosphoglycerate in the glycolytic/gluconeogenic pathways that are present in the majority of cellular *** can be classified as cofactor-dependent PGM(dPGM)or cofactor-independent PGM(iPGM).Vertebrates,yeasts,and many bacteria have only dPGM,while higher plants,nematodes,archaea,and many other bacteria have only iPGM.A small number of bacteria,including Escherichia coli and certain archaea and protozoa,contain both *** silencing of ipgm in Caenorhabditis elegans(***)has demonstrated the importance of this enzyme in parasite viability and,therefore,its potential as an anthelmintic drug *** this study,the role of the Brugia malayi(***)ipgm in parasite viability,microfilaria release,embryogenesis,and in vivo development of infective larvae post-gene silencing was explored by applying ribonucleic acid(RNA)interference ***:The in vitro ipgm gene silencing by small interfering RNA(siRNA)leads to severe phenotypic deformities in the intrauterine developmental stages of female worms with a drastic reduction(~90%)in the motility of adult parasites and a significantly reduced(80%)release of microfilariae(mf)by female worms in *** half of the in vitro-treated infective L3 displayed sluggish *** in vivo survival and development of siRNA-treated infective larvae(L3)was investigated in the peritoneal cavity of jirds where a~45%reduction in adult worm establishment was ***:The findings clearly suggest that iPGM is essential for both larval and adult stages of *** parasite and that it plays a pivotal role in female worm *** results thus validate the Bm-iPGM as a putative anti-filarial drug target.

A Case Study in the Use of Capsaicin in the Treatment of Neuropathic Chest Wall Pain and Post-thoracotomy Intercostal Neuralgia

Off label use of Quetenza? (capsaicin 8%) patch in relieving post-thoracotomy neuropathic pain proved temporarily effective, but to a degree indicating limited usefulness in a clinical setting.A 57 year-old male with chronic post-thoracotomy pain was treated with 8% capsaicin topically. Pre-procedure pain was rated as 8 on a 10 point scale, the 30 minute post procedure pain was reported as 2 - 3 on a 10 point scale. Neuralgia was completely resolved for the initial 48 hours post-procedure, then returned gradually over the next 5 days. Because pain transmission pathways are the same in post herpetic and post traumatic neuralgias, the use of 8% capsaicin merits additional study.

Nonunion of coronal shear fracture of femoral condyle

Isolated coronal fractures of femoral condyle are rare in adults and nonunion of Hoffa fracture is reported only a few times in the literature. We analyzed six cases of nonunion of Hoffa fractures over a period of three years. Three patients were treated conservatively and three patients had fixation failures. Delay of presentation was 2 months to one year. Treatment protocol consisted of open reduction, excision of pseudoarthrosis, bone grafting and internal fixation along with knee arthrolysis. Union wasachieved in all patients at mean 16 weeks. The treatment of nonunion of Hoffa fractures requires careful preoperative planning and meticulous surgical technique. The literature regarding the controversies in fracture management and surgical technique are reviewed.

Camera and Voice Control Based Location Services and Information Security on Android

Increasing popularity of Android is making its security issue more crucial nowadays. This paper focuses on one-stop solution to secure Android device against information security and theft. Proposed application protects Android device against theft and helps to control Android device by SMS or using internet connection. By this application once the user has configured the account for anti theft, user can remotely track, sound a loud siren, lock, secretly capture photo of an intruder who tries to break in, get randomly recorded voice of intruder, get thief identity using device web history and can able to wipe all your private data. This data and tracking information will be stored in one central web server database and one can access it anytime through login.

Objective:To evaluate the antioxidant activity of aqueous extract of Atoringa oleifeta ***) young leaves by in vivo as well as in vitro ***:In vitro study included estimation of total phenolic,total ilavonol,total flavonoid and total antioxidant power(FRAP assay).Tn addition, in vivo study was done with the identified most effective dose of 200 nig/kg of its lyophilized powder on normal and diabetic *** effect on different oxidative free radical scavenging enzymes,viz,superoxide dismutase(SOD),catalase(CAT),glutathione-S-transferase(GST),lipid peroxide(LPO) contents were ***:Significant increase in activities of ***, GST while,a decrease in LPO content was ***,total phenolic,flavonoid and ilavonol contents in the extract were found to be 120 mg/g of CAK,40.5 mg/g of QEK and 12.12 mg/g of QE,*** the other *** assay results of *** leaves was(85.00±5.00)μM of Fe^+/g of extract ***:The significant antioxidant activities of *** leaves from both in vivo as well as in vitro studies suggests that the regular intake of its leaves through diet can protect normal as well as diabetic patients against oxidative damage.
