Hemodynamic effects of propranolol with spironolactone in patients with variceal bleeds: A randomized controlled trial

AIM: To study the hemodynamic effects of spironolactone with propranolol vs propranolol alone in the secondary prophylaxis of variceal bleeding. METHODS: Thirty-five cirrhotics with variceal bleeding randomly received propranolol (n = 17: Group A) or spironolactone plus propranolol (n = 18: Group B). Hemodynamic assessment was performed at baseline and on the eighth day. RESULTS: Spironolactone with propranolol caused a greater reduction in the hepatic venous pressure gradient than propranolol alone (26.94% vs 10.2%; P < 0.01). Fourteen out of eighteen patients on the combination treatment had a reduction in hepatic venous pressure gradient to ≤ 12 mmHg or a 20% reduction from baseline in contrast to only six out of seventeen (6/17) on propranolol alone (P < 0.05). CONCLUSION: Spironolactone with propranolol results in a better response with a greater reduction in hepatic venous pressure gradient in the secondary prophylaxis of variceal bleeding. A greater number of patients may be protected by this combination therapy than by propranolol alone. Hence, this combination may be recommended for secondary prophylaxis in patients with variceal bleeding.

Coronary arteries supply blood and nutrients to the heart. Coronary Artery Disease (CAD) develops by narrowing of the arteries. Once the inner walls of the arteries are damaged, fatty deposits made of cells, connective tissue elements, lipids and debris decrease their lumens which block blood flow to distal tissues. This process is called arteriosclerosis. This study intends to detect the composition of human coronary atheroma collected from the arteries during bypass surgeries in Bangladesh. It shows that all lesions are present in the advanced types, namely, type 4, 5 and 6. Infrequently thrombotic materials and necrotic debris were found in type 6 plaque. The mean length of plaque was 3.36 ± 1.71 cm and mean weight was 0.13 ± 0.12 gm. Insufficient lipid was obtained from each atheroma. Few raised areas were lipid laden whereas most of the strands were fibrous. Biochemical assessment was formidable. The cholesterol composition of coronary plaque ranged between 0.20% - 9.83% with mean 3.06% ± 2.09% and total fatty acids ranged between 0.47% - 3.04% with mean 1.31% ± 0.63%. The most abundant fatty acid was oleic acid (unsaturated) closely followed by palmitic acid (saturated). The mean calcium content was 41,180.20 ± 34,918.59 ppm and ranged from 1148.00 - 140,311.00 ppm or, 0.115% - 14.031%. This study was undertaken to detect the composition of human coronary atheroma in Bangladesh which revealed that it depended upon the type of lesion.

An Approach to the Identification and Characterisation of a Psychrotrophic Lipase Producing <i>Pseudomonas</i>sp ADT3 from Arctic Region

The aim of this research work was to explore psychrotrophic microbes from soil sample of NyAlesund, Svalbard, arctic region and to investigate their potential use as an effective tool for industrial application. A novel psychrotrophic bacterial strain showed good growth on minimal medium containing lipid as the only carbon source. Microbiological characterisation of the isolate showed that it was a gram negative rod. The strain was tested for the production of extracellular lipase enzyme. The enzymes were partially purified by 90% saturated ammonium sulfate and dialysis for desalting. The bacterium was identified as Pseudomonas sp ADT3 by 16S rRNA amplification and sequencing which had been deposited in the NCBI GenBank with accession number JX914667. Phylogenetic tree was also constructed with MEGA5 software and showed the highest level of sequence similarity with Pseudomonas sp. HC3-13 strain. The microorganism had a growth optimum at pH 8.0 and temperature 22°C. Optimization of different parameters e.g. temperature, pH, incubation time, cofactors etc. was performed for the extracellular lipase activity. The hydrolytic activity of the enzyme was enhanced 5 times by Pb2+ but strongly inhibited by heavy metals Hg2+ as well as EDTA and β-mercaptoethanol. For the molecular weight estimation of enzyme SDS-PAGE was done which showed an inducible band of approximately 13.9 KDa. Activity staining and mass spectrometry techniques were also performed.

Mineralogical and Physical Characterization of Clay of Sitakunda Anticline: Used for Ceramic Industries

Efficiency of ion exchange, electro membrane, and electrochemical ion-exchange(EIX) processes on arsenic removal were evaluated in laboratory. Batch studies were performed with chloride(IRA410) as well as hydroxyl(modified IRA410) exchangeable forms of anion exchange resins(AER), whereas, column study was done with only hydroxyl exchangeable AER to evaluate its performance on arsenic removal. A plate and frame type electrolytic cell with ruthenium dioxide coated titanium plates(RuO/Ti) as anode, and stainless steel plates as cathode, was fabricated to do experiment to evaluate the performance on arsenic removal by electro membrane as well as EIX processes. Though, As(V) removal was significant during batch kinetic study, As(III) removal was poor by both the AER. Breakthrough curves were developed by passing As(V) contaminated water at a flow rate of 5 ml/min through 3 different bed depths of 5 cm, 7.5 cm, and 10 cm of modified IRA410 columns having 1 cm internal diameter. The columns could efficiently remove from 500 μg/l of As(V) contaminated water to below 10 μg/l. In electro membrane process, effects of electrolyte concentration, effects of applied voltage, and initial concentration of As(V) were evaluated. EIX process was found to be inefficient in removing As(V) from contaminated water.

This study focuses on mineralogical and chemical characterization of clay of Sitakunda anticline, Bangladesh and potential suitability of this shale as raw materials in ceramic industries. The shale which is clay in texture is composed of chlorite and kaolinite with common illite, smectite, quartz, mica and feldspar. There are two types of samples in color, grey and red. Chemical compositions of clay samples are analyzed by XRF and mineralogical composition of sample analyzed by XRD. Clay samples contain significant amount SiO2, Al2O3 and Fe2O3 with other minor compounds. The plasticity index (12.08 and 16.37) indicates medium ranges of the plasticity of clay. To determine the suitability of ceramic materials, samples were made in a rectangular shape for firing 850°C to 1100°C temperature. The samples were tested for different physical properties such as compressive strength, linear shrinkage, water absorption, porosity and bulk density. Low linear firing shrinkage (0.7% to 2.69%) and water absorption (1.5% to 11%) of the developed ceramic bodies were observed. Scanning Electron Microscope photographs confirm the vitrification of the ceramic bodies. The chemical, physical and ceramic properties of these clays indicate their suitability as raw materials for the production of a wide range of ceramic materials.

Background: Sepsis was a life-threatening situation affecting roughly 27 million individuals globally every year. Devoid of proper management, sepsis could develop to austere sepsis and septic shock that reason certain eight million expiries every year. Initial identification and analysis of contaminations and quick introductions of antimicrobial management had long been predictable as essential principles of treatment. Methods: All patients were evaluated thoroughly by history, clinical and laboratory findings. Serum lactate levels were collected at 0 hr and 24 hrs of the total admission in the ICU. Blood culture and sensitivity was performed prior to antibiotic administration in this study. Results: In this research, total sample was 1367 patients. Among these sepsis patients were 305 (nearly 22%). These 305 studied sepsis cases, majority 128 (42%) of the patients were within the age group of 41 - 65 years, followed by 18 - 30 years consisting of 116 (38%) and the least number of sepsis patients were in the age group of >65 years (20%). This study found that 129 (42.3%) out of 305 blood samples screened from assumed sepsis infection were affirmative for the presence of sepsis causing pathogens. Among the 129 culture positive samples, majority of the isolated micro-organisms were gram negative bacteria 58% (75), followed by gram positive bacteria 38% (49), and 3.8% (5) were the fungi Candida albicans. Conclusion: At present, mortality & morbidity of sepsis subject was too big due to late identification, wrong & inadequate management of sepsis in the ward and also in the intensive care unit of Bangladesh.

In using risk-informed approaches for ensuring safety of operating NPPs(nuclear power plants),risk importance measures obtained from PRAs(probabilistic risk assessments)of the plants are integral elements of consideration in many *** PSA models and applications associated with NPPs the risk importance of a particular feature(***,system,component,failure mode or operator action)can be,most generally,divided into two categories:importance with respect to risk increase potential and importance with respect to risk decrease *** representative of the first category,as used for practical purposes,is RAW(risk achievement worth).Representative of the second category,as mentioned in consideration of risk importance,is RRW(risk reduction worth).It can be shown that the two risk importance measures,RAW and RRW,are dependent on each *** only parameter in this mutual dependency is probability of failure of the considered *** paper discusses the relation between RAW and RRW and some of its implications,including those on the general strategies for the reduction of risk imposed for the operation of the considered *** general risk reduction strategies which are considered in the discussion are:a)risk reduction by decreasing the failure probability of the considered feature;and b)risk reduction while keeping the failure probability of the considered feature at the same *** examples are provided to illustrate the differences between two strategies and point to main issues and conclusions.
