Chronic heart failure （CHF） is a complex clinical syndrome resulting from inefficient myocardial pumping, and is characterised by a typical progressively deteriorating trajectory punctuated by serious episodes of acute disease *** prevalence of hospital admission has been reported.[2] However, around 40% of hospital admissions was avoidable if patients demonstrate consistent self-care.

Bacterial quorum sensing（QS） molecules are involved in the coordination of certain behaviors such as biofilm formation, virulence and antibiotic resistance. QS molecules（autoinducers） and their corresponding receptors have been recognized as important therapeutic targets for drug-resistant infections and biofilm-associated infections（BAI）. This study assessed the multiple biological effects of homogentisic acid y-lactone（HgAL）, a furanone derivative. The anti-QS and anti-biofilm effects of HgAL against PAO1 strain were evaluated using CLSM, SEM, HPLC and other biochemical methods The results showed that HgAL could effectively inhibit the production of pyocyanin and extracellular matrix, as well as reduce the adherence ability and biofilm formation of Pseudomonas aeruginosa. Inhibition of virulence is attributed to the suppressive effect of HgAL on biosynthesis of 3-oxo-C12-HSLand C4-HSL（two kinds of QS signaling molecules in P. aeruginosa）. Our results support HgAL as a potential agent for prevention of BAI in the healthcare settings.

The dynamics of a single strain HIV model is studied. The basic reproduction number R0 used as a bifurcation parameter shows that the system undergoes transcritical and saddle-node bifurcations. The usual threshold unit value of R0 does not completely determine the eradication of the disease in an HIV infected person. In particular, a sub-threshold value Rc is established which determines the system＇s number of endemic states： multiple if Rc 〈 Ro 〈 1, only one if Rc=Ro = 1, and none if R0 〈 Rc 〈 1.

The evolution of a local fragmented model of services for children with autism in Hong Kong emerged gradually over the past three decades with lack of government funding or support. This had been due to increasing number of children with autism being detected and referred for earlier assessment. With increasing pressure from parents due to long waiting time for assessment and training services and the increasing polarization by mass media there had been a gradual increasing public awareness over the past five years. Though still highly fragmented in the availability of services, there is a growing "business model" available in the community due to increasing need and lack of public funding for support. There is a lack of strategic planning for medical diagnostic and management issues in Hong Kong. Our University of Hong Kong based Autism Research Program was pioneered in 1985 based on the increasing load of autism cases referred for assessment for other developmental problems and diagnosed as Autism in the Duchess of Kent Children's Hospital. As the first author has been the staff of the University of Hong Kong, this program flourished as a research based program. The benefits of early identification and intervention of autism spectrum disorder(ASD) had been increasingly recognized, and with the increased public awareness and increasing trend of earlier diagnosis, there has been a continuously high demand from parents for earlier assessment and training for children suspected to have ASD. This model had not received extra funding for this integrated program for research, teaching and training in autism. We had to apply for various donations and grants to support the development of this pioneer program. The research output and organization of forums for public education and awareness are reviewed. AIM： To investigate the association between individual or combined use of non-steroidal anti-inflammatory drugs （NSAIDs） or statins and colorectal cancer ***： In a population-based case-control study in women, we examined the association between NSAIDs and statin use and the risk of colorectal cancers. We further investigated whether the use of statins modifies the protective effect of NSAIDs. Female cases （n = 669）of colorectal cancer aged 50-74 years were identified from a storewide registry in Wisconsin during 1999-2001. Community control women （n = 1375） were randomly selected from lists of licensed drivers and Medicare beneficiaries. Medication use and risk factor information were gathered during a structured telephone interview. A multivariable logistic regression model was used to calculate odds ratio （OR） and 95% confidence interval （CI）.RESULTS： Overall, NSAIDs users had a 30% reduction in risk of colorectal cancer （95% CI： 0.56-0.88）. Statin use was not associated with colorectal cancer risk （OR = 1.17, 95% CI： 0.74-1.85）, regardless of structural type （lipophilic or hydrophilic）, duration of use, or recency. There was no evidence of an interaction between NSAIDs and statins and colorectal cancer risk （P-interaction = 0.28）.CONCLUSION： Although our results confirm the inverse association between NSAIDs use and colorectal cancer risk, they do not support a risk reduction in statin users, or an interaction effect of combined NSAIDs and statin use.

In this paper, a mathematical model of competition between plasmid-bearing and plasmidfree organisms in a chemostat with an inhibitor is investigated. The model is in the form of a system of nonlinear differential equations. By using qualitative methods, the conditions for the existence and local stability of the equilibria are obtained. The existence and stability of periodic solutions of the Hopf type are studied. Numerical simulations about the Hop f bifurcation value and Hopf limit cycle are also given.

The widespread adoption of bead-based multiplexed bioassays requires the ability to easily synthesize encoded microspheres and conjugate analytes of interest to their ***,we present a simple method(MRBLEs 2.0)for the efficient high-throughput generation of microspheres with ratiometric barcode lanthanide encoding(MRBLEs)that bear functional groups for downstream surface *** production in MRBLEs 2.0 relies on the manual mixing of lanthanide/polymer mixtures(each of which comprises a unique spectral code)followed by droplet generation using single-layer,parallel flow-focusing devices and the off-chip batch polymerization of droplets into *** streamline downstream analyte coupling,MRBLEs 2.0 crosslinks copolymers bearing functional groups on the bead surface during bead *** the MRBLEs 2.0 pipeline,we generate monodisperse MRBLEs containing 48 distinct well-resolved spectral codes with high throughput(>150,000/min and can be boosted to 450,000/min).We further demonstrate the efficient conjugation of oligonucleotides and entire proteins to carboxyl MRBLEs and of biotin to amino ***,we show that MRBLEs can also be magnetized via the simultaneous incorporation of magnetic nanoparticles with only a minor decrease in the potential code *** the advantages of dramatically simplified device fabrication,elimination of the need for custom-made equipment,and the ability to produce spectrally and magnetically encoded beads with direct surface functionalization with high throughput,MRBLEs 2.0 can be directly applied by many labs towards a wide variety of downstream assays,from basic biology to diagnostics and other translational research.

Distance caregivers(DCGs) are a growing phenomenon in the United States Family members are struggling to provide care to loved ones with chronic illnesses such as cancer,from a *** local caregiving research,distance caregiving research is limited and inconsistent definitions of distance make it difficult to compare *** date,DCGs have not been afforded the opportunities for educational and emotional support that local caregivers have received from the health care *** they are not usually present at medical appointments,DCGs do not receive first-hand information from the health care team about the patient's condition,disease progression,and/or treatment *** caregivers report feeling left out of important family *** experience anxiety related to the uncertainty of the family members' well-being and guilt related to not being available to help local caregivers *** challenges of distance caregiving are especially evident when the distance caregiver has a parent with advanced ***-centered care,attending to the needs of the whole family regardless of their geographic location is critical for quality cancer *** this manuscript,the sparse literature on distance caregiving is *** for future research and for the development of creative technologically advanced interventions for this underserved caregiving population are suggested.

T Objective： To examine the Eastem-Westem difference in the interpretation of Integrative Medicine Attitude Questionnaire （IMAQ） by assessing the psychometric properties of a revised Chinese medicine （CM）- specific version of IMAQ （CM-IMAQ）. Methods： Factor and Rasch analysis were performed with data collected from a mail survey of 165 Hong Kong Western medical doctors （WMD） randomly sampled from the official registry. The structural validity, unidimensionality, item fit, and differential item functioning （DIF） of the Hong Kong CM-IMAQ were evaluated. Results： Confirmatory factor analysis （CFA） demonstrated that the original IMAQ factor structure was not concordant with our data on Chinese WMD, and subsequent explanatory factor analysis （EFA） validated a new three-factor model for CM-IMAQ：（1） attitude towards tonification , （2） attitude towards the effectiveness of CM, and （3） attitude towards CM knowledge. The original IMAQ factor on holism and doctor-patient relationship disappeared. Rasch analysis confirmed the unidimensionality of tonification and the effectiveness domains, but further refinement of the knowledge domain is needed. Coaclusions： Cultural adaptation of the IMAQ has demonstrated differences between Eastern and Westem doctors trained in allopathic medicine in their interpretations of hoiism in healthcare. For Chinese WMD, the emphasis of holistic care is placed on tonifying＇ the body rather than on nurturing the mind and spidt. Confucian and Taoist conceptualizations of mental health as well as the persistent stigma towards mental illness within modern Chinese culture may explain why Chinese WMD do not regard mental health promotion as part of routine healthcare.
