Nomogram to predict permanent stoma in rectal cancer patients after sphincter-saving surgery

BACKGROUND Approximately 20 percent of patients with a tumour localized in the low rectum still encounter the possibility of requiring permanent stoma(PS), which can cause drastic changes in lifestyle and physical *** To determine the risk factors for PS and to develop a prediction model to predict the probability of PS in rectal cancer patients after sphincter-saving *** A retrospective cohort of 421 rectal cancer patients who underwent radical surgery at Taipei Medical University Hospital between January 2012 and December 2020 was included in this study. Univariate and multivariate analyses were performed to identify the independent risk factors for PS. A nomogram was developed according to the independent risk factors obtained in the multivariate analysis. The performance of the nomogram was assessed using a receiver operating characteristic curve and a calibration *** The PS rate after sphincter-saving surgery was 15.1%(59/391) in our study after a median follow-up of 47.3 mo(range 7–114 mo). Multivariate logistic regression analysis demonstrated that local recurrence, perirectal abscess, anastomosis site stenosis, perineural invasion, tumor size and operative time were independent risk factors for PS. These identified risk factors were incorporated into the nomogram, and the concordance index of this model was 0.903(95%CI: 0.851-0.955). According to the calibration curves, the nomogram represents a perfect prediction *** Several risk factors for PS after sphincter-saving surgery were identified. Our nomogram exhibited perfect predictive ability and will improve a physician’s ability to communicate the benefits and risks of various treatment options in shared decision making.

Conversion of labile Pb species into chloropyromorphite(CPY)using phosphorus-bearing amendments was considered to be an ideal strategy in soil passivation ***,the fate and transport of CPY in the soil are poorly *** study aims to fill the knowledge gap by evaluating the fate and transport of CPY under environmentally relevant conditions of humic acid(HA),pH,electrolyte concentration,and species through the saturated sandy *** showed that bare CPY colloids are basically immobile in sandy porous media while the co-existence of HA made the transport of CPY improved by30%-93.5%.Facilitated transport of CPY was attributed to the increased stability of CPY and the repulsive interaction between CPY particles and sands due to HA *** mobility of CPY was also increased with increasing pH from 5.0 to *** the pH was 9 with a10 mmol/L NaCl background solution,the stronger energy barrier between CPY and sand led to enhanced transport *** divalent Ca^(2+)had a more dramatic effect than monovalent Na^(+)on the aggregation and sedimentation of CPY colloids due to its effectivescreening of the surface charge of CPY and bridging interaction with CPY ***-LandauVerwey-Overbeek theory and attachment efficiency calculation indicated that high energy barriers were responsible for the high mobility of CPY colloids,while the retention of CPY in sands was mainly caused by secondary energy minimum and physically *** findings of this work can help to evaluate the fate of soil passivation remediation products in natural water and soil.

Action of circulating and infiltrating B cells in the immune microenvironment of colorectal cancer by single-cell sequencing analysis

Effect of droplet characteristics on liquid-phase distribution in spray zone of internal mixing air-mist nozzle

In continuous casting production,droplet characteristics are important parameters for evaluating the nozzle atomization quality,and have a significant impact on the secondary cooling effect and the slab *** order to study the behavior of atomized droplets after reaching the slab surface and to optimize the spray cooling effect,the influence of droplet diameter and droplet velocity on the migration behavior of droplets in the secondary cooling zone was analyzed by FLUENT *** show that the droplets in the spray zone and on the slab surface are mainly concentrated in the center,thus,the liquid volume fraction in the center is higher than that of either *** the droplet diameter increases,the region of high liquid volume fraction on the slab surface becomes wider,and the liquid phase distribution in the slab width direction becomes *** increasing the droplet velocity at the nozzle exit has little effect on droplet diffusion in the spray zone,the distribution becomes more uneven due to more liquid reaches the slab surface per unit time.A prediction formula of the maximum water flow rate on the slab surface for specific droplet characteristics was proposed based on dimensionless analysis and validated by simulated data.A nozzle spacing of 210 mm was recommended under the working conditions in this study,which ensures effective coverage of the spray water over the slab surface and enhances the distribution uniformity of water flow rate in the transverse direction.

Improved Approximation Schemes for Early Work Scheduling on Identical Parallel Machines with a Common Due Date

We study the early work scheduling problem on identical parallel machines in order to maximize the total early work,i.e.,the parts of non-preemptive jobs that are executed before a common due *** preprocessing and constructing an auxiliary instance which has several good properties,for any desired accuracy,we propose an efficient polynomial time approximation scheme with running time O(f(1/ε)n),where n is the number of jobs and f(1/ε)is exponential in 1/ε,and a fully polynomial time approximation scheme with running time O(1/ε^(2m+1)+n)when the number of machines is fixed.

The auto-parametric resonance of a continuous-beam bridge model subjected to a two-point periodic excitation is experimentally and numerically investigated in this *** auto-parametric resonance experiment of the test model is conducted to observe and measure the auto-parametric resonance of a continuous beam under a two-point excitation on *** parametric vibration equation is established for the test model using the finite-element *** auto-parametric resonance stability of the structure is analyzed by using Newmark's method and the energy-growth exponent *** effects of the phase difference of the two-point excitation on the stability boundaries of auto-parametric resonance are studied for the test *** with the experiment,the numerical instability predictions of auto-parametric resonance are consistent with the test phenomena,and the numerical stability boundaries of auto-parametric resonance agree with the experimental *** a continuous beam bridge,when the ratio of multipoint excitation frequency(applied to the columns)to natural frequency of the continuous girder is approximately equal to 2,the continuous beam may undergo a strong auto-parametric *** with the present experiment and analysis,a hypothesis of Volgograd Bridge's serpentine vibration is discussed.

Non-Convex Optimization of Resource Allocation in Fog Computing Using Successive Approximation

Fog computing can deliver low delay and advanced IT services to end users with substantially reduced energy ***,with soaring demands for resource service and the limited capability of fog nodes,how to allocate and manage fog computing resources properly and stably has become the ***,the paper investigates the utility optimization-based resource allocation problem between fog nodes and end users in fog *** authors first introduce four types of utility functions due to the diverse tasks executed by end users and build the resource allocation model aiming at utility ***,for only the elastic tasks,the convex optimization method is applied to obtain the optimal results;for the elastic and inelastic tasks,with the assistance of Jensen’s inequality,the primal non-convex model is approximated to a sequence of equivalent convex optimization problems using successive approximation ***,a two-layer algorithm is proposed that globally converges to an optimal solution of the original ***,numerical simulation results demonstrate its superior performance and *** with other works,the authors emphasize the analysis for non-convex optimization problems and the diversity of tasks in fog computing resource allocation.
