Hepatitis B and inflammatory bowel disease: Role of antiviral prophylaxis

Hepatitis B virus (HBV) is a very common infection worldwide. Its reactivation in patients receiving immunosuppression has been widely described as being associated with significant morbidity and mortality unless anti-viral prophylaxis is administered. Treatment in inflammatory bowel disease (IBD) patients has changed in recent years and immunosuppression and biological therapies are now used more frequently than before. Although current studies have reported an incidence of hepatitis B in inflammatory bowel disease patients similar to that in the general population, associated liver damage remains an important concern in this setting. liver dysfunction may manifest in several ways, from a subtle change in serum aminotransferase levels to fulminant liver failure and death. Patients undergoing double immunosuppression are at a higher risk, and reactivation usually occurs after more than one year of treatment. As preventive measures, all IBD patients should be screened for HBV markers at diagnosis and those who are positive for the hepatitis B surface antigen should receive antiviral prophylaxis before undergoing immunosuppression in order to avoid HBV reactivation. Tenofovir/entecavir are preferred to lamivudine as nucleos(t)ide analogues due to their better resistance profile. In patients with occult or resolved HBV, viral reactivation does not appear to be a relevant issue and regular DNA determination is recommended during immunosuppression therapy. Hepatitis B and immunosuppressive therapies for chronic inflammatory diseases: When and how to apply prophylaxis, with a special focus on corticosteroid therapy

Currently immunosuppressive and biological agentsare used in a more extensive and earlier way in patients with inflammatory bowel disease, rheumatic or dermatologic diseases. Although these drugs have shown a significant clinical benefit, the safety of these treatments is a challenge. Hepatitis B virus(HBV) reactivations have been reported widely, even including liver failure and death, and it represents a deep concern in these patients. Current guidelines recommend to preemptive therapy in patients with immunosuppressants in general, but preventive measures focused in patients with corticosteroids and inflammatory diseases are scarce. Screening for HBV infection should be done at diagnosis. The patients who test positive for hepatitis B surface antigen, but do not meet criteria for antiviral treatment must receive prophylaxis before undergoing immunosuppression, including corticosteroids at higher doses than prednisone 20 mg/d during more than two weeks. Tenofovir and entecavir are preferred than lamivudine because of their better resistance profile in long-term immunosuppressant treatments. There is not a strong evidence, to make a general recommendation on the necessity of prophylaxis therapy in patients with inflammatory diseases that are taking low doses of corticosteroids in short term basis or low systemic bioavailability corticosteroids such as budesonide or beclomethasone dipropionate. In these cases regularly HBV DNA monitoring is recommended, starting early antiviral therapy if DNA levels begin to rise. In patients with occult or resolved hepatitis the risk of reactivation is much lower, and excepting for Rituximab treatment, the prophylaxis is not necessary.

Remediation of amide pesticide-polluted soils by combined solarization and ozonation treatment

Agriculture has a close relationship with nature,but it can also be the source of negative and permanent environmental *** use of pesticides in modern agriculture is a common practice,but their side effects on the environment cannot be *** this study,we evaluated a combination of solarization and ozonation techniques for the elimination of six amide pesticides(boscalid,chlorantraniliprole,cyflufenamid,fluopyram,napropamide,and propyzamide)in *** experiments were performed with four different soils to assess the efficiency of this methodology at different soil temperatures and ozone dosages under laboratory conditions,and then a greenhouse pot experiment was conducted under controlled conditions during *** days after the onset of the experiments,higher degradation percentages of amide pesticides were observed in ozonized soils than in other treated soils,particularly when ozone was applied at 10 cm soil *** results show that the utilization of ozonation,along with solarization,represents a valid method for degrading residues of the studied pesticides and suggest that this combined technology may be a promising tool for remediating pesticide-polluted soils.

Objective: Extracorporeal shockwave lithotripsy (SWl) currently plays an important role in the treatment of urinary tract lithiasis. The purpose of this article was to describe new concepts and procedural strategies that would improve results using SWl as a treatment for urolithiasis, thereby achieving better clinical ***: A systematic review process was carried in PubMed/PMC from January 2003 to March 2023. A narrative synthesis of the most important aspects has been ***: The important recommendations for the adequate selection of the candidate patient for treatment with SWl are summarized, as well as the new strategies for a better application of the technique. Aspects about intraoperative position, stone localization and monitoring, analgesic control, machine and energy settings, and measures aiming at reduced risk of complications are ***: To achieve the therapeutic goal of efficient stone disintegration without increasing the risk of complications, it is necessary to make an adequate selection of patients and to pay special attention to several important factors in the application of treatment. Technological development in later generation devices will help to improve current SWl results.

Social media is commonly used by governments to disseminate information related to transparency and institutional promotion,but its potential for the dissemination of cultural resources is often *** research proposes a strategy for adopting social media to achieve cultural dissemination in the municipality of lamas,Peru,which has tourism potential and has been awarded the“Best Tourism Villages”seal of the World Tourism ***,we propose a case study at a propositional exploratory *** start by determining the current state of dissemination of tangible and intangible resources,as well as the practical use of social media,through a survey of 20 *** results reveal that crafts are the most widely disseminated tangible resource in the municipality(according to 55%of officials),and intangible resources are occasionally disseminated(50%).We also find that Facebook is the only digital channel used to disseminate cultural *** this context,we formulate a strategy for adopting social media to improve cultural dissemination,which consists of six phases:(i)forming a work team,(ii)preparing a budget plan,(iii)strengthening capacities,(iv)taking inventory of cultural resources,(v)coordinating community interests and(vi)preparing a cultural dissemination *** findings can contribute to formulating public initiatives for the institutionalisation of social media in governments for the purposes of preserving,promoting and guaranteeing the accessibility of cultural heritage.

Plant triterpenoid saponins function as susceptibility factors to promote the pathogenicity of Botrytis cinerea

The present study aimed to evaluate multi-element concentrations(Al,As,Ca,Cd,Co,Cr,Cu,Fe,Mg,Mn,Mo,Ni,Pb,Sr)in suspended particulate material(SPM)collected from Ensenada de la Paz(ElP)lagoon,Baja California Sur,Mexico in two different periods(September and May)to comprehend their origin,geochemical behavior,mass transfer and associated ecotoxicological *** 24 hr variation coefficient of volumetric SPM levels were found to be 51.7%in September and 40.5%in May,signifying the effects of oceanic *** calculated enrichment factor(EF)values for all the studied elements were of higher magnitude because of the high surface area and oxide nature of SPM,and in this study,Mo had the highest EF of 46.77 probably due to its origin from continental *** the ecotoxicological perspective,the integrated toxic risk index revealed low toxic risk to the benthic ***,the mean-ERM-Quotient calculated using the particulate concentrations of As,Cd,Cr,Cu,Ni,Pb indicated 9%probability of toxicity to *** comprehensive geochemical and ecotoxicological assessment of particulate metal concentrations in the ElP lagoon signify low to moderate contamination.

Phage lytic proteins: a natural approach to agro-food safety

Sustainability is a leading trend in the context of food production. Additionally， consumers increasingly demand safer and less-processed products. Among the different technologies used to maintain the quality and extend the shelf-life of fresh and minimally-processed food， natural antimicrobial agents offer a promising strategy to replace conventional compounds. In this regard， phage lytic proteins or lysins， such as endolysins and virion-associated peptidoglycan hydrolases (VAPGHs)， have been proposed as a viable option for the avoidance and elimination of undesirable bacteria within the food production chain. Even when applied exogenously， these proteins can degrade the bacterial cell wall maintaining their lytic activity. This feature， alongside their modular structure， which can be exploited for bioengineering， provides significant biotechnological potential. However， despite the promising properties of lysins， the main obstacle for their commercialization is the limited legal information regulating their use. This challenge underscores the need to navigate complex regulatory pathways. The primary objective of this review is to address this crucial gap and summarize the many prospective applications of endolysins during the different stages of food production. By doing so， we aim to provide clarity and insight into the regulatory challenges that must be overcome for the successful utilization of lysins.
