Incidence of Primary Open-Angle Glaucoma at the Flamboyants Communal Medical Center of Conakry, Guinea

Background: Primary Open-Angle Glaucoma (POAG) is a chronic, progressive optic neuropathy of multifactorial origin defined by alterations in the visual field, progressive loss of optic nerve fibers and ganglion cells, and an open iridocorneal angle in gonioscopy. It is the world’s leading cause of irreversible blindness. There is no epidemiological study in Guinea that allows us to locate the real level of the problem. The objective of this study is to determine the hospital incidence of POAG at the Centre Médical Communal (CMC) in the Flamboyants. Patients and Method: This was a descriptive cross-sectional study with a prospective collection for one year, from April 1, 2020 to March 31, 2021, in patients aged at least 40 years, received in consultation at the CMC in the Flamboyants. The diagnostic criterion for a new case of glaucoma was the presence in a patient of the following two signs: (excavation of the optical disc C/D ≥ 0.5 and alteration of the visual field) with an open angle at least stage 4 of the Shaffer and Etienne classification associated or not with ocular hypertonia and having never received anti glaucoma treatment. We described variables related to sociodemographic characteristics, visual acuity, intraocular pressures, papillary excavations, and visual field surveys. Informed consent from patients was sought and obtained. The confidentiality of the files was guaranteed. Results: 68 new cases of POAG were diagnosed out of 3220 consultations, an incidence rate of 2.11%. The average age was 58.2 years (10.64 with a male predominance of 63.23%. A family history of glaucoma was present in 11.36% of cases. Visual acuity was poor Conclusion: This study has shown us that POAG in our environment is relatively common, early onset, and rapidly evolving. The implementation of a rapid and reliable screening strategy by the Eye Health Program will make it possible to manage glaucoma from the earliest stages.

Functional Results without Correction of Cataract Surgery during a Free Campaign at Siguiri Prefectural Hospital in Guinea

Background: Cataract is the leading cause of blindness in the world and its treatment is exclusively surgical. However, the results of cataract surgery are not always as good as is assumed and this aspect of surgical services needs a lot of attention. The purpose of this study is to evaluate the functional results of the free cataract surgery campaign carried out at the Siguiri Prefectural Hospital in Guinea. Patients and Method: This was a descriptive and retrospective cross-sectional study over a period of three months (1 October-31 December 2017). Records of cataract-operated patients during the study period were reviewed. The complete files for the main variables sought were retained. Sociodemographic data and visual acuity without pre- and post-operative correction were assessed. Functional outcomes were analysed according to WHO guidelines and recommendations for post-operative functional outcomes of cataract surgery. The data was collected and processed using Excel version 2010 software. Results: A total of 108 eyes from 102 patients were the subject of this present study on 314 eyes from 289 operated patients. 64 (%) men and 38 women (%) had a follow-up of 6 weeks after cataract surgery, a sex ratio of 1.68 in favor of women. The average age was 62.45 ± 15.20 years with extremes of 25 and 90 years. The age group from 60 to 69 years is the most represented with 46 cases or 45.45%. Housewives and farmers are the most represented strata with 42 cases (41.18%) and 31 cases (30.39%) respectively. The operative technique was Phaco A in 4/5 of the cases. No implant calculations were performed. Functional results indicate that 66% of patients had good visual acuity (≥3/10);29% of patients had borderline visual acuity (between 1/10 and 2/10) and 13.30% had poor visual acuity (Conclusion: The importance of the rate of poor results in our study should draw our attention to the conduct of free cataract surgery campaigns. Introduction: Eye trauma represents all morbid lesions on the eyeball due to external violence. In children, they are an important cause of morbidity and the leading cause of monocular blindness. The aim of this study was to determine the sociodemographic, clinical and therapeutic characteristics of eye trauma in children aged 0 to 16 at the Application Centre for the Diploma of Specialized Higher Studies in Ophthalmology (CADESSO) in Donka, Guinea. Patients and Method: This was a prospective longitudinal study with descriptive purposes from January 1 to December 31, 2019 involving 205 children victims of eye trauma. Variables related to sociodemographic, clinical and therapeutic characteristics were studied. Informed consent from patients and/or their parents was sought and obtained. The confidentiality of the files was guaranteed. The data entry was done on EPI-Info version 7 and the analysis using the Stata software. The tables were made in Excel 2010. Pearson’s Chi2 test was used for the comparison of proportions. Results: Pediatric eye trauma accounted for 9.31% of pediatric visits and 30.92% of all eye trauma cases. There was a male predominance with a sex ratio of 1.59. Age groups from 0 - 4 years;5 - 8 years and 9 - 12 were the most affected with 26.8%, 28.8% and 27.3% respectively and a decrease in the number of cases in the 13 - 16 age group (17.1%). Gambling and sports accidents accounted for 33.2%, followed by domestic accidents for 16.6% and physical injury for 8.2%. Eye contusion was the most represented at 66.3% followed by wounds of the globe 23.9% and burns at 5.9%. One hundred and twenty-six patients had received a determination of visual acuity (VA) at admission;99 were assessed at discharge and 27 were lost to follow-up. The VA ≥ 5/10th increased from 43.4% at admission to 79.8% after treatment. We had found 67% of vision ≥ 5/10 in eye bruises against 5% in eyeball wounds. The Séchilienne (France) Landslide:a Non-typical Progressive Failure Implying Major Risks

The National Road RN 91 has been threatened for about twenty-five years by a huge landslide, located 25 km south-east to the town of Grenoble (France). If several million cubic meters of rock fall down, the debris will dam the valley. Then the failure of the dam by overtopping and rapid erosion might result in a catastrophic flood and dramatic consequences for human life, environment and economy throughout the valley. The paper presents the hazard assessment based on geological and hydrological surveys, including small scale hydraulic tests, as well as the risk evaluation that has been performed. The risk management relies first upon a high level monitoring and an emergency plan; various mitigation strategies have been considered.

Experimental evidence of obesity as a risk factor for severe acute pancreatitis

The incidence of acute pancreatitis, an inflammation of the pancreas, is increasing worldwide. Pancreatic injury is mild in 80%-90% of patients who recover without complications. The remaining patients may develop a severe disease with local complications such as acinar cell necrosis, abscess and remote organ injury including lung injury. The early prediction of the severity of the disease is an important goal for physicians in management of patients with acute pancreatitis in order to optimize the therapy and to prevent organ dysfunction and local complications. For that purpose, multiple clinical scale scores have been applied to patients with acute pancreatitis. Recently, a new problem has emerged： the increased severity of the disease in obese patients. However, the mechanisms by which obesity increases the severity of acute pancreatitis are unclear. Several hypotheses have been suggested：（1） obese patients have an increased inflammation within the pancreas; （2） obese patients have an increased accumulation of fat within and around the pancreas where necrosis is often located; （3） increase in both peri- and intra-pancreatic fat and inflammatory cells explain the high incidence of pancreatic inflammation and necrosis in obese patients; （4） hepatic dysfunction associated with obesity might enhance the systemic inflammatory response by altering the detoxiflcation of inflammatory mediators; and （5） ventilation/perfusion mismatch leading to hypoxia associated with a low pancreatic flow might reduce the pancreatic oxygenation and further enhance pancreatic injury. Recent experimental investigations also show an increased mortality and morbidity in obese rodents with acute pancreatitis and the implication of the adipokines leptin and adiponectin. Such models are important to investigate whether the inflammatory response of the disease is enhanced by obesity. Dimensioning a stockpile operation using principal component analysis

Mineral processing plants generally have narrow tolerances for the grades of their input raw materials,so stockpiles are often maintained to reduce material variance and ensure ***,designing stockpiles has often proven difficult when the input material consists of multiple sub-materials that have different levels of variances in their *** this paper,we address this issue by applying principal component analysis(PCA)to reduce the dimensions of the input *** study was conducted in three ***,we applied PCA to the input data to transform them into a lower-dimension space while retaining 80% of the original ***,we simulated a stockpile operation with various geometric stockpile configurations using a stockpile simulator in *** used the variance reduction ratio as the primary criterion for evaluating the efficiency of the ***,we used multiple regression to identify the relationships between stockpile efficiency and various design parameters and analyzed the regression results based on the original input variables and principal *** results showed that PCA is indeed useful in solving a stockpile design problem that involves multiple correlated input-material grades.

Novel imaging biomarkers of response to transcatheter arterial chemoembolization in hepatocellular carcinoma patients

Hepatocellular carcinoma common cause of cancer death （HCC） is the third most worldwide （1）. Most patients present with intermediate or advanced disease that is not amenable to curative treatment, and the median survival in this group is 6-8 months （2）. Several studies and well- designed randomized trials have shown a positive effect of transcatheter arterial chemoembolization （TACE） on patient outcome and survival （3-8）. As nicely described in the present article from Wring et al., assessment of tumor response is of extreme importance in patients undergoing locoregional treatments of liver cancer （9）. Early assessment of the effectiveness of TACE and monitoring of tumor response are paramount to the identification of treatment failure, guidance of future therapy, and determination of the interval for repeat treatment.

Ultrafast Synthesis of Calcium Vanadate for Superior Aqueous Calcium-Ion Battery

Recently,multivalent aqueous calcium-ion batteries(CIBs)have attracted considerable attention as a possible alternative to Li-ion ***,traditional Ca-ion storage materials show either limited rate capabilities and poor cycle life or insufficient specific ***,we tackle these limitations by exploring materials having a large interlayer distance to achieve decent specific capacities and one-dimensional architecture with adequate Ca-ion passages that enable rapid reversible(de)intercalation *** this work,we report the high-yield,rapid,and low-cost synthesis of 1D metal oxides MV_(3)O_(8)(M=Li,K),CaV_(2)O_(6),and CaV_(6)O_(16)·7H_(2)O(CVO)via a molten salt ***,using 1D CVO as electrode materials,we show high capacity 205 mA h g^(-1),long cycle life(>97%capacity retention after 200 cycles at 3.0 C),and high-rate performance(117 mA h g^(-1) at 12 C)for Ca-ion(de)*** work represents a step forward for the development of the molten salt method to synthesize nanomaterials and to help pave the way for the future growth of Ca-ion batteries.
