Understanding pediatric surgical needs in North Korea: a modeling analysis

Background:To examine the baseline morphological characteristics and alterations in intraretinal microvascular abnormalities(IRMAs)in response to anti-vascular endothelial growth factor(anti-VEGF)treatment,documented by optical coherence tomography angiography(OCTA)in diabetic ***:In this retrospective study,IRMAs were evaluated with multimodal imaging(fundus photography,fluorescein angiography,OCTA)in treatment-naïve diabetic eyes before and after anti-VEGF treatment for diabetic macular edema(DME)and/or proliferative diabetic retinopathy(PDR)and compared to diabetic control eyes with similar diabetic retinopathy(DR)severity that did not receive anti-VEGF *** morphological characteristics of IRMAs on enface OCTA imaging were graded by masked readers at baseline,then after anti-VEGF therapy in treated eyes or after observation in control *** of interval changes in an IRMA were based on the following parameters:branching,vessel caliber and area of adjacent capillary ***:The treated group included 45 IRMA foci from 15 eyes of 11 patients,while the control group included 27 IRMA foci from 15 eyes of 14 *** anti-VEGF treatment,enface OCTA demonstrated that 14 foci of IRMA(31%)demonstrated regression with normalization of appearance of the capillary bed,20 IRMAs(44%)remained unchanged,six IRMAs(13%)progressed with enlargement or development of new IRMAs and five IRMAs(11%)demonstrated complete obliteration defined as IRMA disappearance with advancing capillary *** the control group,17 IRMA(63%)remained stable,8 IRMAs(29.6%)progressed and 2 experienced total obliteration(7.4%).The difference in rank order between the two groups was statistically significant(p=0.022).Conclusions:In eyes with DR status post anti-VEGF therapy,foci of IRMAs have a variable course demonstrating one of four possible outcomes:regression,stabilit,progression or complete *** contrast,none of the untreated contr

Density Functional Calculations of the Mechanical, Electronic and Dynamical Properties of Antiperovskite Ca₃BO (B = Pb, Ge, Sn)

An analysis of mechanical, electronic and dynamical properties of antiperovskite Ca3BO (B = Pb, Ge, Sn) in cubic phase space group Pm-3m (221) has been studied using first principle density functional theory (DFT). Ground state energy computation was done using the Projector Augmented Wave (PAW) Pseudo Potentials and the Plane Wave (PW) basis set. The Generalized Gradient Approximation (GGA) was used for the exchange correlation. The open source code QUANTUM ESPRESSO (QE) was used in this study in which plane wave basis sets are applied for the expansion of the electronic structure wave function. Thermo_pw as a post-processing code was used for the computation of mechanical properties including bulk modulus and elastic constants with their derivatives. The lattice parameters are here calculated to be 4.87 Å, 4.86 Å and 4.84 Å for Ca3BO (B = Pb, Ge, Sn) respectively which compares well with other works. This also shows that the three crystals are similar in size and in most of their properties. In addition to this, projected density of states and band structure are also computed both showing that these materials are of semi-metallic nature and are stable in cubic phase. The COVID-19 Pandemic: Present or Gone, Where Do Adolescents Stand?

Introduction: The initial global response to the SARS-CoV-2 pandemic focused on adults as adolescents mainly had subclinical infections. Adolescents, however, could transmit the virus and hence be a risk to the elderly, young children and immunocompromised. Adolescents are also at risk of long-term complications. This study sought to determine the prevalence of antibodies to SARS-CoV-2 among adolescents in Southern Nigeria, and to determine the association between SARS-CoV-2 seropositivity and age, gender, socioeconomic class, school type and nutritional status. Methods: This descriptive cross-sectional study was carried out in Edo state of southern Nigeria. We recruited 270 in-school adolescents aged 10 - 19 years. A WHO-adapted, pre-tested questionnaire was used to obtain data on participant history and risk factors. The questionnaire was interviewer-administered. Adolescents whose parents gave consent were recruited through a multi-stage randomized sampling method. Capillary blood for the assay was collected and analysed using a lateral-flow immunoassay device. The primary outcome was serostatus of the study. The positive predictors of serostatus were finally determined using a multiple logistics regression model. Statistical significance was set at p Results: The seroprevalence of SARS-CoV-2 was 45.9%. IgM antibodies were found in 5.6% of participants. Older adolescents had higher seropositivity rates than younger adolescents (2 = 8.101, p = 0.017). There was no association between gender and seropositivity. Adolescents in public schools were more likely to be seropositive than those in private schools (2 = 25.242, p = 0.001). A higher seroprevalence was noticed among study participants in the lower socioeconomic class (x2 = 6.928, p = 0.031), and those in overcrowded classes. (2 = 79.303, p = 0.001). Development of a Long-Term Operational Optimization Model for a Building Energy System Supplied by a Geothermal Field

In order to reduce energy consumption and CO_(2) emissions in the building sector, more and more renewable energy sources are integrated into energy systems. Especially geothermal fields combined with heat pumps are able to supply buildings with heat and cold at low carbon emissions. However, using geothermal fields as heat and cold source influences the ground temperature. Consequently, the ground temperature can change dramatically over a building’s lifetime, leading to less efficient operation of the energy system. Therefore, a sustainable operation is required to ensure the long-term efficiency of geothermal fields. In this paper, we develop an optimization model to derive operating strategies for an efficient long-term operation of a building energy system coupled to a geothermal field. The investigated energy system is the main building of the *** Energy Research Center in Aachen, Germany, which includes a heat pump, two boilers, a combined heat, and power unit, a glycol cooler, and a geothermal field with 41 probes. For each component, we develop energy-based sub-models, which are connected to form the overall system. The geothermal field is modeled by using a g-functions approach as well as a simplified resistance-capacitance approach. To achieve short computing times and realize an optimization horizon of several years, the optimization problem is formulated as mixed-integer linear programming(MILP). The developed model is optimized regarding two different objectives: the minimization of energy costs and the minimization of long-term temperature changes in the ground. Conclusions for an efficient and sustainable operation of the field, especially for the cooling supply, can be derived from the optimization results. It is shown that a state of equilibrium should be aimed to achieve an energy-efficient operation, in which the temperature of the field is close to the initial ground temperature.
