背景：有关药物和介入治疗对ST段抬高型和非ST段抬高型急性冠脉综合征（non—ST-segment elevation acute coronary syndrome，NSTE ACS）患者的作用，随机试验已经提供了足够的证据，但是是否已经转化为相应的临床变化还不清楚。目的：确定ST段抬高型心肌梗死（ST-segment elevation myocardial infarction，STEMI）和NSTE ACS患者的院内治疗改变是否伴随临床结果的改善。设计、地点及患者：全球急性冠脉事件注册研究（Global Registry of Acute Coronary Events，GRACE）是一项多国家队列研究。自1999年7月1日至2006年12月31日，该研究共入选并随访了来自14个国家、113家医院的44372例ACS患者。主要观测指标：当前循证药物和介入治疗应用的趋势及患者预后（死亡、充血性心力衰竭、肺水肿、心源性休克、卒中和心肌梗死）。结果：随着研究时间的进展，相关药物的应用增加（β-受体阻滞剂、他汀类药物、血管紧张素转换酶抑制剂、行或未行经皮冠状动脉介入治疗[percutaneous coronary intervention，PCI]患者应用噻氯毗啶、糖蛋白Ⅱb／Ⅲa抑制剂和低分子肝素；P〈0．001）。STEMI患者药物再灌注治疗下降了22个百分点（95％可信区间[confidence interval，CI]，-27～-17），而直接PCI增加了37个百分点（95％CI，33—41）。在NSTEMI患者中，PCI的比例显著升高了18个百分点（95％CI，15～20）。充血性心力衰竭和肺水肿在STEMI患者中降低了9个百分点（95％CI，-12～-6），在NSTEACS患者中降低了6．9个百分点（95％CI，-8．4～-4．7）。STEMI患者的院内死亡率降低了18个百分点（95％CI，-5．3～-1．9），心源性休克降低了24个百分点（95％CI，-4．3～-0．5）。校正了危险评分后，NSTEACS患者的院内死亡率降低了0．7个百分点（95％CI，-1．7～0．3）；STEMI患者6个月随访时卒中发生率降低了0．8个百分点（95％CI，-1．7～0．1），心肌梗死发生率降低了2．8个百分点（95％CI，-6．4～0．9）。NSTEACS患者6个月死亡率降低了1．6个百分点（95％CI，-3．0～-0．1），卒中发生率降低了0．7个百分点（95％CI，-1．4～0．1）。结论：在本项多国家观察性研究中，ACS患者治疗的改善与6个月新发心力衰竭、死亡率、卒中和心肌梗死发生率显著降低相关。

AIM:To study if impaired renal function is associated with increased risk of peri-infarct heart failure (HF) in patients with preserved ejection fraction (EF).METHODS:Patients with occluded infarct-related arteries (IRAs) between 1 to 28 d after myocardial infarction (MI) were grouped into chronic kidney disease (CKD) stages based on estimated glomerular filtration rate (eGFR).Rates of early post-MI HF were compared among eGFR *** regression was used to explore independent predictors of ***:Reduced eGFR was present in 71.1% of 2160 patients,with significant renal impairment (eGFR < 60 mL/min every 1.73 m2) in 14.8%.The prevalence of HF was higher with worsening renal function:15.5%,17.8% and 29.4% in patients with CKD stages 1,2 and 3 or 4,respectively (P < 0.0001),despite a small absolute difference in mean EF across eGFR groups:48.2 ± 10.0,47.9 ± 11.3 and 46.2 ± 12.1,respectively (P=0.02).The prevalence of HF was again higher with worsening renal function among patients with preserved EF:10.1%,13.6% and 23.6% (P < 0.0001),but this relationship was not significant among patients with depressed EF:27.1%,26.2% and 37.9% (P= 0.071).Moreover,eGFR was an independent correlate of HF in patients with preserved EF (P=0.003) but not in patients with depressed EF (P=0.181).CONCLUSION:A significant proportion of post-MI patients with occluded IRAs have impaired renal *** renal function was associated with an increased rate of early post-MI HF,the association being strongest in patients with preserved *** findings have implications for management of peri-infarct HF.

Impact of the Intermittency of Photovoltaic Power Plants on the Frequency Management: Case of the Senegalese Electricity Grid

This paper investigates the impact of the intermittency of the photovoltaic (PV) power plants on the electric grid frequency management and the consequences that can occur. To meet these purposes, three years data of the Senegalese grid frequency are used, combined with three PV power plants data since their starting-up. Using these data, we analyzed two days of heavily disturbed operation of the Senegalese grid in order to emphasize the real impact of the presence of PV power plants on the electrical grid. Our study has highlighted that how the intermittency of photovoltaic power plants affects frequency management. It obviously appears that the PV production curves follow the irradiation variations. Instantaneous variations of PV production make it difficult to manage the grid frequency leading to more frequent load shedding as we have shown with the data after the commissioning of the PV power plants. We also estimate the power then energy losses during the load shedding induced by the PV integration on the power grid by comparing a period corresponding to one month before (February 2017) and one month after (February 2018) the integration of variable PV production.

高龄、糖尿病(DM)和慢性肾脏疾病(CKD)不仅会增加慢性冠状动脉综合征(CCS)患者缺血事件的风险,还会增加其接受抗血小板治疗期间的出血风险。此类特殊人群可能需要调整治疗方案,尤其是亚洲的特殊人群,往往表现出与西方人群不同的临床特征。关于亚洲人群CCS高风险的分类和急性冠状动脉综合征(ACS)后使用新一代强效P2Y12受体抑制剂(如替格瑞洛和普拉格雷)的国际指南已相继发表。本共识总结了在特殊人群中使用强效P2Y12受体抑制剂的相关证据,提出了在冠状动脉疾病(CAD)特殊人群中应用标准疗程双联抗血小板治疗(DAPT)、短期DAPT和单一抗血小板治疗的推荐意见。本共识的特殊人群包括从ACS过渡至CCS,老年,或患有CKD、DM、多血管CAD和治疗期间发生出血并发症等特征的患者。

Tunnel junctions in a III-V nanowire by surface engineering

We demonstrate a simple way of fabricating high performance tunnel devices from p-doped InAs nanowires by tailoring the n-doped surface accumulation layer inherent to InAs surfaces. By using appropriate ammonium sulfide based surface passivation before metallization without any further thermal treatment, we demonstrate characteristics of tunnel p-n junctions, namely Esaki and backward diodes, with figures of merit better than previously published for InAs homojunctions. The further optimization of both the surface doping, in a quantitative way, and the device geometry allows us to demonstrate that these nanowire-based technologically-simple diodes have promising direct current characteristics for integrated high frequency detection or generation.

Context: Atherothrombosis is the leading cause of cardiovascular morbidity and mortality around the globe. To date, no single international database has characterized the atherosclerosis risk factor profile or treatment intensity of individuals with atherothrombosis. Objective: To determine whether atherosclerosis risk factor prevalence and treatment would demonstrate comparable patterns in many countries around the world. Design, Setting, and Participants: The Reduction of Atherothrombosis for Continued Health(REACH) Registry collected data on atherosclerosis risk factors and treatment. A total of 67 888 patients aged 45 years or older from 5473 physician practices in 44 countries had either established arterial disease(coronary artery disease[CAD], n=40 258; cerebrovascular disease, n=18843; peripheral arterial disease, n=8273) or 3 or more risk factors for atherothrombosis(n=12 389) between 2003 and 2004. Main Outcome Measures: Baseline prevalence of atherosclerosis risk factors, medication use, and degree of risk factor control. Results: Atherothrombotic patients throughout the world had similar risk factor profiles: a high proportion with hypertension(81.8% ), hypercholesterolemia(72.4% ), and diabetes(44.3% ). The prevalence of overweight(39.8% ), obesity(26.6% ), and morbid obesity(3.6% ) were similar in most geographic locales, but was highest in North America(overweight: 37.1% , obese: 36.5% , and morbidly obese: 5.8% ; P< .001 vs other regions). Patients were generally undertreated with statins(69.4% overall; range: 56.4% for cerebrovascular disease to 76.2% for CAD), antiplatelet agents(78.6% overall; range: 53.9% for ≥ 3 risk factors to 85.6% for CAD), and other evidence-based risk reduction therapies. Current tobacco use in patients with established vascular disease was substantial(14.4% ). 背景：目前，有关社区中动脉粥样硬化血栓形成稳定患者心血管（cardiovascular，CV）事件发生率的资料比较少，既往也无国际性队列研究对冠状动脉病（coronary artery disease，CAD）、脑血管病（cerebrovascular disease，CVD）、外周动脉病（peripheral arterial disease，PAD）确诊患者以及此类疾病高危患者不同事件的发生率进行评估。目的：在患有动脉疾病的门诊患者或存在多个动脉粥样硬化血栓形成危险因素的门诊患者中确定当今世界1年内CV事件的发生率。设计、地点及参试者：“减少动脉粥样硬化血栓形成维系健康（Reduction of Artherotllrombosis for Continued Health，REACH）”注册研究是一项国际前瞻性队列研究。研究人员于2003—2004年从44个国家、5587名医生入选68236例已确诊的动脉粥样硬化性血管病患者（CAD、PAD、CVD；n=55814）以及至少存在3个动脉粥样硬化血栓形成危险因素的患者（n=12422）。主要观测指标：CV死亡、心肌梗死（myocardial infarction，MI）及卒中的发生率。结果：截至2006年7月，共有95．22％的患者（n=64977）提供了1年内的结局。心血管死亡、MI及卒中的总体发生率为4．24％；动脉粥样硬化性血管病患者为4．69％，仅存在多个危险因素的患者为2．15％。CAD、CVD及PAD患者CV死亡、MI及卒中的总体发生率分别为4．52％、6．47％和5．35％。CAD、CVD及PAD患者发生终点事件（即CV死亡、MI、卒中）以及因动脉粥样硬化血栓形成事件入院的几率分别为15．20％、14．53％和21．14％。上述事件发生率随症状性动脉疾病病变部位数量的增加而增加，仅存在危险因素的患者为5．31％，有1处症状性动脉疾病的患者为12．58％，有2处症状性动脉疾病的患者为21．14％，有3处症状性动脉疾病的患者为26．27％（趋势P〈0．001）。结论：在这项当代大型国际研究中，患有动脉粥样硬化性血管病的门诊患者以及有动脉粥样硬化血栓形成危险的门诊患者CV事件的年发生率相对较高。存在多部位病变者CV事件的1年发生率升高。

背景：动脉粥样硬化血栓形成是导致心血管疾病和死亡的主要原因。至今，尚无国际数据库阐述动脉粥样硬化血栓形成患者动脉硬化危险因素的特征和治疗强度。目的：确定动脉粥样硬化危险因素的发生及治疗在世界许多国家是否具有可比性。设计、地点及参试者：“降低动脉粥样硬化血栓形成保持健康研究”（Reduction of Atherothrombosis for Continued Health，REACH）搜集了动脉粥样硬化危险因素及治疗情况的数据。从2003年到2004年，44个国家、5473位内科从业医师总共发现67888例年龄≥45岁的动脉病患者（冠心病[coronaryartery disease，CAD]，n=40258；脑血管病，n=18843；周围动脉病，n=8273）或≥3个动脉粥样硬化血栓形成危险因素（n=12389）。主要观测指标：动脉粥样硬化危险因素的基线发生率、药物应用情况及危险因素控制程度。结果：全世界动脉粥样硬化血栓形成患者具有相似的危险因素，即高血压（81．8％）、高胆固醇血症（72．4％）以及糖尿病（44．3％）的比例高。超重（39．8％）、肥胖（26．6％）以及病态肥胖（3．6％）的发生率在大部分地区相似，但是以北美最高（超重：37．1％，肥胖：36．5％，病态肥胖：5．8％；与其他地区比较P〈0．001）。患者采用他汀（总共69．4％；范围：从脑血管病的56．4％到CAD的76．2％）、抗血小板治疗（总共78．6％；范围：从≥3个危险因素的53．9％到CAD的85．6％）及其他循证（降低危险）普遍不足。确诊有血管病的患者在吸烟者为数可观（14．4％）。高血压病治疗不足（基线50．0％血压升高）、高血糖诊断不足（4．9％）以及空腹血糖受损（其中36．5％不知已有糖尿病）均很常见。在有症状的动脉粥样硬化血栓形成患者中，15．9％是有症状的多血管疾病患者。结论：这项当代大型国际数据库研究显示，在世界许多地区典型心血管危险因素的发生是一致且常见的，但是大多治疗不足、控制不得力。

Pelagic fish,including sardines and anchovies(Order Clupeiformes),are the most common species taken by artisanal marine fisheries along the Togolese *** investigated fisher involvement as well as fish captures over a period of 10 years,particularly of the European anchovy(Engraulis encrasicolus).Our results indicate that 60%of fishers operated from the Loméfishing harbour,most fishers being Ghanaians working seasonally in Togo.63.7%of all the fishers used canoes with outboards,a higher percentage compared to the previous *** fishing gear type were identified,with bottom gillnet and surface gillnet being the most commonly ***,in the most important fishing camp in the country in terms of fish production(Loméfishing harbour),all fishers used shark ***,fisheries catches did not change significantly across years,but anchovy fishing effort and catch per unit of effort(CPUE)declined over the study period,suggesting some depletion of the species stocks on the Togolese coast and a demotivation of fishers.
