management of recto-vaginal fistulas after prosthetic reinforcement treatment for pelvic organ prolapse

AIm: To communicate our findings on successful treat-ment of recto-vaginal fistulas (RVFs) after prosthetic reinforcement surgery of pelvic organ prolapse (POP). mETHODS: A retrospective single center study between 1998 and 2008 was performed. A total of 80 patients with RVF were identified, of which five patients (6%), with a mean age of 65 years (range: 52-73), had undergone previous surgery for POP with pros-thetic reinforcement. RESULTS: All patients complained about ongoing vaginal infections and febrile episodes. These symptomswere reported after a mean period of 18 mo after POP repair. As a first intervention, three patients underwent ablation of the prosthetic material (Pm). As a second intervention, open proctectomy with a primary anas-tomosis, an omental patch, and a protective ileostomy were performed in two patients. One patient required a terminal colostomy due to complete destruction of the anal sphincters. In two other patients, ablation of the Pm and proctectomy was performed as a one-step procedure. The postoperative course in all patients was uneventful, with a mean length of hospitalization of 20 d (range: 15-30). Closure of the ileostomy was achieved in all four patients within four months. After a mean period of 35 mo (range: 4-60) of follow-up, no recurrence was observed with normal continence in four ***: In our experience, the definitive treat-ment of high RVFs after Pm repair for POP necessitates ablation of the Pm, proctectomy with a primary colo-rectal anastomosis, an omental patch interposition, and a temporary ileostomy.

The ectodermal dysplasias are rare diseases with hypodontia, hypotrichosis and hypohidrosis. The subject's life is considerably constrained and this from an early age, with major difficulties for the integration and acceptance of conventional prosthetic occlusal rehabilitation. The use of implants is an integral part of early treatment, in the regions of stable growth, that is to say symphysis. In two childs of 5 and 6 years we have made implant-borne prosthetic rehabilitation in the maxilla and the mandible. Aesthetic and social evaluation were positive. We have restored the normal oro-facial functions for the correct development of skeletal bases. They acted as an external fixator intraoral, stimulating the growth by the function. Our question was: can we leave a child throughout his childhood and adolescence with a not suitable removable prosthesis, under the pretext of growth unfinished?

Pattern formation in 2-μm Tm mamyshev oscillators associated with the dissipative Faraday instability

We investigate numerically the pattern formation in 2-μm thulium-doped mamyshev fiber oscillators, associated with the dissipative Faraday instability. The dispersion-managed fiber ring oscillator is designed with commercial fibers, allowing the dynamics for a wide range of average dispersion regimes to be studied, from normal to nearzero cavity dispersion where the Benjamin–Feir instability remains inhibited. For the first time in the 2-μm spectral window, the formation of highly coherent periodic patterns is demonstrated numerically with rates up to ~100 GHz. In addition, irregular patterns are also investigated, revealing the generation of rogue waves via nonlinear collision processes. Our investigations have potential applications for the generation of multigigahertz frequency combs. They also shed new light on the dissipative Faraday instability mechanisms in the area of nonlinear optical cavity dynamics.

Bacteria are known to communicate with each other and regulate their activities in social networks by secreting and sensing signaling molecules called autoinducers,a process known as quorum sensing(QS).This is a growing area of research in which we are expanding our understanding of how bacteria collectively modify their behavior but are also involved in the crosstalk between the host and gut *** is particularly relevant in the case of pathologies associated with dysbiosis or disorders of the intestinal *** review will examine the different QS systems and the evidence for their presence in the intestinal *** will also provide clues on the role of QS molecules that may exert,directly or indirectly through their bacterial gossip,an influence on intestinal epithelial barrier function,intestinal inflammation,and intestinal *** review aims to provide evidence on the role of QS molecules in gut physiology and the potential shared by this new *** understanding the impact of intestinal bacterial social networks and ultimately developing new therapeutic strategies to control intestinal disorders remains a challenge that needs to be addressed in the future.

Greenhouse gas （GHG） emissions from the surface soils and surface water receiving animal excreta may be important components of the GHG balance of terrestrial ecosystems, but the associated processes are poorly documented in tropical environments, especially in tropical arid and semi-arid areas. A typical sylvo-pastoral landscape in the semi-arid zone of Senegal, West Africa, was investigated in this study. The study area （706 km2 of managed pastoral land） was a circular zone with a radius of 15 km centered on a borehole used to water livestock. The landscape supports a stocking rate ranging from 0.11 to 0.39 tropical livestock units per hectare depending on the seasonal movements of the livestock. Six landscape units were investigated （land in the vicinity of the borehole, natural ponds, natural rangelands, forest plantations, settlements, and enclosed plots）. Carbon dioxide （CO2）, nitrous oxide （N2O） and methane （CH4） fluxes were measured with static chambers set up at 13 sites covering the six landscape units, and the 13 sites are assumed to be representative of the spatial heterogeneity of the emissions. A total of 216 fluxes were measured during the one-year study period （may 2014 to April 2015）. At the landscape level, soils and surface water emitted an average 19.8 t C-CO2 eq/（hm2.a）（CO2： 82%, N2O： 15%, and CH4： 3%）, but detailed results revealed notable spatial heterogeneity of GHG emissions. CO2 fluxes ranged from 1148.2 （±91.6） mg/（m2.d） in rangelands to 97,980.2 （±4,861.7） mg/（m2.d） in surface water in the vicinity of the borehole. N2O fluxes ranged from 0.6 （±0.1） mg/（m2.d） in forest plantations to 22.6 （±10.8） mg/（m2.d） in the vicinity of the borehole. CH4 fluxes ranged from -3.2 （±0.3） mg/（m2.d） in forest plantations to 8788.5 （±2295.9） mg/（m2.d） from surface water in the vicinity of the borehole.
