Durability of viral response after off-treatment in HBeAg positive chronic hepatitis B

AIM:To evaluate the durability in hepatitis B e antigen (HBeAg) positive chronic hepatitis B patients who discontinued antiviral treatment. METHODS:A total of 48 HBeAg positive chronic hepatitis B patients who were administered nucleoside analogues and maintained virological response for ≥ 6 mo [hepatitis B virus (HBV) DNA 104 copies/mL after HBeAg seroconversion with previously undetectable HBV-DNA level. RESULTS:During the median follow-up period of 18.2 mo (range:5.1-47.5 mo) after cessation of antiviral treatment, the cumulative serological recurrence rate was 15 % at 12 mo. The median duration between the cessation of antiviral treatment and serologic recurrence was 7.2 mo (range:1.2-10.9 mo). Of the 48 patients with HBeAg positive chronic hepatitis, 20 (41.6%) showed virological recurrence. The cumulative virologic recurrence rates at 12 mo after discontinuing the antiviral agent were 41%. The median duration between off-treatment and virologic recurrence was 7.6 mo (range:4.3-27.1 mo). The mean age of the virological recurrence group was older than that of the non-recurrence group (46.7 ± 12.1 years vs 38.8 ± 12.7 years, respectively; P = 0.022). A Strategy of Signal Detection for Performance Improvement in Clipping Based OFDM System

In this paper,the supervised Deep Neural Network(DNN)based signal detection is analyzed for combating with nonlinear distortions efficiently and improving error performances in clipping based Orthogonal Frequency Division Multiplexing(OFDM)*** of the main disadvantages for the OFDM is the high Peak to Average Power Ratio(PAPR).The clipping is a simple method for the PAPR ***,an effect of the clipping is nonlinear distortion,and estimations for transmitting symbols are difficult despite a Maximum Likelihood(ML)detection at the *** DNN based online signal detection uses the offline learning model where all weights and biases at fully-connected layers are set to overcome nonlinear distortions by using training data ***,this paper introduces the required processes for the online signal detection and offline learning,and compares error performances with the ML detection in the clipping-based OFDM *** simulation results,the DNN based signal detection has better error performance than the conventional ML detection in multi-path fading wireless *** performance improvement is large as the complexity of system is increased such as huge Multiple Input Multiple Output(MIMO)system and high clipping rate.

Streptococcus mutans activates the AIM2, NLRP3 and NLRC4 inflammasomes in human THP-1 macrophages

Streptococcus mutans(S. mutans), a major aetiologic agent of dental caries, is involved in systemic diseases, such as bacterial endocarditis, if it enters the bloodstream through temporary bacteraemia. Interleukin(IL)-1β, a proinflammatory cytokine, is related to the host defences against pathogens, and its synthesis, maturation, and secretion are tightly regulated by the activation of the inflammasome, an inflammatory signalling complex. This study examined the signalling mechanism of IL-1β secretion and the inflammasome pathway induced by S. mutans to explain the molecular mechanism through which systemic infection by oral streptococci can occur. After infection of THP-1 cells with S. mutans, the expression of inflammasome components was detected using various methods. S. mutans induced IL-1β secretion via caspase-1 activation, and S. mutans-induced IL-1β secretion required absent in melanoma(AIM2), NLR family pyrin domain-containing 3(NLRP3) and NLR family CARD domain-containing 4(NLRC4)inflammasome activation. In particular, the S. mutans-induced NLRP3 inflammasome was mediated by adenosine triphosphate(ATP) release, potassium depletion and lysosomal damage. Our study provides novel insight into the innate immune response against S. mutans infection.

This paper proposes the multiple-input multiple-output(MIMO)detection scheme by using the deep neural network(DNN)based ensemble machine learning for higher error performance in wireless communication *** the MIMO detection based on the ensemble machine learning,all learning models for the DNN are generated in offline and the detection is performed in online by using already learned *** the offline learning,the received signals and channel coefficients are set to input data,and the labels which correspond to transmit symbols are set to output *** the online learning,the perfectly learned models are used for signal detection where the models have fixed bias and *** performance improvement,the proposed scheme uses the majority vote and the maximum probability as the methods of the model combinations for obtaining diversity gains at the MIMO *** simulation results show that the proposed scheme has improved symbol error rate(SER)performance without additional receive antennas.

In this paper,a novel precoding scheme based on the Gauss-Seidel(GS)method is proposed for downlink massive multiple-input multiple-output(MIMO)*** GS method iteratively approximates the matrix inversion and reduces the overall complexity of the precoding *** addition,the GS method shows a fast convergence rate to the Zero-forcing(ZF)method that requires an exact invertible ***,to satisfy demanded error performance and converge to the error performance of the ZF method in the practical condition such as spatially correlated channels,more iterations are necessary for the GS method and increase the overall *** efficient approximation with fewer iterations,this paper proposes a weighted GS(WGS)method to improve the approximation accuracy of the GS *** optimal weights that accelerate the convergence rate by improved accuracy are computed by the least square(LS)*** the computation of weights,the different weights are applied for each iteration of the GS *** addition,an efficient method of weight computation is proposed to reduce the complexity of the LS *** simulation results show that bit error rate(BER)performance for the proposed scheme with fewer iterations is better than the GS method in spatially correlated channels.

The recent rapid development in information systems （ISs） has resulted in a critical need for integration and interoperability between heterogeneous ISs in various domains, using specific commonalities. However, stovepipe systems have been caused due to inconsistencies in planning IS architecture among stakeholders. So far, there has been no research on an enterprise architecture framework （EAF） that can satisfy with the coefficient factors of system architecture （SA） and enterprise architecture （EA）. This paper proposes a new EAF that can resolve the problems inherent in existing legacy EAFs and their features. EAFoC （Enterprise Architecture Framework based on Commonality） is based on commonality that can be satisfied as the coefficient factors in both SA and EA within a common information technology （IT） domain. Thus, it should be possible to integrate an established heterogeneous framework for each stakeholder＇s view. Consequently, the most important contribution of this paper is to establish the appropriate EAFoC for the development of consistent IS architecture, smooth communication among stakeholders, systematic integration management of diversified and complicated new IT technologies, interoperability among heterogeneous ISs, and reusability based on commonality with other platforms.

Effects of lysophosphatidic acid on human periodontal ligament stem cells from teeth extracted from dental patients

Despite their potential applications in future regenerative medicine, periodontal ligament stem cells(PDLSCs) are difficult to obtain in large amounts from patients. Therefore, maintaining sternness while expanding the cell numbers for medical use is the key to transitioning PDLSCs from the bench to the clinic. Lysophosphatidic acid(LPA), which is present in the human body and saliva, is a signaling molecule derived from phospholipids. In this study, we examined the effects of LPA on sternness maintenance in human PDLSCs. Several spindle-shaped and fibroblast-like periodontal ligament stem-like cell lines were established from PDLSC isolation. Among these cell lines, the most morphologically appropriate cell line was characterized. The expression levels of OCT4, NANOG(a stem cell marker), and CD90(a mesenchymal stem cell marker) were high. However, CD73(a negative marker of mesenchymal stem cells) expression was not observed. Notably, immunofluorescence analysis identified the expression of STRO-1, CD146(a mesenchymal stem cell marker), and sex determining region Y-box 2 at the protein level. In addition, lipid droplets were stained by Oil red O after the induction of adipogenesis for 21 days, and mineralized nodules were stained by Alizarin Red S after the induction of osteogenesis for 14 days. Alkaline phosphate staining also demonstrated the occurrence of osteogenesis. In summary, we established a human PDLSC line, which could be applied as a cell source for tissue regeneration in dental patients. However, further studies are needed to determine the detailed effects of LPA on PDLSCs.

BACKGROUND Midazolam is commonly used for sedation during gastrointestinal ***,some patients experience paradoxical reactions characterized by excessive movement or *** To investigate the rate of recurrence of paradoxical reactions to midazolam during an upper *** We retrospectively reviewed 122152 sedative endoscopies among a total of 58553 patients at the Seoul National University Hospital,Healthcare System Gangnam Center,from July 2013 to December *** them,361 patients with a history of paradoxical reaction during sedative upper endoscopy were *** characteristics of patients in the recurrent and non-recurrent groups were compared via multivariable analysis using logistic *** Paradoxical reactions occurred in 0.86%(1054/122152)of endoscopies,and in 1.51%(888/58553)of *** the 361 subjects with previous paradoxical reactions in sedative endoscopies,111(30.7%)experienced further paradoxical *** analysis revealed that the total midazolam dose used was higher in the recurrent group(6.74±2.58 mg)than in the non-recurrent group(5.49±2.04 mg;P<0.0001).Patients were administered a lower dose of midazolam than previous doses:1 mg less in the recurrent group and 2 mg less in the non-recurrent *** analysis showed that the midazolam dose difference was an independent risk factor for recurrent paradoxical reaction(odds ratio:1.213,95%CI:1.099-1.338,P=0.0001).CONCLUSION The rate of recurrence of paradoxical reactions is significantly associated with midazolam *** dose of midazolam administered to patients with previous paradoxical reactions should be less than that previously used.
