The optimal timing of post-prostate biopsy magnetic resonance imaging to guide nerve-sparing surgery

The goal of our study was to evaluate the impact of the interval between prostate biopsy and magnetic resonance imaging （MRI） on the accuracy of simple tumor localization, which is essential information that enables nerve-sparing surgery. We also sought to determine the optimal timing of a post-biopsy MRI, A total of 184 patients who had undergone MRI before radical prostatectomy at an institution without a predetermined schedule for MRI after a prostate biopsy were enrolled. The mean interval from the biopsy to the MRI was 30.8 ± 18.6days. The accuracy of the MRI for simplified tumor location （right, left, bilateral and none） was 44.6%. In the group with discordant pathologic and MRI findings, the most common reason recorded was ＇MRI predicted a unilateral lesion, but pathology revealed bilateral lesions＇（58.3%）, followed by ＇MRI predicted no lesion, but pathology revealed the presence of a lesion＇（32.0%）. Multivariable analysis showed that the discordant group had a shorter interval （25.0 ± 14.3 vs 38.1 ± 20.6days, P 〈 0.01） preceding the MRI and a higher rate of hemorrhage as observed by MRI （80.4% vs 54.8%, P 〈 0.01） in comparison with the accordant group. In receiver operating characteristics analysis, the area under the curve of the MRI interval in accurate prediction of the tumor location was 0.707 （P 〈 0.001）. At the MRI interval＇s cutoff of 28.5days, the sensitivity was 73.2% and the specificity was 63.7%. When the MRI was performed within 28days, the accumulated accuracy was only 26.1% （23/88）; however, when it was performed after 28days, the reversely accumulated accuracy was 61.5% （59/96）. These data support a waiting period of at least 4weeks after a biopsy before performing an MRI for the purposes of surgical refinement.

Anticipating the lifespan: Predicting the durability of an anode-supported solid oxide fuel cell short stack over 50,000 h

In the present study, the operational lifetime of a solid oxide fuel(SOFC) short stack is predicted by investigating the performance degradation of both the short stack and its cells throughout 1000 h at 800°C. The short stack and integral cell voltages are continuously measured during the long-term test, with electrochemical impedance spectroscopy(EIS) conducted every 200 h. The short stack voltage decreased rapidly for the initial 200–300 h and afterwards, it decreased at a slow rate due to the increase in the Ohmic and polarization resistances in the same manner. Scanning electron microscopy results show that there is no delamination or cracking among constituent layers of the short-stack cells. The single degradation effects of the Ni coarsening in the anode, cation migration and surface segregation in cathode and oxide scale growth in metallic interconnect mesh are successfully integrated into a comprehensive lifetime prediction model. The experimentally measured voltage degradation data of the short stack fits well with the developed mathematical model and allows the successful prediction of the lifetime up to 50,000 h.

Design of a novel 50kW fast charger for electric vehicles

A novel 50 kW fast charger was proposed for electric vehicles. The proposed fast charger is divided into two main sections an AC-DC converter performing a PFC function and a DC-DC converter performing a charging function. A transformer including leakage inductances was used in the AC-DC converter in order to obtain isolation and inductance. A series-connection topology was used in the DC-DC converter between the DC-bus and outlet. This topology enables high power conversion efficiency up to 95% for the DC-DC converter. In order to reduce the impact of the 50 kW charging on the AC grid, the proposed fast charger system includes a buffering battery unit between the two main power conversion units. This leads to reductions in the power installation costs of power companies and to improvements in the power quality were verified through simulations and experimental results. on the AC grid. Landscape pattern and its effect on ecosystem functions in Seoul Metropolitan area:Urban ecology on distribution of the naturalized plant species

During land transformation process in the human history, naturalized plants were introduced to several land use patterns by the different ways of plant itself. Including some naturalized plants that had been contribute to land restoration, many naturalized plants have been invaded to original habitat or landscape for native plants. Once the plants were colonized, they extend their area and population size. Urban developed areas often give an important role of source habitat for naturalized plants and expanding their population size. In recent, this situation is appearing as one of environmental problems about the urban landscape management controlling the naturalized plants that invaded in the developed area and conserving the native vegetation. This paper is focusing on relationships between distribution of habitat of naturalized plants and landscape patch in urban area in Seoul. Gangdong-Gu, one of the administrative areas in Seoul was selected for this study. We examined the recent land use change using LANDSAT TM data and spreading of the representative naturalized plants (Robinia pseudoacacia and Eupatorium rugosum) by Seoul Biotope Mapping Project and field survey in 1999. As a result, these two species were often occurred in the same habitat and distributed in forest edge disturbed by man. Their distribution patterns were related to landscape indices (patch size and shape) in the forest edge.

Oxcarbazepine for trigeminal neuralgia may induce lower extremity weakness: A case report

BACKGROUND Although few studies have reported hyponatremia due to carbamazepine or oxcarbazepine in patients with epilepsy,no study has investigated cases of carbamazepine-or oxcarbazepine-induced hyponatremia or unsteady gait in patients with neuropathic ***,we report a case of oxcarbazepineinduced lower leg weakness in a patient with trigeminal neuralgia and summarize the diagnosis,treatment,and changes of clinical *** SUMMARY A 78-year-old male with a history of lumbar spinal stenosis was admitted to the hospital after he experienced lancinating pain around his right cheek,eyes,and lip,and was diagnosed with trigeminal neuralgia at the right maxillary and mandibular *** was prescribed oxcarbazepine(600 mg/d),milnacipran(25 mg/d),and oxycodone/naloxone(20 mg/10 mg/d)for four *** years later,the patient experienced symptoms associated with spinal stenosis,including pain in the lower extremities and unsteady *** serum sodium level was 127 mmol/*** oxcarbazepine to be the cause of the hyponatremia,oxcarbazepine administration was put on hold and the patient was switched to *** subsequent visit,the patient’s serum sodium level had normalized to 143 mmol/L and his unsteady gait had *** Oxcarbazepine-induced hyponatremia may cause lower extremity weakness and unsteady gait,which should be differentiated from those caused by spinal stenosis.

Expression of prothymosin α predicts early recurrence and poor prognosis of hepatocellular carcinoma

BACKGROUND：Prothymosinα（PTMA）is a nuclear oncoprotein-transcription factor essential for cell cycle progression and *** was overexpressed in several human malignancies including hepatocellular carcinoma（HCC）.However,the prognostic significance of PTMA protein expression in HCC remains *** the present study,we evaluated PTMA protein expression by immunohistochemistry in order to elucidate the prognostic roles of PTMA in HCC ***： By immunohistochemistry, we investigated the expression of PTMA protein in tumor tissue from 226 HCC patients who underwent curative hepatectomy. Univariate and multivariate analyses were performed to evaluate its predic- tive value for tumor recurrence and survival of patients. The median follow-up period was 120 months. RESULTS： PTMA expression was observed in 162 （71.7%） of the 226 HCC patients and was significantly associated with higher Edmondson grade, microvascular invasion, intrahe- patic metastasis, higher American Joint Committee on Cancer （AJCC） T-stage, and lower albumin level. PTMA expression was an independent predictor of early recurrence （P=0.001）. PTMA expression showed an unfavorable influence on recurrence- free survival （RFS）（P〈0.001）. Subgroup analysis showed that among patients with tumor size _〈5.0 cm （140 patients）, patients at AJCC T-stage I （95 patients） and patients with a-fetoprotein ≤20 ng/mL （83 patients）, the differences in RFS between PTMA- positive and PTMA-negative groups were also statistically sig- nificant （P=0.017, P=0.002 and P=0.002, respectively）. In addi- tion, PTMA expression was an independent predictor of shorter RFS （P=0.011）. PTMA expression showed an unfavorable influ- ence on overall survival （P=0.014）, but was not an independent predictor of shorter overall survival （P=0.161）. CONCLUSIONS： PTMA protein expression might be a novel predictor of early recurrence and RFS in HCC patients, even those at early stage or with a-fetoprotein-negative after curative hep
