BACKGROUND Amino-acid based medical foods have shown promise in alleviating symptoms of drug induced gastrointestinal side effects;particularly,diarrhea-predominant *** bowel syndrome(IBS)is a gastrointestinal disorder that affects up to 9% of people globally,with diarrhea predominant IBS(IBS-D)being the most prevalent *** trials are needed to explore potential added benefits when integrated into standard care for *** To assess the effectiveness of an amino acid-based medical food as an adjunct to standard of care for adults with *** This is a pragmatic,real world,open label,single arm study comparing a 2-week baseline assessment to a 2-week intervention *** hundred adults,aged 18 to 65 years,with IBS-D,according to Rome IV criteria,were enrolled after completing a 2-week baseline assessment period and received a 2-week supply of an amino acid based medical food which was consumed at home twice daily on top of their standard of *** primary outcome was an assessment of tolerability after 2-weeks of consumption,while secondary outcomes included changes in stool consistency(Bristol Stool Form Scale),severity of abdominal pain&discomfort,symptoms of urgency,Global Improvement Survey(GIS),and the IBS severity scoring system(IBS-SSS).RESULTS The test product was well-tolerated as each participant successfully completed the full 14-day trial,and there were no instances of dropouts or discontinuation of the study product *** percent of participants achieved a 50% or more reduction in the number of days with type 6-7 bowel movements(IBS-D stool consistency responders).Fifty-three percent of participants achieved a clinically meaningful reduction of 30% in mean weekly pain scores,and 55%experienced the same for mean weekly discomfort scores(IBS-D pain and discomfort responders).Participants experienced a mean-109.4(95% confidence interval:-130.1,-88.8)point reduction on the IBS-SSS and 52% experienced a minimal

Microjet-Based Active Flow Control on a Fixed Wing UAV

A fixed wing, remote controlled, unmanned aerial vehicle (UAV), has been retrofitted with a system of microjet-based actuators to test microjet efficacy for flow control during flight. This allows one to evaluate the flow control system in a more complex and uncontrolled environment relative to the “clean” laboratory flowfield. The system is composed of off-the-shelf components and provides some appreciation of the challenges associated with implementing such an active control scheme in more “practical” configurations. A wing section with actuators was first tested in a low-speed wind tunnel to characterize microjet control efficacy in the laboratory using surface flow visualizations and particle image velocimetry (PIV). The laboratory results show that the microjet actuators are an effective means of controlling separation with fairly low supply pressures and flow rates. Only relatively robust and straightforward diagnostics can be used to determine the flow conditions on the UAV during flight. As such, tufts are installed on the aircraft’s wing to serve as a qualitative way of measuring control efficacy. Results from the flight tests confirm that this flow control system is capable of delaying flow separation in the complex flows occurring during flight.

Clinical outcomes of isolated renal failure compared to other forms of organ failure in patients with severe acute pancreatitis

AIM To assess differences in clinical outcomes of isolated renal failure(RF) compared to other forms of organ failure(OF) in patients with severe acute pancreatitis(SAP).METHODS Using a prospectively maintained database of patients with acute pancreatitis admitted to a tertiary medical center between 2003 and 2016, those with evidence of persistent OF were classified to renal, respiratory, cardiovascular, or multi-organ(2 or more organs). Data regarding demographics, comorbidities, etiology of acute pancreatitis, and clinical outcomes were prospectively recorded. Differences in clinical outcomes after development of isolated RF in comparison to other forms of OF were determined using independent t and Mann-Whitney U tests for continues variables, and χ~2 test for discrete *** Among 500 patients with acute pancreatitis, 111 patients developed persistent OF: mean age was 54 years, and 75(67.6%) were male. Forty-three patients had isolated OF: 17(15.3%) renal, 25(21.6%) respiratory, and 1(0.9%) patient with cardiovascular failure. No differences in demographics, etiology of acute pancreatitis, systemic inflammatory response syndrome scores, or development of pancreatic necrosis were seen between patients with isolated RF vs isolated respiratory failure. Patients with isolated RF were less likely to require nutritional support(76.5% vs 96%, P = 0.001), ICU admission(58.8% vs 100%, P = 0.001), and had shorter mean ICU stay(2.4 d vs 15.7 d, P < 0.001), compared to isolated respiratory failure. None of the patients with isolated RF or isolated respiratory failure *** Among patients with SAP per the Revised Atlanta Classification, approximately 15% develop isolated RF. This subgroup seems to have a less protracted clinical course compared to other forms of OF. Isolated RF might be weighed less than isolated respiratory failure in risk predictive modeling of acute pancreatitis.

本文综述《中国应用语言学》2013 年特刊所探讨的主要理论构念,包括外语和二语教育的自主性(autonomy)、能动性(agency)与身份认同(identity)。我们首先深入探讨和比较自主性和能动性这两个相似概念的复杂意义。自主性的讨论从 Benson (2011) 对自主性的宽阔定义("capacity to take control of one’s own learning")展开,目的在于深入发掘 capacity和 control 各自多层面的涵义以及同时简要解释相关理论及教育实践问题如"可用特性"(affordances)和学习者过程规划(learner agendas)等。在深入审视能动性这一概念的基础上,我们提出自主性和能动性最好被视为相关而相异的两个概念,相异之处在于对教—学过程的有效控制程度。自主性和能动性两个密切相关的概念经过仔细梳理后,我们转向讨论另一个重要相关概念,即外语和二语教育的身份认同。文章最后探讨自主性、能动性和身份认同这些主要概念在语言教育中的相互关系。
