Retinal manifestations of patients with human immun- odeficiency virus, a multiethnics study in malaysia

· AIm: To investigate the fundus findings of patients infected with human immunodeficiency virus (HIV) in correlation to Highly Active Antiretroviral Therapy (HAART) and CD4 count. · mETHODS: Two hundred and two patients of the three major races (Chinese, malay and Indian) in malaysia were recruited in this population-based cross-sectional study. This consisted of confirmed HIV sero-positive patients with HAART treatment (n=95) or without HAART therapy (n=107) from December 2007 to march 2008. They were further classified into the HIV infected group, AIDS related complex (ARC) group and AIDS group. Each group was then subdivided according to their CD4 count. Clinical fundus findings were recorded. · RESULTS: Sixty six patients (32.7%) showed presence of fundus manifestation, majority of which was HIV microangiopathy (89%) and the rest being Cytomegalovirus (CmV) retinitis (11%). The most common fundus lesion was cotton wool spot (34%). There was a higher incidence of fundus manifestation in the non HAART group than the HAART group (P =0.04) and in patients with CD4 count less than 200 cells/ml in both groups (P =0.01). The HAART therapy had remarkably reduced the percentage of fundus manifestation by 20% but CD4 count remains the marker for fundus manifestations. There were no significant differences noted in the retinal manifestation among the different races. (ANOVA, P =0.25). · CONCLUSION: The fundus manifestations were higher in patients with CD4ml and in the non HAART group. Hence the HAART therapy is capable of reducing the incidence of fundus manifestations, however the CD4 count determines the occurrence of fundus manifestations. ·

In this paper, we prove some fixed point theorems for generalized contractions in the setting of G-metric spaces. Our results extend a result of Edelstein [m. Edelstein, On fixed and periodic points under contractive mappings, J. London math. Soc., 37 （1962）, 74-79] and a result of Suzuki [T. Suzuki, A new type of fixed point theorem in metric spaces, Nonlinear Anal., 71 （2009）, 5313-5317]. We prove, also, a fixed point theorem in the setting of G-cone metric spaces.

The deposition of asphaltenes on the inner wall of oil wells and pipelines causes flow blockage and significant production loss in these *** major underlying mechanism(s)for the deposition of asphaltene particles from the oil stream are still under investigation as an active research topic in the *** this work,a new deposition model considering both diffusional and inertial transport of asphaltene toward the tubing surface was *** predictions were compared and verified with two sound experimental data available in the literature to evaluate the model's performance.A parametric study was done using the validated model in order to investigate the effect of the asphaltene particle size,flow velocity and oil viscosity on the magnitude of asphaltene deposition *** of the study revealed that increasing the oil velocity causes more drag force on wall's inner surface;consequently,particles tend to transport away from the surface and the rate of asphaltene deposition is *** addition,the developed model predicts that at low fluid velocity(~0.7 m/s),the less viscous oil is more prone to asphaltene deposition problem.

Depositional environments and sequence stratigraphy of the Bahram Formation(middleelate Devonian) in north of Kerman, south-central Iran

This study is focused on sedimentary environments, facies distribution, and sequence stratigraphy. The facies and sequence stratigraphic analyses of the Bahram Formation（middleelate Devonian） in southcentral Iran are based on two measured stratigraphic sections in the southern Tabas block. The Bahram Formation overlies red sandstones Padeha Formation in sections Hutk and Sardar and is overlain by Carboniferous carbonate deposits of Hutk Formation paraconformably, with a thickness of 354 and386 m respectively. mixed siliciclastic and carbonate sediments are present in this succession. The field observations and laboratory studies were used to identify 14 micro/petrofacies, which can be grouped into 5 depositional environments： shore, tidal flat, lagoon, shoal and shallow open marine. A mixed carbonate-detrital shallow shelf is suggested for the depositional environment of the Bahram Formation which deepens to the east（Sardar section） and thins in southern locations（Hutk section）. Three 3rdorder cyclic siliciclastic and carbonate sequences in the Bahram Formation and one sequence shared with the overlying joint with Hutk Formation are identified, on the basis of shallowing upward patterns in the micro/pertofacies.

The Relationship between Fe mineralization and magnetic Basement Faults using multifractal modeling in the Esfordi and Behabad Areas(BmD), Central Iran

multifractal modeling is a mathematical method for the separation of a high potential mineralized background from a non-mineralized background. The Concentration-Distance to Fault structures(C-DF) fractal model and the distribution of the known iron(Fe) deposits/mines seen in the Esfordi and Behabad 1:100,000 sheets from the Bafq region of central Iran are used to distinguish Fe mineralization based on their distance to magnetic basement structures and surface faults, separately, using airborne geophysical data and field surveys. Application of the C-DF fractal model for the classification of Fe mineralizations in the Esfordi and Behabad areas reveals that the main ones show a correlation with their distance from magnetic basement structures. Accordingly, the distances of Fe mineralizations with grades of Fe higher than 55%(43% m, whereas for surfacial faults with grades of 43% ≤ Fe ≤ 60%, the distances are 3162m from the faults. Thus, there is a positive relationship between Fe mineralization and magnetic basement structures. Also, the proximity evidence of Precambrian high-grade Fe mineralization related to magnetic basement structures indicates syn-rifting tectonic events. Finally, this C-DF fractal model can be used for exploration of magmatic and hydrothermal ore deposits.

Lipid and polymer stabilized microbubbles are used in medicine as contrast agents for ultrasound imaging and are being developed for the delivery of water soluble drugs to diseased areas of the ***,many new therapeutics exhibit poor water solubility or stability,which has led to the requirement for the development of effective hydrophobic drug delivery *** study presents a new method to produce microbubbles coated with an oil layer capable of encapsulating hydrophobic drugs and suitable for targeted,triggered drug *** new method utilizes highly controllable flow-focusing microfluidics with lipid oil nanodroplets self-assembling and spreading at gas–aqueous *** layer inside microbubbles were produced with diameters of 2.4±0.3μm(s.d.,1.6μm)and at concentrations up to 10^(6) bubbles per *** mechanism of oil layer inside microbubble assembly and stability were characterized using methods including contact angle measurements,quartz crystal microbalance with dissipation monitoring and fluorescence resonance energy transfer imaging.

AIm To investigate changes in adiposity and cardio-metabolic risk profile following Roux-en-Y gastric bypass in patients of middle Eastern ethnicity with severe *** This prospective cohort study involved 92 patients who met the indications of bariatric surgery. Post-procedure markers of obesity and cardiometabolic profile were monitored regularly for a *** mean body mass index decreased by 29.5% from 41.9 to 29.5 kg/m^2 between baseline and 12-mo follow-up, while mean fat mass decreased by 45.9% from 64.2 kg to 34.7 kg. An improvement was also observed in the glucometabolic profile with both fasting glucose and HbA 1csubstantially decreasing(P medium term(1 year) health benefits of bariatric surgery for patients of middle Eastern ethnicity.

Effect of Bromocriptine on Insulin Resistance in Patients with PCO

Introduction: Poly cystic ovary (PCO) is one of the most common endocrine disorders in women. All patients with PCO are at risk of insulin resistance, IFG and diabetes. Recently, bromocriptine is used in treatment of diabetes mellitus type II to improve insulin resistance. Objective: The aim of this study is the evaluation of bromocriptine on insulin resistance in PCO people. Patients and methods: In this single-blind controlled clinical trial with placebo, 44 patients with PCO referring to endocrinology clinic were evaluated. Inclusion criteria were BmI > 25 kg/m2 and diagnosed PCO patients according to Rotterdam criteria and rule out other causes. Blood samples were obtained for FBS, Fasting Insulin, Prolactin, TSH and 17(OH)P. They divided two groups: Case group was given bromocriptine 2.5 mg daily and placebo was given to control group. Patients were treated for 8 days and in day 9th blood sample was obtained for FBS, Fasting insulin, HOmA-IR index. mann-Whitney method is used for mean comparison. Results: Data analysis using showed in pre diabetes range, mean changes of FBS, insulin level and IR in Groups 1 and 2 had significant differences(P = 0.004), but no significant different was found in FBS mg/dl (P = 0.92). In group with BmI 2, no significant differences were found in changes in FBS mI ≥ 30 kg/m2, no significant differences were found in changes in insulin level and IR (P = 0.69, 0.089 respectively). mean systolic blood pressure changes in Group 1 and 2 in FBS > 100 mg/dl with (P = 0.036) were significant, but no significant difference showed in mean change of diastolic blood pressure in FBS > 100 mg/dl (P = 0.99). In FBS mg/dl mean changes in systolic and diastolic blood pressures were not significant (P = 0.6). Age showed no difference changes in effect on treatment in Groups 1 and 2 (P = 0.1). 关于头痛这种常见神经症状的记载最早可追溯到9 000年前,许多古老的医学体系都对头痛的症状及其治疗方法有一定阐述,可对现代医学治疗头痛提供一定的参考。查阅9-19世纪伊朗传统医学古籍及文献中对头痛分类及相应治疗方法的记载,发现头痛被分为20余种类型,部分类型与现代医学中的分类十分相似;160多种草药被用于头痛的治疗,60%以上用于治疗头痛的草药被证实具有抗炎镇痛作用;给药方式包括口服、局部用药以及鼻部吸入。从传统医学中可窥探古代伊朗人民的智慧,或可为现代医学治疗头痛提供新的思路。
