Cervical myelopathy: Diagnostic Aspects, Surgical Treatment and Evolution of Patients Operated upon in Yaounde

Introduction: Although cervical myelopathy is the most common degenerative cervical spine pathology in adults, the indications and approaches of surgical treatment have not yet been clearly defined in the literature. Very few studies exist regarding these aspects in our setting, and they are mostly outdated. This study aimed to describe the diagnostic aspects, current surgical treatment with technical improvements, and progression in patients. Patients and methods: We conducted a multicentre retrospective descriptive study over a 10-year period between January 2011 and January 2020 in three referral centres in Yaoundé. All patients who underwent surgery for cervical myelopathy were included. Results: Fifty-two patients were recruited. The rate of operated cervical myelopathies among all degenerative spinal pathologies was 14.05%. The m/F sex ratio was 3/1, and the average age was 52 ± 10 years. All included patients had gait problems, 90.38% demonstrated motor deficiency, and 67.30% experienced at least three levels of compression. Surgery was decided based on the Nurick grade;a posterior approach was applied to 86.54% of patients. Postoperative progression showed a neurological improvement of 82% with an average follow-up of 4 years. Conclusion: In this study, the patients who underwent surgery were relatively young, their clinical presentations were mostly advanced, and surgical management showed good results in well-selected cases.

Influences of Biochar and Biochar-mineral Complex on mycorrhizal Colonisation and Nutrition of Wheat and Sorghum

The high price of synthetic fertilisers and the price barrier for biochar as a soil amendment have encouraged the exploration of using biochar in fertiliser replacement formulations. Biochars coupled with fertilisers can be applied at lower application rates to achieve benefits in plant growth and nutrition, as well as soil biological fertility. It is necessary to evaluate the use of biochar as a fertiliser substitute. Therefore, this study investigated the comparative influences of biochars, including Acacia saligna （AS）, Simcoa jarrah （S J） and Wundowie jarrah （W J）, mineral fertiliser with microbes （mF ＋ m）, biochar-mineral complex （BmC） and their combination on mycorrhizal colonisation, growth and nutrition of wheat in a glasshouse experiment and sorghum in field conditions. BmC ＋ mF ＋ m treatment produced higher mycorrhizal colonisation than mF ＋ m alone, indicating that BmC had a significant role in increasing mycorrhizal colonisation. SJ （treated with acetic acid） and mF ＋ m treatments, as well as AS ＋ mF ＋ m application, showed similar effects on mycorrhizal colonisation, but lower colonisation than the BmC ＋ mF ＋ m treatment. Ovcrall~ the BmC ＋ mF ＋ m treatment supported the maximum shoot, root and total plant dry weight followed by AS ＋ mF ＋ m and WJ ＋ mF ＋ m. The mF ＋ m treatment had the maximum shoot N and K concentrations, while BmC ＋ mF ＋ m application had the maximum shoot P concentration. AS ＋ mF -4- m and WJ ＋ mF ＋ m treatments supported the maximum N uptake by wheat shoots, while BmC ＋ mF ＋ m supported the maximum P uptake. The results showed that biochars and BmCs could increase mycorrhizal colonisation, plant growth and nutrient uptake of wheat, particularly N, P, K, S and Zn. The field experiment confirmed that BmC application at a rate of 300 kg ha-1 could increase the yield of irrigated sorghum on a loam soil and provide better applied P use efficiency compared to a water-soluble fertiliser alone. An Efficient Tide-Surge Interaction model for the Coast of Bangladesh

The numerical method of lines(mOLs)in coordination with the classical fourth-order Runge Kutta(RK(4,4))method is used to solve shallow water equations(SWEs)for foreseeing water levels owing to the nonlinear interaction of tide and surge accompanying with a storm along the coast of *** SWEs are developed by extending the body forces with tide generating forces(TGFs).Spatial variables of the SWEs along with the boundary conditions are approximated by means of finite difference technique on an Arakawa C-grid to attain a system of ordinary differential equations(ODEs)of initial valued in time,which are being solved with the aid of the RK(4,4)*** grid technique is adopted to solve coastal complexities closely with least computational cost.A stable tidal solution in the region of our choice is produced by applying the tidal forcing with the major tidal constituent m2(lunar semi-diurnal)along the southern open-sea boundary of the outer *** experimentations are carried out to simulate water levels generated by the cyclonic storm AILA along the coast of *** model simulated results are found to be in a reasonable agreement with the limited available reported data and observations.

morphometric studies of 3D micro CT-scanned images can provide insights into the evolution of the brain and sensory structures but such data are still scarce for the most diverse mammalian order of *** reviewed and new data,we tested for convergence to extreme aridity and high elevation in the sensory and brain morphology of rodents,from morphometric data from micro-CT X-ray scans of 174 crania of 16 species of three distantly re-lated African murid(soft-furred mice,Praomyini,laminate-toothed rats,Otomyini,and gerbils,Gerbillinae)clades and one North American cricetid(deer mice and white-footed mice,Peromyscus)*** studies demon-strated convergent evolution acting on the oval window area of the cochlea(enlarged in extremely arid-adapted species of Otomyini and Gerbillinae)and on endocranial volume(reduced in high elevation taxa of Otomyini and Peromyscus).However,contrary to our predictions,we did notfind evidence of convergence in brain struc-ture to aridity,or in the olfactory/respiratory system(turbinate bones)to high *** structure differed,particularly in the petrosal lobules of the cerebellum and the olfactory bulbs,between Otomyini and Gerbillinae,with extreme arid-adapted species in each clade being highly divergent(not convergent)from other species in the same *** observed greater“packing”of the maxillary turbinate bones,which have important respiratory functions,in Peromyscus mice from high and low elevations compared to the high-elevation African Praomyini,but more complex patterns within Peromyscus,probably related to trade-offs in respiratory physiology and heat exchange in the nasal epithelium associated with high-elevation adaptation.

Pediatric Nephrotic Syndrome in a Cameroonian Cohorte: The Beast to Slaughter

Background: Idiopathic nephrotic syndrome (INS) is a frequent pathology in children. There is little data on the future of NS in children in sub-Saharan Africa, particularly Cameroon. The aim of our study is to report the prognosis of children treated for nephrotic syndrome in the city of Yaoundé. method: This was an analytical cross-sectional study with retrospective collection in 4 reference hospitals in the Cameroonian capital over a period of five years from January 1, 2018 to December 31, 2022. We included all medical records of patients treated for idiopathic INS. We excluded incomplete records and those with a history of chronic kidney disease. The sociodemographic, clinical, paraclinical, and therapeutic data, as well as the short-term evolution were collected in the files. Data was analysed using the software statistical package for social sciences version 25.0. Statistical significance was set at a p-value Results: A total of 131 children (58% boys) were included in our study over a period of 5 years. The median age was 8 [6 - 11] years. median proteinuria was 5 g/24h [3 - 8.4], median serum protein was 39 [34 - 46] g/l and median estimated glomerular filtration rate was 130.36 [68 - 174.6] ml/min/1.73m2. During steroid therapy, 45.07% were in partial remission at 2 months, 16.9% were in complete remission at 4 and 6 months, and 37.25% had relapsed. Steroid sensitivity was reported in 28.17% of cases, steroid resistance in 64.78% of cases and steroid dependent in 7.04% of cases. The mortality rate was 12.97%. Survival time averaged 48.2 months, with an overall crude survival rate of 99.2% at 3 and 6 months and 98.4% at 1 year. Regarding renal survival, renal function was impaired in 8.33% of patients at 6 months and 9% at 12 months. Conclusion: Idiopathic nephrotic syndrome is a common disease in children. Introduction: metastatic spinal tumors (mST) refer to secondary involvement of the vertebral column by hematogenously-disseminated metastatic cells. They could affect either the bony structures or the spinal cords. mechanical instability and neurologic deficits resulting from spinal cord compression are the most common manifestations. Surgical intervention remains the most effective treatment for about 20% of patients who present with spinal cord compression. The prognosis is relatively poor. This work has as objectives to describe: the diagnostic tools, the different modalities of management and the prognostic elements of spine metastasis. methodology: We conducted an ambispective cross-sectional descriptive study;with retrospective data collection from January 2015 to December 2021 and prospective collection from January to April 2022 in the “Neurosurgery” unit of the Yaounde Central Hospital and the “Oncology and Neurosurgery” units of Yaounde General Hospital. Result: We included 101 patients. The m/F sex ratio was 1.66. The average age of the participants was 56.44 years (±14.19 SD) with a median of 58 years. metastatic spinal tumors were discovered in 61.39% of patients with a previously known primary tumor and 21.78% of patients had newly discovered tumors. The neurologic examination revealed a vertebral syndrome in 79.21% of cases, radicular syndrome in 60.40% and sub-lesional syndrome in 59.89%. Sensory disorders accounted for 39.60% and sphincter disorders accounted for 34.65%. According to the degree of severity, the lesions were classified as Frankel E (37.62%) followed by Frankel D (21.78%). metastatic lesions were mostly found at the thoracic vertebrae (68.25%) and lumbar vertebrae (22.22%). The most represented primary tumors were: prostate tumors (41.58%) and breast tumors (23.76%);followed by malignant hemopathies (15.84%).
