Surgical management of facial dog bite trauma:A contemporary perspective and review

Objective:to perform a review evaluating management of and complications stemming from dog bite trauma sustained to the head and neck over the past *** Sources:PubMed and Cochrane ***:the authors searched the PubMed and Cochrane Library databases for relevant published literature.A total of 12 peer-reviewed canine-exclusive series inclusive of 1384 patient cases describing facial dog bite trauma met inclusion *** including fractures,lacerations,contusions,and other soft-tissue injuries were *** related to clinical course and management,operating room requirements,and antibiotic usage were compiled and *** trauma and surgical management complications were also ***:75.5%of patients sustaining dog bites required surgical *** these patients,7.8%suffered from postsurgical complications,including hypertrophic scarring(4.3%),postoperative infection(0.8%),or nerve deficits and persistent paresthesias(0.8%).Prophylactic antibiotics were administered to 44.3%of patients treated for facial dog bites and the overall infection rate was 5.6%.Concomitant fracture was present in 1.0%of ***:Primary closure,often in the OR may be necessary,with few cases requiring grafts or *** should be aware that the most common complication is hypertrophic *** research is needed to elucidate the role of prophylactic antibiotics.

Classical recommender systems provide users with a list of recommendations where each recommendation consists of a single item, e.g., a book or DVD. However, sev- eral applications can benefit from a system capable of recom- mending packages of items, in the form of sets. Sample appli- cations include travel planning with a limited budget （price or time） and twitter users wanting to select worthwhile tweeters to follow, given that they can deal with only a bounded num- ber of tweets. In these contexts, there is a need for a system that can recommend the top-k packages for the user to choose from. Motivated by these applications, we consider composite recommendations, where each recommendation comprises a set of items. Each item has both a value （rating） and a cost associated with it, and the user specifies a maximum total cost （budget） for any recommended set of items. Our composite recommender system has access to one or more component recommender systems focusing on different do- mains, as well as to information sources which can provide the cost associated with each item. Because the problem of deciding whether there is a recommendation （package） whose cost is under a given budget and whose value exceeds some threshold is NP-complete, we devise several approximation algorithms for generating the top-k packages as recommen- dations. We analyze the efficiency as well as approximation quality of these algorithms. Finally, using two real and two synthetic datasets, we subject our algorithms to thorough ex- perimentation and empirical analysis. Our findings attest tothe efficiency and quality of our approximation algorithms for the top-k packages compared to exact algorithms.

Introduction: Individuals with cerebral palsy (CP) often suffer from speech impairments and dysphagia due to impaired motor control of the tongue and lips. Aim: to investigate the effect of singing on peak flow, speech- and swallowing abilities in individuals with CP. Method: ten adults with CP received one-to-one singing lessons by a professional singing teacher for one hour twice a week for 6 months. Methods: the peak flow, swallowing-, speech-, and eating abilities were investigated pre, and after 3 months and 6 months of singing. Results: the meal analysis revealed that an average of eleven factors was improved after 6 months of singing. Nine out of ten participants improved their pronunciation of the letter “N” and seven participants improved their pronunciation of the letter “G”. the singing lessons increased the peak flow significantly (p = 0.02;Pre = 246 ± 36, 3 months = 287 ± 34, and 6 months = 336 ± 48). Self-reported difficulties on a 1 - 10 scale reduced significantly for both swallowing (p = 0.039) and speaking (p = 0.049). Conclusion: 6 months of singing lessons improve the peak flow, speech-, swallowing- and eating abilities in adults with CP. the present study indicates that singing is a very promising intervention for the treatment of speech and eating impairments in individuals with CP.

Bio-glycerol was synthesized from Cameroon palm kernel oil (PKO) through the transesterification procedure. Palm kernel oil extracted from palm kernel seeds using mechanical expression and solvent extraction was purified and characterized by physico-chemical methods and used in the transesterification process to give biodiesel and bio-glycerol. the biodiesel was purified and characterized as reported in previous articles. Our focus in this article is on glycerol, an important by-product of the transesterification process which has potential pharmaceutical, cosmetic and engineering applications. the bio-glycerol was purified by acidification and the purified glycerol was subjected to physical and chemical characterization. the specific gravity of glycerol was obtained as 1.2 kg/L, viscosity at 40°C gave 1500 cSt and 500 cSt at 100°C;pH was 7.4;the flash point was 160°C, and the AStM color was 2.0 before purification and zero after purification. the sulfur content was 0.016%w/v. this sulfur content is low thus posing no environment threat. the chemical composition of the synthesized bio-glycerol determined using IR spectroscopy and gas chromatography-mass spectrometry (GC-MS) confirmed the known chemical structure of glycerol. the purification and analysis of bio-glycerol is important as it can find applications in the pharmaceutical, cosmetic and food industries inter alia.

Edge preserved smoothing techniques have gained importance for the purpose of image processing applications A good edge preserving filter is given by nonlocal-means filter rather than any other linear model based approaches. this paper explores a different approach of nonlocal-means filter by using robust M-estimator function rather than the exponential function for its weight calculation. Here the filter output at each pixel is the weighted average of pixels with surrounding neighborhoods using the chosen robust M-estimator function. the main direction of this paper is to identify the best robust M-estimator function for nonlocal-means denoising algorithm. In order to speed up the computation, a new patch classification method is followed to eliminate the uncorrelated patches from the weighted averaging process. this patch classification approach compares favorably to existing techniques in respect of quality versus computational time. Validations using standard test images and brain atlas images have been analyzed and the results were compared with the other known methods. It is seen that there is reason to believe that the proposed refined technique has some notable points.

AIM to describe the development and implementation of a person-centered endoscopy safety checklist and to evaluate the effects of a "checklist intervention".MEtHODS the checklist,based on previously published safety checklists,was developed and locally adapted,taking patient safety aspects into consideration and using a person-centered approach. this novel checklist was introduced to the staff of an endoscopy unit at a Stockholm University Hospital during half-day seminars and team training sessions. Structured observations of the endoscopy team's performance were conducted before and after the introduction of the checklist. In addition,questionnaires focusing on patient participation,collaboration climate,and patient safety issues were collected from patients and staff. RESULtS A person-centered safety checklist was developed and introduced by a multi-professional group in the endoscopy unit. A statistically significant increase in accurate patient identity verification by the physicians was noted(from 0% at baseline to 87% after 10 mo,P mained high among nurses(93% at baseline vs 96% after 10 mo,P = nonsignificant). Observations indicated that the professional staff made frequent attempts to use the checklist,but compliance was suboptimal: All items in the observed nurse-led "summaries" were included in 56% of these interactions,and physicians participated by directly facing the patient in 50% of the interactions. On the questionnaires administered to the staff,items regarding collaboration and the importance of patient participation were rated more highly after the introduction of the checklist,but this did not result in statistical significance(P = 0.07/P = 0.08). the patients rated almost all items as very high both before and after the introduction of the checklist;hence,no statistical difference was *** the intervention led to increased patient identity verification by physicians-a patient safety improvement. Past research has demonstrated that digital Fresnel holograms can be binarized in a non-iterative manner by downsampling the source image with a grid lattice prior to the hologram generation process. the reconstructed image of a hologram that is binarized with this approach is superior in quality compared with that obtained with direct thresholding, half-toning, and error diffusion. Despite the success, the downsampling mechanism results in a prominent texture of regularly spaced voids in the shaded regions. to alleviate this problem, an enhanced non-iterative method for the generation of binary Fresnel holograms is presented. Our method is based on a multi-direction line-sampling formed by a combined grid and cross lattice, which is capable of preserving a more solid texture in the shaded regions and enhancing the visual quality of the reconstructed image. Computer simulations and optical reconstructions are shown to demonstrate the effectiveness of our proposed technique.
